A 10-year-old Tennessee Walking Horse gelding was admitted to the veterinary teaching hospital for evaluation of intermittent fever, lethargy, and anorexia. Initial laboratory analyses revealed anemia and hyperfibrinogenemia. Abdominocentesis and thoracentesis yielded fluid samples with high nucleated cell counts and total protein concentrations. The tentative diagnosis was nonseptic peritonitis. The horse did not improve after 4 days of antimicrobial treatment, and pitting edema of the ventral midline developed. Thoracic radiography and ultrasonography revealed consolidation of the ventral aspect of the lung fields and pleural effusion. Pleuroscopy of the right hemithorax revealed pleural effusion and a soft-tissue mass in the caudal portion of the mediastinum. Findings on biopsy of the liver and mediastinal mass led to a presumptive diagnosis of metastatic cholangiocellular carcinoma. The horse was euthanatized, and the diagnosis was confirmed at necropsy.  相似文献

A leiomyoma of the small colon was discovered incidentally in a 4-year-old Thoroughbred gelding during colic surgery to correct large colon displacement. The mass and 20 cm of small colon were resected, and an end-to-end anastomosis was performed. A postoperative fecal impaction proximal to the anastomosis responded after 5 days to administration of intravenous fluids, analgesics, and stool softeners.  相似文献

Small-colon rupture attributable to granulosa cell tumor in a mare   总被引:1,自引:0,他引:1  
A large granulosa cell tumor was believed to be responsible for causing obstruction and subsequent rupture of the small colon in a 10-year-old Quarter Horse mare. Two months earlier, a mass, tentatively diagnosed as granulosa cell tumor of the left ovary, had been identified by means of rectal palpation and ultrasonography. The mare was evaluated for clinical signs of acute, severe, abdominal pain, increased heart rate, cyanotic mucous membranes, clinical dehydration, with high PCV, leukopenia, and extreme abdominal distension. A large soft tissue mass and taut band that constricted the lumen of the small colon were palpable per rectum. Septic peritonitis was diagnosed on the basis of results of abdominocentesis. Exploratory surgery revealed extensive fecal contamination of the abdominal viscera, and the mare was euthanatized because of the resultant poor prognosis. At necropsy, the small colon was occluded by a taut, left broad ligament and the ovarian mass that was proved to be a granulosa cell tumor. The occlusion had caused impaction of the small colon, with subsequent perforation at the level of the broad ligament.  相似文献

A 15‐year‐old trotter gelding was evaluated because of an acute onset of ataxia in all 4 limbs. There was no known history of trauma. The gelding showed grade 2/5 ataxia in all 4 limbs, which was localised after clinical neurological examination to the cervical vertebral spinal cord. Initial therapy consisted of oral anti‐inflammatory doses of prednisolone and antimicrobial treatment with potentiated sulphonamides. The ataxia progressed to grade 3/5 at Day 10 of hospitalisation. Additionally, the horse was slightly depressed and showed spontaneous yawning during examination. Facial sensation was blunted. Blood chemistry revealed a marked elevation of liver specific enzymes and blood ammonia levels. Transcutaneous abdominal ultrasonography revealed hepatomegaly. Due to a guarded prognosis, the horse was subjected to euthanasia. At necropsy the left lateral liver lobe was markedly enlarged and showed a firm texture, whereas the cranial part and the right and quadratic liver lobe displayed a severe and diffuse atrophy. Histopathologically, the left lateral liver lobe revealed a moderate to severe cirrhosis with a severe, diffuse hepatocellular iron‐accumulation. Increased numbers of Alzheimer type II astrocytes in the cerebral cortex and cerebral white matter vacuolisation were indicative for encephalopathy. These findings were interpreted as haemosiderosis and cirrhosis of the liver with consecutive hepatic encephalopathy. Aetiologically, haemosiderosis should be considered as a cause of liver cirrhosis with consecutive hepatic encephalopathy. Although hepatic encephalopathy in horses usually presents with predominating cerebral signs, it has to be taken into account as a differential diagnosis in cases of acute onset generalised ataxia.  相似文献

