A 3-day-old Quarter Horse colt was examined because of signs of severe depression, discomfort, and abdominal straining. The foal seemed disoriented, and the abdomen was tense and distended ventrally. The differential diagnoses included ruptured urinary bladder, retained meconium, septicemia/bacteremia, and neonatal maladjustment syndrome. Serum biochemical analysis revealed marked hyponatremia, hypochloremia, and moderate hyperkalemia, as well as mildly high urea, creatinine, and phosphorus concentrations. The primary differential diagnosis at this time was ruptured urinary bladder. Abdominocentesis was performed to confirm this diagnosis. Microscopic examination of abdominal fluid revealed calcium carbonate crystals, which originated from the urine of the foal. Biochemical analysis also confirmed the diagnosis of ruptured urinary bladder, because the ratio of peritoneal fluid creatinine to serum creatinine was 2.8:1. The foal died before surgical correction could be attempted.  相似文献

Nonsurgical management of ruptured urinary bladder in a critically ill foal   总被引:1,自引:0,他引:1  
A small tear in the urinary bladder of a severely debilitated 4-day-old foal was managed with an indwelling urinary catheter connected to a urine collecting system. Fluid therapy, parenteral nutrition, and antimicrobial agents were used during the initial management of the ruptured bladder. Aseptic technique for catheter care and systemic administration of antimicrobial agents prevented the development of bacterial cystitis. Catheter management required constant monitoring, but the bladder defect was sealed within 5 days. Fungal arthritis caused by Candida tropicalis, immune-mediated anemia, diarrhea, constipation, venous thrombosis, and pneumonia were observed during 43 days of hospitalization. Although the foal died at 3 months of age, serum creatinine concentration and electrolyte values remained within normal limits during the 3-month period.  相似文献

A 2-day-old filly foal presented with signs of depression, recumbency and inappetence. Blood analyses revealed hypoalbuminaemia, hyperfibrinogenaemia, hyperglycaemia and hyperkalaemia. The foal deteriorated despite intensive treatment and was subjected to euthanasia. At post mortem examination, the urinary bladder, ureters and kidneys appeared normal grossly. Histologically both kidneys showed disorganised development with the presence of structures inappropriate for a foal of this age, including primitive glomeruli, immature renal tubules and persistent metanephric ducts. Based on these findings a diagnosis of bilateral renal dysplasia was made.  相似文献

Laparoscopic Repair of a Bladder Rupture in a Foal   总被引:1,自引:0,他引:1  
Ruptured bladder was diagnosed in a 90-day-old Thoroughbred colt that had suffered a open, comminuted tibial fracture 2 days earlier. The bladder rupture was identified by laparoscopic examination of the abdomen and was repaired using a laparoscopic stapling instrument. This technique provided good visualization and allowed repair of the rupture with minimal intervention. Ten months after surgery, the foal was admitted to a referral surgical practice because of colic and stanguria. A urinary calculus was removed from the penile urethra by urethrotomy. Laparoscopic repair of the bladder with nonabsorbable staples may be contraindicated because of possible urolith formation.  相似文献

Three mares were treated for vaginal evisceration of a portion of the small intestine. Evisceration occurred in 2 mares shortly after breeding accidents in which the stallion's penis penetrated the vaginal fornix dorsal to the cervix. The affected bowel was replaced through the laceration, and the vaginal defects were sutured with the mares standing, utilizing epidural anesthesia. One mare recovered without complications, was subsequently bred, and delivered a foal with no difficulty. The other mare developed signs of a strangulating small intestinal obstruction 24 hours after the injury, necessitating exploratory celiotomy. Two meters of small intestine had become devitalized secondary to avulsion of the mesentery and adjacent vascular supply during the evisceration. The affected portion was resected and a jejunocecostomy was performed. Recovery was uncomplicated and 21 months later the mare was 9 months in foal. A third mare was treated unsuccessfully for evisceration of several meters of small intestine through the external urethral orifice after rupture of the urinary bladder.  相似文献

