OBJECTIVES: Peripheral parenteral nutrition is an option for short-term nutritional support in dogs which cannot be supported with enteral nutrition. The objective of this study was to examine the use of a three-in-one, 840 mOsmol/l peripheral parenteral nutrition product containing amino acids, lipids and glucose in separate compartments in dogs. METHODS: Nine dogs were administered the three-in-one product, and two dogs were administered the amino acid part of the product, via a peripheral vein. Dogs were monitored for mechanical and metabolic complications. RESULTS: Mechanical complications (apparent thrombus or thrombophlebitis) caused failure of infusion at a median of 36 hours. None of the dogs appeared to develop catheter-related sepsis. Using a 10-hour infusion period appeared to decrease the incidence of line failure. Mild and clinically non-significant hyperglycaemia was the only metabolic complication. In four of the dogs, serum folate, cobalamin and homocysteine concentrations were determined before and after peripheral parenteral nutrition administration. Oral and parenteral administration of methionine has been previously associated with lowered serum folate concentrations. Low serum folates and the subsequent hyperhomocysteinaemia have been associated with venous endothelial damage and venous thrombus in other species. Serum cobalamin also affects homocysteine metabolism. Median serum folate, cobalamin and homocysteine concentrations were not affected by the short-term administration of this three-in-one product. CLINICAL SIGNIFICANCE: Using the product for 24 hours/day may require catheter replacement due to line failure. Other than line failure, which may be improved by 10- to 12-hour infusion times, this product was found to be safe and practical for short-term peripheral parenteral nutrition in dogs.  相似文献

Five ponies and one donkey with hyperlipaemia that occurred secondarily to a variety of primary clinical conditions were treated with lipid-free partial parenteral nutrition comprising equal volumes of 50 per cent glucose and 15 per cent amino acids. The infusion supplied energy and protein at rates of 2.6 kJ/kg per hour and 34.3 mg/kg per hour, respectively. In all six cases there was a prompt and sustained decrease in serum concentrations of triglyceride. In four of the six cases a good response to treatment of the primary condition was also seen and the subjects were discharged successfully. In the remaining two cases, poor clinical response of the primary condition resulted in euthanasia, although hyperlipaemia was nevertheless resolved. The main complication of parenteral nutrition was hyperglycaemia.  相似文献

The purpose of this study was to investigate clinical and metabolic effects of combined parenteral and oral nutrition compared with parenteral nutrition in young dogs with haemorrhagic gastroenteritis in a prospective clinical study. Dogs with acute gastroenteritis received either parenteral nutrition (group PN, n = 9) or combined parenteral and early enteral nutrition (group EN, n = 10). Infusions were compounded from amino acids, lipids, glucose and electrolyte/glucose solutions [149 g/l glucose, 20 g/l triglycerides, 40 g/l amino acids and 4009 kJ metabolizable energy/l (957 kcal ME/l)], and supplemented with potassium, phosphate and trace elements. Group EN received additionally a hydrolysed diet (74 kJ/kg BW(0.75) on day 2 and 148 kJ/kg BW(0.75) on days 3 and 4). Glucose, triglycerides, protein, albumin, fibrinogen, urea, creatinine, alkaline phosphatase, glutamate dehydrogenase and glutamate pyruvate transaminase were measured before and during the infusions, haematological traits only before the infusions. Statistics included two-factorial anova and subsequent t-test or Wilcoxon test (P < 0.05). All dogs of group EN survived compared with seven of nine patients in group PN. Most dogs in the EN group vomited within half an hour after introduction of oral feeding on day 2 but tolerance for food increased on days 3 and 4. The general health status and faecal and blood parameters of the surviving dogs were similar (P > 0.05) between the groups. In all dogs leucocytes increased during the treatment period, haematocrit and haemoglobin levels declined. Infusions increased blood glucose and triglycerides (P < 0.05); however, no adverse signs were observed. Early enteral nutrition was possible after a short period of adaptation, however, vomiting can be a severe problem. The evaluation of clinical benefits of early enteral nutrition in young dogs with haemorrhagic gastroenteritis requires further investigations.  相似文献

