OBJECTIVE: To determine whether a laryngeal mask airway (LMA) provides a better airway than a facemask in spontaneously breathing anesthetized rabbits, and to test if it can be used for mechanically controlled ventilation. STUDY DESIGN: Randomized prospective experimental trial. ANIMALS: Sixteen young, healthy, specific pathogen-free Giant Flemish cross Chinchilla rabbits (10 females and 6 males) weighing 4.1 +/- 0.8 kg. METHODS: Rabbits were assigned randomly to one of three treatment groups: facemask with spontaneous ventilation (FM-SV; n = 5), LMA with spontaneous ventilation (LMA-SV; n = 5), and LMA with controlled ventilation (LMA-CV; n = 6). In dorsal recumbency, and at 2.3% end-tidal isoflurane concentration, Fé isoflurane, Fi isoflurane, partial pressure of expired isoflurane (PECO(2)), partial pressure of inspired carbon dioxide (PiCO(2)), heart rate, respiratory rate, minute volume, arterial oxygen tensions (PaO(2)), arterial carbon dioxide tensions (PaCO(2)), arterial pH (pH(a)), arterial standard base excess (SBE(a)) values were measured for 120 minutes. Results Two individuals in the FM-SV group had PaCO(2) > 100 mm Hg. One rabbit in the FM-SV had PaO(2) < 80 mm Hg. All FM-SV rabbits showed signs of airway obstruction, and two were withdrawn from the study at 45 and 90 minutes, respectively, because cyanosis was observed. No signs of airway obstruction were observed in either LMA group. Four rabbits in the LMA-CV group developed gastric tympanism, one of which refluxed gastric contents after 110 minutes. There were no differences between FM-SV and LMA-SV in any variable tested. PaCO(2) and PECO(2) were decreased, while PaO(2) and minute volume were increased in the LMA-CV group compared to the LMA-SV group. CONCLUSIONS: An LMA provided a better airway than a facemask during spontaneous breathing in rabbits, as the use of a facemask was associated with hypercapnia and low partial pressures of oxygen. Although an LMA can be used for intermittent positive pressure ventilation (IPPV), gastric tympanism may develop, especially at a peak inspiratory pressure of 14 cm H(2)O. CLINICAL RELEVANCE: The LMA can be used in rabbits but further work is needed before it is applied routinely.  相似文献

OBJECTIVE: To investigate the use of the laryngeal mask airway (LMA) in dogs. STUDY DESIGN: Prospective experimental study. ANIMALS: Eight healthy adult mixed breed dogs weighing from 15 to 20 kg. METHODS: The dogs were anesthetized with intravenous pentobarbital. An LMA was introduced after the induction of anesthesia and 1 L/min O2 plus 1 L/min air was delivered using a circle anesthetic system. Respiratory rate, tidal volume, arterial O2 saturation (pulse oximetry), end tidal CO2, inspired fraction of O2, pulse rate, and mean arterial blood pressure were measured after the insertion of the LMA and 30, 60, 90, and 120 minutes afterwards. RESULTS: There were no changes in respiratory rate, tidal volume, arterial O2 saturation, and pulse rate during anesthesia. End tidal CO2 decreased significantly by the end of anesthesia and ventilation appeared satisfactory. CONCLUSIONS: An LMA appeared to be an alternative option to maintain the patency of the airway in dogs. CLINICAL RELEVANCE: This device may allow safe maintenance of an airway in dogs when intubation is difficult or when it interferes with the procedure (eg, cervical myelography).  相似文献