A 9-year-old Saddlebred gelding was referred to the University of Wisconsin Veterinary Care for decreased energy levels, weight loss, persistently elevated liver enzymes, hyperglobulinaemia and leukocytosis. Abdominal ultrasonography showed a large volume of peritoneal fluid of mixed echogenicity, and the presence of focally distributed, circular to oval (approximately 1–2 cm), hyperechoic areas within the parenchyma of an enlarged liver. Cytological analysis of the peritoneal fluid revealed sterile neutrophilic/macrophagic inflammation, with haemosiderophages. A liver biopsy confirmed the presence of hepatic amyloidosis and focal fibrosis. Medical therapy with trimethoprim-sulfamethoxazole for 30 days and long-term dexamethasone was associated with both a clinical and biochemical response for 7 months, at which time the horse died from presumed hepatic rupture. To the authors’ knowledge, this is the first report describing potential successful management of hepatic amyloidosis in a horse.  相似文献

A quarter horse gelding with intermittent colic was diagnosed with a perirectal abscess and dyschezia. Rectal ultrasonography identified a multiloculated, fluid-filled mass. A perirectal abscess was diagnosed when the mass ruptured and drained into the rectum. The abscess was treated successfully with warm soapy water enemas and trimethoprim and sulfamethoxazole.  相似文献

A 14-year-old Trakehner gelding was evaluated for recurrent colic, with episodes occurring over 1 year. Signs were consistent with intermittent ascending colon obstruction and hematochezia. Necropsy examination revealed an ulcerated mass extending into the lumen of the right dorsal ascending colon. Gross and histologic appearance and immunoreactivity to c-kit (CD117), desmin, vimentin, and smooth muscle actin, were consistent with a diagnosis of gastrointestinal stromal tumor.  相似文献

A 14-year-old Quarter Horse gelding presented for a 2-week history of increased respiratory rate, fever and weight loss. The referring veterinarian performed an ultrasonographic examination on the farm and considered a differential diagnosis of pleuritis and pleuropneumonia. On presentation, transtracheal wash showed suppurative septic inflammation with bacteria within macrophages. Thoracic ultrasonography showed increased hypoechoic fluid in the pleural and peritoneal cavities. Cytologic analysis of pleural and peritoneal fluids revealed lymphorrhagic effusion with atypical lymphocytes, suggestive of a lymphatic flow obstruction or lymphoma. Treatment consisted of drainage of the pleural fluid, anti-inflammatory agents, fluid therapy and antimicrobial therapy. Continued fluid accumulation in the thoracic cavity, tachypnoea, lack of response to pain medication and progressive lethargy resulted in the horse being subjected to euthanasia. At necropsy, bicavitary effusion was confirmed and histopathological diagnosis of multicentric lymphoma was made. Horses presenting with pleural effusion should also be evaluated for peritoneal effusion, as bicavitary effusion may indicate noninflammatory conditions such as neoplasia.  相似文献

A 23-year-old Thoroughbred gelding was referred for the evaluation of acute onset of ataxia and depression, and a 2-day history of fever. On physical examination, the gelding was profoundly depressed and 10-12% dehydrated. The horse appeared very unstable, with a wide-based stance in the hind limbs, severe symmetric ataxia in all 4 limbs, and proprioceptive deficits in both hind limbs. Nasogastric intubation produced 4 L of brown, fetid reflux, and rectal examination revealed mild small intestinal and cecal distention. Hematologic abnormalities included neutropenia with toxic change, compatible with acute inflammation and endotoxemia, and prolonged coagulation times. Serum biochemical abnormalities included prerenal azotemia. metabolic acidosis, and electrolyte abnormalities consistent with enteritis. Blood ammonia concentration was markedly increased (406 micromol/L; reference interval 4-49 micromol/L), however, serum bile acids concentration and hepatic enzyme activities were within reference intervals. Histopathologic examination of a liver biopsy revealed no abnormalities and results of tests for several infectious agents were negative. Clinical signs resolved with correction of the dehydration and electrolyte abnormalities and with antibiotic therapy. The horse was diagnosed with hyperammonemic neuropathy associated with gastrointestinal disease. In such cases, hyperammonemia is caused by increased production of ammonia by organisms in the gastrointestinal tract in combination with increased gut permeability that facilitates ammonia absorption.  相似文献