Parenteral nutrition (PN) is used to support intensive care patients. The risk for adverse metabolic effects depends on the composition of infused solutions and the duration of application. The present study in dogs compares metabolic and endocrine effects of two infusion solutions, with either triglycerides or glucose being the major energy sources, administered in a comparatively short infusion period (10 h/day). PN was administered for 9 days to two groups of five adult dogs to meet energy maintenance requirements. In group PN-LIP 61% of the total energy was derived from lipids and 22% from carbohydrates, compared with 21 and 62% in group PN-GLUC. Among routine haematology and clinical chemistry the plasma levels of glucose, triglycerides, insulin, insulin-like growth factor-I (IGF-I), glucagon, 3,5,3'-triiodothyronine and thyroxin were measured in non-infused dogs and at 2, 4, 6, and 8 h after the start of infusion at days 2 and 8 of the study. Infusions protocols did not cause gross metabolic aberrations. During the actual infusions glucose, triglyceride and insulin concentrations were elevated, each depending on the infusion solution. Concentrations of IGF-I, glucagon, 3,5,3'-triiodothyronine, thyroxin and cortisol did not change significantly. In conclusion short infusion periods of 10 h per day were tolerated by healthy dogs without adverse signs, which could improve practicability of PN also in clinical cases.  相似文献

OBJECTIVE: To determine nitrogen balance in clinically normal dogs receiving parenteral nutrition solutions. ANIMALS: 8 clinically normal female Beagles. PROCEDURE: Dogs were randomly assigned to receive 4 treatments in random order. Treatment A consisted of IV administration of nonlactated Ringer's solution. Treatments B, C, and D consisted of IV administration of isocaloric parenteral solutions containing 0, 1.36, and 2.04 g of amino acids/kg of body weight/d, respectively, for 7 consecutive days. Urine and feces were collected on days 5, 6, and 7 of each treatment period, and Kjeldahl analysis was used to determine nitrogen balance. RESULTS: Mean nitrogen balance was negative with treatments A and B but was not significantly different from 0 with treatments C and D. Dogs had the lowest nitrogen balance values and lost the most weight while receiving treatment A. Dogs were able to conserve protein and had higher nitrogen balance values when receiving treatment B, compared with treatment A. Dogs lost the least amount of weight while receiving treatment D. Regression analysis indicated that an IV amino acid intake of 2.32 g/kg/d (95% confidence interval, 2.00 to 2.81 g/kg/d), as supplied by the commercial product used in this study, would result in zero nitrogen balance in clinically normal dogs. CONCLUSIONS AND CLINICAL RELEVANCE: Results suggest that IV amino acid requirement of clinically normal dogs is approximately 2.3 g/kg/d.  相似文献

Newborn piglets were fed either a 5 per cent glucose/electrolyte solution or a commercial milk replacer for 24 or 36 hours before they were given colostrum, or they were given colostrum after birth. The effects of these treatments on their ability to absorb macromolecules was assessed by measuring the uptake of 125I-polyvinylpyrrolidone (125I-PVP) and the increases in the concentrations of total proteins and immunoglobulins in serum. The piglets which received colostrum at birth absorbed 50 per cent of a test dose of 125I-PVP within six hours, and there was a rapid increase in the concentration of immunoglobulins in their blood. The piglets which received the 5 per cent glucose/electrolyte solution absorbed 40 per cent of the test dose of 125I-PVP administered 24 or 36 hours after birth and nearly normal quantities of immunoglobulins, but the piglets given the milk replacer absorbed only 10 per cent of the test dose of 125I-PVP, and virtually no immunoglobulins.  相似文献

