Hired farm workers provide the majority of the workforce for California's labor-intensive agricultural sector Agriculture is one of the most hazardous occupations, but there has been little research into the etiology of poor health outcomes that occur disproportionately in hired farm worker populations. MICASA is a cohort investigation of occupational and environmental health risks in hired farm worker households in Mendota, California, that employed a two-stage sampling process, including random selection of census blocks and door-to-door enumeration. The aim of this analysis was to evaluate the success of the sampling process and compare demographics of the enumerated population to other regional samples of Latino populations. In the enumeration, 1257 addresses were mapped and 729 hired farm worker households were enumerated. Findings showed no significant differences between the enumerated population and the resulting MICASA study sample; however, the MICASA population was more likely to be male, from Central America, work in agriculture, and have fewer years residency in the U.S. than California Health Interview Survey (CHIS) respondents. Additionally, 9.4% of the enumerated dwellings were back houses or unofficial dwellings and may have been missed by the U.S. Census 2000. Demographic comparisons between the enumerated population, census data, and CHIS data highlight the differences in these sampling methods and suggest possible demographic changes in hired farm workers in California. While difficulties in accessing hired farm workers often account for the lack of population-based research, the MICASA cohort provides an opportunity to examine occupational health patterns relevant to other farm worker populations.  相似文献

OBJECTIVE: To assess socio-economic indicators, nutritional status and living conditions of farm workers and their families, with the purpose to develop research and intervention programmes aimed at enhancing nutritional status and quality of life. DESIGN AND SETTING: Three farm schools in two districts of the North-West Province and farming communities were selected. Anthropometrical measurements, structured face-to-face questionnaires and focus group discussions were carried out in 2002 and 2003 by a multidisciplinary research team. RESULTS: Access to electricity, water and sanitation, as well as monthly food rations or subsidies, vary and depend on farm owners. The majority of adults have education below or up to grade four, farm schools provide only up to grade seven. Distance to farm schools and intra-household issues hamper children's attendance and performance at school. Household food security is compromised due to a lack of financial resources, infrastructure and also household resource allocation. This impacts negatively especially on children, with half of them being underweight, stunted or wasted. Employment is usually linked to men, while most women have access to casual jobs only. Insecurity of residence and the perceived disempowered position towards farm owners add to feelings of hopelessness and stress. CONCLUSIONS AND RECOMMENDATIONS: This study highlights destitute living conditions of farm worker families. Apart from structural and financial constraints, paternalistic structures of the past might also hamper development. Based on these findings, follow-up research projects and in-depth investigations into underlying social issues with regard to nutrition insecurity and livelihoods of farm workers were initiated.  相似文献

While childhood agricultural injury has long been recognized as an important public health issue, most research has focused on family farms and there have not been many interventions targeting hired youth. This study evaluated the impact of a high school English as a Second Language (ESL) curriculum, designed to provide teen agricultural workers with the knowledge and tools to protect their health and safety in the fields. Using a quasi-experimental design, the research consisted of two intervention groups and a comparison group, and included over 2,000 students from communities that lead California in agricultural production. The research findings revealed that the curriculum had significant impact in terms of increases in knowledge and attitudes, and nearly half of those interviewed after a summer of working in the fields reported implementing new behaviors to protect their health and safety. The curriculum also had extended effects in the broader community, as the majority of students reported sharing the new information with others. The study found that a school-based ESL curriculum is an effective intervention to reach and educate teen farm workers and that ESL classes can serve as a much-needed access point for young farm workers.  相似文献

A cluster of campylobacteriosis cases occurred at a pheasant farm in rural Wyoming during the summer of 2000. This study examined the potential causes of the outbreak. A cohort study of all farm workers was conducted to assess foodborne and occupational exposures at the facility. Eight of fifteen workers (53%) became ill, and four were stool-culture positive for Campylobacter jejuni. High attack rates were noted among workers who had direct contact with pheasant feces and first-time workers at the farm. This investigation suggests an association between campylobacteriosis and occupational animal exposure to pheasants. Enhanced educational efforts targeting occupations with direct animal contact are critical, particularly in rural communities.  相似文献