A 6‐day‐old foal was evaluated for depression and inappetence. After initial stabilization, the filly developed a hypochloremic metabolic alkalosis and persistent hypercreatinemia. Abdominal ultrasound revealed fluid accumulation around the left kidney and a well‐defined rounded fluid filled structure dorsal to the urinary bladder. Computed tomography revealed a partial tear of the left ureter with distension of the retroperitoneal membrane. Exploratory celiotomy was performed to allow left kidney nephrectomy. At 6 months follow‐up, the filly was growing normally without complications. Ultrasound and CT imaging in this case provided an accurate diagnosis and a presurgical aid to select the appropriate therapeutic approach.  相似文献

A urachal remnant, causing pollakiuria and dysuria, was diagnosed by rectal palpation of a urinary bladder adhesion and endoscopic visualization of a urinary bladder diverticulum. Surgical excision of the remnant resulted in relief of abnormal micturition, but the filly was euthanatized 9 months after surgery because of a chronic ventral midline incisional infection. Pollakiuria and dysuria associated with urachal abscessation occur most commonly in calves. This report documents the syndrome in a horse.  相似文献

A 7-week-old Appaloosa filly was admitted for persistent urinary incontinence since birth. Vaginal speculum examination revealed urine flowing from an opening in the right vaginal wall. Cystoscopy demonstrated that the ureters did not terminate at the bladder. The endoscope passed easily from the vagina directly into a dilated right ureter. An excretory urogram confirmed the vaginal termination of at least one ureter, based upon extensive filling of the vagina with contrast media in the absence of bladder filling. Bilateral hydroureter and dilated renal pelves were demonstrated both by excretory urography and by ultrasonography. Euthanasia was requested by the owner in lieu of attempted surgical correction. At necropsy, it was confirmed that the right ureter entered the vagina and the left ureter terminated at the urethra. The diagnosis of bilateral ectopic ureter in this foal was suggested by the history and clinical signs, supported by endoscopy and ultrasonography, and confirmed by excretory urography and necropsy. This case establishes the value of diagnostic imaging techniques in the antemortem diagnosis of ectopic ureter.  相似文献

The purpose of this study was to assess the diagnostic value of ultrasonography in the evaluation of abdominal distension in 52 camels (Camelus dromedarius). The conditions included trypanosomiasis (n=35), intestinal obstruction (n=12) and ruptured urinary bladder (n=5). Fifteen clinically normal camels were included as controls. Transabdominal and transrectal ultrasonography was carried out on all camels. In animals with trypanosomiasis, ultrasonographic findings included accumulation of massive amounts of hypoechoic abdominal fluids where liver, intestine, kidney, spleen and urinary bladder were imaged floating. Except in two cases of bile duct calcification and one of hepatic abscessation, no detectable abnormal sonographic lesions were detected while imaging the hepatic and renal parenchyma, and the heart and its valves and major blood vessels. In camels with intestinal obstruction, ultrasonographic findings included distended intestinal loops with markedly reduced or absent motility. In one camel, the intestinal lumen contained localised hyperechoic material that was consistent with a foreign body. Hypoechoic fluid with or without fibrin was seen between intestinal loops. In camels with ruptured urinary bladder, ultrasonographic findings included collapsed and perforated bladder, echogenic blood clots within the urinary bladder and peritoneal cavity, increased thickness of the bladder wall, floating intestines in hypoechogenic fluid and echogenic calculi within the urethra. Ultrasonography was considered a useful tool for the evaluation of dromedary camels with abdominal distension.  相似文献

An omphalocele was detected at birth in a male Arabian foal. The mass contained small intestine and after releasing a constricting band at the body wall, the contents slipped easily back into the abdomen. On the outside was the hairless pink membrane. The interior, now empty of small intestine, contained the umbilical arteries and vein, and a large urachus that extended from the bladder to the opening at the extremity of the mass. Six days after corrective surgery the foal was sent home and remained healthy.  相似文献

