犬急性胰腺炎是因胰腺分泌的胰酶消化自身器官所引起的以胰腺水肿、出血、坏死为主要特征的一种疾病,是犬常见的消化系统疾病之一。该文结合具体临床病例,对一例犬的临床疑似病例进行血液学、B型超声波和X线摄影检查,综合判断为犬急性胰腺炎,最终通过抑制胰腺分泌、补液、纠正低钾血症、改善贫血状况等多种方法进行治疗,治疗效果良好。该文对犬急性胰腺炎的形成原因、临床表现、诊断与治疗方法等方面作了较为详细的阐述,为临床上犬胰腺炎的诊断和治疗提供一定的参考。  相似文献

为探讨丹参注射液联合奥曲肽对犬急性胰腺炎的临床疗效,选取浙江大学附属动物医院2014年7月至2016年7月门诊诊断为犬急性胰腺炎的70例病例,将其分为治疗组和对照组,每组35例。对照组采用奥曲肽进行治疗;治疗组采用奥曲肽联合丹参注射液进行治疗,分析2组治疗后犬特异性胰脂肪酶水平、腹痛缓解时间、治疗时间以及临床综合效果。结果显示,治疗组和对照组的治疗有效率分别为80%和65.7%,两组之间具有显著性差异(P0.05),即治疗组能显著缓解腹痛时间和缩短治疗时间。因此,丹参联合奥曲肽治疗犬急性胰腺炎的临床效果显著优于单独使用奥曲肽,为犬急性胰腺炎的治疗提供一种新的临床方案。  相似文献

胰腺炎可分为急性胰腺炎和慢性胰腺炎,其中以急性胰腺炎对犬的危害较大,因为犬急性胰腺炎在发病时往往会有严重的呕吐和腹痛,有的还伴有腹泻,甚至粪便中带血,严重者可导致患犬昏迷或休克。由于此病发病急,诊断难度大,患犬往往不能得到及时治疗,从而出现急性死亡。犬急性胰腺炎一般侵害成年到中年犬,雌犬发病率比雄犬高,笔者在临床中接诊了一例犬急性胰腺炎病例,现报道如下。  相似文献

Pancreatitis was considered a rare disease in the cat until a couple of decades ago when several retrospective studies of severe acute pancreatitis were published. It was apparent that few of the diagnostic tests of value in the dog were helpful in cats. With increasing clinical suspicion, availability of abdominal ultrasonography, and introduction of pancreas-specific blood tests of increasing utility, it is now accepted that acute pancreatitis is probably almost as common in cats as it is in dogs, although the etiology(s) remain more obscure. Pancreatitis in cats often co-exists with inflammatory bowel disease, less commonly with cholangitis, and sometimes with both. Additionally, pancreatitis may trigger hepatic lipidosis, while other diseases, such as diabetes mellitus, may be complicated by pancreatitis. Therapy is similar to that used in dogs, with added emphasis on early nutritional support to prevent hepatic lipidosis. Less is known about chronic pancreatitis than the acute form, but chronic pancreatitis is more common in cats than it is in dogs and may respond positively to treatment with corticosteroids.  相似文献

In the past decade pancreatitis has become recognised as a significant disease in the cat. Chronic, mild pancreatitis is often associated with more commonly diagnosed diseases such as inflammatory bowel disease or cholangitis/cholangiohepatitis. Furthermore, acute pancreatitis with similar complications to those seen in dogs is now diagnosed more frequently in cats. Unfortunately, the clinical signs and clinicopathological findings in cats with pancreatitis are often non-specific and vague. The lack of specific signs often results in a diagnosis being made only when the veterinary surgeon has a strong index of suspicion for pancreatitis and vigorously pursues that diagnosis. Pancreatitis is an important disease in cats, has been implicated as a potential cause of diabetes mellitus, and when present complicates the treatment of diabetes and other intra-abdominal diseases in cats.  相似文献