A donkey developed abdominal discomfort and distension together with cardiovascular collapse and gastric reflux 24 to 36 hours after ingesting a large amount of poultry feed. Rectal findings prompted a laparotomy that identified extensive gastric dilation, an empty, atonic small intestine, dry colonic content and an easily corrected caecal displacement. These findings were not consistent with the severity of the signs, which were attributed to endotoxaemia. The donkey was euthanased during recovery. Necropsy revealed acute necrotising pancreatitis with massive gastric dilation and right dorsal colon impaction. The clinicopathological features of acute equine pancreatitis associated with grain overload are discussed.  相似文献

A 14‐year‐old Thoroughbred gelding was presented for chronic colic and weight loss. Transcutaneous and transrectal abdominal ultrasonography revealed distended, thickened small intestine with primary thickening of the muscularis and a focally more thickened loop with an echoic structure crossing the wall from the mucosa to the serosa. Visualization of diffuse thickening of the muscularis (muscular hypertrophy of the small intestine) and a focal lesion (pseudodiverticulum) helped clinicians make informed decisions. This case illustrates the importance of transabdominal and transrectal ultrasonography in horses with chronic colic and the relevance of considering the abnormalities in layering pattern of the intestinal wall.  相似文献

Eight cases of feline pancreatic adenocarcinoma and two cases of pancreatic adenoma were reviewed. The adenomas were incidental findings. Most cats with adenocarcinomas had anorexia (75%) and vomiting (63%), while 38% had abdominal pain, a palpable abdominal mass, and/or jaundice. Diagnostic abnormalities included leukocytosis, hyperglycemia, increased alanine aminotransferase activity, poor serosal detail on abdominal radiography, and an abdominal mass effect on ultrasonography. The majority of cats with carcinomas had metastases (mostly to liver, lung, and small intestine), and all were euthanized or died within 7 days of diagnosis. Clinically, feline pancreatic carcinoma may be difficult to distinguish from feline pancreatitis.  相似文献

A 3-year-old Thoroughbred gelding presented with acute signs of colic and tachycardia. Transabdominal ultrasonography revealed a viscus visible on the left and right sides of the cranioventral abdomen with a mural thickness of up to 1.6 cm. Exploratory laparotomy was performed under general anaesthesia. This revealed that the caecum was incarcerated through a rent in the gastrosplenic ligament, and the caecal apex was in the left caudal abdomen. The ventral aspect of the gastrosplenic ligament was ligated and transected. The caecum was freed, and normal orientation confirmed. The horse recovered uneventfully from surgery. Caecal mural thickness was monitored using daily ultrasonography, until normal. The horse was discharged from the hospital 11 days post-operatively. A 14‐year‐old Connemara cross gelding presented with abnormal respiratory noise and exercise intolerance. Upper airway endoscopy, ultrasonography, radiography and computed tomography revealed a large mass within the left guttural pouch causing marked left dorsal nasopharyngeal collapse and displacement and compression of the right guttural pouch. The horse was subjected to euthanasia and a post‐mortem examination confirmed the above findings. Histological and immunohistochemical examinations of the mass confirmed a diagnosis of guttural pouch leiomyosarcoma, a lesion previously unreported at this site.  相似文献