The purpose of this study was to find out if an LMA (#1 LMA‐Classic) would provide a better airway than a face mask in spontaneously breathing anesthetized rabbits, and to test if it could be used for mechanically controlled ventilation. Sixteen rabbits (4.1 ± 0.8 kg, mean ± SD) were assigned randomly to three treatment groups; face mask with spontaneous ventilation (FM‐SV; n = 5), LMA with spontaneous ventilation (LMA‐SV; n = 5), and LMA with controlled ventilation (LMA‐CV; n = 6). Rabbits were anesthetized in dorsal recumbency using a circle circuit at constant ET isoflurane (2.3%, Datex airway gas monitor) and constant rectal temperature (38.85 °C) for 2 hours. PaCO2, PaO2, minute volume, tidal volume (Wright's respirometer), and Pe CO2 were measured at 15 minute intervals. Two individuals in the FM‐SV group had PaCO2 >100 mm Hg (>13.3 kPa). One rabbit in the FM‐SV had PaO2 <80 mm Hg (<10.7 kPa). All FM‐SV rabbits showed signs of airway obstruction and two were withdrawn from the study at 45 and 90 minutes, respectively, because of cyanosis. Tidal volume could not be measured in the FM‐SV group. No signs of airway obstructions were observed in either of the LMA groups. Four rabbits in the LMA‐CV group developed gastric tympany, and one of these refluxed after 110 minutes. The significance of differences between the two spontaneously breathing groups and between the two LMA groups were measured using Wilcoxon's rank sum test (with significance assumed at p < 0.05). There were no statistical differences between FM‐SV and LMA‐SV in any variable tested. PaCO2 and Pe ′CO2 were less in the LMA‐CV group than in the LMA‐SV group, while PaO2, tidal volume, and minute volume were all more. We conclude that biologically, the LMA provides a better airway than the face mask during spontaneous breathing and that it can be used for IPPV, but that gastric tympany is likely to occur during IPPV.  相似文献

OBJECTIVE: To compare the cardiorespiratory effects and incidence of gastroesophageal reflux with the use of a laryngeal mask airway (LMA) or endotracheal tube (ET) in anesthetized cats during spontaneous (SV) or controlled ventilation (CV). STUDY DESIGN: Prospective randomized experimental trial. ANIMALS: Thirty-two adult crossbred cats, weighing 2.7 +/- 0.4 kg. METHODS: The cats were sedated with intramuscular (IM) methotrimeprazine (0.5 mg kg(-1)) and buprenorphine (0.005 mg kg(-1)), followed 30 minutes later by induction of anesthesia with intravenous (IV) thiopental (12.5-20 mg kg(-1)). An ET was used in 16 cats and an LMA in the remaining 16 animals. Anesthesia was maintained with 0.5 minimum alveolar concentration (0.6%) of halothane in oxygen using a Mapleson D breathing system. Cats in both groups were further divided into two equal groups (n = 8), undergoing either SV or CV. Neuromuscular blockade with pancuronium (0.06 mg kg(-1)) was used to facilitate CV. Heart and respiratory rates, direct arterial blood pressure, capnometry (PE'CO2) and arterial blood gases were measured. Gastric reflux and possible aspiration was investigated by intragastric administration of 5 mL of radiographic contrast immediately after induction of anesthesia. Cervical and thoracic radiographs were taken at the end of anesthesia. Data were analyzed using anova followed by Student-Newman-Keuls, Kruskal-Wallis or Friedman test where appropriate. RESULTS: Values for PaCO2 and PE'CO2 were higher in spontaneously breathing cats with the LMA when compared with other groups. Values of PaO2 and hemoglobin oxygen saturation did not differ between groups. Gastroesophageal reflux occurred in four of eight and two of eight cats undergoing CV with ET or LMA, respectively. There was no tracheal or pulmonary aspiration in any cases. CONCLUSIONS AND CLINICAL RELEVANCE: The use of an LMA may be used as an alternative to endotracheal intubation in anesthetized cats. Although aspiration was not observed, gastric reflux may occur in mechanically ventilated animals.  相似文献

