Evaluation of both arterial and central venous blood can be valuable in monitoring the critically ill veterinary patient. The traditional approach, which concentrates on arterial blood analysis only, may miss important aspects of oxygen delivery to tissues, especially in patients with poor perfusion. The advances that have resulted in affordable bedside blood gas analyzers have created a clinical situation in which blood gas analysis should be an integral part of critical care monitoring. Following basic principles of interpretation, blood gas analysis, which has traditionally been viewed as a complex method of monitoring, should become more useful. Assessing both the arterial and central venous samples should result in more efficient and higher quality care for veterinary patients.  相似文献

The clinical hematology practices utilized at veterinary teaching hospitals and private veterinary diagnostic laboratories were surveyed using a questionnaire. The hematology caseload at private diagnostic laboratories was larger, and comprised predominantly of canine and feline submissions. The Coulter S Plus IV and Serono Baker 9000 were the hematology analyzers used most frequently at veterinary medical laboratories. The Abbott Cell-Dyn 3500, a multispecies analyzer capable of leukocyte differential counting, was utilized more by private laboratories. Commercial hematology control reagents were used at all laboratories; teaching hospital laboratories more often used reagents supplied by the manufacturer of the analyzer. A greater percentage of private diagnostic laboratories participated in the external quality assurance programs offered by Veterinary Laboratory Association and College of American Pathologists. While private diagnostic laboratories retained the EDTA blood specimens longer after initial testing, the teaching hospital laboratories retained blood smears and complete blood count reports longer. The complete blood count reports at veterinary teaching laboratories more often included red blood cell volume distribution width, mean platelet volume, manual hematocrit, plasma protein, and leukocyte differentials as absolute concentrations. The laboratory practices utilized by these veterinary medical laboratories were generally similar, and differences were attributed to divergent emphasis on economic accountability and clinical investigation.  相似文献

OBJECTIVE: To estimate the effect of gender on ownership and income in veterinary practice in Australia. METHODS: Questionnaire completed by private veterinary practitioners, and analysed using the SAS System for Windows 7.0. RESULTS: More than three-quarters (78%) of male but 36% of female private practitioners were partial or sole owners of practices. The median annual income for all male practitioners working more than 40 hours/week was $70K, but that for females was $43K. These disparities existed in both city and country practices, and in the case of income it increased with increasing time in the workforce. Male practice owners also reported higher incomes than female owners. CONCLUSIONS: Female veterinary practitioners are less likely to own practices, and more likely to earn low incomes than males. These differentials do not appear to be due to location, hours worked or years since graduation or, in the case of income, to whether they are owners or employees. The evidence points to a lower interest by women than men in the business aspects of veterinary practice.  相似文献

Reptile medicine has emerged as a specialty area within the broader field of zoological medicine. It encompasses the medical needs of approximately 7,500 vertebrate species. This vertebrate class is highly diversified, having biological and medical peculiarities that differ both between and within major groups. Historically, veterinarians who have become recognized specialists with reptiles have had limited formal training in their medical management. The pet reptile trade is a multi-million-dollar business, and the popularity of reptiles as pets has resulted in a need for more veterinarians with training in their medical management. While few private practices have high volumes of reptile cases, many small-animal practices will have the opportunity to see a significant number of reptiles on an annual basis. Most practitioners with reptile medical expertise have merged their experiences as reptile pet owners with the principles of veterinary medicine taught in veterinary college. Several North American veterinary colleges have reptile medicine courses, and most have didactic and clinical courses in exotic and zoo animal medicine that include lectures and practical experience. Most accredited zoological medicine residency training programs include training in reptile medicine. The case load and interest in reptile medicine will probably never be sufficient to lead the average veterinary college to develop much more than what is currently offered. Consequently, those few colleges having more extensive course offerings, both didactic and clinical, will serve as educational centers for this discipline. Future Web-based teaching programs in reptile medicine will allow students nationally and internationally to have access to instructional material that can be continually updated.  相似文献

