This study evaluates the cranial rectus abdominus muscle pedicle flap as the sole blood supply for the caudal superficial epigastric skin flap. This flap was composed of a cranially based rectus abdominus muscle pedicle flap that was attached to the caudal superficial epigastric island skin flap (including mammary glands 2 to 5) via the pudendoepigastric trunk. Selective angiography of the cranial epigastric artery in eight cadaver dogs proved that the arterial vasculature in the cranial rectus abdominus was contiguous with that in the caudal superficial epigastric skin flap. In the live dog study, three of six of the flaps failed because of venous insufficiency. Necrosis of mammary gland 2 occurred in two of six flaps. One of six flaps survived with the exception of the cranial most aspect of mammary gland 2. Angiography of the cranial epigastric artery proved that arterial blood supply to these flaps was intact. Histological evaluation of the failed flaps showed full-thickness necrosis of the skin and subcutaneous tissues, the presence of severe congestion, and venous thrombosis. Retrograde venous blood flow through the flap was inconsistent, and hence resulted in failure of this myocutaneous flap. Use of this flap for clinical wound reconstruction cannot be recommended.  相似文献

OBJECTIVE: To determine the anatomic guidelines and viability of an axial pattern flap based on the cutaneous branch of the superficial temporal artery (STA) in dogs. Application of the flap in a clinical patient is reported. STUDY DESIGN: Flap viability in control and experimental groups was determined 7 days postoperatively. ANIMALS: A total of 14 mature, mesaticephalic dogs; 1 clinical patient. METHODS: The cutaneous branch of the STA and vein were incorporated in the flaps of the experimental groups (group A, n = 5; group B, n = 4) and were ligated in the control group (n = 5). Flap length was extended in experimental group B. Seven days postoperatively, the length and area of tissue that remained viable in each flap was determined and compared with similar measurements performed intraoperatively. Cutaneous fluorescence was also used postoperatively to assess flap perfusion. The flap used in the clinical patient had the same dimensions as flaps developed in group A. RESULTS: Mean survival length (+/- SD) of STA flaps [group A, 9.1 (0.8) cm], was significantly increased (P < .05) compared with control flaps [7.0 (0.6) cm]. Percentage flap length survival (+/- SD) of STA flaps [group A, 91.8 (8.9)%], was significantly increased (P < .05) compared with control flaps [71.6 (7.0)%]. Mean percentage area of survival (+/- SD) of STA flaps [group A, 93.1 (7.5)%], was significantly increased (P < .05) compared with control flaps [73.5 (7.4)%]. Group B flaps had a mean survival length of 10.4 (1.1) cm, percentage flap length survival of 69.5 (4.8)%, and mean percentage area of survival of 69.1 (6.5)%. There was no positive correlation between the area of flap fluorescence at days 0, 1, and 3, and the area of flap survival. Application of the flap in a clinical patient allowed primary wound reconstruction with 100% survival. CONCLUSION: A flap based on the cutaneous branch of the STA may be a source of skin for reconstructive procedures of the maxillofacial region in dogs. CLINICAL RELEVANCE: Knowledge of the anatomic landmarks and expected viability of a STA axial pattern flap is essential before consideration of its use as a reconstructive surgical technique.  相似文献

Intravenous injection of xylenol orange results in generalized violet staining of vascularized skin. The precision of xylenol orange vital staining was compared to fluorescein sodium vital staining and to assessment of skin flap viability by visual observation (color). Skin flaps in the experimental dogs were evaluated by each method at 0, 24, 72, and 120 hours after surgery and subsequently compared to the length of skin flap that remained viable at 216 hours. No significant differences between methods were found at any time interval; however, assessment of skin flap viability was significantly more precise at 24,72, and 120 hours in comparison to the 0 hour assessments. The use of xylenol orange to assess vascular integrity of skin is precise, but no advantage over fluorescein sodium vital staining or assessment by visual observation was noted.  相似文献

