The objective of this paper is to describe clinical behavior, histopathologic features, and immunohistochemical staining of two‐related horses with intraocular teratoid medulloepithelioma. Two‐related Quarter Horses with similar intraocular masses presented to the UF‐CVM Comparative Ophthalmology Service for evaluation and treatment. The first horse, a 3‐year‐old gelding, had glaucoma and a cyst‐like mass in the anterior chamber. Enucleation was performed. Histopathology revealed a teratoid medulloepithelioma. The tumor was considered to be completely excised. Fifteen months later, the gelding presented with swelling of the enucleated orbit and local lymph nodes with deformation of the skull. Cytology revealed neuroectodermal neoplastic cells. Necropsy confirmed tumor metastasis. Six weeks later, a 9‐year‐old mare, a full sibling to the gelding, presented for examination. An infiltrative mass of the iris and ciliary body was found that extended into the anterior, posterior, and vitreal chambers. Uveitis was present, but secondary glaucoma was not noted. Enucleation was performed and the histopathologic diagnosis was also teratoid medulloepithelioma. The mare has had no recurrence to date, 2 years following enucleation. Metastasis of intraocular teratoid medulloepithelioma is possible. Staging is recommended in cases where the diagnosis of teratoid medulloepithelioma is confirmed. Surveillance of full siblings is recommended until more information regarding etiology is known.  相似文献

Enlargement of the left eye, corneal opacification, and blindness were clinical signs of a teratoid medulloepithelioma in a 5-year-old Standardbred mare. Diagnosis was made on histologic examination of the enucleated eye. Medulloepitheliomas are congenital intraocular tumors arising from primitive ciliary body epithelium. ANIMAL STUDIED: A tumor of the left eye with involvement of the brain and kidney was diagnosed in a 4-year-old Neapolitan mastiff. PROCEDURE: The dog presented with acute glaucoma of the left eye. Peripheral corneal vascularization and severe corneal edema obscured examination of deeper structures. Because of concurrent progressive neurologic signs the dog was euthanized and a postmortem examination was performed. The eyes, brain and samples from the lung, heart, liver and kidneys were fixed in 4% neutral buffered formalin and embedded in paraffin wax. Sections were examined by light microscopy including histochemical and immunohistochemical staining. RESULTS: The ocular tumor originated from the ciliary body and was composed of small islets and cords of poorly differentiated, oval to polyhedral cells surrounded by abundant ground substance. Areas with chondroid differentiation were observed. Rosettes were not found. Metastases with the same morphology were present in the brain and in one kidney. CONCLUSION: An intraocular malignant teratoid medulloepithelioma with metastases to the brain and kidney is presented.  相似文献

A 14-year-old gray gelding was presented for investigation of a visible, pale-colored ocular mass in the right eye. An intraocular mass was identified clinically and ultrasonographically as originating from the superior nasal quadrant of the ciliary body and retina. The mass occupied the majority of the vitreous chamber and some of the superior anterior chamber of the eye. The affected eye was blind. Following exenteration, a primary intraocular primitive neuroectodermal tumor (i.e. a retinoblastoma/medulloepithelioma), a rarely described intraocular mass in adult horses, was identified by pathologic examination. The gelding returned to normal use following a short recovery period.  相似文献

A 6-year-old female llama presented with buphthalmos of its right eye owing to the presence of an intraocular mass. The affected globe was enucleated and submitted for microscopic examination. The intraocular mass was diagnosed as malignant medulloepithelioma. Within the following months, the llama developed soft tissue masses, which completely filled the right orbital cavity and expanded the cranial portion of the right mandibular bone, and enlarged mandibular lymph nodes. Euthanasia was elected 30 months after the initial diagnosis. The carcass was submitted for postmortem examination, which revealed the presence of medulloepithelioma metastases within the right orbit, mandible, mandibular lymph nodes, lungs, liver, and mesenteric and sublumbar lymph nodes. The primary intraocular tumor and its metastases were composed of neoplastic undifferentiated neuroepithelial cells, which formed tubules, Flexner-Wintersteiner and Homer Wright rosettes, and rare solid sheets. Electron microscopy showed that tumor cells were connected by desmosome-like junctions and contained rare intracytoplasmic basal bodies. Neoplastic cells were positive for vimentin, nestin, microtubule-associated protein 1B, S-100 protein, and glial fibrillary acidic protein (GFAP). To the best of the authors' knowledge, this is the first report of a malignant nonteratoid ocular medulloepithelioma with distant metastases in a llama and of the ultrastructural and extended immunohistochemical characterization of a nonteratoid medulloepithelioma in this species.  相似文献