Recent trends in urbanization of the population, increased need for bio-security on large farms, and more food-animal or mixed-animal practitioners approaching retirement age are forcing a renewed focus on recruiting and training veterinary students with an interest in production-animal medicine. The increasing number of veterinary students coming from urban backgrounds has led to a need to expose these students to standard animal-production practices and to interest them in a career involving food animals. This article describes one such program developed at Iowa State University, in which 14 students obtained hands-on experience in all aspects of swine and dairy production across a wide sampling of herd size, housing style, bio-security levels, and production phases. The participating students, ranging from senior undergraduates to third-year veterinary students, gained valuable insight not only into daily farming practices but also the knowledge and skills necessary to provide quality veterinary care to these clients. The first year of this program has yielded positive feedback from all participants, including the veterinary practices, private producers, corporate sponsors, and students. Current applicants cite positive comments from past participants as motivating their interest in the program. This program has the potential to expand as an opportunity to educate selected students in the field of food-supply veterinary medicine and to help fill the anticipated void in this area.  相似文献

Renal transplantation is associated with several potential complications. Early selection and appropriate screening of candidates has reduced the morbidity and mortality associated with this procedure in human beings. In veterinary medicine, the important process of identification and screening of candidates for renal transplantation occurs primarily in private general or specialty practice. This article reviews guidelines for screening of candidates for renal transplantation and related publications in both veterinary and human literature in an attempt to provide comprehensive information to practitioners involved in the selection process. It is anticipated that early referral of stable cats for renal transplantation will further improve the likelihood of a successful outcome.  相似文献

In the present market, veterinarians with a strong background in career development, practice management, and business skills have a clear advantage in achieving financial success. Although there is ample evidence that the scientific and clinical skills of veterinary college graduates are high, there are also data that suggest that additional capabilities in the business realm may promote greater economic success. As noted in the KPMG executive summary, the field of veterinary medicine must make changes in its "current business practices and attitudes" to be successful in the future. Furthermore, the KPMG study found that 36% of industry employers reported that some jobs within their companies had specific job requirements that were not met by a veterinarian with only a veterinary medical degree. The areas of additional training most often cited included business, administration, personnel management, sales and marketing, and financial skills. Yet, Lewis and Klausner found that veterinarians reported challenges in the business realm, such as "how business works and how business goals are translated into action. This challenge held true for veterinarians in industry, academia, government, and private practice." The present gender trends in the field of veterinary medicine provide additional impetus to make career development and business skills training more prevalent. Presently, women comprise >65% of the veterinary student population and approximately 45% of all practicing veterinarians. In some areas of practice, the rate is much higher. For example, in 2002, women comprised 48.2% of all small animal exclusive private practitioners. Unfortunately, the KPMG study found that female veterinarians in private practice report lower self-evaluation of business management and financial skills, compared with their male cohorts. Female veterinarians in nonprivate practice report lower self-evaluation in communication, personnel management, business management, and marketing skills than that reported by males. As a result of these pressing needs, CSU CVMBS has undertaken a major initiative to improve the veterinary practice management and business skills training of veterinary students by offering a variety of options to gain this knowledge: a combined MBA/DVM degree program, a Business Certificate Program for Health Professions, and core curriculum courses. In this way, students can select the amount of focus they want to place on career development and business skills as they earn their DVM degree, to best ensure that they become successful veterinarians.  相似文献

Veterinary directories from Canada's 4 western provincial veterinary associations provided source data for compiling demographic profiles of the veterinary profession for the years 1991, 1996, 2001, and 2007. From 1991 to 2007 the number of veterinary practitioners in western Canada increased by 1197 (79%), for a net gain of 75 veterinarians/y. Of these, 786 (66%) were companion animal (CA) practitioners, 184 (15%) mixed animal (MA) practitioners, 96 (8%) food animal (FA) practitioners, and 131 (11%) were classified as "Other." The number of veterinary clinics also increased over this same period by 301 (38%), of which 266 (88%) were CA practices, 5 (2%) MA practices, and 31 (10%) FA practices. The majority of CA (75%), MA (68%), and FA (86%) practices had ≤ 2 veterinarians.  相似文献