Nineteen axial pattern skin flaps were used in 16 dogs and cats to provide skin for repair of extensive cutaneous defects. Retrospective evaluation of medical records was used to determine percentage flap survival, postoperative complications, and long-term outcome of axial pattern skin flaps. The most common indication for use of axial pattern flaps was to augment wound closure following tumor resection (n = 7). Other indications included trauma (n = 5), chronic nonhealing wounds (n = 4), urine-induced cellulitis (n = 1), idiopathic dermal necrosis (n = 1), and chronic lymphoplasmocytic dermatitis (n = 1). Mean flap survival (+/- SD) was 96% (+/- 8). Postoperative complications included wound drainage (n = 15), partial dehiscence of the sutured flap (n = 7), distal flap necrosis (n = 6), infection (n = 3), edema (n = 3), and seroma formation (n = 2). After a median follow-up time of 5 months, evaluation of animals indicated that surgery provided successful wound reconstruction with good cosmetic results. Reconstruction of large cutaneous defects is facilitated by axial pattern flap application regardless of cause of wound. Postoperative complications are common but amenable to standard wound management techniques such as drain placement and surgical debridement of devitalized distal flap skin.  相似文献

The purpose of this study was to identify the angiosome of the medial saphenous artery and vein and to evaluate the use of this cutaneous angiosome as a free skin flap in the dog. In phase 1 of this study, selective angiography of the medial saphenous artery performed in six canine cadavers showed that the skin covering the entire medial femorotibial area, the distal half of the caudal head of the sartorius muscle, and the gracilis muscle were perfused by the medial saphenous artery. In phase 2, a medial saphenous fasciocutaneous island flap was raised and sutured back to the skin edges of the donor wound in three dogs. One hundred percent survival of all of the flaps occurred. In phase 3, a medial saphenous fasciocutaneous microvascular free flap was transferred to a wound that was created over the dorsal metacarpal (n = 3) or metatarsal region (n = 3). The mean length ± SD of the medial saphenous vascular pedicle was 80 ± 13 mm (n = 5); the mean diameter ± SD of the medial saphenous artery was 2.8 ± 0.2 mm (n = 5) and the mean diameter ± SD of the medial saphenous vein was 4.2 ± 0.2 mm (n = 5). One hundred percent of all flaps survived (n = 6). Selective angiography of the distal cranial tibial artery (metatarsal wounds, n = 3) and the median artery (metacarpal wound, n = 3) was performed 3 weeks after surgery. All of the vascular anastomoses were patent and neovascularization of the wound beds was present. This free flap was found to be acceptable for cosmetic reconstruction of wounds located on the distal extremity.  相似文献

Objective— To investigate neutrophil accumulation after ischemia and reperfusion (IR) in microvascular tissue flaps in horses.
Study Design— Randomized controlled experiment.
Sample Population— A total of 8 horses between 1 and 10 years of age, 4 of each sex.
Methods— Control and experimental myocutaneous island flaps based on the superficial branch of the deep circumflex iliac vessels were dissected on each horse. Atraumatic vascular clamps were applied to the pedicle of the experimental flap for 90 minutes and then removed to allow reperfusion. Based on the assumption that rapid infiltration of neutrophils into affected tissues is a hallmark of IR injury, radiolabeled autogenous leukocytes were used to indirectly quantify neutrophil accumulation in flap tissues. Labeled leukocytes were administered through a jugular catheter 30 minutes before flap reperfusion. Biopsies were collected from each flap over a 6 hour postischemia time period; in group 1 (  n = 4  ) from 0 to 6 hours postischemia, and in group 2 (  n = 4  ) from 24 to 30 hours postischemia. Biopsies were examined scintigraphically and histologically for evidence of neutrophil infiltration.
Results— All control flaps survived and 6 of 8 experimental flaps survived. There was no significant evidence of acute neutrophil infiltration into flap tissues after reperfusion in either group.
Conclusions— The results of this study suggest that equine myocutaneous flap tissues can survive a 90-minute ischemic period and reperfusion. No significant evidence of the occurrence of IR injury in flap tissues was found.
Clinical Relevance— The reasons for the previously reported failures of equine free tissue transfer remain uncertain, but they do not appear to be caused by neutrophil mediated injury associated with ischemia and reperfusion.  相似文献