OBJECTIVE: To compare the doses of propofol required for insertion of the laryngeal mask airway (LMA) with those for endotracheal intubation in sedated dogs. STUDY DESIGN: Randomized prospective clinical study. Animals Sixty healthy dogs aged 0.33-8.5 (3.0 +/- 2.3, mean +/- SD) years, weighing 2.2-59.0 (23.4 +/- 13.6, mean +/- SD) kg, presented for elective surgery requiring inhalation anaesthesia. METHODS: Animals were randomly assigned to receive either a LMA or an endotracheal tube. Pre-anaesthetic medication was intravenous (IV) glycopyrrolate (0.01 mg kg(-1)) medetomidine (10 microg kg(-1)) and butorphanol (0.2 mg kg(-1)). Repeated IV propofol injections (1 mg kg(-1) in 30 seconds) were given until LMA insertion or endotracheal intubation was achieved, when the presence or absence of laryngospasm, the respiratory rate (fr) and the total dose of propofol used were recorded. RESULTS: The total propofol dose (mean +/- SD) required for LMA insertion (0.53 +/- 0.51 mg kg(-1)) was significantly lower than for endotracheal intubation (1.43 +/- 0.57 mg kg(-1)). The LMA could be inserted without propofol in 47% of dogs; the remainder needed a single 1 mg kg(-1) bolus (n = 30). Endotracheal intubation was possible without propofol in 3.3% of the dogs, 47% needed one bolus and 50% required two injections (n = 30). The f(r) (mean +/- SD) was 18 +/- 6 and 15 +/- 7 minute(-1) after LMA insertion and intubation, respectively. CONCLUSION AND CLINICAL RELEVANCE: Laryngeal mask airway insertion requires less propofol than endotracheal intubation in sedated dogs therefore propofol-induced cardiorespiratory depression is likely to be less severe. The LMA is well tolerated and offers a less invasive means of securing the upper airway.  相似文献

OBJECTIVE: To determine the effect of desensitization of the laryngeal mucosal mechanoreceptors on upper airway mechanics in exercising horses. ANIMALS: 6 Standardbreds. PROCEDURE: In study 1, videoendoscopic examinations were performed while horses ran on a treadmill with and without topical anesthesia of the laryngeal mucosa. In study 2, peak tracheal and nasopharyngeal pressures and airflows were obtained from horses during incremental treadmill exercise tests, with and without topical anesthesia of the laryngeal mucosa. A nasal occlusion test was performed on each horse while standing during an endoscopic examination for both trials. RESULTS: In study 1, horses had nasopharyngeal collapse while running on the treadmill when the laryngeal mucosa was anesthetized. In study 2, inspiratory upper airway and nasopharyngeal impedance were significantly higher, and peak tracheal inspiratory pressure, respiratory frequency, and minute ventilation were significantly lower in horses when the laryngeal mucosa was anesthetized, compared with values obtained when horses exercised without topical anesthesia. Peak inspiratory and expiratory airflows were lower in horses when the laryngeal mucosa was anesthetized, although differences did not quite reach significance (P = 0.06 and 0.09, respectively). During a nasal occlusion test, horses had episodes of nasopharyngeal collapse and dorsal displacement of the soft palate when the laryngeal mucosa was anesthetized. Upper airway function was normal in these horses without laryngeal mucosal anesthesia. CONCLUSIONS AND CLINICAL RELEVANCE: Receptors within the laryngeal mucosa may be important in maintaining upper airway patency in exercising horses.  相似文献

