Endocardial fibroelastosis in a dog   总被引:1,自引:0,他引:1  
Endocardial fibroelastosis is an uncommon congenital heart disease in dogs that may be manifested by signs of left-sided congestive heart failure. A three-month-old, male, Fila Brasileiro dog developed signs of generalised heart failure. Physical examination revealed normal temperature, ascites, and pale and cyanotic mucous membranes. The pup died just after radiography which revealed ascites, hepatomegaly, severe cardiac enlargement and pulmonary oedema. At necropsy, serosanguineous fluid in the thorax and abdomen, pulmonary oedema, right ventricular dilatation, hypertrophy and dilatation of the left ventricle, and mitral valve incompetence were observed. The histopathological examination demonstrated that the thickening of the endocardium of the left atrium and left ventricle was due to the presence of elastic and collagen fibres, although there were no signs of an inflammatory process.  相似文献

Double outlet right ventricle, a rare congenital heart anomaly of either animals or man, was diagnosed in a three day old Brangus heifer. The clinical course was brief, consisting of terminal recumbency, dyspnea and cyanosis. Upon necropsy, the heart was enlarged as a result of marked dilatation of the right atrium and the right ventricle. The aorta arose from the right ventricle while the left ventricle ended blindly near the ventral cusp of the left atrioventricular valve. The wall of the left ventricle was thickened, reducing the ventricular lumen.  相似文献

M-mode, 2-dimensional, and contrast echocardiographic studies were used to detect tricuspid atresia in 2 foals. M-mode echocardiographic findings included a small right ventricle, large left ventricle, large mitral valve excursion, large left atrium (foal 2), dropout of the cranial aspect of the aortic root, and a thick band of echoes in the tricuspid valve region. These findings were confirmed by 2-dimensional echocardiography. In addition, a large right atrium, persistent foramen ovale, ventricular septal defect, and large mitral valve apparatus were imaged. One foal also had a thick right atrial wall. Contrast echocardiography confirmed the intracardiac flow of blood from right to left atrium and then to the left ventricle, followed by simultaneous opacification of the right ventricle and aorta. The use of these echocardiographic techniques enables accurate antemortem diagnosis and prognosis of tricuspid atresia in the foal.  相似文献

Right atrioventricular atresia with a ventricular septal defect, a rare congenital cardiac anomaly of either animals or man, was diagnosed in a one week old Arabian foal. The foal had been weak and cyanotic since birth, sometimes becoming dyspneic when stressed. Necropsy revealed a dilated and hypertrophied left ventricle with the right ventricle being small. The right atrioventricular valve was absent. The right and left ventricles communicated via a septal defect which was located immediately adjacent to the aortic valve.  相似文献

An 8-month-old entire Miniature Dachshund, weighing 4.2 kg, was presented for examination following delvelopment of a cough. Ventricular septal defect had been diagnosed tentatively in its infancy on the basis of a cardiac murmur detected by auscultation and echocardiography. Echocardiography using a B mode right parasternal long-axis view showed a defect at the atrioventricular junction and a thickened cusp of the aortic valve prolapsing into the defect. Colour-flow Doppler showed shunt blood flow across the defect at the level of the atrioventricular junction, from left to right. The sinus of Valsalva was dilated, with turbulent blood flow. Aortic regurgitation was also observed. Cardiac catheterisation studies confirmed the diagnosis of a supracristal ventricular septal defect with aortic regurgitation. Despite medication with digoxin, enalapril and aminophylin, started from the first admission, left ventricular internal dimensions gradually increased, and fractional shortening of the left ventricle gradually decreased. Surgery, with the aid of extracorporeal circulation, to close the ventricular septal defect, was performed 1 year after the initial examination. The aortic valve was left untreated. Postoperatively, the systolic murmur disappeared. Shunt flow from the left to the right ventricle was no longer observed on echocardiography, however there was still a small amount of aortic regurgitation during diastole visualised with colour-flow Doppler echocardiography. The prolapse of the cusp of the aortic valve on B-mode echocardiography was no longer observed and thickening of the cusp had not progressed. Left ventricular function measurement using M mode echocardiography showed a reduced left ventricular volume overload with reduced left ventricular internal dimensions and increased fractional shortening. The cough was relieved and no follow-up medication was scheduled. Early surgical closure of the ventricular septal defect improved the patient's condition and controlled prolapse and thickening of the aortic valve.  相似文献