The repeatability and sensitivity of Doppler ultrasonographic measurement of lateral digital arterial and venous blood flow has not been previously determined. Doppler ultrasonography was used to measure blood flow within the forelimb lateral digital vessels in one normal adult Thoroughbred horse on six occasions and in six normal adult Thoroughbred horses on three occasions, each occasion being at least 1 h apart, to determine the within- and between-horse variation. The values obtained from the right and left lateral digital vessels did not differ significantly. The within-horse coefficients of variation (CV) for arterial and venous measurements were all acceptable (< 11%); the between-horse CV were acceptable (< 7%) for all parameters except TaVa (average velocity of first peak of arterial waveform) and TaVb (average velocity of remainder of arterial waveform). The within-horse intraclass correlation coefficients (ICC) demonstrated excellent repeatability (> or = 0.71) for all parameters except venous diameter; the between-horse ICC demonstrated good to excellent repeatability (> or = 0.67) for all parameters except TaVb. Doppler ultrasonography can detect differences of 0.005 and 0.01 ml/ min in digital arterial and venous flow, respectively, using measurements from six horses on three occasions (80% power; P < 0.05). Thus, the technique is sufficiently repeatable and sensitive to be able to detect changes in flow during different physiological or pathological states or following pharmacologic intervention.  相似文献

A model for the study of equine cutaneous physiology, pharmacology, and toxicology was developed. Four 4 x 12 cm and twenty-one 6 x 12 cm single-pedicle axial pattern skin flaps based on the caudal superficial epigastric artery, and eight 6 x 12 cm flaps based on the saphenous artery and medial saphenous vein, were raised and sutured in a tubed configuration. On day 2, each flap was removed, the artery was cannulated, and the flap was perfused with a modified Krebs-Ringer's albumin-based medium for at least 6 hours. Flap viability was assessed by glucose use, lactate production, and histologic examination at the end of the perfusion period. The 4 x 12 cm flaps had evidence of skin necrosis, but the 6 x 12 cm flaps remained histologically viable. Results were compared to those previously reported from perfusion of porcine skin flaps based on the caudal superficial epigastric artery. While the ratios of glucose use to lactate production were similar, equine flaps used less glucose and produced less lactate per gram of tissue than similar pig flaps. Equine skin flaps perfused by saphenous vessels used more glucose and produced more lactate than flaps perfused by caudal superficial epigastric vessels. These results indicate that conclusions drawn from cutaneous physiology studies should not be extrapolated across species lines and that site-specific skin should be used for cutaneous physiology, pharmacology, and toxicology studies. The identified skin flaps may have applications in equine reconstructive surgery.  相似文献

An axial pattern cutaneous free flap in the dog based on superficial cervical vessels was identified. Consistent anatomic landmarks were proposed. The vessels consistently emerged from the angle formed by the omotransversarius and trapezius muscles, and coursed in a craniodorsal direction. Eight orthotopic transfers were performed to evaluate the flap's potential as a free tissue transfer. Six of eight (75%) flaps survived. Arterial insufficiency was the likely cause of one failure. The cause of the other failure was considered to be venous occlusion. We conclude that the superficial cervical flap can be used successfully as a microvascular free tissue transfer in the dog.  相似文献

The stability of blood ionized calcium (Ca2+) and acid-base variables in equine, bovine, ovine, and canine venous blood samples (n = 15, in each group) stored at 4 C for 3, 6, 9, 24, or 48 hours was studied. Variables included blood Ca2+ and standard ionized calcium (Ca2+ corrected to pH 7.4) concentrations, pH, blood carbon dioxide and oxygen tensions, base excess, bicarbonate concentration, and total carbon dioxide content. Results indicate that storage of blood samples at 4 C for up to 48 hours, despite appreciable acid-base changes, is associated with less than 1.5% change in equine, bovine, and ovine blood Ca2+ concentrations. Similar changes were observed in canine blood during the first 9 hours' storage. After 24 and 48 hours' storage, clinically relevant decrease (10.5 and 15.5%) in canine blood Ca2+ concentration was measured. Therefore, Ca2+ concentration in equine, bovine, and ovine venous blood samples stored up to 48 hours, and in canine blood samples stored up to 9 hours at 4 C is of diagnostic use.  相似文献