The efficacy of a nerve muscle pedicle (NMP) graft in restoring upper airway function was evaluated in exercising horses with induced left laryngeal hemiplegia. The NMP graft was created from the first cervical nerve and the omohyoideus muscle and transplanted into the left cricoarytenoideus dorsalis muscle. Seven adult Standardbreds were trained to exercise on a treadmill inclined at 6.38 degrees. With the horses at rest and exercising at 4.2 and 7.0 m/s, the following variables were recorded: peak inspiratory and expiratory transupper airway pressures (defined as the pressure difference between a lateral tracheal catheter and a mask catheter), peak inspiratory and expiratory air flow, inspiratory and expiratory impedance, tidal volume, minute ventilation, heart rate, and respiratory frequency. Measurements were made before left recurrent laryngeal neurectomy (LRLN), 28 days after LRLN, and 12, 24, and 52 weeks after the NMP graft (n = 5) or sham operation (n = 2). Before LRLN, exercise increased inspiratory and expiratory air flow and transupper airway pressure, whereas the impedance was unchanged. After LRLN, transupper airway inspiratory pressure and impedance were significantly greater and inspiratory air flow was significantly less than baseline values at 7.0 m/s. The sham operation did not improve airway function. Twelve weeks after insertion of the NMP graft, inspiratory impedance and inspiratory air flow were significantly different (improved) from LRLN values. OBJECTIVES: To compare isoflurane (ISO) and sevoflurane (SEVO) short-term anesthesia in piglets during castration. STUDY DESIGN: Prospective, randomized study. ANIMALS: A total of 114 male piglets aged 6-10 days, body weight 1.3-5.0 kg. METHODS: Piglets were randomly selected from multiple litters and randomly assigned to being anesthetized with ISO or SEVO prior to castration. To calculate appropriate doses for induction and maintenance of anesthesia, a square root of time model was used, with calculations based on metabolic size and attainment of 1.3x minimum alveolar concentration. The equipotent target alveolar concentration of ISO was 1.82% and for SEVO 4.03%. After doses were calculated, a table listing piglet weights and agent requirements was produced. Anesthetics were delivered via liquid anesthetic injection into a previously developed rebreathing inhaler that was filled with oxygen prior to use. Piglets were anesthetized, castrated and allowed to recover prior to return to the sow. Times for induction, recovery and total time to standing were recorded, and end-tidal carbon dioxide (Pe'CO2) tensions were measured by capnography immediately after mask removal. Each response variable was analyzed in sas using the Proc Mixed procedure, with piglet weight and days of age as covariates. Castration problems and mortality were assessed relative to unanesthetized littermates. RESULTS: There were no statistically significant differences in age, weight or total anesthetic time between the anesthetics. Induction time was shorter, recovery time longer, and Pe'CO2 lower with ISO. No morbidity or mortality was associated with either group. CONCLUSION AND CLINICAL RELEVANCE: Isoflurane and SEVO, delivered in a novel inhaler, provided economical, safe, rapid anesthetic induction and maintenance. Optimal conditions were provided for castration and recoveries were brief and smooth. Statistically significant differences in times would be of minor clinical importance. The cost of anesthesia was much less with ISO than with SEVO.  相似文献

OBJECTIVE: To evaluate the effects of the alpha2-adrenoceptor agonist medetomidine on respiratory rate (RR), tidal volume (V(T)), minute volume (V(M)), and central respiratory neuromuscular drive as determined by inspiratory occlusion pressure (IOP) during increasing fractional inspired concentrations of carbon dioxide (FiCO2) in conscious dogs. ANIMALS: 6 healthy dogs (3 males and 3 females). PROCEDURE: Dogs were administered 0, 5, or 10 microg of medetomidine/kg i.v. We measured RR, V(T), V(M), and IOP for the first 0.1 second of airway occlusion (IOP0.1) during FiCO2 values of 0%, 2.5%, 5.0%, and 75% at 15 minutes before and 5, 30, and 60 minutes after administration of medetomidine. RESULTS: Increases in FiCO2 significantly increased RR, V(T), and V(M). The i.v. administration of 5 and 10 microg of medetomidine/kg significantly decreased RR and V(M) at 5, 30, and 60 minutes for FiCO2 values of 2.5% and 5.0% and at 30 and 60 minutes for an FiCO2 value of 75%. The IOP0.1 was decreased after 30 minutes only for an FiCO2 value of 7.5% in dogs administered 5 and 10 microg of medetomidine/kg. The IOP0.1 was decreased at 60 minutes after administration of 10 microg of medetomidine/kg for an FiCO2 value of 7.5%. CONCLUSIONS AND CLINICAL RELEVANCE: The i.v. administration of medetomidine decreases RR, V(M), and central respiratory drive in conscious dogs. Medetomidine should be used cautiously and with careful monitoring in dogs with CNS depression or respiratory compromise.  相似文献

