Radiographs of 74 dogs (84 stifles) presented with a cranial cruciate ligament rupture and surgically treated using a lateral extracapsular wire (LEW) were reviewed. A strand of orthopaedic wire was surgically placed caudally around the lateral fabella and through a predrilled hole in the tibial crest. At six week follow-up, the LEW was broken at least once in 26 of 33 stifles, predominantly in the area of the lateral fabella. In five stifles, the LEW had slipped off the fabella and was displaced distally along the gastrocnemius muscle. Six months after surgery, the LEW was often broken at several sites. LEW migration was rare. Osteolysis and sclerosis at the site of LEW penetration through the tibial crest occurred in about two-thirds of the stifles examined.  相似文献

Locations of 3 instantaneous centers of joint rotation (ICR) were found in the stifles of 6 healthy dogs. A novel algorithm, verified by computer simulation, was used to determine ICR from radiographs. Comparisons of ICR locations were made between flexion and extension, between left and right limbs, and between living and dead dogs, using analysis of variance. All stifles were examined for evidence of lesions, but all were normal. The normal location of the ICR in the stifle is near the joint surface, indicating that there is a predominantly rolling type of motion. As the limb flexes, the ICR moves caudally; as the limb extends, the ICR moves cranially. Significant differences in ICR position were not found between left and right limbs or between living and dead dogs. The average ICR position was significantly more caudal during extension than it was during flexion. Knowing the normal ICR position is a preliminary to diagnosing joint abnormalities, using the ICR.  相似文献

The levels of tartrate resistant acid phosphatase (TRAP), matrix metalloproteinase-2 (MMP-2), and tissue inhibitor of matrix metalloproteinase-2 (TIMP-2) in synovial fluid (SF) and serum in cases of canine osteoarthritis (OA) were measured. OA was induced by a surgically-created medial patellar luxation in the left stifle of 24 dogs. SF and blood samples were collected at 1.5- and 3-month intervals, respectively. Every 3 months, one dog was euthanatized to collect tissue samples from both stifles. TRAP levels in SF and serum were measured using a spectrophotometer, and TRAP-positive cells in joint tissues were identified by enzyme histochemistry. MMP-2 and TIMP-2 in SF and serum were detected by Western blotting and ELISA, respectively. TRAP in SF from the stifles and serum was significantly increased (p < 0.05) after 3 months. TIMP-2 in SF and serum was significantly decreased (p < 0.05), whereas MMP-2 in SF was significantly increased (p < 0.05) during the progression of OA. Histochemistry revealed an increased number of TRAP-positive cells in tissues from OA-affected joints. Assays measuring TRAP, MMP-2, and TIMP-2 in SF and serum, and methods that detect increased numbers of TRAP-positive cells in the joint tissues can play an important role in identifying the early phases of degenerative changes in canine joint components.  相似文献

This study evaluated the effect of notchplasty (enlargement of the intercondylar fossa) in stable and unstable canine stifles. Bilateral notchplasty and unilateral cranial cruciate ligament (CrCL) transection were performed in 6 dogs. Exercise, consisting of walking 1.5 miles three times a week, began 1 month after surgery and continued until euthanasia 6 months after surgery. Evaluation methods included orthopedic examination, serial radiographs, thin section radiography, histopathology, and gross pathology. Notchplasty in the stable stifle did not cause lameness beyond 3 weeks, joint instability, or degenerative joint disease. In the stable stifle, smooth resurfacing of the notchplasty site with fibrous and osseous tissue occurred. Stifles with notchplasty and CrCL transection exhibited persistent lameness, instability, and degenerative joint disease. In CrCL deficient stifles osteophytes formed within the notchplasty site, resulting in a rough surface. Our observations indicated significant refilling in notchplasties of both stable and unstable stifles ( P <.05). However, the intercondylar fossa (ICF) width 6 months after notchplasty was significantly smaller in unstable stifles compared with stable stifles ( P <.05) indicating that greater refilling of the notchplasties occurred in the unstable stifles. In clinical cases, notchplasty should be larger than the desired final result to accomodate the partial refilling that occurs even in stable stifles.  相似文献

The effects of surgical and nonsurgical therapy on the development of osteoarthritis were compared in 12 dogs with bilateral medial patellar luxation and unilateral surgical repair. Evaluations included severity of lameness and patellar luxation, ligamentous stability, range of motion, and radiographic evidence of osteoarthritis before surgery and at a mean of 33 months after surgery. Stifles without surgical treatment served as controls for the contralateral stifles with surgery. All stifles treated surgically had reduced patellofemoral joints, normal range of motion, and improved limb use. Osteoarthritis progressed significantly and comparably in both groups of stifles. Progression of osteoarthritis was not correlated with luxation grade, body weight, or interval from surgery to follow-up. Age at surgery was correlated positively with severity of osteoarthritis in the stifles treated surgically.  相似文献