The relationship between pedicle flap width and viable length was characterised for skin flaps of the flank in ponies. Four dorsally based, pedicle type skin flaps of 20 cm in length and 3, 6, 9 or 12 cm in width were created in a random sequence on one flank in each of 10 ponies. Flap survival length was assessed by skin texture and appearance, depilation of hair and wound healing at 14 days after surgery. There was considerable variation between animals in the viable length of flaps of the same width; however, a significant difference in the viable length of flaps of different widths was detected (P = 0.002). The viable length of the 3 cm flaps was significantly different from that of the 12 cm flaps (9.69 +/- 1.1 cm vs 12.97 +/- 1.0 cm, mean +/- se P less than 0.05). There was a positive correlation between flap width and viable length (r2 = 0.141, P = 0.017). There was no effect of flap order (cranial to caudal positioning relative to the other flaps) on viable length (P = 0.286). The results of this study demonstrate a significant relationship between flap width and viable length, confirming a previously unsupported assumption that flap width and viable length are related. These findings suggest that the clinician should employ broad pedicles when using local flaps to reconstruct skin defects on the flank of the horse.  相似文献

BACKGROUND: Associations of age and heart rate with blood flow velocities and durations assessed by pulsed-wave (PW) Doppler echocardiography in cats are incompletely understood. OBJECTIVES: To determine the effects of age and heart rate on blood flow velocities and durations of cardiac events obtained by PW Doppler echocardiography in healthy, nonsedated cats. ANIMALS: A convenience sample of 87 healthy, nonsedated cats aged 3 months to 19 years. METHODS: Prospective, observational study. PW Doppler measurements were obtained by echocardiography. Association of age and heart rate with PW Doppler values was evaluated by simple and multiple linear regressions and ANCOVA. RESULTS: Significant weak positive relationships were found between age and isovolumic relaxation time (IVRT) (R2= 0.18; P< or = .001), and between age and duration of pulmonary venous retrograde flow (R2= 0.07; P= .041). There was a significant weak negative relationship between age and transmitral peak early diastolic velocity (R2= 0.19; P< or = .001). Age and heart rate were significantly related to pulmonary venous peak systolic velocity (R2= 0.13; P= .008). Heart rate affected transmitral peak late diastolic velocity (R2= 0.11; P= .006). After adjusting for heart rate effect, the PW Doppler variables that were significantly different between age groups were transmitral peak early diastolic velocity (P< or = .001), duration of transmitral late diastolic flow (P< or = .001), IVRT (P< or = .001), and the ratio of duration of transmitral late diastolic flow to duration of pulmonary venous retrograde flow (P= .029). CONCLUSIONS AND CLINICAL IMPORTANCE: The association of several PW Doppler-derived variables and age and heart rate is weak and not clinically important.  相似文献

OBJECTIVE: To evaluate the efficacy of a percutaneously controlled hydraulic occluder (HO) in a rat model of gradual venous occlusion. STUDY DESIGN: Experimental study. ANIMALS: Ten male Sprague-Dawley rats. METHODS: HOs and perivascular transit time ultrasound flow probes were applied to the caudal vena cava (CVC). Occluders in group I rats (n=6) were gradually attenuated over 8 weeks, whereas occluders in group II rats (n=4) were not attenuated. Vena caval blood flow measurements were performed weekly in all rats for 8 weeks. After euthanasia, the CVC was evaluated grossly and by histopathology. RESULTS: Premature occlusion of the vena cava occurred during the 1st week in 3 rats (1 in group I, 2 in group II) because of kinking of the vena cava between the HO and the flow probe. For the remaining rats, mean blood flow in group I decreased significantly from 40.71 mL/min/kg at baseline to 4.68 mL/min/kg at 8 weeks (P=.0094, power=0.91). Group II rats maintained vena caval blood flow at all times during the study. Complete occlusion of the vena cava was confirmed at necropsy in all group I rats. CONCLUSIONS: Gradual occlusion of the CVC of rats was achieved with HOs over 8 weeks. This model is limited by the size of the experimental animals and comparatively large implants. CLINICAL RELEVANCE: HOs may provide a method for gradual occlusion of congenital portosystemic shunts.  相似文献