OBJECTIVE: Evaluation of the LMA-ProSeal for positive pressure ventilation (PPV) in the pig. STUDY DESIGN: Prospective observational study. ANIMALS: Twelve German country pigs, weighing 25-62 kg. METHOD: Lungs of pigs were mechanically ventilated under general anaesthesia using the LMA-ProSeal. The ease of insertion, number of attempts and total time until placement of the LMA-ProSeal and gastric tube were recorded. Bronchoscopy was performed to determine the position of the LMA-ProSeal and to detect signs of aspiration. Ventilation variables and the leak airway pressure (P(leak)) were measured. An arterial blood gas sample was taken to determine the adequacy of ventilation. RESULTS: The airway was secured in all pigs within 39 +/- 19 seconds (27-51). Different sizes of LMA-ProSeal were used; up to 30 kg: size 3, up to 43 kg: size 4; and above 43 kg: size 5. In all but one animal the P-LMA and gastric tube were inserted at the first attempt. In nine animals gastric fluid was drained through the gastric tube. There was no evidence of aspiration in any animal. The mean [+/-SD (95%CI)]P(leak) was 28.8 +/- 7.5 cm H(2)O (24.06-33.60) and normal ventilation was achieved in all animals. CONCLUSIONS: The results of this study indicate that the airway of pigs weighing 25-62 kg can be secured safely and reliably with the sizes 3, 4 and 5 LMA-ProSeal. CLINICAL RELEVANCE: Endotracheal intubation in pigs can be difficult so there is a risk of hypoxemia in the apnoeic animal. With the LMA-ProSeal the airway can be secured rapidly, safely and reliably. Use of the Standard-LMA under PPV can be associated with gas leakage into the stomach and the subsequent risk of gastric distension and regurgitation. Both the ability to drain the stomach and the high P(leak) of the LMA-ProSeal could contribute to improved protection against aspiration under PPV.  相似文献

OBJECTIVE: To evaluate the effect of medetomidine on minimum alveolar concentration (MAC), respiratory rate, tidal volume, minute volume (V(M)), and maximum inspiratory occlusion pressure (IOCP(max)) in halothane- and isoflurane-anesthetized dogs. ANIMALS: 6 healthy adult dogs (3 males and 3 females). PROCEDURE: The MAC of both inhalants was determined before and 5, 30, and 60 minutes after administration of medetomidine (5 microg/kg, IV). Dogs were subsequently anesthetized by administration of halothane or isoflurane and administered saline (0.9% NaCl) solution IV or medetomidine (5 microg/kg, IV). Respiratory variables and IOCP(max) were measured at specific MAC values 15 minutes before and 5, 30, and 60 minutes after IV administration of medetomidine while dogs breathed 0% and 10% fractional inspired carbon dioxide (FICO2). Slopes of the lines for VM/FICO2 and IOCP(max)/FICO2 were then calculated. RESULTS: Administration of medetomidine decreased MAC of both inhalants. Slope of V(M)/FICO2 increased in dogs anesthetized with halothane after administration of medetomidine, compared with corresponding values in dogs anesthetized with isoflurane. Administration of medetomidine with a simultaneous decrease in inhalant concentration significantly increased the slope for V(M)/FICO2, compared with values after administration of saline solution in dogs anesthetized with halothane but not isoflurane. Values for IOCP(max) did not differ significantly between groups. CONCLUSIONS AND CLINICAL RELEVANCE: Equipotent doses of halothane and isoflurane have differing effects on respiration that are most likely attributable to differences in drug effects on central respiratory centers. Relatively low doses of medetomidine decrease the MAC of halothane and isoflurane in dogs.  相似文献

OBJECTIVE: To evaluate the effect of positive end-expiratory pressure (PEEP) on oxygen delivery (DO(2)) with 1-lung ventilation during thoracoscopy in normal anesthetized dogs. STUDY DESIGN: Prospective, controlled experimental study. ANIMALS: Eight, adult, intact Walker Hound dogs weighing 25.6-29.2 kg. METHODS: Anesthetized dogs had 1-lung ventilation during an open-chest condition. A Swan-Ganz catheter was used to measure pulmonary hemodynamic variables and to obtain mixed venous blood samples for blood gas analysis. A dorsal pedal catheter was used for measurement of systemic arterial pressure and to obtain arterial blood samples for blood gas analysis. Oxygen delivery was calculated and used to assess the effect of 0, 2.5, and 5 cm H(2)O PEEP during 1-lung ventilation on cardiopulmonary function. Each dog was its own control at 0 cm H(2)O PEEP. A randomized block ANOVA for repeated measures was used to evaluate the effect of the treatment on hemodynamic and pulmonary variables. RESULTS: Use of 5 cm H(2)O PEEP induced a significant augmentation in the arterial partial pressure of oxygen (PaO(2)). Shunt fraction (Q(s)/Q(t)), physiologic dead space (V(D)/V(T)), and the alveolar-arterial oxygen difference (P(A-a)O(2)) decreased significantly after 5 cm H(2)O PEEP, compared with 1-lung ventilation without PEEP. Use of 2.5 cm H(2)O PEEP had no significant effect on cardiopulmonary variables. Use of PEEP had no significant effect on arterial oxygen saturation (SaO(2)), DO(2), and hemodynamic variables in normal dogs. CONCLUSIONS: PEEP had no effect on DO(2) in normal dogs undergoing open-chest 1-lung ventilation because it had no adverse effect on hemodynamic variables. CLINICAL RELEVANCE: PEEP in normal dogs during open-chest 1-lung ventilation for thoracoscopy is not detrimental to cardiac output and can be recommended in clinical patients.  相似文献