While there is no single diagnostic approach, other than direct examination of the pancreas, that is completely reliable for the diagnosis of pancreatitis, a presumptive diagnosis of acute pancreatitis can be made with a good degree of confidence by careful evaluation of the entire clinical picture - history and clinical signs, results of physical examination, abdominal imaging and laboratory testing. The approach to treatment is to rest the pancreas, by withholding food, while providing general supportive care by fluid therapy including, in severe cases, plasma transfusion to replace plasma protease inhibitors and albumin. Additional specific interventions in an effort to halt the progression of spontaneous pancreatitis have not yet been shown to be effective, but control of obesity and avoidance of high fat diets may help prevent pancreatitis or lessen its severity when it occurs. Pancreatitis is an unpredictable disease of widely varying severity. While many patients with uncomplicated pancreatitis recover spontaneously following several days of supportive care, other patients die in spite of prolonged intensive care.  相似文献

In summary, pancreatitis is common in dogs and cats, but it seems that most cases remain undiagnosed. Serum amylase and lipase activities are useful as a quick screening test for pancreatitis in the dog only. Serum amylase or lipase activity must be at least three to five times the upper limit of the reference range to suggest a diagnosis of pancreatitis. Furthermore, the diagnosis must be confirmed by other diagnostic modalities, and normal test results do not eliminate the possibility of pancreatitis. Abdominal ultrasound is highly specific for pancreatitis in dogs and cats but is not particularly sensitive, especially in cats. Serum cPLI concentration is highly specific for exocrine pancreatic function and is also highly sensitive for pancreatitis. Similarly, initial data would suggest that serum fPLI is the most sensitive and specific diagnostic test for feline pancreatitis. Until further data are available, however, serum fPLI should be used in conjunction with other diagnostic tests to arrive at a diagnosis of feline pancreatitis. Histopathologic evidence of pancreatitis is conclusive for a diagnosis of pancreatitis. In most cases, however, lesions are localized, and the lack of histopathologic evidence of pancreatitis does not eliminate a diagnosis of pancreatitis.  相似文献

A clinical diagnosis of acute pancreatitis is often difficult to obtain. Histopathology remains the gold standard, whereas clinical signs, diagnostic imaging and laboratory testing, even in combination, may be insufficient. In a prospective study, lipase activity in ascitic fluid of various aetiologies was determined in 44 dogs in order to investigate its performance in cases of acute pancreatitis. Data of simultaneously determined blood lipase activities were available in 27 dogs. Lipase activity was measured by a colorimetric assay. A complete peritoneal fluid analysis was performed. Dogs were divided into four groups, according to their final diagnosis: acute pancreatitis (A), abdominal trauma (B), abdominal neoplasia (C) and others (hepatic or cardiac diseases) (D). Dogs with acute pancreatitis had a significantly higher peritoneal lipase activity than those in other groups (P < or = 0.024), while no significant difference was found between the other groups (P > or = 0.734). Blood lipase activity as well as protein content and total cell count of the ascitic fluid did not show any significant difference between groups. Data show that determination of lipase activity in dogs that develop ascites may be useful in complementing the diagnosis of acute pancreatitis.  相似文献

Sideroblastic anemia is an anemic condition characterized by chronic hypochromic anemia and the presence of large iron deposits in erythroid cells. Seven dogs with sideroblastic anemia were evaluated retrospectively. Historical, clinical, and clinicopathologic findings were reviewed to determine whether the condition was idiopathic or associated with disease conditions or drug or toxin exposure. Associated diseases were identified in 6 affected dogs and included acute hepatitis, pancreatitis, acute hepatitis and pancreatitis, inflammatory disease, glomerulonephritis, and myelofibrosis. None of the dogs had a history of recent exposure to drugs or toxins. One dog had no evidence of associated disease. Regardless of the associated disease condition, sideroblastic anemia was characterized by moderate to severe nonregenerative and frequently hypochromic anemia with prominent dysplastic features in bone marrow that were most prominent in the erythroid series. Survival varied from days to years. Identification of large numbers of siderocytes or sideroblasts in blood or bone marrow is inconsistent with a diagnosis of iron deficiency and should prompt a search for inflammatory disease conditions, including hepatitis, pancreatitis, and glomerulonephritis.  相似文献