A 3-year-old cockatiel was presented for evaluation of a buphthalmic right eye (OD). The history included a traumatic event 5 months prior to presentation, and the referring veterinarian diagnosed cataract and secondary glaucoma. Computed tomography of the bird's orbital region revealed a large right orbital soft tissue mass that extended lateral to the orbital rim with obliteration of the globe without bone involvement. There was also no evidence of metastatic disease within the lungs or any abnormalities within the celomic cavity, therefore enucleation was recommended to achieve a histopathologic diagnosis and prognosis, and to palliate the clinical signs. Final diagnosis was malignant intraocular teratoid medulloepithelioma.  相似文献

Objective To describe the long‐term histologic and intraocular pressure (IOP) lowering effects of diode laser transscleral cyclophotocoagulation (TSCP) on the normal equine eye. Animals Eight normal adult horses. Procedures TSCP was performed in one randomly assigned eye. Sixty spots were treated at settings of 1500 ms and 1500 mW. Two horses were randomly selected for euthanasia at 2, 4, 12, or 24 weeks post‐TSCP. Both eyes were enucleated and histologically evaluated. Intraocular pressure was measured by applanation tonometry prior to TSCP, immediately post‐TSCP, twice daily for 7 days post‐TSCP and then monthly until study conclusion. A longitudinal model estimated the average IOP values for the treated and untreated eyes at 1 week, 1, 3, and 6 months post‐TSCP. Results All treated eyes at all time periods exhibited four characteristic histologic lesions: scleral collagen hyalinization, ciliary body pigment dispersion and clumping, focal disruption of the ciliary body epithelium, and focal ciliary process atrophy. After TSCP, there were no significant changes in IOP from baseline for the control eyes, while the IOP in treated eyes was significantly decreased from baseline (P < 0.05) at all time periods. The estimated decrease in IOP in the treated eyes compared to baseline IOP at 6 months was ‐3.76 mmHg for an average decrease in IOP of 20% from baseline. Conclusion  Diode laser TSCP produces histologic lesions in the equine ciliary body that result in a significant and sustained decrease in IOP. TSCP may be an effective management for equine glaucoma.  相似文献

An indoor‐only, 5‐year‐old, spayed female domestic shorthair cat presented for an ophthalmic examination of the left eye. An intraocular tumor with secondary glaucoma and blindness was diagnosed; the globe was enucleated and sent for histopathological examination. Gross examination revealed a solid white mass filling the entire vitreous space and replacing the iris and ciliary body. The lens and retina appeared to be similarly replaced by the neoplasm. Histological examination revealed a complete loss of the internal ocular structures, with a ruptured capsule as the only remnant of the lens within an extensive malignant mesenchymal neoplastic cell proliferation. The cells were polygonal, with well‐defined cytoplasmic borders and abundant weakly basophilic cytoplasm, embedded within the islands of chondroid matrix. No neoplastic invasion of the sclera was apparent. The animal died 6 months after the enucleation due to respiratory distress. Gross examination revealed numerous firm, white to tan nodular masses with smooth to mildly irregular surfaces dispersed throughout the parietal pleura, thoracic surface of the diaphragm, tracheobronchial and mediastinal lymph nodes, pericardium, and lungs. On cross‐section, the neoplastic nodules were solid and variably translucent, resembling hyaline cartilage. Histologically, these nodules were similar to the neoplasm identified earlier in the left globe. Metastasis of post‐traumatic ocular chondrosarcoma has not yet been described in cats. This is therefore believed to be the first report of metastases of this type of neoplasm in cats. This case adds to the limited set of data on the outcome of this type of tumor.  相似文献

A lesion was identified in the eye of a juvenile llama, and preliminary clinical findings included anterior uveitis and an exudative retinal detachment suggestive of infectious disease. However, histopathologic evaluation of the enucleated globe revealed an intraocular neoplasm composed of primitive neuroepithelium forming ribbons, cords, and rosettes, heteroplastic elements including spindle cells in a loose myxomatous matrix, and islands of well-differentiated hyaline cartilage. Immunohistochemically, neoplastic cells were positive for vimentin and neuron-specific enolase. Spindle cells were multifocally positive for desmin and muscle specific actin, indicating differentiation towards myofibers. These findings are consistent with a diagnosis of malignant teratoid medulloepithelioma, an extremely rare ocular neoplasm that affects children and young animals.  相似文献