Discussions with groups of North Carolina farmers identified farm vehicle public road safety as their primary occupational health and safety concern. Findings of a mail survey of North Carolina growers participating in a North Carolina Department of Labor migrant housing inspection program indicated that over 97% of them felt less safe on North Carolina public roads now (1999) than five years prior (1995), and over 79% currently (1999) felt unsafe transporting farm vehicles on North Carolina public roads. Using both primary and secondary data, we explore the context of farm vehicle public road crashes, identify contributing individual and environmental risk factors, and estimate the public health cost. Recommendations and suggestions for future farm vehicle public road safety research and interventions are proposed.  相似文献

The Australian cotton industry has been a leader in the adoption of farm health and safety approaches in the Australian agricultural sector. In order to analyze factors associated with occupational health and safety performance, a review of available information and key stakeholder interviews were conducted A recently defined model for adoption of farm safety, emphasizing individual behavioral, environmental, community, and governmental factors, was used as the framework for the assessment. This hazard-based examination of changes describes improvements that have positively impacted health and safety. Improvements include: reduction in vehicle and on-farm traffic systems, safer irrigation systems, changes in cultivation technology, changes in pesticides and application technology, changes in harvest technology, adoption of gene technology, adoption of occupational health and safety (OHS) management systems, and government regulation. The introduction of genetically modified cotton has directly reduced potential exposure to pesticides, and it has indirectly impacted safety by, for example, reducing traffic flow on farms. This study demonstrates the considerable innovation and positive safety enhancements across the Australian cotton industry with respect to the interaction of management and production systems. These lessons should be used develop effective interventions in other agricultural industries.  相似文献

A trend in consolidating livestock and poultry operations into concentrated animal feeding operations (CAFOs) potentially increases farm worker exposure to the hazards associated with high animal density conditions. The two main contributors of documented injury (fatal and non-fatal) are related to accidents with machinery and animals. Tractor rollovers are the leading accident in the area of farming machinery issues; kicks, bites, and workers being pinned between animals and fixed objects are non-machinery issues typically caused by inadequate precautions taken in the vicinity of livestock. These types of accidents are well documented; however, recommended safety strategies continue to be studied to reduce the risks and numbers of injuries associated with both machines and animals. Unlike accidents involving machinery and animals, air emission exposure and potential health effects from CAFOs are not well documented. CAFOs have the potential to show higher gaseous and particulate matter emissions compared to smaller farms. Pollutants like hydrogen sulfide, ammonia, volatile organic compounds, particulate matter, and endotoxin are emitted on CAFOs and can potentially affect worker health. These specific air emissions, their sources, and some of their harmful capabilities have been identified, and regulations have been implemented to create improved work environments on CAFOs. Despite such precautions, farm workers continue to report respiratory health symptoms related to their work environment. Air pollutant exposure and its health effects on farm workers require focused research to arrive at improved safety strategies that include mitigation techniques and protective gear to minimize adverse effects of working in CAFOs.  相似文献

In order to increase farm safety awareness and teach children good safety habits, farm safety day camp programs have been initiated in many communities. For the most part, the effectiveness of the programs is unknown because few evaluations have been performed. The purpose of this project was to qualitatively assess the overall strengths and weaknesses of farm safety day camp programs and to generate recommendations for future programs. Interviews, a literature search, and observations were used to identify strengths and weaknesses of the farm safety day camp programs. Major strengths of the programs are that: 1) positive attention is given to agriculture and agricultural safety; 2) programs are hands-on, interactive, and fun; 3) children gain knowledge about farm safety; 4) programs bring a community together to work toward a common goal; and 5) children talk to parents about safety. Key weaknesses of farm safety day camp programs are that: 1) there is a lack of parental involvement in many of the programs; 2) it is hard to change patterns of behavior in one day; 3) programs are expensive and time consuming; 4) curricula may include content that is inaccurate or inappropriate for the age of participants; and 5) evaluation is difficult. When done well, farm safety day camp programs can teach children about safety and influence safety practices affecting children on the farm. Recommendations for future farm safety day camp programs are: 1) ensure that child-development principles are applied to all aspects of program activities and curricula; 2) provide session leaders with accurate and relevant content; 3) increase parental involvement; and 4) address safety issues throughout the year.  相似文献

