OBJECTIVE: To report short- and long-term clinical and radiographic outcomes in dogs with humeral condylar fractures repaired using self-compressing Orthofix pins. STUDY DESIGN: Retrospective evaluation with solicited long-term clinical and radiographic evaluations. ANIMALS: Twenty-three dogs with humeral condylar fractures. METHODS: Medical records and radiographs were reviewed. Owners were asked to return dogs for long-term clinical and radiographic evaluation. RESULTS: Dogs were aged 1.5-26 months (mean+/-SD, 5.8+/-5.0 months) with 20 dogs weighing <5 kg (mean, 3.6+/-2.8 kg). Of 21 dogs with immediate post-operative radiographs, 10 had anatomic reduction with the rest having either a step and/or gap at the articular surface. Kirschner wire migration and implant loosening were the most common post-operative complications. All fractures with adequate follow-up radiographic evaluations achieved union. Twelve of 13 dogs returned for long-term evaluation (mean, 19.0+/-18.5 months) were either sound (10 dogs) or had subtle, weight-bearing lameness (2). Fourteen dogs had radiographs >/=75 days after surgery (mean, 18.7+/-18.3 months), 8 dogs (57%) had no radiographic evidence of osteoarthritis (OA; mean OA score, 0.8; median 0), and all dogs had good or excellent limb function. CONCLUSIONS: Self-compressing Orthofix pins are suitable implants for the stabilization of humeral condylar fractures in small breed dogs. Implants were convenient and simple to use and complications were easily resolved. Dogs consistently had good long-term clinical and radiographic outcomes with no or minimal lameness and OA. CLINICAL RELEVANCE: Self-compressing Orthofix pins should be considered for the repair of humeral condylar fractures in small breed dogs.  相似文献

Humeral condylar fractures have fracture lines that extend through the joint surface and through one or both epicondyles. One hundred and eighty-nine dogs with humeral condylar fractures are presented in this study. Lateral condylar fractures were the most frequent (67 per cent), occurring nearly 10 times as often as the medial condylar fracture. Intercondylar fractures represented about 35 per cent of these. Condylar fractures occur as a result of moderate trauma and only rarely due to traffic accidents. The histories and circumstances of the three fracture types are not essentially different. Dogs, three and four months old, were strongly over-represented among the lateral and medial condylar fractures, compared with the age distribution of other types of fractures in the extremities of the dogs. Humeral condylar fractures were recorded in 58 breeds. All dogs are vulnerable to humeral condylar fractures. All sizes and both sexes are equally at risk. Some breeds seem to be more vulnerable to lateral condylar fractures: English and French bulldogs, Yorkshire terriers, miniature schnauzers, Gordon setters and German short-haired pointers. The St Bernard seems to be more vulnerable to medial and intercondylar fractures and the German shepherd dog and beagle to intercondylar fractures. A retrospective case control study does not provide wholly reliable statistical information on risk factors, and the statistics concerning breed and breed size must be judged accordingly. The method was considered to be the best statistical model available for this study.  相似文献

OBJECTIVE: To compare shear stability of simulated humeral lateral condylar fractures reduced with either a self-compressing pin or cortical bone screw. STUDY DESIGN: In vitro biomechanical tests. SAMPLE POPULATION: Bilateral cadaveric canine humeri (n=18) without evidence of elbow disease. METHODS: Lateral condylar fracture was simulated by standardized osteotomy. Bone fragments were stabilized with a self-compressing pin or a cortical bone screw (2.7 or 3.5 mm) inserted in lag fashion. Specimens were mounted in a materials testing system and the condylar fragment displaced in a proximal direction until failure. Mechanical testing variables derived from load-deformation curves were compared between stabilization methods using a Student's paired t-test. RESULTS: There were no statistically significant differences for mechanical testing variables between pin and screw stabilized specimens at expected walk and trot loads. Three yield points subjectively coincided with yield of the interfragmentary interface (Y1), bone at the implant interface (Y2), and implant deformation (Y3). Displacements at Y1 were 48-156% greater for pin than screw stabilized specimens. Y2 and Y3 loads were higher for screw than pin stabilized specimens, but likely supraphysiologic for dogs convalescing after surgical repair. CONCLUSIONS: A self-compressing pin or a cortical bone screw inserted in lag fashion both provided adequate strength in applied shear to sustain expected physiologic loads through the repaired canine elbow during postoperative convalescence. CLINICAL RELEVANCE: Because self-compressing pins were easy to implant and mechanical properties were not significantly different than cortical screws at expected physiologic loads, pins should be considered for the repair of traumatic humeral condylar fractures.  相似文献