Case 1 A two‐year old, 462 kg Standard bred horse was anesthetized for arthroscopy and castration. During anesthesia, hyperemia of the mucosal membranes and urticaria were noticed. During 5 hours of anesthesia subcutaneous edema of the eyelids and neck region developed. In the recovery box, the orotracheal (OT) tube was left in situ and secured in place with tape. Following initial attempts to stand, the horse became highly agitated and signs consistent with pulmonary edema developed subsequently. Arterial hypoxemia (PaO2: 3.7 kPa [28 mmHg]) and hypocapnia (PaCO2: 3.1 kPa [23 mmHg]) were confirmed. Oxygen and furosemide were administered. The horse was assisted to standing with a sling. Therapy continued with bilateral intra‐nasal oxygen insufflation. Ancillary medical therapy included flunixin meglumine, penicillin, gentamycin and dimethylsulfoxide. Following 7 hours of treatment the arterial oxygen tensions began to increase towards normal values. Case 2 An 11‐year old, 528 kg Paint horse was anesthetized for surgery of a submandibular mass. The 4‐hour anesthetic period was unremarkable. The OT tube was left in situ for the recovery. During recovery, the horse was slightly agitated and stood after three attempts. Clinical signs consistent with pulmonary edema and arterial hypoxemia (PaO2: 5 kPa [37.5 mmHg]) subsequently developed following extubation. Respiratory signs resolved with medical therapy, including unilateral nasal oxygen insufflation, furosemide, flunixin meglumine and dimethylsulfoxide. The diagnosis of pulmonary edema in these horses was made by clinical signs and arterial blood‐gas analysis. While pulmonary radiographs were not taken to confirm the diagnosis, the clinical signs following anesthesia support the diagnosis in both cases. The etiology of pulmonary edema was most likely multifactorial.  相似文献

Distribution of the immunoglobulin (Ig) classes G, A and M within the anterior uvea of eight clinically normal equine eyes was examined using indirect immunoperoxidase labelling. Increased staining intensity of stromal IgG and IgA was observed within the ciliary processes, the iris stroma being relatively free of immunoglobulin. This may reflect anatomical variation in the permeability of the uveal microvasculature to lipid insoluble plasma macro-molecules. Intracellular IgG and IgA were observed within the non-pigmented ciliary epithelium in seven and four of the eight eyes respectively, although wide variation between numbers of Ig bearing cells in each eye was noted. It was suggested that these cells may have a role in the removal of Ig from the posterior chamber aqueous. Plasma cells, of IgG isotype, were observed in only one eye, suggesting that intraocular production of Ig is not a feature of the normal equine eye.  相似文献

Glaucoma is a serious complication of anterior uveitis that can result in significant ocular discomfort and loss of vision. As glaucoma is insidious and mostly asymptomatic until late in the course of disease, it is often undiagnosed. Equine recurrent uveitis is the most common underlying disorder: it is a disease of high prevalence, characterised by recurrent‐remitting episodes of ocular inflammation. Here, the inflammatory cells and mediators they release alter the normal anatomic structure of the anterior chamber and aqueous drainage pathways of the eye. These changes alter the homeostatic mechanisms of intraocular pressure control. Management of glaucoma secondary to uveitis can be challenging due to the difficulties of managing the underlying disorder and its effects on the eye. This article reviews the pathogenesis of uveitic glaucoma in the horse and medical and surgical management options.  相似文献

This case report describes the findings in a 10-year-old goat with metastasis of an adenocarcinoma in the iris. Two weeks before admission, the owner had noticed blepharospasm of the left eye. Clinical examination by the referring veterinarian revealed unilateral intraocular hemorrhage. The goat was referred to our clinic for further work-up. The rectal temperature was 40 degrees C. The most important haematological result was severe thrombocytopenia. There was mild corneal oedema of the left eye. Approximately 75 per cent of the anterior chamber was filled with non-coagulated blood. The fluid in the anterior chamber dorsal to the blood was cloudy, and the corpora nigra could not be seen clearly. All other internal parts of the eye could not be seen. Ultrasonography of the left eye confirmed cloudiness of the anterior chamber and revealed moderate thickening of the iris. The right prescapular lymph node was markedly enlarged. Cytological examination of a fine needle aspirate of the lymph node showed a mixed population of neoplastic cells. Based on immunohistochemical evaluation of the cells metastasis of a carcinoma was diagnosed. The goat was subjected to euthanasia, and a postmortem examination was carried out. The anterior chamber of the left eye contained blood, and the iris was thicker than normal and adhered to the posterior surface of the cornea. There were neoplastic alterations in the iris, the oesophagus, the lung lobes, the liver, the kidney and in the prescapular, retropharyngeal, mediastinal and hepatic lymph nodes. Histologically, a diagnosis of carcinoma was confirmed, but the origin of the tumour could not be determined.  相似文献