Background: The use of soybean oil-based lipid emulsion (SO-based LE) in parenteral nutrition has been reported to impair neutrophil functions in humans and rodents. As yet, little is understood about the effects of SO-based LE on canine immune responses.
Hypothesis: A short-term infusion with SO-based LE affects the phagocytic responses of canine peripheral blood polymorphonuclear neutrophilic leukocytes (PMNs).
Animals: Twenty-four healthy Beagle dogs.
Methods: Experimental study. Dogs were randomly assigned into groups of six and administered a 2-hour IV infusion with 0.9% NaCl solution or sufficient SO-based LE (INTRALIPOS 20%) to supply 40, 100, and 200% of the basal energy requirement (BER). PMN functions were determined after collecting blood samples before, immediately after, and 24 hours after the infusion.
Results: None of the treatments significantly affected the phagocytic capacity of PMNs or circulating leukocyte numbers. The infusion providing 200% of BERs significantly reduced PMN oxidative burst activity, filamentous actin polymerization, and Cdc42 Rho guanosine triphosphatase activity immediately after its delivery. However, these functions were restored to pre-infusion values 24 hours after the infusion. The lower calorie infusions did not have these effects.
Conclusions and Clinical Importance: These results suggest that short-term infusions with a supraphysiological dose of SO-based LE may decrease the immune functions of canine PMNs. However, more long-term studies will be needed to extrapolate the effect of SO-based LE with clinically relevant doses in a practical situation.  相似文献

The objective of this experiment was to evaluate the influence of glucose infusion during lactation on LH secretion in the energy-restricted sow. Ten primiparous Landrace x Yorkshire sows (152 kg postfarrowing) were fed a low-energy (6.5 Mcal of ME/d), high-lysine (45 g/d), corn-soybean meal diet throughout lactation. On d 18 of lactation, sows received a continuous infusion (1 L/12 h) of glucose (50% dextrose solution) or .9% saline from 1200 to 2400. Blood samples were drawn every 15 min for an 18-h period on d 18 to evaluate concentrations of plasma glucose, serum insulin, and serum LH before (600 to 1200) and during (1215 to 2400) the infusions. The glucose infusion immediately increased (P less than .001) plasma glucose and serum insulin relative to preinfusion levels. Glucose and insulin concentrations in sows receiving the glucose infusion were higher (P less than .001) throughout the infusion period relative to concentrations in sows receiving a saline infusion. Glucose infusions had no influence on LH pulsatility during the 12-h infusion period. In contrast to the response observed in the nutrient-restricted gilt, these results indicate that glucose infusions do not result in an immediate increase in pulsatile LH secretion in the energy-restricted, lactating sow.  相似文献

To assess the leptin response to metabolic challenges, three Italian Simmental cows were infused for 6 h: with (a) saline (control); (b) glucose; and (c) amino acid solutions according to a 3 x 3 Latin square experiment. The infusions were carried out at the 36th week of pregnancy, and the second and 12th week of the following lactation. At each of the three infusion periods, blood samples were collected from the jugular vein before and 15, 30, 120, 180, 240, 300 and 360 min after the beginning of each infusion. All samples were analysed for leptin, insulin, glucagon, growth hormone (GH), glucose, non-esterified fatty acids (NEFA) and urea. The physiological phase of the cows significantly affected the basal concentrations of insulin, glucagon, urea and NEFA. The infusion of both glucose and the amino acid solutions did not affect leptin concentrations. Insulin response was significantly increased when animals were infused with the glucose solution and, within treatment, the greatest response was observed at the 12th week of lactation. The greatest glucagon response was observed when infusing the amino acid solution. Urea response to all treatments increased from the dry period to the 12th week of lactation. The GH and NEFA responses were not affected by treatments. The Multi Species radio-immunoassay used in this study showed a lower sensitivity for ruminant leptin which may partially explain the lack of significant leptin variations. However, it can be hypothesized that leptin variations around parturition can be affected by the negative energy balance, and leptin release is not acutely affected by glucose and amino acid availability. In addition, no short-term relationship were found between insulin, glucagon and GH and leptin release in Italian Simmental cows during the dry period and early lactation.  相似文献