Farm families have been identified traditionally with a strong family bond resulting from both living and working together. When a farming fatality occurs, surviving family members are left to deal with not only the tragedy of losing a loved one, but also the loss of a coworker. Although every family experiencing a loss will deal with bereavement issues, farm families are faced with additional challenges that differentiate them from other family situations. A qualitative research methodology was employed to understand the complex mix of challenges facing farm families after the death of a family member. Semi-structured interviews were conducted with farming widows in Kentucky to explore the stresses and challenges related to the farm business, family relationships, and the mental health of the individual members. Becoming the primary decision maker for the farm and household was a difficult role for the widows. Economic issues were an underlying consideration in many aspects of the experiences and changes they encountered. The need to make economic decisions almost immediately while continuing the necessary chores to maintain crops and livestock was very stressful and left little time for bereavement. Often the support from family, friends, and neighbors went beyond emotional comforting to providing help with farm chores and guidance on financial decisions. In developing resources for farm families in similar circumstances, it is important to understand how intertwined their lives are with their environment and the economics of the business.  相似文献

In 1999, project leaders from seven states (i.e., Iowa, Kansas, Minnesota, Nebraska, North Dakota, South Dakota, and Wisconsin) began to share ideas and resources for providing behavioral health assistance (i.e., mental health counseling and addictions services) to stressed farmers, ranchers, farm workers, and their families. The seven states are among those most impacted by the farm crisis of the 1980s and again by low commodity prices and disasters such as droughts and floods in the 1990s. Project leaders conferred in monthly telephone conference calls and by 2001 began meeting in semi-annual face-to-face meetings to formally agree on a mission, program components, and management structure. Administrative functions were transferred from the Wisconsin Office of Rural Health and Wisconsin Primary Healthcare Association, to AgriWellness, Inc., a regional nonprofit corporation founded to provide technical assistance, grant writing, training of service providers, and other administrative supports. The Sowing the Seeds of Hope program has become a model for the provision of behavioral health supports for the agricultural population, including development of farm stress telephone hotlines, provision of confidential and affordable outpatient mental health and substance abuse counseling, training of professional providers in agricultural behavioral health, training of indigenous farm and rural residents as outreach workers who can respond to disasters of all types, and weekend educational retreats for farm residents. The program has achieved economy of scale by sharing expertise across state boundaries and the formation of a regional administrative structure. Yet, many challenges exist, the greatest of which is obtaining ongoing permanent support for the increasing numbers of uninsured and underinsured people involved in agriculture.  相似文献

In order to estimate the one-year cumulative incidence of work-related injuries in teenage agricultural workers and to characterize the patterns and types of agricultural work performed by teenagers, we performed a community-based random-digit dialing telephone survey. We used a sampling frame of farm and non-farm households to access information on teenage agricultural workers. An agricultural community in eastern Washington State was selected due to the high number of teenage youth and its agricultural economy. Teenage agricultural workers who worked for an agricultural business owned by one of their family members were more likely to work a greater number of seasons, work fewer hours per week, perform tasks involving driving, animal care, and mechanic work, and were less likely to be seasonal workers or work in the harvest than teenagers who did not work for a family member. A similar pattern of work differences was seen when comparing non-Hispanic to Hispanic agricultural workers. Teenagers working for a family member in agriculture had a higher injury rate than teenagers working for an agricultural business not owned by a family member. The injury rate among Hispanic teenage agricultural workers was also higher than that of non-Hispanics.  相似文献

This article examines various environmental alternatives within the context of forest resource-dependent communities of the State of Oaxaca, Mexico. Two main objectives were to describe particular rural development routes among different Mexican communities, and to explain why certain environmental options for rural development are selected over others. While many communities choose either sustainable or illegal logging options in Oaxaca, some may decide against logging of any kind. Four principal forest-based community categories, according to a government forest classification scheme, are discussed in the context of environmental alternatives for this article. Based on this typology, two "integrated forest management" communities in the Sierra Norte of Oaxaca are described and compared. One community's decision not to log within a shared land arrangement has caused significant tensions in the region. Key findings illustrate the extent to which rural communities can make appropriate environmental decisions and examines their effects on environmental and social sustainability. Increased rural involvement in environmental decision-making is called for, since rural residents are those most likely to appreciate nearby natural resources as a source of sustainable livelihoods. It is expected that this research may be applicable to rural areas of other countries.  相似文献