Using a hand-held optical fiber, a surgical neodymium:yttrium, aluminum, and garnet laser was used transsclerally on the left ciliary body of 25 dogs. Dogs were assigned to groups and were given low (100 J) and high (238 J) energy levels. In 12 dogs given 100-J energy, the intraocular pressure decreased a mean value of 6 mm of Hg below that in the untreated right eye. However, intraocular pressure returned to the pretreatment value 1 week after treatment. Intraocular lesions were not found by histologic examination 28 days later. In the second group of 13 dogs treated with 238-J energy, 6 were euthanatized 7 days after treatment and 7 were euthanatized 28 days after treatment. A mean decrease in intraocular pressure of 10 mm of Hg in the treated left eye, relative to the untreated right eye, persisted throughout the evaluation period. In eyes examined histologically 7 days after treatment, ciliary hemorrhage and necrosis were prominent. Other histologic changes were minimal and consisted principally of iris stromal hemorrhage. Of 7 dogs examined for 28 days after treatment, 6 remained sighted; in 1 dog, extensive intraocular hemorrhage occurred, eventually resulting in phthisis bulbi. Ciliary atrophy and fibrosis were the important histologic lesions observed 28 days after treatment. Laser energy delivered transsclerally by this optical system effectively induced ciliary necrosis and may be an effective treatment for canine glaucoma.  相似文献

Objective Vascular tumors of the conjunctiva in the horse are rare. We present a unique case of an intravascular papillary endothelial hyperplasia of the conjunctiva. Animal studied Horse. Procedures Case report. A 6‐year‐old‐mare presented with a red mass in the conjunctiva of the left eye. After complete ophthalmologic examination the lesion was excised. The tissue was processed for light microscopy and studied histopathologically. Results Pathologic examination revealed a nonencapsulated vascular lesion composed of confluent vascular spaces filled by multiple papillary structures composed of a central collagenous core lined by hyperplastic endothelial cells. There was neither atypical endothelial cell nor mitotic activity. Conclusions Intravascular papillary endothelial hyperplasia is a benign proliferative lesion that should be differentiated from malignant vascular tumors.  相似文献

A 3½‐year‐old, female (spayed) working black Labrador Retriever was referred to the Unit of Comparative Ophthalmology at the Animal Health Trust after a penetrating lead gunshot injury to the right eye. Clinical examination revealed penetrating injuries to the right cornea, left lateral canthus, and left forelimb. Radiography revealed the presence of multiple metallic foreign bodies in the head and one in the left forelimb. B‐mode ocular ultrasonography confirmed the presence of an intraocular metallic foreign body within the posterior tunics of the right eye. Medical treatment of the right eye over a period of 1 month resulted in a comfortable, visual eye. Long‐term follow‐up over 4½ years revealed a slow deterioration in vision because of progressive lens opacification and the presence of a localized area of chorioretinopathy.  相似文献

Ciliary body adenocarcinomas are well-recognized tumors in dogs; however, invasion of the lens by the tumor has not been described in veterinary species. Evaluation of the right eye of a 10-year-old Labrador Retriever revealed a ciliary body mass spanning from the 10 o'clock to 2 o'clock position that was impinging on the lens. Histologic evaluation of the enucleated eye revealed a ciliary body adenocarcinoma with lens invasion. Where the tumor encroached on the lens, the lens capsule was absent and there was scrolling of the broken ends of the capsule. Tumor cells were juxtaposed to the lens fibers and dissecting between the lens fibers. There were no inflammatory cells within the lens associated with the breaks in the capsule or within the anterior chamber. To our knowledge this is the first report of lens invasion by a tumor in any veterinary species.  相似文献