OBJECTIVE: To assess the effects of epidural anesthesia using lidocaine on microcirculatory blood flow, volume, and velocity in free fasciocutaneous flaps in dogs. Study Design-In vivo experimental investigation. Animal Population-Ten adult dogs weighing 20 to 25 kg. METHODS: A medial saphenous fasciocutaneous free flap was removed and an orthotopic transfer was performed by anastomosing the primary flap vessels back to the medial saphenous vessels. Blood flow (mL(LD)/min/100 g), volume (%volume or tissue hematocrit) and velocity (mm/s) in the flap were recorded throughout the procedure. After epidural anesthesia, blood flow, volume, and velocity values were again recorded. RESULTS: Microcirculatory blood flow, volume, and velocity, as measured by a laser-Doppler flowmeter, failed to reveal any significant changes over time. Immediately after epidural anesthesia, mean arterial pressure was significantly reduced and remained depressed throughout the experimental procedure. CONCLUSIONS: Epidural anesthesia combined with general anesthesia does not improve microcirculatory flow in free flaps in the pelvic limbs of dogs. No significant change in blood flow to the medial saphenous fasciocutaneous free flap occurred after division and anastomosis of the vascular pedicle. CLINICAL RELEVANCE: We recommend that epidural anesthesia with 2% lidocaine be used with caution in dogs undergoing microvascular free-flap transfer.  相似文献

OBJECTIVE: To determine applicability and size limits of an axial pattern flap based on the lateral caudal arteries in dogs to reconstruct caudodorsal trunk skin defects. STUDY DESIGN: Experimental study. ANIMALS: Ten mature, mixed breed dogs. METHODS: The lateral caudal vessels of the tail were incorporated in the flaps of the treatment group (n=5) and were ligated in the control group (n=5). Flaps were rotated and placed into experimentally created caudodorsal skin defects on the trunk. The length and area of tissue that remained viable in each flap were determined. RESULTS: Mean (+/-SD) survival area (222.8+/-32.9 cm2) and length (25.1+/-4.5 cm) of vascularized flaps were significantly greater (P<.05) compared with control flaps (94.9+/-13.4 cm2 and 14.61+/-4.7 cm). Necrosis occurred in all control flaps, resulting in lower percentage flap area (48.8%) and length (47.1%) survival compared with vascularized flaps (78.1%, 77.5%). CONCLUSIONS: Axial pattern flap based on lateral caudal arteries of the tail was successfully elevated and transferred in a single procedure, with 78% survival for closure of large experimentally created caudodorsal trunk defects in dogs. CLINICAL RELEVANCE: Large cutaneous defects of the caudodorsal trunk area in dog could be reconstructed with tail axial pattern flap. Limitations in terms of size and changes in animal appearance have to be considered before flap elevation.  相似文献

A neutered male German shorthaired pointer sustained severe bite wounds to the left caudal flank and thigh area. Thorough wound lavage and debridement was performed immediately and also three days after presentation. Daily wound dressing resulted in the production of a mature granulation tissue bed. Prior to wound closure, colour flow Doppler ultrasonography was used to confirm blood flow through the right and left caudal superficial epigastric arteries and veins. Sixteen days after presentation, right and left caudal superficial epigastric axial pattern flaps were simultaneously elevated to cover the defect. The right flap was elevated as an island flap, rotated 120 degrees and used to cover the caudodorsal aspect of the defect. The left flap was elevated and rotated dorsally to cover the cranioventral aspect of the defect. Ninety per cent wound coverage was achieved and flap survival was total. The donor site defect was closed primarily and no dehiscence occurred. Three months postsurgery, the entire defect was closed and limb function was normal.  相似文献