A workforce survey of private veterinary practices in western Canada was conducted in 2020. Data were obtained on 526 practices (response rate = 39.5%) and 1445 individual veterinary practitioners. Overall, 68.4% of practitioners identified as female, with 4 times as many females as males comprising the youngest age cohorts (26 to 35 y) of the profession. The majority of practices (67.9%) were companion animal, followed by mixed animal (21.9%) and food animal (10.2%). Most females (77.2%) and males (57.8%) were engaged in companion animal practice, whereas 23.5% of males and 6.0% of females were food animal practitioners. During an average work week, practitioners devoted 77.4% of practice time to small animals, 15.1% to food animals, and 7.5% to equine animals. A greater proportion of males (75.2%) versus females (63.2%) worked on a full-time equivalent basis (P < 0.001). Whereas males were 1.7 times (95% CI = 1.3 to 2.3; P < 0.001) more likely to be practice owners than females, 54.5% of females were owners. Practice ownership was lower than in previous surveys, a trend that may have long-term implications with respect to the corporatization of the veterinary profession.  相似文献

Veterinary clinicians can make a significant contribution to the knowledge of the practical value of using alternative treatment methods by conducting clinical trials. By following a logical sequence of steps the clinician can design and participate in a clinical trial in his clinic without interfering with his normal practice. The successive stages of a field trial were presented along with an example to demonstrate the application of the principles involved. These stages are: design and planning, implementation, collection of data, and analysis of data.

The veterinary clinician who initiates clinical trials to answer questions plaguing him provides two services to the veterinary profession. First, he helps answer questions facing private practitioners throughout the profession. Second, he can demonstrate that not all research must be done in a laboratory setting isolated from “real world” complications. Seeing the results that can be derived from individual practicing veterinarians participating in clinical trials should act as a stimulus to other veterinarians to logically organize the data coming from their practices.

We have previously defined a set of 62 attributes-12 in the area of professional characteristics, 28 addressing knowledge and understanding, and 22 delineating skills-that veterinary students should be expected to have demonstrated by the time of their graduation (Walsh DA, Osburn BI, Christopher MM. Defining the attributes expected of graduating veterinary medical students. J Am Vet Med Assoc 219:1358-1365, 2001). We have used this set of attributes as the basis of an outcomes assessment completed by California practitioners to determine whether graduates from the University of California School of Veterinary Medicine are meeting these expectations. Based upon this assessment, these 62 defined attributes appear to reflect very well practicing veterinarians' views and expectations of DVM graduates. The survey results also indicate that, overall, the recent University of California graduates are meeting these set of expectations. Simultaneously, the outcomes assessment focused attention on several areas, including private practice management, work expectations for successful practice, and surgical capabilities. For each, California practitioners recommended that the definition of the expectation be expanded and that the level of achievement by graduates be improved. Defining a set of attributes expected of veterinary graduates is a key step in obtaining an effective outcomes assessment of a professional educational program.  相似文献

Increasing fiscal constraints on the government, a lackadaisical performance by public sector animal health and breeding services and pressure from donor partners have prompted the governments of various developing countries to rethink the role of the public sector in the provision of veterinary services. Various countries have started to implement, or have already implemented, privatization of some veterinary services. The results are mixed. It is established that private provision alone is not optimal, and a blend of private and public sector veterinary services is required to utilize the virtues of both.The privatization process has also begun in India. Certain state governments in India are pursuing a cost recovery approach and are encouraging private practitioners to cope with the financial constraints and to deliver broad and effective animal health and breeding services. This paper considers the global aspects of the privatization of veterinary services as well as the scenario in India, so as to gain an insight into the very complex and debatable issue of privatization of veterinary services.  相似文献

Although access to veterinary blood banks has increased, practitioners may prefer to create their own blood-donor program to provide for their blood-product needs or to respond to an emergent need. Non-payment of bills is a phenomenon that has grave economic consequences for all entrepreneurs, veterinary practitioners included. Little in depth research has been performed into the non-payment of veterinary bills. The aim of this study was to quantify the non-payment of bills in Dutch veterinary practices and to investigate the procedures implemented in the case of non-payment and their effectiveness. Non-payment of bills is a potential source of a substantial loss of income in Dutch veterinary practices. Even though veterinarians actively tried to collect the money owed them, often bills were paid very late or were paid in part only.  相似文献