The effects of three dietary protein intakes (1–9, 1–3, and 0–6 g protein/kg bodyweight/day) on selected aspects of protein, mineral and electrolyte balance as well as renal functions was studied in normal dogs with moderate chronic renal failure. Dietary phosphate levels varied according to the quantity of phosphate in protein sources. Within the ranges of protein intake examined in this study, protein intake did not have a significant effect on glomerular filtration rates as measured by inulin clearance or 24-hour creatinine clearance. Restricting protein intake to less than 1–9 g protein/kg bodyweight/day further reduced retention of nitrogenous waste products as measured by serum urea nitrogen concentrations, but at the expense of adequate protein nutrition. Diets providing less than 1–9 g protein/kg bodyweight/day promoted protein malnutrition characterised by hypoproteinaemia and hypoalbuminaemia in both normal dogs and dogs with renal failure. Serum protein concentrations were similar in normal and renal failure dogs, suggesting that, under the conditions of this study, normal and renal failure dogs may have similar protein requirements. Progressive dietary protein/phosphate restriction improved but did not normalise phosphate balance in renal failure dogs.  相似文献

Fasting has been shown to decrease chemotherapy‐associated adverse events (AEs), in part through insulin‐like growth factor (IGF‐1) reduction, and may induce a protective effect on normal cells during chemotherapy treatment in mice and people. The purpose of this study was to evaluate the effect of fasting on constitutional, bone marrow and gastrointestinal (GI) AEs, and serum glucose, IGF‐1 and insulin levels in dogs receiving vincristine. The study was a prospective, crossover clinical trial in tumour‐bearing dogs. Dogs were randomized to be fasted for 24 to 28 hours prior to and 6 hours following their first or second vincristine treatment, and fed normally for the alternate dose. A significant reduction in nausea, anorexia, lethargy and serum insulin was observed when dogs were fasted; however, no significant differences were found in other GI symptoms, neutrophil count, serum glucose or IGF‐1. Fasting prior to vincristine therapy is a safe and effective treatment modality that helped mitigate constitutional and GI AEs in tumour‐bearing dogs.  相似文献

The purpose of this retrospective study was to evaluate the use of partial parenteral nutrition (PPN) in dogs and cats. The medical records of all dogs and cats receiving PPN between 1994 and 1999 were reviewed to determine signalment, reasons for use of PPN, duration of PPN administration, duration of hospitalization, complications, and mortality. Complications were classified as metabolic, mechanical, or septic. One hundred twenty-seven animals (80 dogs and 47 cats) were included in the study, accounting for 443 patient days of PPN. The most common underlying diseases were pancreatitis (n = 41), gastrointestinal disease (n = 33), and hepatic disease (n = 23). Median time of hospitalization before initiation of PPN was 2.8 days (range, 0.2-10.7 days). Median duration of PPN administration was 3.0 days (range, 0.3-8.8 days). Median duration of hospitalization was 7 days (range, 2-20 days). In the 127 animals receiving PPN, 72 complications occurred. These included metabolic (n = 43), mechanical (n = 25), and septic (n = 4) complications. The most common metabolic complication was hyperglycemia (n = 19), followed by lipemia (n = 17) and hyperbilirubinemia (n = 6). Most complications were mild and did not require discontinuation of PPN. Ninety-three (73.2%) of the 127 patients were discharged. All 4 animals with septic complications were discharged from the hospital. The presence, type, and number of complications did not impact the duration of hospitalization or outcome. However, animals that received supplemental enteral nutrition survived more often than those receiving PPN exclusively. Although PPN seems to be a relatively safe method of providing nutritional support, future studies are warranted to determine its efficacy.  相似文献