AIM: To characterise the prevalence and distribution of radiographic changes in the hocks and stifles of Thoroughbred yearling colts and fillies in New Zealand and compare them with other populations of young horses. METHODS: Repository radiographs taken in New Zealand for the 2003-2006 Thoroughbred national yearling sales were evaluated by two individual readers. The distribution of radiographic changes was classified as left side only, right side only, or bilateral. Lesions were categorised by type, location, and sex of the yearling. Complete sets of hock and stifle radiographs of 1,505 yearlings were evaluated. RESULTS: Osteophytes or enthesophytes were seen radiographically in the distal tarsal joints of 460/1,505 (31%) horses. Osteochondrosis was seen in the tibiotarsal joint of 66/1,505 (4%) horses, and in the femoropatellar joint of 40/1,505 (3%) horses. Radiographic lucency in the distal or axial aspect of the medial femoral condyle was seen in 247/1,505 (16%) horses, and lucencies consistent with subchondral cyst-like lesions were seen in 26/1,505 (2%) horses. No significant difference was seen in the proportion of colts and fillies with radiographic changes in the hock or stifle. The prevalence of osteochondrosis and subchondral cyst-like lesions in the stifles of the yearlings examined were similar to those reported in Thoroughbred yearling sale horses in the United States of America (USA). The prevalence of changes in the distal tarsal joints was similar to those reported in Standardbred and Thoroughbred yearlings from Scandinavia and the USA. CONCLUSIONS: There was no significant difference in the prevalence of osteochondrosis in the hock and stifle, lucencies in the distal medial femoral condyle, or radiographic changes in the distal tarsal joints between colts and fillies. CLINICAL RELEVANCE: Establishment of the normal prevalence and distribution of radiographic changes in the hocks and stifles of Thoroughbred yearlings in New Zealand will allow comparison with populations of young horses in other countries. Knowledge of the normal prevalence will assist veterinarians to identify abnormally high or low prevalences on individual farms, to further investigate the pathogenesis of the lesions.  相似文献

Twenty stifles (10 dogs) were studied for a period of 1 year after various lesions of the cranial cruciate ligament and medial meniscus were produced surgicaoy. Through serial arthroscopic evaluations, degenerative processes In stifles with a "torn" cranial cruciate ligament were documented. Intra-articular changes were minimal after partial meniscectomy and were severe after total meniscectomy. Multiple arthroscopies caused no demonstrable changes.  相似文献

Unilateral medial patellar luxation was diagnosed in 10, and bilateral medial patellar luxation in six, large and giant-breed dogs (22 stifles). Lameness occurred in five dogs after trauma or surgery, and 11 dogs had no known predisposing history. The mean age at presentation was 25 months, and the mean time from initial onset of clinical signs to diagnosis was 13 weeks. All traumatic or iatrogenic luxations (five dogs) were unilateral. Luxations presumed to be congenital were unilateral in five dogs and bilateral in six. The grades of medial patellar luxation were I (1 stifle), II (11 stifles), III (9 stifles), and IV (1 stifle). Preoperative function was good (1 dog), fair (9 dogs), and poor (6 dogs). Surgical correction was performed in dogs with grades II, III, and IV luxations (21 stifles). Complications included one wound dehiscence and trochlear wedge migration, one pin loosening, and one persistent lameness caused by lymphoplasmacytic synovitis. Long-term follow-up was available in 13 dogs (18 stifles). Function was judged by owners to be excellent in seven dogs, good in five dogs, and poor in one dog. Surgical treatment of grades II and III luxations yielded good (8 stifles) and excellent (9 stifles) results, while one grade IV luxation had a poor long-term outcome.  相似文献