There was only limited emphasis on anesthesia and pain management in veterinary colleges in Russia until the last ten years. There is now great interest in improvement by Russia practitioners. This effort is hampered by the lack of approved and available medications and limited instructions. At this time, the small animal practitioners have tranquilizers, alpha2 agonists local anesthetics and NSAID's for their use. Propofol is available through pharmacies and efforts are being made for veterinarians to use ketamine. Only a few practices have inhalant anesthetic units and halothane. During a recent lecture tour of Russia, general clinical principles and specific anesthetic and pain management protocols were taught. The audience, primarily young and progressive veterinarians, actively participated and showed great desire to practice quality anesthesia and pain management for their patients. Teaching young veterinarians how to manage their cases without the availability of opioids, ketamine advanced anesthetic equipment and monitors was indeed challenging. Protocols for perioperative management consisting of medetomidine with or without local anesthesia followed by carprofen for post-operative pain is an improvement. We sometimes spend so much time discussing which of many drug combinations we will use but seldom consider what we would do if we were practicing our specialty in parts of the world where we would have to modify dosages and combinations to achieve humane care with few drug choices.  相似文献

As efforts to reduce the overpopulation and euthanasia of unwanted and unowned dogs and cats have increased, greater attention has been focused on spay-neuter programs throughout the United States. Because of the wide range of geographic and demographic needs, a wide variety of programs have been developed to increase delivery of spay-neuter services to targeted populations of animals, including stationary and mobile clinics, MASH-style operations, shelter services, feral cat programs, and services provided through private practitioners. In an effort to ensure a consistent level of care, the Association of Shelter Veterinarians convened a task force of veterinarians to develop veterinary medical care guidelines for spay-neuter programs. The guidelines consist of recommendations for preoperative care (eg, patient transport and housing, patient selection, client communication, record keeping, and medical considerations), anesthetic management (eg, equipment, monitoring, perioperative considerations, anesthetic protocols, and emergency preparedness), surgical care (eg, operating-area environment; surgical-pack preparation; patient preparation; surgeon preparation; surgical procedures for pediatric, juvenile, and adult patients; and identification of neutered animals), and postoperative care (eg, analgesia, recovery, and release). These guidelines are based on current principles of anesthesiology, critical care medicine, microbiology, and surgical practice, as determined from published evidence and expert opinion. They represent acceptable practices that are attainable in spay-neuter programs.  相似文献

We used an exploratory study in two districts of West Bengal State, India to find out the constraints of private veterinary practice. Privatization of veterinary services is being seen as an alternative to government-sponsored veterinary services to livestock. In December 2000 to February 2001, 50 private veterinary practitioners were interviewed from the districts. Their opinion was that weak regulations about competition from illegal practitioners and the use of veterinary drugs, competition by animal-health technicians (who are likely to charge less than veterinarians), absence of efficient diagnostic-laboratory support, and lack of commercial livestock farming were the main constraints faced.  相似文献

The complete blood count is one of the basic building blocks of the minimum database in veterinary medicine. Over the past 20 years, there has been a tremendous advancement in the technology of hematology analyzers and their availability to the general practitioner. There are 4 basic methodologies that can be used to generate data for a complete blood count: manual methods, quantitative buffy coat analysis, automated impedance analysis, and flow cytometric analysis. This article will review the principles of these methodologies, discuss some of their advantages and disadvantages, and describe some of the hematology analyzers that are available for the in-house veterinary laboratory.  相似文献

The bovine practitioner has a critical role to play in promoting biosecurity at both the farm level and the national level. Successful exclusion of exotic diseases, biocontainment of endemic diseases, and emergency preparedness rest soundly on bovine practitioners as part of the national biosecurity team. Bovine practitioners must voice their opinions on the strengths and weaknesses of existing and proposed national biosecurity programs. Healthy debate about national biosecurity programs and consideration of biosecurity issues by national veterinary organizations provide valuable feedback for the continual improvement of the programs and enhance their credibility.