The use of an intravenous glucose tolerance test (IVGTT) is described in 11 confirmed cases of insulinoma. Basal plasma glucose and serum insulin concentrations, glucose half-life (t.J and fractional clearance rate (k-value) were determined in the affected animals and seven healthy dogs. A fasting plasma glucose concentration of 3 mmol/litre or less with a coexisting serum insulin concentration greater than 20 mU/litre, and an insulin:glucose ratio (IGR) greater than 4-2 U/mol were considered to be diagnostic for insulinoma. Although there was a narrow area of overlap in the U2 and k-values between normal and affected animals, a t.,2 of less than 20 minutes with a k-value of more than 3 per cent/minute were also highly suggestive of insulinoma. The insulinogenic index (δI/δG) was too variable to be of diagnostic significance. Contrary to previous reports, it appears that insulin-secreting tumours retain a degree of responsiveness to a glucose challenge and that the IVGTT, using a dose of 0–5 g glucose/kg bodyweight, is a useful diagnostic procedure in dogs with insulinoma.  相似文献

In fasting dogs between 29 and 53 per cent (median 42 per cent) of newly produced bile was stored in the gall bladder, with the remainder being released directly into the duodenum. Pretreatment with methadone and atropine resulted in the complete closure of the sphincter of Oddi for three hours, and the complete retention of bile in the gall bladder. This effect could be abolished with nalorphine. The minimum dose of methadone to prevent bile from entering the intestines was 0.25 mg/kg, in combination with 0.1 mg/kg atropine. A meal of canned dog food resulted in the release of between 5 and 65 per cent of gall bladder bile (median 31 per cent) into the duodenum. Infusion with cholecystokinin octapeptide emptied the gall bladder more effectively and predictably and a median of 80 per cent (range 62 to 93 per cent) of the bile had been emptied into the duodenum after one hour. It was concluded that the induction of bile storage in the gall bladder with methadone and the subsequent stimulation of its release into the duodenum by the infusion of cholecystokinin produced an endogenous bile acid load that was three to four times larger than that induced by a meal after a period of fasting.  相似文献

The insulin and glucose responses to glucagon infusions (27 microgram/hr) were determined in sheep before and after parenteral lead treatment (6 mg/kg intravenously). Glucose production was measured by primed continuous infusion of [6-3H]glucose. Glucagon and insulin concentrations before and during glucagon infusions were not significantly different between lead treatment and control experiments. Lead administration did not affect the concentration or production of glucose in the preinfusion period. However, depressed hyperglycemia during glucagon infusion in lead treated experiments tended to be associated with decreased glucose production. The reduced glucogenic response to glucagon may be the result of reduced function of pyruvate carboxylase, a key hepatic gluconeogenic enzyme in sheep, from lead induced impairment of mitochondrial function.  相似文献

OBJECTIVE: To evaluate the clinical and immunologic response in healthy dogs to infusions of human serum albumin (HSA). ANIMALS: 9 healthy purpose-bred mixed-breed dogs. PROCEDURES: Each dog was administered a 25% HSA solution once or twice. Various physical examination and laboratory variables were serially evaluated. Antibody against HSA was assayed before and after infusion by use of an ELISA. Intradermal testing was also conducted. A repeated-measures ANOVA or Friedman repeated-measures ANOVA on ranks was used to compare results for the variables. RESULTS: Adverse clinical reactions were observed after the first or second infusion in 3 dogs. Anaphylactoid reactions were observed in 1 of 9 dogs during the first infusion and in 2 of 2 dogs administered a second infusion. Two dogs developed severe edema and urticaria 6 or 7 days after an initial infusion. All dogs developed anti-HSA antibodies. Positive responses for ID tests were observed in 8 of 9 dogs. Short-term increases were detected in blood protein, total bilirubin, and calcium concentrations after HSA infusion. Serum cholesterol concentrations and platelet counts decreased after HSA infusion. CONCLUSIONS AND CLINICAL RELEVANCE: Administration of HSA resulted in profound reactions in 2 of 9 dogs administered a single infusion and in 2 of 2 dogs administered a second infusion. This indicates that there is risk of life-threatening adverse reactions to HSA infusion in healthy dogs.  相似文献