OBJECTIVES: To describe bilateral fixation of Y-T fractures of the humeral condyle via combined medial and lateral approaches, and to determine the technique's clinical and radiographic short-term outcomes. METHODS: Details of 30 consecutive fractures in 29 dogs were reviewed. These included signalment, method of fixation, complications, and follow-up limb function and range of elbow joint motion. RESULTS: The age of the dogs ranged from three months to nine years, and bodyweight ranged from 1.9 to 48 kg. The humeral condyle was reattached to the shaft using medial and lateral bone plates in 18 fractures, a medial plate and lateral Kirschner wire(s) in six fractures, and medial and lateral Kirschner wire(s) in six fractures. Major complications were recorded in four fractures and minor complications in two fractures. Limb function at follow-up was graded as excellent in 12, good in 15 and fair in three fractures. The range of elbow flexion was normal in seven, mildly reduced in 18, moderately reduced in four and severely reduced in one fracture. CLINICAL SIGNIFICANCE: In contrast to the caudal approach, combined medial and lateral approaches decrease the extent of periarticular soft tissue dissection, avoid complications associated with olecranon osteotomy and enable exposure of the entire humeral diaphysis for fixation. Bilateral fixation is likely to be better at counteracting bending and torsional forces compared with unilateral fixation.  相似文献

Femur fractures occur commonly in cats and dogs following substantial trauma. Fractures of the femur may be categorized as capital physeal, femoral neck, trochanteric, subtrochanteric, diaphyseal, supracondylar or condylar or distal physeal. Most femur fractures are closed due to the heavy overlying muscle, unless due to a penetrating injury such as a gunshot wound. Femur fractures are generally not amenable to conservative repair, and some kind of internal fixation is generally required. Implant systems suitable for repair of femur fractures include bone plates, interlocking nails, plate-rod construct, lag screws, pins and wires and external fixators. Fractures may be repaired using anatomic reduction and rigid fixation or using the principles of biologic osteosynthesis. Biologic osteosynthesis is particularly effective for highly comminuted fractures because vascular supply and soft tissue attachments to bone fragments are preserved, speeding the formation of bone callus. Articular fractures should be anatomically reduced and rigidly stabilized to reduce the chance of progressive osteoarthritis.  相似文献

OBJECTIVE: To report a technique for fluoroscopically guided closed reduction with internal fixation of fractures of the lateral portion of the humeral condyle (FLHC) and determine the long-term results in 10 clinical cases. STUDY DESIGN: Prospective clinical case study. ANIMALS: Ten dogs with 11 fractures. METHODS: Fractures of the lateral portion of the humeral condyle were stabilized with transcondylar screws and Kirschner wires. Closed reduction and implant placement were achieved using intraoperative fluoroscopic guidance. After fracture repair, postoperative radiographs were evaluated for articular alignment and implant placement. Dogs were evaluated after surgery by means of lameness scores, elbow range of motion (ROM), radiographic assessment, and owner evaluation of function. RESULTS: Postoperative reduction was considered anatomic in 6 fractures with all other fractures having <1.5 mm of malreduction. Follow-up was available for 9 patients from 9 to 21 months after surgery. All of the fractures had healed. One minor (wire migration) and one major (implant failure) complication occurred. Mean lameness scores were 0 (n = 6), 0.5 (n = 2), and 1 (n = 1) at the time of final follow-up. No significant differences were found in follow-up ROM values between affected and unaffected elbows. All of the dogs in this study regained 90-100% of full function, based on owner assessment. CONCLUSIONS AND CLINICAL RELEVANCE: Fluoroscopic guidance for closed reduction and internal fixation of FLHC in dogs is an effective technique.  相似文献