Conservation justice, a concept analogous to environmental justice, suggests that local communities are entitled to receive fair treatment and meaningful involvement in the development and implementation of conservation policy. In this study, of an urban conservation project in Cape Town, South Africa, we contribute to the ongoing conversation about the importance of community-based conservation approaches. Conservationists must work to plan and implement projects in ways that are not only acceptable to stakeholders, but inspire local community involvement in achieving conservation goals. Given its location in the impoverished Cape Flats region of metropolitan Cape Town and its unique ecological and conservation value, the Macassar Dunes Conservation Area warrants a conservation justice approach. We conducted semi-structured interviews with members of interested and affected communities, then analyzed stakeholder perspectives on biodiversity protection, fencing, and informal housing. The results suggest that despite disparity among groups in needs and perspectives, conservation projects have potential to deliver tangible benefits to all stakeholder groups. A belief in conservation is universal across stakeholder lines, but contrasting needs and perspectives of the studied groups lead to conflicting views on important issues of implementation. An understanding of different stakeholder groups’ specific needs and interests is therefore essential for successful implementation of sustainable urban conservation projects.  相似文献

Little is known about the magnitude of occupational health problems among migrant farm workers. A community-based cross-sectional survey was conducted in two migrant farm worker communities: Homestead, Florida, and Kankakee, Illinois. Camp Health Aides (CHAs) interviewed 425 workers about job tasks, personal protective equipment (PPE), field sanitation, work exposures, and selected health conditions. Limited provision of personal protective equipment was reported among those reporting early re-entry tasks: 35% in Kankakee and 42% in Homestead were provided gloves, and 22% in Homestead and 0% in Kankakee were provided protective clothing. About two-thirds were provided toilet facilities and water for hand-washing. Workers reported high prevalences of health conditions consistent with exposure to ergonomic hazards and pesticides. The prevalence of back pain in the past 12 months was 39% in Homestead and 24% in Kankakee. Among Homestead participants, 35% experienced eye symptoms, while 31% reported skin symptoms. These symptoms were less prevalent among Kankakee participants (16% for both eye and skin symptoms). Specific areas of concern included back pain associated with heavy lifting and ladder work; eye and skin irritation associated with fertilizer application tasks and with working in fields during or after spraying of chemicals, especially early re-entry of sprayed fields; and skin irritation associated with a lack of access to hand-washing facilities. In both Kankakee and Homestead, better adherence to safety standards is needed, as well as greater efforts to implement solutions that are available to help prevent work-related musculoskeletal problems.  相似文献

The California Agricultural Workers Health Survey was a statewide cross-sectional household survey of 970 hired farm laborers. Randomly selected participants residing in randomly selected dwellings were recruited in seven communities representing all of the state's agricultural regions. Participants were interviewed in their preferred language by professional staff. The response rate was 83%. The comprehensive interview included self-reported health conditions, doctor-reported health conditions, work history, workplace health conditions, field sanitation, and work-related injuries. A farm workplace injury during the twelve-month period prior to the interview was reported by 6% of male workers (95% CI: 4% - 8%) and 2% of female workers (95% CI: 1% - 3%). Significant numbers of both male (41%) and female (40%) workers reported persistent pain (every day for more than one week) in the back, neck, knees, shoulders, hands, feet, or multiple body parts. The number of body parts in which female workers reported persistent pain correlated with increased years of U.S. hired farm work (Spearman r = 0.24, p < 0.01). Direct contact with pesticides from being sprayed or drifted upon among both male and female workers was associated with multiple workplace health conditions such as irritated, itchy, or water eyes (male: OR 2.9, 95% CI: 1.6 - 5.0; female: OR 13.8, 95% CI: 4.3 - 44.7). Persistent stomach aches among male and female participants was associated with being required to taste unwashed grapes for sweetness while picking (male: OR 4.6, 95% CI: 2.1 - 9.9; female: OR 5.8, 95% CI: 2.6 - 12.6).  相似文献