OBJECTIVE: To determine ultrasonographic findings in cats with clinical, gross pathologic, and histologic evidence of acute pancreatic necrosis. DESIGN: Retrospective study. ANIMALS: 20 cats. PROCEDURE: Ultrasound reports and permanent ultrasonographic images were reviewed, and ultrasonographic findings were recorded. Thoracic and abdominal radiographs were also reviewed, when available. Anatomic localization of pancreatic necrosis was determined from the gross pathology report; duration and severity of pancreatic necrosis were determined by reviewing histologic specimens. The presence of concurrent disease was recorded from the final pathology report. RESULTS: The pancreas was considered ultrasonographically normal in 10 cats and was not observed in 3. Ultrasonographic findings were considered compatible with pancreatitis in the remaining 7 cats. Gross pathologic findings indicated that pancreatitis was multifocal in all 7 of these cats; histologically, pancreatitis was acute or subacute in 5 and associated with severe or moderate necrosis in 6. In the remaining 13 cats, gross pathologic findings indicated that pancreatitis was multifocal (n = 8) or focal (2), or gross pathologic findings were normal (3). Histologically, pancreatitis was peracute or acute in 11 of these 13 cats and associated with severe or moderate necrosis in 8. Thoracic and abdominal radiographic findings were nonspecific. CONCLUSIONS AND CLINICAL RELEVANCE: Results of ultrasonography were consistent with a diagnosis of pancreatitis in only 7 of 20 cats with acute pancreatic necrosis in the present study. This suggests that new diagnostic criteria must be established if abdominal ultrasonography is to be an effective tool in the diagnosis of pancreatitis in cats.  相似文献

A female, mixed-breed dog was presented with signs of abdominal discomfort and vomiting of 24 h duration following an episode of dietary indiscretion. Clinical signs, previous medical history, and diagnostic tests supported a diagnosis of acute pancreatitis. Acute pancreatitis is a challenging medical condition, with the potential for high mortality and comorbidity in companion animal patients. The anatomic position and relationships of the pancreas make direct observation of pancreatic pathology difficult, so the diagnosis of pancreatic disease is typically based on the measurement of biochemical markers in serum and imaging studies of the abdomen. The digestive enzymes typically used in the diagnosis of pancreatitis, amylase and lipase, have poor sensitivity and specificity. Recently, pancreas specific lipases have been purified from dogs and cats, leading to the availability of specific immunoassays for these proteins that show great promise in the diagnosis of acute pancreatitis. After the diagnosis of acute pancreatitis is established, there is a need to assess the severity of the disease to give an accurate prognosis for the patient. This article will review currently available methods used to diagnose acute pancreatitis and the ability of these methods to assess accurately the severity of the disease.  相似文献

BACKGROUND: Increased serum lipase activity has been used historically to support the diagnosis of acute pancreatitis, a common disease in dogs. Most of the lipase assays that are currently in use lack optimum sensitivity and specificity. OBJECTIVE: The objectives of this study were to 1) validate the 1,2-o-dilauryl-rac-glycero-3-glutaric acid-(6'-methylresorufin) ester (DGGR) assay for determination of lipase activity in canine serum and 2) compare results, reference intervals, sensitivity, and specificity of the DGGR assay with a standard 1,2-diglyceride (1,2 DiG) assay for diagnosing acute pancreatitis in dogs. METHODS: Precision, linearity, and interference studies were performed for method validation on a Hitachi 911 analyzer. Lipase results from the DGGR and 1,2 DiG assays were compared by linear regression analysis. Sensitivity, specificity, and diagnostic efficacy were determined for both assays on a population of 30 dogs, 15 of which had acute pancreatitis based on history, clinical signs, and ultrasound findings. RESULTS: Within-run and within-day coefficients of variation (CVs) were low (<3%), with higher day-to-day CVs (< or =14 %). The assay was linear between 8 and 2792 U/L. No significant interference by hemolysis and lipemia was found. Poor correlation was found between the assays (r(s)=0.84). The lipase reference interval was 8-120 U/L for the DGGR assay and 30-699 U/L for the 1,2 DiG assay. Sensitivity and specificity for the diagnosis of pancreatitis were 93% and 53%, respectively, for the DGGR assay and 60% and 73% for the 1,2 DiG assay. Receiver operating characteristic curve analysis showed similar areas under the curve. CONCLUSIONS: On the basis of this study, the DGGR method is considered adequate for assaying serum lipase activity in dogs. The high sensitivity of the DGGR assay suggests it may be a useful screening test for canine pancreatitis.  相似文献