Reasons for performing study: There have been many studies that document radiographic findings in young Thoroughbred and Standardbred horses. No such studies have been performed in Quarter Horses. Objective: To describe the prevalence of radiographic changes in the stifles, tarsi, carpi and fetlocks of young Quarter Horses intended for cutting. Methods: Radiographs of yearling and 2‐year‐old Quarter Horses were obtained from a radiograph repository and a private farm. The carpi, tarsi, fetlocks and stifles were evaluated and radiographic changes categorised by type and location. The frequency of changes was calculated and comparisons were made between the 2 age groups. Results: Of 458 included horses, 408 (89.1%) had radiographic changes, most of which were in the tarsi (304, 69.4%) followed by the stifles (202, 44.5%), hind fetlocks (155 of 355, 43.7%), fore fetlocks (131 of 361, 36.3%) and carpi (27 of 342, 7.9%). Of the horses with stifle changes, 188 (93.1%) were in the medial femoral condyle (MFC). There was a significant difference between the age groups for changes on the distal intermediate ridge of the tibia (DIRT), hindlimb middle phalanx (P2) osteophytes and proximal tibial osteophytes. Conclusions: There is a high prevalence of radiographic changes in presale survey radiographs, especially in the stifles and tarsi, of young Quarter Horses intended for cutting. Potential relevance: Veterinarians examining presale radiographs at cutting horse sales should expect a high prevalence of radiographic changes in this population of horses. Work to determine the clinical relevance of these radiographic changes is currently ongoing.  相似文献

Seven dogs with torn cruciate ligament were used to evaluate the "over-the-top" repair procedure, using instantaneous center of rotation (ICR). Normal and cruciate-torn stifles of dogs were radiographed before surgery, using an image intensifier videotape system. The injured stifle was reradiographed 6 weeks after surgery. Images of the stifles were photographed from video monitor. Photographs were analyzed by a digitizer hooked to a computer to find the ICR at various angles. Data were analyzed, using multivariate analysis of variance. Relative to those of normal stifles, ICR of the damaged stifles were located significantly (P less than 0.05) proximal and caudal. After surgery, the ICR were still proximal and caudal, but not significantly so. We conclude that the cruciate-torn stifle deviates significantly from normal and that such deviation is restored to normal limits by use of the over-the-top procedure.  相似文献

OBJECTIVE: To assess functional outcome in dogs with cranial cruciate ligament (CrCL) disease after tibial tuberosity advancement (TTA) using force plate gait analysis, and to evaluate parameters potentially influencing outcome. STUDY DESIGN: Prospective clinical study. ANIMALS: Consecutive clinical patients (n = 37) with CrCL-deficient stifles (n = 40). METHODS: The stifle joints were examined arthroscopically prior to TTA. Meniscal release was not performed if the medial meniscus was intact. Open medial arthrotomy and partial meniscectomy were performed in the presence of meniscal tears. Vertical ground reaction forces were measured preoperatively and at follow-up examinations four to 16 months postoperatively (mean: 5.9 months). The ground reaction forces of a group of 65 healthy dogs were used for the comparison. The potential effects of clinical parameters on functional outcome were evaluated statistically. RESULTS: Complete CrCL rupture was identified in 28 joints, and partial CrCL rupture in 12 joints. The medial meniscus was damaged in 21 stifles. Vertical ground reaction forces were significantly higher at follow-up (P < 0.01), but remained significantly lower than those of control dogs (P < 0.01). Complications were identified in 25% of joints, and the dogs with complications had significantly lower peak vertical forces at follow-up than the dogs without complications (P = 0.04). Other clinical parameters did not influence outcome. CONCLUSIONS: Tibial tuberosity advancement significantly improved limb function in dogs with CrCL disease, but did not result in complete return to function. Complications adversely affected functional outcome. CLINICAL SIGNIFICANCE: A return to a function of approximately 90% of normal can be expected in dogs with CrCL disease undergoing TTA.  相似文献

Objective To investigate the clinical outcomes, complications and owners' evaluation of the tibial tuberosity advancement (TTA) procedure in canine stifles. Methods A retrospective study of hospital records was performed to identify dogs diagnosed with partial or complete cranial cruciate ligament (CCL) rupture that had undergone TTA repair. Information obtained included signalment, period of lameness, surgical report, evidence of meniscal injury, postoperative recovery and peri-operative complications. Owners were asked to assess the long-term outcome. Results In a total of 72 dogs (median age, 6 years; median body weight, 34.8 kg), TTA was performed in 92 stifles. Twenty breeds were represented, with Labrador Retrievers and Rottweilers the most common. The period of lameness ranged from 3 days to 24 months. The median pre-operative lameness score was 3/4 and meniscal injury was present in 51 stifles. Minor complications occurred in 29% of cases. Major complications occurred in 6.5% of cases and consisted of meniscal injury and two tibial tuberosity fractures. All were successfully managed, with good limb function when subsequently assessed. In the owner evaluation, 96% reported moderate to great improvement postoperatively, with no lameness at rest and mild to no lameness after vigorous exercise. Conclusion Clinical outcome and owner evaluations in this case series indicate favourable results can be expected when CCL-deficient stifles are treated with TTA.  相似文献