Objective  To compare the incidence of gastro-oesophageal reflux (GOR) during anaesthesia in the kitten when using a laryngeal mask airway (LMA) or an endotracheal tube (ET).
Study design  Prospective randomized cross-over experimental study.
Animals  Forty Domestic Short Hair laboratory cats, 19 females and 21 males, aged 12–15 weeks and weighing 0.57–1.73 kg (mean 1.13 ± SD 0.26).
Methods  Kittens were anaesthetized twice, once using the LMA and once the ET. Following induction of anaesthesia with isoflurane in an anaesthetic chamber and intubation of the trachea with the ET or placement of the LMA, a pH-electrode was introduced into the lower oesophagus. Monitoring of the oesophageal pH was performed for 45 minutes while anaesthesia was maintained with isoflurane. At the end of the experiment, gastric pH was measured. Kittens that had GOR during the experiment were treated with sucralfate, cisapride and ranitidine for 15 days.
Results  Oesophageal pH was 6.51 ± 0.76 and gastric pH was 1.54 ± 0.59. GOR was observed in nine kittens when the ET was used, and in 20 kittens when the LMA was used, the difference being significant ( p  =   0.013). The refluxate nearly always was acidic, being alkaline in only one kitten. Most of the GOR episodes occurred shortly after induction of anaesthesia and the oesophageal pH remained below 4.0 until the end of the experiment. No regurgitation was observed.
Conclusions and clinical relevance  The use of the LMA is associated with an increased incidence of GOR during anaesthesia in the kitten, which is not detected by observation. That this may have occurred should be considered if the kitten demonstrates signs of oesophagitis in the postoperative period.  相似文献