Gastrointestinal failure occurs as a consequence of a number of clinical syndromes in neonates including ischemic, distention, inflammatory, and degenerative injury. Failure of this organ system may manifest as diarrhea, ileus, abdominal pain, obstipation, and weight loss. There are numerous causes of gastrointestinal failure in foals; however, the principles of assessment are similar for most causes. The history can be helpful in formulating and focusing the differential diagnosis list. For example, rupture of the urinary bladder is a more likely cause of colic in a foal with progressive abdominal distention that is less than one week of age than in a foal that is 1 month of age. A farm history of current or past enteritis indicates that a medical cause of colic is more likely than a surgical cause; however, distinguishing between medical and surgical causes is challenging. An onset of clinical signs at or just after birth indicates the possibility of a congenital problem. Colic caused by retained meconium must be considered because of the high prevalence of the condition. This article considers the methods used to assess foals with gastrointestinal failure and highlights age-related differences. The principles of initial treatment of foals with gastrointestinal failure are also discussed. Because the clinical condition of neonates can deteriorate rapidly, initial assessment may entail therapeutic intervention to stabilize the patient before a diagnosis is confirmed. Therapeutic intervention may include intravenous or enteral administration of fluids and electrolytes, provision of caloric requirements, prevention or treatment of hypogammaglobulinemia, prevention of infection, and alleviation of pain. The treatment and supportive care of a foal with diarrhea differ from those of a foal with acute abdominal pain associated with other causes. In cases of diarrhea, nonspecific treatments can be implemented before a causative diagnosis is established.  相似文献

Diagnostic imaging, including computed tomography, of a two-month-old foal with renal failure indicated that its right kidney was probably absent and that its left kidney was abnormal in shape. The foal was stabilised and released, but three days later its clinical signs recurred. Postmortem examination revealed renal hypoplasia and dysplasia, the first reported case of this condition in an American miniature horse.  相似文献

Melanosis of the urinary bladder is a very rare condition characterized by an abnormal black or brownish-black pigmentation of the organ. The pigmentary disorder can involve both the urothelial cell layers and/or the submucosa. The biologic potential of the melanosis of urinary bladder remains unknown because only a few cases have been reported in medical literature. So far melanosis of the urinary bladder is not known to occur in cattle. Here we describe the first case of melanosis of the urinary bladder in an inbred red-spotted, 7-year-old cow. Light, ultrastructural, and immunohistochemic investigations demonstrated melanin pigment in the submucosa and lamina propria but not the urothelium of the bladder. In addition, biochemical characterization of the pigment-laden cells demonstrated that the pigment of this disorder consisted mainly of eumelanin, thus corroborating the morphologic studies. Finally, virologic examination revealed the presence of bovine papillomavirus type 2.  相似文献

A 2‐day‐old male Quarter Horse foal was diagnosed with uroperitoneum and ruptured bladder. Intravenous fluid therapy was initiated prior to anaesthesia and repeated unsuccessful attempts were made to drain the abdomen of accumulated urine. Prior to anaesthesia the foal exhibited clinical signs of abdominal compartment syndrome. When anaesthetised the accumulated urine was drained by free flow through a small abdominal stab incision after aseptic site preparation. A few minutes later electrocardiography indicated second degree atrioventricular block which progressed into third degree atrioventricular block and ventricular asystole. The foal was resuscitated by closed chest compressions, mechanical ventilation, sympathomimetic and antimuscarinergic drugs. When anaesthetising these neonatal patients one must be prepared to handle potentially fatal cardiac arrhythmias and, prior to surgery, the danger posed by increased intra‐abdominal pressure should be weighed against the need for diluting plasma potassium.  相似文献

This Case Report describes a 5‐week‐old male Arabian foal of Egyptian bloodline with generalised seizures due to hypocalcaemia. The laboratory changes in this foal were consistent with an abnormal regulation of calcium as occurs with hypoparathyroidism. Prolonged transient neonatal hypoparathyroidism as reported in man was suspected. The foal received oral calcium and vitamin D3 supplementation combined with the calcium sparing diuretic carbonic anhydrase inhibitor acetazolamide. Calcium concentrations increased gradually over 6 months. In Arabian foals, especially those of Egyptian bloodline suffering suspected juvenile epilepsy, hypocalcaemia should be ruled out as a cause of seizures.  相似文献