A 20-year-old gelding Lipizzaner horse was referred for onset of profuse sweating and increased heat on the right side of the head, neck, and thoracic limb. On physical examination, the horse presented tachycardia, muffled heart sounds with a pansystolic murmur, and signs of heart failure such as ventral edema, jugular distention, and pulsation. Ultrasonography examination revealed a bilateral pleural effusion and a large intrathoracic mass extending into the anterior mediastinum, which shifted the heart dorsocaudally. Thoracocentesis revealed serosanguineous fluid indicative of a melanocytic tumor. Thoracoscopy revealed a large dark mass extending into and filling the cranioventral mediastinum. The mass appearance was consistent with a melanoma, and because prognosis was poor, the owner elected euthanasia. A necropsy supported the presence of disseminated melanomas with a greater thoracic mass involving the right cervicothoracic ganglion, leading to clinical signs of sympathetic denervation.  相似文献

A seven-year-old standardbred gelding developed marked signs of colic associated with an acute small intestinal obstruction. Surgical exploration revealed three intramural, circumferential constricting lesions in the small intestine, the two most severe of which were in the jejunum and were resected. The horse was euthanased owing to postoperative complications. Histopathological examination confirmed the diagnosis of idiopathic multifocal eosinophilic enteritis.  相似文献

Pleural effusion, secondary to a metastasis from a malignant hepatoblastoma, was diagnosed in a 3-year-old Appaloosa gelding. Severe hemorrhagic transudate in both pleural cavities resulted in acute onset of labored breathing, tachypnea, tachycardia, and jugular vein pulsation. Results of ultrasonography and radiography of the ventral lung field and cranial portion of the abdomen initially were nondiagnostic, as were results of cytologic examination of peritoneal fluid and tracheal wash specimens. Moderately high serum gamma-glutamyl transferase and alkaline phosphatase activities, despite normal hepatocyte-specific enzyme (sorbital dehydrogenase) activity, were indicative of biliary stasis without hepatocyte destruction. The horse was euthanatized. Necropsy revealed a 47-kg hepatoblastoma, with metastases in the lungs and intestines.  相似文献

An 8-year-old Arabian gelding with septic cholangitis and peritonitis was successfully treated with trimethoprim/sulfadiazine. The gelding was referred for evaluation of signs of abdominal pain, icterus, fever, and weight loss. Peritoneal fluid analysis revealed septic and suppurative peritonitis. Culture of the peritoneal fluid yielded Escherichia coli and Klebsiella pneumoniae, which were sensitive to trimethoprim/sulfadiazine. On the basis of results of hepatic ultrasonography, a diagnosis of septic cholangitis also was made. The horse was treated with 30 mg of trimethoprim/sulfadiazine/kg, PO, q 12 h for approximately 6 weeks. The horse improved steadily, and telephone follow-up with the owner 1 year later disclosed that the horse had complete return to normal condition, appetite, and attitude. On the basis of our findings, aggressive, long-term anti-inflammatory and antibiotic treatment may result in complete return to health and normal athletic function in horses with septic cholangitis and concurrent septic peritonitis.  相似文献

A 13-year-old Thoroughbred gelding presented with acute onset respiratory distress, muscle fasciculations and ataxia during ridden exercise. On physical examination, the horse was extremely distressed and had a markedly increased respiratory rate of 92 breaths/min and heart rate of 82 beats/min. Auscultation of the thorax revealed adventitious sounds throughout the lung fields. A continuous, loud, splashing (‘water-wheel’) murmur was evident on cardiac auscultation. A diffuse, marked interstitial and patchy alveolar pattern was present on thoracic radiographs and comet tail artefacts were found on thoracic ultrasonography. Echocardiography showed air embolisation, with the right ventricle and atrium markedly opacified with air bubbles. The horse deteriorated despite treatment with intranasal oxygen therapy, anti-inflammatories, antimicrobials, diuretics and glycopyrrolate. Post-mortem examination identified an expansile haematoma of the sphenopalatine sinus secondary to a rostral fracture of the skull, a previously unreported differential for vascular air embolism in the horse.  相似文献

A 12-year-old Clydesdale gelding was presented for colic and dysuria. Obstructive urolithiasis and chronic renal disease were diagnosed via transurethral endoscopy and percutaneous ultrasonography. Nephroliths, hydronephrosis, and peri-ureteral fibrosis were present. Surgical intervention was declined and the gelding was managed medically with antibiotics and dietary modification.  相似文献