Parenteral nutrition has been an important adjunct to therapy of abdominal diseases in calves, with chronic diarrhea and wasting being the most common indication. Parenteral nutrition is administered on a short-term basis to prevent further protein-energy malnutrition in debilitated calves that cannot or will not consume adequate quantities of milk. Parenteral nutrition solutions consist of a protein source (amino acids) and energy sources (glucose and lipid emulsions), supplemented as needed with balanced electrolytes and vitamins. Complications due to PPN are rare, and it is the authors' clinical impression that survivability is enhanced when PPN is employed, although enhanced survivability was not demonstrated in one controlled experimental trial.  相似文献

Two experiments were conducted to study the effects of alterations in ruminal pH and volatile fatty acid (VFA) concentrations on utilization of Mg and other minerals. In Exp. 1, two metabolism trials were conducted with 12 ruminally cannulated crossbred wethers fed 800 g/d of orchard-grass (Dactylis glomerata, L.) hay. After each feeding, wethers were ruminally infused with 500 ml (4.2 ml/min) or either 1) deionized water, 2) 40% (w/v) glucose solution, 3) .26 M propionic and .17 M butyric acid solution or 4) .35 M HCl. The pH of the VFA solution was adjusted to 6.8 with 10N NaOH. In Exp. 2, a metabolism trial was conducted with 12 ruminally cannulated crossbred wethers fed 600 g of orchard-grass hay and infused with a buffered VFA solution prepared as in Exp. 1 or with an unbuffered solution. In both experiments each trial consisted of a 5-d adaption period followed by four 5-d collections of feed, feces and urine. Compared with the glucose treatment, infusion of the buffered VFA solution produced similar acetic and propionic and higher (P less than .05) butyric acid concentrations (Exp. 1). The HCl solution produced changes in ruminal and pH values similar to those of the glucose infusion. In Exp. 1, apparent absorption of Mg was increased over twofold by the glucose infusion (P less than .05), but the other infusions had no effect. Apparent absorption of P was decreased (P less than .05) by HCl infusion, and K absorption was decreased by HCl and glucose infusions. In Exp. 2, infusion of the unbuffered VFA solution decreased apparent Mg absorption by 15.7%, compared with infusion of the buffered solution. These experiments suggest that the increased Mg absorption observed with carbohydrate supplementation is not due to alterations in ruminal pH or VFA levels.  相似文献

In six normal‐weight and six obese cats, the metabolic effect of propionate absorbed from the colon was assessed. Two colonic infusions were tested in a crossover design with intervals of 4 weeks. The test solution contained 4 mmol sodium propionate per kg ideal body weight in a 0.2% NaCl solution. Normal saline was given as control solution. Solutions were infused into the hindgut over 30 min. Blood samples were obtained prior to and at various time points after starting the infusion. As body condition did not affect evaluated parameters, all data were pooled. Plasma glucose concentrations showed differences neither over time nor during or after infusion with propionate or control. Plasma amino acid concentrations rose over time (p < 0.001), but were similar for both infusions. Plasma propionylcarnitine rose markedly towards the end of the propionate infusion and decreased afterwards (p < 0.001), whereas 3‐hydroxy‐3‐methylglutarylcarnitine was lower 30 (p = 0.005) and 60 min (p = 0.032) after ending propionate infusions and acetylcarnitine tended to fall at the same time points (p = 0.079; p = 0.080), suggesting inhibition of gluconeogenesis from pyruvate and amino acids, but initiation of propionate‐induced gluconeogenesis. In conclusion, propionate absorbed from the colon is hypothesized to act as gluconeogenic substrate, regardless of the cat’s body condition.  相似文献