Bone plate fixation was reviewed in 29 distal radial fractures of small- and miniature-breed dogs. Twenty-two fractures in 18 dogs were available for follow-up. Number of complications and return to function were evaluated. Complications occurred in 54% of the fractures. Catastrophic complications occurred in 18% of fracture repairs with follow-up, while minor complications occurred in 36%. Sixteen (89%) of 18 dogs had a successful return to function. Bone plate fixation is a successful repair method for distal radius and ulna fractures in small-breed dogs, compared to previously reported methods.  相似文献

A simulated lateral humeral condylar fracture was created in each of the 52 humeri collected from 26 dogs. One humerus from each pair was stabilized with a 2.0 mm cortical bone screw which was inserted in lag fashion. The other humerus from each pair was stabilized with a 2.2 mm threaded diameter Orthofix pin inserted across the condyle. Prior to each repair, an antirotational K-wire was placed and then the Pressurex Sensitive film was inserted in the osteotomy site in order to determine the compressive pressure (MPa), compressive force (KN), and area of compression (cm(2)) achieved during fixation. The maximum insertional torque achieved before stripping was measured for each implant. The mean compression generated by insertion of a 2.0 mm lag screw was 20.36 +/- 1.51 MPa compared to 18.88 +/- 1.76 MPa generated by a 2.2 mm Orthofix pin (p < 0.003). The mean area of compression generated by insertion of a 2.0 mm lag screw was 2.39 +/- 1.29 cm(2), compared to 1.16 +/- 0.84 cm(2) generated by insertion of a 2.2 mm Orthofix pin (p < 0.0001). The mean compressive force (compression x area compressed) generated by insertion of a 2.0 mm lag screw was 4.96 +/- 2.90 Kn, compared to 2.20 +/- 1.65 Kn generated by insertion of a 2.2 mm Orthofix pin (p < 0.0001). The mean insertion torque to failure for the lag screws was 0.49 +/- 0.07 NM, compared to 0.91 NM +/- 0.18 NM generated by the Orthofix pins (P < 0.0001). Both repair methods are likely to be acceptable for the repair of similar fractures in small breed dogs.  相似文献

Methods of repair of central tarsal bone (Tc) fractures in the racing greyhound can involve coaptation, single-screw fixation, or 2-screw fixation. Successful management, defined as a return to competitive racing, was obtained in 71% of 114 affected dogs treated by these methods. Both 1- and 2-screw fixation, with interfragmentary compression, was used to repair Tc fractures in 81 dogs. It was concluded that satisfactory results can be expected by use of open reduction and screw fixation in all types of Tc fractures except those that are severely comminuted.  相似文献

Deep-Frozen Allogeneic Cancellous Bone Grafts in 10 Dogs: A Case Series   总被引:1,自引:0,他引:1  
Deep-frozen, aseptically collected and processed allogeneic cancellous bone was implanted in eight dogs during the surgical repair of diaphyseal long bone fractures and in two dogs during arthrodeses. A combined allogeneic and autogeneic cancellous bone graft was used in two fractures with a segmental bone loss of more than 5 cm. Bone union occurred in five fractures and in both arthrodeses. Failure of fixation occurred in two dogs with nonunion fractures and in a third dog with an open, infected fracture. Biopsies from the fracture sites were obtained from these dogs following failure of their fracture fixation. The cancellous bone graft appeared to be in the process of normal incorporation in each case. Failure of fixation was attributed to technical or case management errors or both, in each of the three fractures that failed to achieve bony union. Frozen allogeneic cancellous bone grafts were effectively incorporated when used in the primary repair of fractures and arthrodeses. Combined autogenous and allogeneic cancellous bone grafts may be particularly useful in the repair of fractures with large segmental diaphyseal bone defects. The use of allogeneic cancellous bone grafts in nonunion fractures requires further investigation before it can be recommended.  相似文献