Strangulation obstruction was induced in anesthetized ponies for periods of 2 and 3 hours by clamping 45-cm segments of jejunum and associated veins (venous strangulation obstruction) and arteries and veins (arterial and venous strangulation obstruction). Four segments were studied in each of 7 ponies allowed to survive 12 hours, 2 segments in a pony that was allowed to survive 1 hour, and 1 segment in each of 10 ponies allowed to survive 42 days after the strangulation periods ended. Fifteen minutes after the periods of strangulation obstruction ended, the viability of test segments was assessed by clinical judgment (40 segments), fluorescein fluorescence (40 segments), and Doppler ultrasound (32 segments). Because the test segments were normal at necropsy in long-term survivors, all segments were designated as viable. The overall accuracy of the methods used to predict viability was 88% for Doppler ultrasound and 53% each for clinical judgment and fluorescein fluorescence (P less than 0.005). Failures in the last 2 techniques could be attributed to their tendency to score venous strangulation obstruction segments as nonviable (90% for each). Doppler ultrasound was 94% accurate in these segments.  相似文献

Objective — The purpose of this study was to determine the surgical guidelines for and the survivability of an axial pattern flap based on the superficial temporal artery in cats.
Study Design — The mean survival length and mean percentage area of survival of flaps after 7 days in control and experimental groups, are assessed and compared.
Animals — Ten mature, domestic shorthair cats.
Methods — The superficial temporal artery and vein were incorporated in the flaps of the experimental group (n = 5) and were ligated in the control group (n = 5). Seven days postoperatively, the length of tissue that remained viable in each flap was determined by measurement of the length of the grossly devitalized tissue and subtracting this from total flap length. Results — Mean width and length of all flaps was 2.0 ± 7.0 cm, respectively. Mean survival length (±SD) of experimental and control flaps was 6.9 (0.2) cm and 4.4 (2.2) cm, respectively. Necrosis occurred in all control flaps, resulting in a mean percentage area of survival of 62.8 (11.7)%, compared with 98.6 (3.2)% for experimental flaps. Results were significantly different (P <.05) between the experimental and control groups.
Conclusions — A flap based on the superficial temporal artery may be a source of skin for reconstructive procedures of the maxillofacial region in cats.
Clinical Relevance — Knowledge of the anatomic landmarks and survivability of an axial pattern flap are essential considerations when planning a reconstructive technique.  相似文献

A randomized, placebo-controlled, parallel study was conducted to investigate the effectiveness of oral carprofen for the control of postoperative pain in dogs undergoing knee surgery for stabilization of ruptured cranial cruciate ligaments. Dogs were randomly assigned to treatment with carprofen (n = 10) or placebo (n = 9). Pain was assessed at 1, 2, 4, 6, 24, and 48 hours and 10 and 21 days postoperatively. Eight of 10 dogs treated with carprofen and five of nine dogs treated with placebo were given at least one dose of morphine as rescue therapy. The mean relative dose of morphine given at 1 hour (P =.01) and 24 hours (P =.02) after surgery was greater for dogs treated with carprofen than for dogs given a placebo. There were no significant postoperative differences in cortisol levels or any measured variable. It appears that the scoring system used was not sensitive enough to detect differences in pain between a known analgesic and a placebo.  相似文献

Caudally based axial pattern and random pattern skin flaps, 5 cm x 10 cm, were raised on the abdomens of seven foals. Blood flow was measured 7.5 cm from the flap bases during incremental increases in applied tension. At tensions less than 1400 g, cutaneous blood flow was more than four times higher in axial pattern flaps than in random pattern flaps. Blood flow in axial pattern flaps at tensions up to 1500 g was greater than baseline flow in random pattern flaps. After removal of applied tension, perfusion in axial pattern flaps gradually increased but remained below baseline values. A short period of hyperperfusion after removal of tension in random pattern flaps may have been indicative of accumulation of waste products and localized acidosis. All flaps healed in their donor beds without complications.  相似文献