OBJECTIVE: To determine the effects of adenosine infusion on the minimum alveolar concentration (MAC) of isoflurane in dogs. STUDY DESIGN: Prospective, randomized crossover study. ANIMALS: Seven adult male and female Beagles weighing 10.9 (7.5, 13.6) kg [median (minimum, maximum)]. METHODS: Each dog was anesthetized with isoflurane in oxygen and randomly assigned to receive either an intravenous (IV) adenosine (0.3 mg kg(-1) minute(-1)) or saline (6 mL kg(-1) hour(-1) IV) infusion. After an interval of 7 days or more, each dog was re-anesthetized and treated with the alternative infusion. Using a tail-clamp technique, MAC was determined before (pre-infusion), during (infusion), and 2 hours after the infusions (post-infusion). RESULTS: The pre-infusion MAC of isoflurane was 1.25 (1.15, 1.35) [median (minimum, maximum)] vol.% for the saline treatment group and 1.25 (1.05, 1.45) vol.% for the adenosine treatment group, and did not differ significantly between the two treatments. The infusion MAC values were not significantly different (p = 0.16) and were 1.25 (0.95, 1.35) vol.% and 1.05 (1.00, 1.25) vol.%, respectively. The post-infusion MAC values differed significantly (p = 0.016); MAC was 1.15 (1.15, 1.35) vol.% and 1.05 (1.05, 1.25) vol.% for the saline and adenosine treatment groups, respectively. During infusion, mean arterial blood pressure decreased significantly (p = 0.008) during adenosine treatment compared with the saline 66 mmHg (52, 72) and 91 mmHg (68, 110), respectively. End-tidal CO2 (Pe'CO2), urine production, hematocrit, and plasma total solids did not differ significantly between the two treatments at any time (all p > 0.05). CONCLUSION: Although the MAC of isoflurane in dogs was not decreased significantly during infusion with adenosine (0.3 mg kg(-1) minute(-1)), it was significantly decreased post-infusion, but only by 0.1 vol.%, an amount not considered clinically important. ObjectiveTo compare airway management using the v-gel supraglottic airway device (v-gel SGAD) to that using an endotracheal tube (ETT), with respect to practicability, leakage of volatile anaesthetics and upper airway discomfort in cats.Study designProspective, randomized clinical trial.AnimalsTwenty European Shorthair cats (9 males, 11 females), weighing 3.3 ± 0.7 kg.MethodsCats were randomly allocated to one of two groups, in which the airway was managed by either the v-gel SGAD or a cuffed ETT, and anaesthetized for neutering procedures. The dose of propofol necessary to insert the ETT or v-gel SGAD; time from the first injection of propofol to the first clinically acceptable reading on the capnograph; leakage of isoflurane around the airway device; and upper airway discomfort scores during recovery and during the first 24 hours after anaesthesia were recorded. Continuous and discrete variables were analyzed with the Mann–Whitney U-test and the Pearson chi-squared test, respectively. Results were considered statistically significant if p < 0.05.ResultsTime from the first injection of propofol to the first clinically acceptable reading on the capnograph was significantly shorter in the v-gel group. The ETT group showed significantly more stridor during recovery. No other significant differences were found.Conclusions and clinical relevanceAirway management with the v-gel SGAD is a sound and practicable alternative to endotracheal intubation with an ETT. However, larger prospective trials will be needed to draw firm conclusions on the benefits and/or drawbacks of the use of v-gel SGAD for airway management in cats.  相似文献

OBJECTIVE: To evaluate the effect of the mode of mechanical ventilation (MV) on the dose of intravenous anesthetic during 3 hours of ketamine/xylazine anesthesia. STUDY DESIGN: Prospective laboratory study. ANIMALS: Sixty-one adult male New Zealand White rabbits. METHODS: Rabbits were anesthetized (ketamine/xylazine 35 + 5 mg kg(-1), IM), the trachea was intubated and randomized to four groups - (1) CMV-1 (n = 14), ventilated with traditional conventional volume-cycled MV [V(T) = 12 mL kg(-1), RR = 20, positive end-expiratory pressure (PEEP) = 0 cmH(2)O]; (2) CMV-2 (n = 13), ventilated with a modern lung-protective regimen of volume-cycled MV (V(T) = 6 mL kg(-1), RR = 40, PEEP = 5 cmH(2)O); (3) HFPV (n = 17) ventilated with high-frequency percussive ventilation [high-frequency oscillations (450 minute(-1)) superimposed on 40 minute(-1) low-frequency respiratory cycles, I:E ratio = 1:1], oscillatory continuous positive airway pressure (CPAP) of 7-10 cmH(2)O, and demand CPAP of 8-10 cmH(2)O. (4) A fourth group, spontaneously ventilating (SV, n = 17), was anesthetized, intubated, but not ventilated mechanically. FiO(2) in all groups was 0.5. Anesthesia was maintained at a surgical plane by IV administration of a ketamine/xylazine mixture (10 + 2 mg kg(-1), as necessary) for 3 hours after intubation. Total dose of xylazine/ketamine administered and the need for yohimbine to facilitate recovery were quantitated. RESULTS: The total dose of xylazine/ketamine was significantly higher in the HFPV and SV groups compared with CMV-1 (p < 0.01). Fewer animals required yohimbine to reverse anesthesia in the HFPV than CMV-1 group (p < 0.05). CONCLUSIONS: The HFPV mode of MV led to higher doses of ketamine/xylazine being used than the other modes of MV. CLINICAL RELEVANCE: In rabbits, anesthetic dose for the maintenance of anesthesia varied with the mode of MV used. Investigators should be aware of the possibility that changing the mode of ventilation may lead to an alteration in the amount of drug required to maintain anesthesia.  相似文献