An evaluation of 157 dogs with humeral fractures was performed. Cocker spaniels were more likely to have humeral condylar fractures (HCFs) than other breeds ( P < .0001). Male cocker spaniels were at increased risk ( P < .001). Cocker spaniels had more bilateral HCFs than other breeds of dogs ( P < .001). Eighteen dogs (17 purebred spaniels and 1 crossbred spaniel) with HCFs of unknown cause or occurring with normal activity were further studied, using radiography of their humeral condyle bilaterally (n = 18), computed tomography (n = 3), biopsy (n = 2), bone scintigraphy (n = 2), and genetic evaluation (n = 8). Fourteen of these 18 dogs had a nonfractured contralateral condyle. Twelve (86%) of the 14 nonfractured humeral condyles had a radiolucent line within the center of the condyle, 13 (93%) had radiographic signs of degenerative joint disease and an abnormal medial coronoid process, and six (43%) had periosteal proliferation involving the lateral epicondyle. Examination of biopsy samples from the fracture sites of two cocker spaniels showed fibrous tissue present at the fracture surfaces. The results of this study suggest an association between incomplete ossification of the humeral condyle in cocker spaniels and Brittany spaniels and a high prevalence of HCFs. Eight affected cocker spaniels with available pedigree information were found to be genetically related, suggesting that incomplete ossification of the humeral condyle may be a genetic disease with a recessive mode of inheritance.  相似文献

Case histories of 130 dogs and cats with humeral fractures were reviewed. The different types of fractures were classified. Most animals with proximal, shaft, and supracondylar fractures had excellent results. The poor prognosis associated with distal articular fractures was due most often to failure of the fixation device in the supracondylar area; best results were achieved with a plate on the caudal and medial surface of the distal humerus.  相似文献

A 2-year-old sexually intact male Paint horse weighing 427 kg (940 lb) was admitted for examination and treatment of intermittent non-weight-bearing lameness of the right hind limb of 1 week's duration. Radiography revealed a displaced Salter-Harris type-III fracture of the right femoral condyle with the sagittal component of the fracture line located in the intercondylar space and the transverse component exiting on the medial aspect of the femur. The fracture was repaired with a condylar screw plate designed for repair of femoral condylar fractures in humans. The owner reported by telephone 9 months after surgery that the horse was sound. To our knowledge, use of this particular implant system for fixation of a Salter-Harris type-III fracture on the medial side of the femur in a horse has not been described. Results in this horse suggest that this implant can be successfully used for repair of femoral condylar fractures in selected adult horses.  相似文献

The purpose of this study was to evaluate the occurrence of humeral condylar fractures in Vietnamese pot-bellied pigs and to postulate a possible predisposing cause for these fractures. Thirteen Vietnamese pot-bellied pigs (Group A) were evaluated over an eight year period (1990-1998), each with a history of either a unilateral or bilateral forelimb lameness. The cause of lameness was localized to the elbow region. Of the thirteen pigs, twenty-one elbows were evaluated radiographically. Pigs ranged in age from six months to four years old. All pigs over the age of seven months showed radiographic evidence of elbow degenerative joint disease. Fractures involving the medial aspect of the humeral condyle were identified in 8/21 studies (38%). A well-defined linear intracondylar articular lucency was identified in 7/21 studies (33%) on the craniocaudal projection. The site of this lucency corresponded to the location of the articular component of the fractures seen involving the humeral condyle. The elbows of five pot-bellied pigs with no known history of forelimb lameness or trauma (Group B) were evaluated radiographically following euthanasia. All five pigs were of unknown age and gender. An intracondylar vertical linear lucency was identified bilaterally in three pigs (60%). Concurrent degenerative joint disease was present in all instances. The remaining two pigs were radiographically normal. Computed tomography of the elbows was performed in one affected pig from Group B. The radiographic findings in this pig were verified. Histopathology of the right elbow of this affected pig was diagnostic for incomplete endochondral ossification of the humeral condyle. A similar condition involving the humeral condyle has been previously described in Cocker and Brittany Spaniels. These canine breeds also have a high incidence of humeral condylar fractures. It is postulated that Vietnamese pot-bellied pigs are similarly prone to humeral condylar fractures, even in the absence of known trauma, due to incomplete ossification of the humeral condyle.  相似文献