Autogenous periosteal grafts were harvested and placed in the trochlear grooves of the stifles of 32 rabbits. Postoperative immobilization periods of 0, 7, and 14 days were compared. Grafted areas were evaluated for gross appearance, histologic morphology, attachment of cartilage to subchondral bone, cartilage depth, chondrocyte density, proteoglycan presence, collagen fiber orientation, and mitotic activity. Nonimmobilized stifles had increased gross coverage of subchondral bone, better histologic quality, increased safranin O stain uptake, and more normal collagen fiber orientation at 60 days than stifles immobilized for 7 ir 14 days. Postoperative immobilization did not affect attachment of the graft to the subchondral bone.  相似文献

The structural integrity of subchondral bone in fresh and frozen osteochondral autografts was investigated at month 3 in 10 horses. Two osteochondral autografts were harvested from the lateral aspect of the lateral trochlear ridge of the left talus in each of 10 anesthetized horses. Grafts were frozen in 7.5% DMSO. After 14 days, the thawed grafts were press-fitted into drill holes in the trochlear ridges of the right stifles. A fresh graft from the right hock was implanted in each left stifle. To control for the effects of surgery, a fresh graft was transferred from the right stifle to the left stifle. The end result was two grafts in each femoropatellar joint. Fresh and frozen bone grafts maintained a structurally intact support for the cartilage surface. Graft stability and surface congruency were determining factors in the outcome of the grafts. Incorporation of both types of graft was complete at month 3, but remodeling of the fresh grafts was more active.  相似文献

Synovial fluid (SF) volume was calculated using various methods in the stifles of goats, in which the cranial cruciate ligament had been transected on one side. Measurements were performed prior to surgery and again 4,8, and 18 weeks following surgery, by measuring the dilution of an injected radioactive tracer diluted by the SF. Later, 7 months following surgery, SF volume measurements using simple arthrocentesis were performed on stifles in 9 of the goats, and the SF that could not be aspirated, was calculated using 2 indirect methods simultaneously on identical fluids in 3 of these goats. SF was also collected directly during staged arthrotomy of the stifles in 4 goats.There were conflicting results between methods, but the resulting calculated SF volumes seemed to be larger in the operated stifles compared to the controls for all the methods at about the same degree. The 2 indirect methods used to calculate the fluid remaining in the joints following arthrocentesis gave disparate volume calculations. The experiments revealed sources of error in all methods. Direct methods failed to acquire the total fluid volume, and indirect methods were subject to improper mixing and escape of the injected fluid or synovial fluid or both. It was concluded that none of the methods could be used to measure the “true” volume of SF, if such a concept exists and can be defined. None of the methods were considered reliable to compare volumes in different type of joints containing this type of fluid. It was, however, concluded that all the methods gave indication of increased SF volume present on a relative basis when paired joints were compared.  相似文献

REASONS FOR PERFORMING STUDY: Radiography in presale examinations of TB yearlings has become standard practice in recent years. OBJECTIVES: To describe the prevalence and distribution of radiographic changes in the fetlocks, carpi, tarsi, stifles and fore feet of Thoroughbred yearlings in central Kentucky when these joints were examined as part of routine pre- and post sale evaluations. METHODS: Horses subjected to radiographs included the fore (n = 1127) and hind (n = 1102) fetlocks, carpi (n = 1130), tarsi (n = 1101), stifles (n = 660) and fore feet (n = 300). Radiographic changes were categorised by location and type of change present (e.g. lucency, fragment) for each series. RESULTS: In the fore fetlocks 1.6% had fragmentation of the proximal dorsal first phalanx and 0.5% fragmentation of the proximal palmar aspect. In the hind fetlocks 5.9% had fragmentation at the plantar aspect and 3.3% fragmentation dorsally. Lucencies, fragments or loose bodies were detected at the dorsal aspect of the distal third metacarpus in 2.8% and, at the same location on the third metatarsus, 3.2%. Most yearlings (98%) had vascular channels in the proximal sesamoid bones and irregular vascular channels (> 2 mm wide or with nonparallel sides) were more common (79%) than regular vascular channels (56%). The intermediate ridge of the distal tibia was the most common location for fragmentation in the tarsus (4.4%). CONCLUSIONS: While some radiographic changes, e.g. vascular channels in proximal sesamoid bones, are very common in Thoroughbred yearlings, others, e.g. fragmentation or subchondral lucency within joints are quite rare usually affecting less than 5% of the population. POTENTIAL RELEVANCE: Veterinarians should expect to find radiographic changes in Thoroughbreds presented for examination prior to the yearling sales. The rarity of some changes thought to affect soundness or racing performance will make further invesigation of these conditions in horses without clinical signs more difficult.  相似文献