The Amish and other Old Order Anabaptists have been inseparably linked with agriculture since coming to America. However few efforts have been identified which analyze the issues involved with Old Order Anabaptist farm injuries or present best practices for addressing these problems. As part of an effort to develop culturally appropriate and effective injury prevention strategies for use within the Old Order Anabaptist community, this article identifies important cultural issues that should be considered in understanding and attempting to reduce farm injures in this population, summarizes statistics concerning farm-related fatalities among Old Order Anabaptists, and discusses methods which have been effectively used to address farm injuries within these communities. In the statistical analysis, 92 cases were identified that were classified as being work-related or occurring in a farm work-related setting during the period 1980 to 2000. Approximately 64% of the identified cases were children 15 years and under; and approximately 77% were male. Run-over incidents were the most frequent primary cause of the fatal injuries, while animal-related behavior was the most frequent secondary cause, reflecting agricultural practices that remain highly dependent upon the use of horses and mules. The authors believe evidence suggests that certain Old Order Anabaptist choices concerning farm safety issues may be directly related to their socio-religious beliefs, and that effective intervention strategies must also be sensitive to socio-religious beliefs.  相似文献

OBJECTIVE: Previous research on the health transition and nutrition security in black South African households revealed the need for further research among farm workers who belong to the poorest population categories. This article gives an overview of the link between nutrition security, livelihoods and HIV/AIDS in South Africa, drawing conclusions for research among farm worker households. DESIGN AND SETTING: A literature review, observations on farms and interviews with farmers and key informants were carried out in 2003 and 2004. Peer examination was done with South African and German researchers from the natural and social sciences. RESULTS: Farm workers face poverty and nutrition insecurity and continue to be a tragically underserved population group, also in terms of research. There is furthermore a lack of in-depth research on underlying causes for nutrition security in South Africa and on the link with livelihoods, poverty and HIV/AIDS. Micro-social qualitative research from the household and gender perspective is required, as valid data on households, their internal dynamics and therefore the reflection of social realities are missing. A multidisciplinary research approach based on a new conceptual framework was developed to address the situation of farm worker households. CONCLUSION: The outlined research contributes to existing programmes on farms in the North West Province, with the findings being valid also for other parts of southern Africa facing poverty, nutrition insecurity and HIV/AIDS.  相似文献

Surveillance of agricultural work-related illness and non-fatal injuries is limited and considered to be a major gap in prevention and policy-making capacity. This pilot study evaluated the utility of the Marshfield Epidemiologic Study Area (MESA) and its farm residency cohort to identify and monitor potential priorities for prevention and control. MESA is a defined geographic region that serves as a resource for conducting population-based health research. Nearly all of the residents receive their health care from the Marshfield Clinic and affiliated hospitals, providing archived electronic medical information to characterize past and present diagnoses. Based on scientific literature and national research priorities, five broad health issues (injuries, back problems, hearing loss, respiratory conditions, and dermatologic conditions) were selected. To estimate age-specific and age-adjusted incidence and prevalence, we followed a fixed cohort of 1995 MESA residents through 2002. Standardized incidence ratios and standardized prevalence ratios were also estimated to quantify the potential impact of farm residency. Linking MESA, its farm residency cohort, and the clinic's data archives allowed successful identification of significant associations with agricultural exposure for a few conditions and subconditions related to consensus priorities in agricultural health, particularly among men. These included extrinsic alveolitis, organic dust toxic syndrome, back pain, osteoarthrosis, and certain injuries. However, the system was generally not able to demonstrate strong influence of farm residency on disease occurrence for many conditions specifically selected for their likely capacity to show such influence. Future surveillance systems should supplement clinical data with other sources of information on health events and should be adequately powered to focus on narrower ranges of health conditions.  相似文献

Biotechnology applications such as the use and production of genetically modified organisms (GMOs) have been widely promoted, adopted, and employed by agricultural producers throughout the world. Yet, little research exists that examines the implications of agricultural biotechnology on the health and safety of workers involved in agricultural production and processing. Regulatory frameworks do exist to examine key issues related to food safety and environmental protection in GMO applications. However, based on the lack of research and regulatory oversight, it would appear that the potential impact on the safety and health of workers is of limited interest. This article examines some of the known worker health and safety implications related to the use and production of GMOs using the host, agent, and environment framework. The characteristics of employers, workers, inputs, production practices, and socio-economic environments in which future agricultural workers perform various tasks is likely to change based on the research summarized here.  相似文献