In a series of 20 dogs, 23 fractured ilia were reduced and stabilized with a variety of bone plates. The results indicated that fixation was rigid enough for fracture healing and for the patient to be ambulatory soon after surgery. Plate fixation can therefore be recommended as a reliable method of repair in selected fractures of the canine ilium.  相似文献

OBJECTIVE: To assess the suitability of the intramedullary interlocking nail to stabilise humeral diaphyseal fractures in dogs and cats. METHOD: This multi-centre study retrospectively examined medical records, between June 1994 and May 2001, of 19 dogs and one cat, in which a total of 21 humeral fractures were stabilised with intramedullary interlocking nails. RESULTS: Animals ranged in body-weight from 4 to 97 kg. Eighteen (86%) of the fractures were comminuted. Adjunctive stabilisation was used in twelve (57%) fractures and bone grafts in nine (43%) fractures. A rapid return of function was noted in the majority of animals, with 14 (67%) having good or excellent function within four days of surgery. In two fractures the repair collapsed when a single proximal transcortical screw was placed cranial to the tricipital line of the humerus. This suggests that if a single transcortical screw is placed proximally the screw should be distal or caudal to the tricipital line in order to engage sufficient cortical bone. Eighteen (86%) of the fractures healed when stabilised with intramedullary interlocking nails. Three fractures did not heal. One was in a dog where a pathological fracture was temporarily stabilised with an intramedullary interlocking nail, one in a dog that died of an abdominal crisis three weeks after surgery and one in a dog in which fracture stabilisation collapsed due to incorrect implant selection. CONCLUSION: Intramedullary interlocking nails are well suited to the stabilisation of humeral diaphyseal fractures in dogs and cats.  相似文献

REASONS FOR PERFORMING STUDY: Complications of osteosynthesis including superficial and deep infections continue to affect adversely outcome in equine surgery of the lower limb. OBJECTIVE: To describe a technique for minimally invasive plate fixation in horses and evaluate clinical results of the technique. HYPOTHESIS: Lower complication rates would occur in horses treated with a minimally invasive approach compared with conventional techniques. METHODS: Cases of minimally invasive plate fixation from January 1999 to December 2003 were evaluated. Case records, radiographs and race records for horses that sustained distal third metacarpal/metatarsal (Mc/tIII) condylar fractures were assessed. Follow-up information was obtained and results analysed. In addition, results for horses that received arthrodesis by a conventional, open approach during the same period were evaluated and statistical analysis was performed. RESULTS: Thirty-two cases of minimally invasive plate fixation were identified (22 incomplete distal Mc/tIII condylar fractures with a spiralling or diaphyseal component, 10 metacarpophalangeal arthrodeses and 4 pastern arthrodeses). For both types of arthrodesis, surgery times and infection rates were lower and survival rates higher in the minimally invasive group, but no statistical difference was found. Spiralling condylar fractures were successfully managed, without catastrophic fracture. CONCLUSIONS AND POTENTIAL RELEVANCE: Minimally invasive percutaneous plate osteosynthesis (MIPPO) can be successfully used for select applications in horses, and may reduce morbidity and mortality in equine fracture repair.  相似文献