Surgical management of feline osteoarthritis is currently limited to joint immobilization or amputation. A method of implanting a readily available Swanson Design Silastic finger joint implant in feline stifles was developed and tested in four cats.
The surgical procedure was uncomplicated. The cats ambulated well and resumed normal activities. The joints were stable and retained normal ranges of motion. No undesirable gross pathology resulted from 3 months of implant residence.
We believe that the results of this preliminary study justify investigation of prolonged implant times and clinical trials in arthritic cat stifles.  相似文献

OBJECTIVES: To evaluate the high incidence of tibial tuberosity avulsion fracture diagnosed in skeletally immature Staffordshire bull terriers presenting to a UK animal welfare charity hospital. METHODS: A retrospective review of tibial tuberosity avulsion fractures treated by the hospital between 2002 and 2007. RESULTS: Sixty-five tibial tuberosity avulsion fractures were recorded in 59 dogs. Fifty-one tibial tuberosity avulsion fracture-affected dogs (86 per cent) were Staffordshire bull terriers. Dogs sustaining tibial tuberosity avulsion fractures had median and mean ages of five and 4.9 months, respectively (range three to 10 months). Where recorded, injury was associated with a short fall or jump (typically 3 to 4 feet) in 29 of 50 dogs. Three fracture patterns were recorded: 37 stifles sustained isolated tibial tuberosity avulsion fracture; 15 stifles sustained tibial tuberosity avulsion fracture accompanied by separation of the proximal tibial epiphysis; in 13 stifles epiphyseal separation extended to produce Salter-Harris type II fracture of the caudal tibial metaphysis. On analysis of the hospital database, tibial tuberosity avulsion fracture was a reason for presentation in 51 (3.3 per cent) of 1536 Staffordshire bull terriers, but only five (0.18 per cent) of 2815 other breed dogs, registered under the age of 12 months during the study period (P<0.001). CLINICAL SIGNIFICANCE: Staffordshire bull terriers commonly present with tibial tuberosity avulsion fracture, with or without concurrent separation of the proximal tibial epiphysis, to this urban charity hospital.  相似文献

SUMMARY The duration of electrocardiograph wave forms and intervals were determined in 179 Spanish-bred (Andalusian) horses aged from 1 month to 17 years. The values were compared with those of other breeds, and the relationship between electrocardiographic data and age was examined. High correlation coefficients were found between PR, ST and QT intervals and the age of the horses, and an inverted relation between heart rate and age was found. A multiple range analysis was made and the results suggest that significant changes in duration values and heart rate ocurred at the age of 6 months and in the second year of life.  相似文献

Objective— To evaluate after 12 weeks the effects of caudal medial meniscal release (MR) in the cranial cruciate ligament-intact canine stifle.
Study Design— Blinded, prospective in vivo study.
Animals— Purpose-bred hound dogs (n=10).
Methods— Either MR (n=5) or a sham (SH) surgery (n=5) was performed via arthroscopy. Orthopedic examination and subjective lameness evaluation were performed in each dog preoperatively and at 4, 8, and 12 weeks after surgery. Twelve weeks postoperatively, ultrasonographic, radiographic, and arthroscopic examinations were performed on the operated stifles. Gross pathology of the articular cartilage, cruciate ligaments, and menisci was assessed. India ink staining of the femoral and tibial articular surfaces was performed to determine the percent area of articular cartilage damage.
Results— At 8 and 12 weeks after surgery, MR dogs were lamer than SH dogs. At 12 weeks, the degree of radiographic OA was significantly higher in MR stifles than in SH stifles. Gross and sonographic meniscal pathology was more severe in MR stifles compared with SH stifles. MR stifles had significantly more severe articular cartilage pathology compared with SH stifles 12 weeks after surgery; pathology was most severe in the medial compartment.
Conclusions— MR alone is associated with articular cartilage loss, further meniscal pathology, degenerative joint disease, and lameness.
Clinical Relevance— Subsequent osteoarthritis and dysfunction of the stifle joint should be considered when making clinical decisions regarding MR in dogs.  相似文献