OBJECTIVES: To determine temporal variations of pulmonary function in horses without respiratory tract disease (controls) and horses with chronic obstructive pulmonary disease (COPD) and determine whether reversibility of airway obstruction after environmental control can be predicted by response to atropine administration. ANIMALS: 7 COPD-affected and 5 control horses. PROCEDURES: Pulmonary function testing was performed monthly during 3 consecutive months, daily for 5 consecutive days, and at 6-hour intervals for 24 hours before and after administration of atropine (0.02 mg/kg of body weight, i.v.) and after 5 consecutive months at pasture. Respiratory rate, tidal volume (VT), minute ventilation (VE), maximal change in transpulmonary pressure (deltaP(L)), pulmonary resistance (R(L)), and pulmonary elastance (E(L)) were calculated. RESULTS: COPD-affected horses had a significantly higher expiratory to inspiratory time ratio (T(E)/T(I)) and deltaP(L), E(L), and R(L) than horses without respiratory tract diseases during all periods and higher V(E) during monthly and daily evaluations. Daily variation in VT and monthly and circadian variation in E(L) were significant in COPD-affected horses. In control horses, significant changes were apparent only in T(E)/T(I) during daily recordings. In COPD-affected horses, reduction in deltaP(L), R(L), and E(L) was significant after atropine administration and after maintenance on pasture. CONCLUSIONS AND CLINICAL RELEVANCE: Despite variations in measurements of respiratory mechanics in both groups of horses, values remained significantly different between groups over time. Despite individual variation, measurements were repeatable during short and long periods. Response to administration of atropine to COPD-affected horses underestimated improvement in respiratory tract function that resulted from maintenance on pasture.  相似文献

The purpose of this study was to investigate the cardiopulmonary influences of sevoflurane in oxygen at two anaesthetic concentrations (1.5 and 2 MAC) during spontaneous and controlled ventilation in dogs. After premedication with fentany-droperidol (5 microg/kg and 0.25 mg/kg intramuscularly) and induction with propofol (6 mg/kg intravenously) six dogs were anaesthetized for 3 h. Three types of ventilation were compared: spontaneous ventilation (SpV), intermittent positive pressure ventilation (IPPV), and positive end expiratory pressure ventilation (PEEP, 5 cm H2O). Heart rate, haemoglobin oxygen saturation, arterial blood pressures, right atrial and pulmonary arterial pressures, pulmonary capillary wedge pressure and cardiac output were measured. End tidal CO2%, inspiratory oxygen fraction, respiration rate and tidal volume were recorded using a multi-gas analyser and a respirometer. Acid-base and blood gas analyses were performed. Cardiac index, stroke volume, stroke index, systemic and pulmonary vascular resistance, left and right ventricular stroke work index were calculated. Increasing the MAC value during sevoflurane anaesthesia with spontaneous ventilation induced a marked cardiopulmonary depression; on the other hand, heart rate increased significantly, but the increases were not clinically relevant. The influences of artificial respiration on cardiopulmonary parameters during 1.5 MAC sevoflurane anaesthesia were minimal. In contrast, PEEP ventilation during 2 MAC concentration had more pronounced negative influences, especially on right cardiac parameters. In conclusion, at 1.5 MAC, a surgical anaesthesia level, sevoflurane can be used safely in healthy dogs during spontaneous and controlled ventilation (IPPV and PEEP of 5 cm H2O).  相似文献

This study investigated volumetric capnography (VC) in detecting airway responsiveness following airway challenge using carbachol in seven sedated dogs via face mask. Nebulised saline was administered, followed by increasing concentrations of nebulised carbachol until airflow limitation occurred (EP). Dead space (DS) variables and shape indices of the VC curve were calculated automatically after entering arterial carbon dioxide tension. Airway DS, airway DS to tidal volume (V(T)) ratio and the intercept of slope 2 of the VC curve decreased significantly at EP by 10%, 13% and 16%, respectively, minute ventilation, V(T) and alveolar DS increased significantly at EP by 49%, 22% and 200%, respectively. We conclude that VC and derived indices may be used to verify a reaction to airway challenge caused by carbachol in sedated dogs.  相似文献