A 3-year-old male pygmy goat was evaluated because of a grade III/V plateau pansystolic murmur that was auscultated over the tricuspid valve area and a grade II/V plateau pansystolic murmur that was auscultated over the left heart base. Echocardiography revealed a large right atrium and ventricle, an atrial septal defect, and a dysplastic tricuspid valve displaced apically into the right ventricle. Contrast echocardiography and color-flow Doppler echocardiography revealed flow from right to left through the atrial septal defect and severe tricuspid regurgitation. A diagnosis of tricuspid valve dysplasia and severe tricuspid regurgitation was made. This anomaly (Ebstein's anomaly) of the tricuspid valve should be considered in goats that have systolic, with or without diastolic, murmurs over the tricuspic valve. Two-dimensional echocardiography can confirm the diagnosis of the anomaly. The associated tricuspid regurgitation and interatrial right-to-left shunt flow can be detected and semiquantitated by use of contrast, continuous wave, and color-flow Doppler echocardiography.  相似文献

Echocardiography was used to identify a flail left atrioventricular valve cusp caused by ruptured chordae tendineae in each of 4 dogs; two-dimensional echocardiography was superior to M-mode echocardiography in identifying the flail cusps. The following findings on two-dimensional imaging were characteristic: the tip of the flail cusp extended beyond the line of left atrioventricular valve cusp closure and pointed toward the left atrium in systole; the tip was thrust into the left ventricle, and then toward the left ventricular outflow tract in diastole, forming a convex surface to the cusp, which faced toward the left ventricle. The flail motion of the left atrioventricular valve cusp was best observed in the right parasternal long axis or left apical four-chamber views, in a plane parallel to the long axis of the left ventricle and left atrium. Rupture of chordae tendineae leading to flail cusp was attributed to chronic valve degeneration (endocardiosis) in all 4 dogs. Echocardiographic or clinical diagnoses were confirmed by postmortem gross and microscopic studies in all dogs.  相似文献

A 3-day-old Hereford heifer calf presented for evaluation of lethargy and dyspnea, with persistent hypoxia despite supplemental oxygen therapy. A grade III/VI right apical systolic murmur was noted during cardiac auscultation. Echocardiography revealed a double-outlet right ventricle with an intact interventricular septum and concurrent hypoplastic left ventricle and tricuspid valve dysplasia. Post-mortem examination revealed additional congenital anomalies of ductus arteriosus, patent foramen ovale, and persistent left cranial vena cava. This report illustrates the use of echocardiographic images to diagnose a double-outlet right ventricle with an intact interventricular septum and a hypoplastic left ventricle in a calf.  相似文献

Porcine bioprosthetic valves cross-linked with glutaraldehyde and polyepoxy compound were newly developed for mitral valve replacement (MVR) in dogs. Five beagle dogs were performed a left thoracotomy and underwent MVR using the porcine bioprosthetic valves during cardiopulmonary bypass (CPB). A vein catheter inserted into right atrium and a vent catheter inserted into the right ventricle to drain. The hemodynamic conditions of CPB were excellent during surgery. The left atrial pressure was measured before and after MVR; there was no significant difference and it was normal. Thrombosis and the prosthetic valve regurgitation were not observed one week after MVR. Pressure half time (PHT) prolonged significantly (P<0.05) from 31.40 +/- 4.0 msec presurgery to 99.20 +/- 19.4 msec at seven days after MVR, although it indicated the normal range as the bioprosthetic valve. The symptom of the prosthetic valve failure was not observed. This study indicated that the MVR using porcine bioprosthetic valves under CPB might have been effective in dogs as a short-term evaluation.  相似文献

Gerbode type defects are rare left ventricular outflow tract–right atrial communications in people that may be congenital or acquired; they have been reported only once previously in dogs. Acquired forms in humans have been reported secondary to bacterial endocarditis, trauma, and valve replacement surgery, among other causes. We report a case of left ventricular outflow tract to right atrium and right ventricle communications (Gerbode type defect) in association with aortic and tricuspid valve bacterial endocarditis in a geriatric dog. The dog also developed third degree atrioventricular block and had underlying subaortic stenosis. The authors hypothesize that the Gerbode type defect in this case was acquired secondary to invasion and destruction of the membranous interventricular septum due to bacterial endocarditis.  相似文献

Congestive heart failure was diagnosed in a 27-year-old Indian ringneck parakeet with exercise-induced dyspnea. A grade IV/VI holosystolic murmur that radiated to the right sternal area was auscultated over the left side of the sternum. Radiography revealed progressive cardiomegaly, hepatomegaly, pulmonary edema, and accumulation of fluid within the coelomic cavity. Echocardiography revealed biatrial enlargement and enlargement of the right ventricle. Doppler recording revealed high velocity left and right atrioventricular valve regurgitation. Treatment with digoxin and furosemide alleviated clinical signs for approximately 10 months. Gross postmortem examination revealed cardiac enlargement and eccentric hypertrophy of both ventricles on cross-section. Pulmonary congestion and edema, hepatomegaly, hepatic congestion, and ascites were also evident. Histologic examination of the heart revealed myxomatous degeneration of the left atrioventricular valve, muscular hypertrophy of the right atrioventricular valve, and biventricular chronic myofiber degeneration and necrosis.  相似文献