Percutaneous fine needle aspiration biopsy (FNAB) was used to diagnose a urinary bladder carcinoma in an aged cat. The cytologic appearance of specimens collected initially was similar to that reported for canine transitional cell carcinoma. However, impression smears of the tumor made at necropsy 7 weeks later consisted predominantly of atypical squamous epithelial cells compatible with squamous cell carcinoma. Histologically, the malignancy was noted to have intermixed areas of abnormal squamous and transitional cell proliferation. The neoplasia was interpreted as a transitional cell carcinoma with extensive transformation to squamous cell carcinoma. This report examines the use and limitations of FNAB in the diagnosis of feline urinary bladder carcinoma and the incidence and behavior of these tumors in the cat.  相似文献

Bilateral renal dysplasia and nephron hypoplasia was diagnosed in a Quarter Horse foal with clinical signs of lethargy, convulsions, and diarrhea. Laboratory evaluation revealed anemia, hypoproteinemia, leukopenia, hyponatremia, hypochloremia, and hyposmolality. The foal also had high concentrations of serum creatinine, BUN, and phosphorus. Evaluation of urinary indices revealed a high ratio of urinary gamma-glutamyl-transferase activity to concentration of creatinine, as well as a high fractional clearance ratio of sodium and potassium. Intravenous treatment with saline solution (0.9% NaCl) and antimicrobials provided only temporary resolution of some of the abnormalities. Diagnosis was partly established by histologic evaluation of renal tissue obtained via an ultrasonographically guided biopsy and was confirmed at necropsy. Pathologic changes in the kidney were unique in that the size of the kidneys, along with the appearance and number of glomeruli, were essentially normal despite marked hypoplasia of nephron tubules in the medulla.  相似文献

A 24‐hour‐old Welsh‐Thoroughbred foal presented for routine surgical repair of an abdominal and body wall hernia. The colt became acutely colicky and emergency celiotomy was performed revealing abnormal anatomical configuration of the gastrointestinal tract (GIT). Incomplete gastrointestinal rotation is a rare congenital anomaly only described in man. This report describes the clinical and diagnostic findings, surgical and medical treatment, and outcome of a neonatal foal with nonrotation of the GIT.  相似文献

Possible correlations were investigated between the clinical and urodynamic findings in 77 neutered female dogs with urinary incontinence refractory to standard medical management. The clinical data used in the analysis were age, bodyweight, breed, the characteristics of the dribbling of urine and whether it developed before or after neutering, the type of response to previous treatments, signs of polyuria/polydipsia, and the results of urinalysis and culture. In 50 of the dogs, a standardised contrast radiographic examination was made to locate the neck of the bladder and to assess the morphology of the bladder. These findings were analysed for their correlation with the results of multichannel urethral pressure profilometry and diuresis cysto-urethrometry. Logistic regression analysis revealed that neither the clinical nor the radiographic findings could predict any of the abnormal urodynamic findings. Functional abnormalities of the bladder, such as a low compliance, detrusor instability, or a low bladder capacity may therefore be particularly difficult to detect in dogs with urinary incontinence.  相似文献

A 4.5-month-old Standard-bred filly was referred for evaluation of pigmenturia. Initially, the pigmenturia had resolved with the administration of antibiotics, only to recur after their withdrawal. A dark red urine sample contained numerous RBC, WBC, and gram-negative rods (Escherichia coli). Ultrasonography revealed the right kidney to be large, with multiple cystic structures and a dilated renal pelvis and calices. Cystoscopy revealed a large blood clot within the bladder and urine coming from the left ureteral opening. Urine was not observed coming from the right ureter. It was suspected that the primary infection within the urinary tract was coming from the right kidney, with secondary ureteral obstruction and cystitis. Trimethoprim-sulfamethoxazole treatment was initiated. However, acute depression and abdominal pain developed several days later, and the foal died before assistance could be provided. Necropsy revealed a large abscess that had eroded into the right ureter and aorta and had ruptured, resulting in acute blood loss and death. The location and extensive nature of the lesion would have precluded surgical intervention.  相似文献