A 7-week-old Wire Fox Terrier was admitted with pulmonary atresia with intact ventricular septum. The right ventricle and tricuspid valve were hypoplastic, and venous return to the right atrium reached the left side through an atrial septal defect. Oxygenation was via hyperplastic bronchial arteries. There was no evidence of the ductus arteriosus. Physical examination, plain and contrast radiography, and electrocardiography were performed. Clinical findings for this combination of defects were similar to those of more common defects (tetralogy of Fallot, patent ductus arteriosus.  相似文献

Patent ductus arteriosus (PDA) is a rare congenital cardiac defect in foals causing left to right shunting from the aorta to pulmonary artery. In extremely rare conditions, complications with pulmonary hypertension can result in right to left shunting (Eisenmenger's physiology); however, reversed or right to left shunting has not yet been precisely described in PDA-affected equids. This report describes a unique and unusual case of right to left PDA associated with acute respiratory distress syndrome in a 20-day-old male foal. A holosystolic murmur over the tricuspid valve area and adventitious pulmonary sounds respectively on cardiac and thoracic auscultations, caudodorsally interstitial and bronchointerstitial pulmonary opacities on thoracic radiography, right atrial and ventricular dilatations with paradoxical ventricular septal motion, increased ratio of pulmonary artery internal diameter to the aorta and tricuspid valve regurgitation on echocardiography were detected. Post-mortem and histopathological examinations revealed consolidated, dark red and diffusely enlarged lungs with granular appearance, marked enlargement of the right atrium and ventricle, pulmonary artery thickening and enlargement, patency of the ductus arteriosus, hyaline membrane formation, type II pneumocyte proliferation, hypertrophy and thickening of the pulmonary arteries medial layers and right ventricular myocardial cells distortion and hypertrophy. The findings suggest a right to left shunting through the persistent patency of the ductus arteriosus, pulmonary hypertension and cor pulmonale. We proposed pulmonary hypertension associated with bronchointerstitial pneumonia as a cause of this unusual case of PDA with reversed shunt direction.  相似文献

A six-year-seven-month-old female neutered Cavalier King Charles Spaniel was referred for the investigation of progressive dyspnea and hyphema in the right eye with secondary glaucoma. Previous medical history included a high-grade soft tissue spindle cell sarcoma removed from the cranial sternal region one year before.On presentation at the referral hospital, the dog was tachypneic and dyspneic. The heart rhythm was regular and there was a soft left-sided systolic murmur. Echocardiography identified the presence of a mass significantly occluding left heart inflow, with no other lesions identified. Thoracic radiographs documented a localized alveolar pattern within the left caudal lung lobe. The size of the heart and pulmonary vessels were within normal limits, indicating a non-cardiogenic alveolar pattern.Given the clinical presentation of dyspnea and high index of suspicion of intra-cardiac neoplasia, the dog was considered to have a grave prognosis and therefore euthanized. Post-mortem gross and histopathologic examination revealed the presence of a metastatic osteosarcoma tumor thrombus in the left atrium and pulmonary vein, metastatic osteosarcoma infiltrating the myocardium, lungs, the uveal tract of the right eye, and both adrenal glands. Whitney grade II myxomatous changes were noted on the mitral and tricuspid valve leaflets.This report describes an unusual intra-cardiac tumor thrombus in a dog presenting with dyspnea. Cavalier King Charles Spaniels presenting with dyspnea often raise suspicion for myxomatous mitral valve disease. However, as demonstrated in this case, other more unusual causes of dyspnea should also be considered in the absence of classic clinical findings.  相似文献

A five-month-old, intact female Domestic Shorthair cat presented to the Kansas State Veterinary Health Center for evaluation of a murmur and exercise intolerance. Physical exam revealed a grade V/VI right, parasternal, holosystolic murmur. On echocardiogram, there was an abnormality in the membranous interventricular septum and tricuspid valve, allowing blood to shunt from the left ventricle to the right atrium. This lesion is consistent with an infravalvular Gerbode defect. The Gerbode defect is a rare, left ventricle-to-right atrial shunt that can be congenital or acquired, and in cats, has only been reported once. At the time of submission, the patient is alive and is maintaining a good quality of life.  相似文献

After 2 years of medical management with a beta-adrenoreceptor blocking agent, a 30-month-old castrated male Golden Retriever with subaortic stenosis was treated surgically because of progression of its condition. In an attempt to achieve complete relief of the left ventricular outflow obstruction, a modified Konno procedure consisting of right ventriculotomy and septal myectomy from the infundibular portion of the right ventricle was performed; this combination of procedures allowed wide resection of the septal portion of the left ventricular outflow obstruction. Two years after surgery, the mass of the dog's left ventricle had decreased and the peak calculated pressure gradient across the aortic valve had decreased to 40 mm Hg, compared with a preoperative value of 240 mm Hg; at that evaluation, the dog had gained weight and was able to play normally. It is suggested that use of this modified approach to the outflow tract may have a positive effect on long-term survival time in dogs with subaortic stenosis.  相似文献

本研究采用心电图描记技术动态检测低温环境下肉鸡右心功能的变化,同时利用电镜技术观察心肌超微结构的病理变化。研究表明低温处理1周后心电图Ⅱ导联、AVF导联S波幅显著升高,主要呈rS波型;超微结构显示肌原纤维部分断裂、线粒体体积增大为特征;而右 心室病变较轻。低温处理3周、4周后左右心室肌微丝受损, 肌原纤维Z线模糊或消失,线粒体明显肿胀,严重者出现空泡样变,左心室的病理变化比右心室重。研究发现低温处理后肉鸡 右心功能减退,左右心室病理组织学变化有所不同,提示低温处理后肉鸡早期左心主要呈 代偿性变化,中期右心出现代偿功能变化,左心代偿功能增强,而后期左心室出现失代偿的 病变,左心室病变可能对肉鸡腹水症的形成有直接影响。  相似文献

Normal and paradoxical ventricular septal motions were studied in dogs, using M-mode and 2-dimensional echocardiography. Normal ventricular septal motion was evaluated, using 35 clinically healthy dogs (group I), and was compared with characteristics of septal motion measured in 12 dogs with right ventricular overload (group II) and 14 dogs with left-sided heart disease (group III). Normal ventricular septal motion consisted of a series of active and passive anterior (right cranial) and posterior (left caudal) movements that were subjectively evaluated and quantitated during segments of the cardiac cycle. Maximum excursion of the ventricular septum occurred during systole, was directed toward the left ventricle, and was related to body size. Determination of a normalized radius of septal curvature from analysis of 2-dimensional echocardiogram indicated that the septal arc formed part of a generally circular left ventricle in group I dogs. In contrast to these findings, group II dogs with right ventricular pressure and volume overloads exhibited reduced systolic septal excursion, diminished or flat septal motion, and paradoxical systolic septal motion. Normalized radius of septal curvature was significantly greater in group II dogs than in group I or group III dogs. Septal motion and radius of septal curvature in dogs comprising group III were not significantly different from normal during most phases of the cardiac cycle. Results of this study support the concept that septal motion is a reflection of overall left ventricular shape and that abnormalities in ventricular septal motion, namely flat or paradoxical septal motion, should cause the clinician to suspect right ventricular volume or pressure overload.  相似文献

The QRS amplitude and polarity were determined in 12-lead electrocardiograms recorded from 22 Boxers with ventricular arrhythmias. Eighty-one percent (18/22) of dogs displayed a positive QRS morphology in the caudoventral leads (II, III, and aVF) and 77% (17/22) of dogs displayed a positive QRS morphology in the left precordial leads (V2-V6). In leads I and V1, the polarity of the QRS complex was variable (positive or negative). To determine if these morphologic features were suggestive of ventricular complexes arising from the right or left ventricle, a comparison was made to the QRS complexes in a pace-mapping study performed in 7 healthy mixed-breed dogs. A total of 3 right and 4 left ventricular sites were paced. None of the left ventricular paced sites resulted in a QRS morphology similar to the most common spontaneous ventricular arrhythmia in the Boxers. In contrast, QRS morphology in each of the 3 right ventricular sites was similar to that observed in the Boxers (P < .033). Each of these produced positive deflections in the caudoventral and left precordial leads, but both positive and negative QRS complexes were observed in leads I and V1 only when the right ventricular septum was paced. This finding suggested that the right ventricular septum might be a site of origin for the ventricular rhythm observed in the Boxers because in the Boxers the polarity of leads I and V1 also varied. Pacing the right ventricular outflow tract always resulted in a negative QRS complex in lead 1, whereas pacing the right ventricular apex always resulted in a positive QRS complex in lead I and a negative QRS complex in V1. However, these locations cannot be excluded as possible sites of origin for the spontaneous ventricular arrhythmias in the Boxers because the arrhythmias could be originating from both of these locations. The spontaneous ventricular arrhythmia of the Boxer is most similar to that of paced ventricular rhythms arising from the right ventricle. More precise localization to a region of the right ventricle such as outflow tract, septal, or apical could not be made.  相似文献
