Traditional cystostomy tubes (used for temporary or permanent diversion of urine in dogs and cats) are long (> or = 22 cm) and cumbersome to stabilize, requiring sutures or bandages to hold the tube against the body. Use of a low-profile gastrostomy port system as a low-profile cystostomy tube (LPCT) in 4 dogs and a cat was investigated; owner satisfaction with the device was assessed. Technical difficulty associated with placement and management of LPCTs was similar to that for traditional cystostomy tubes; with LPCTs, activity and mobility of pets was not compromised, and bandaging was not required. Complications included lower urinary tract infection, mild peristomal leakage of urine and leakage from components of the system, and subcutaneous peristomal infection. Four of 5 owners considered the tube to be easy to use; all owners said they would be comfortable repeating their decision to use the LPCT in their pet.  相似文献

Dogs and cats that had a percutaneous endoscopic gastrostomy (PEG) tube or surgically placed gastrostomy (SPG) tube inserted were retrospectively analyzed to compare complication rates and the severity of complications. Complication rates and severity scores were not significantly different when the PEG tube group was compared to the SPG tube group in either dogs or cats. Only when data from dogs and cats were combined did PEG tubes have a significantly higher complication rate and significantly greater complication severity scores.  相似文献

The purpose of this study was to evaluate complications and owner impressions of a low profile gastrostomy device (LPGD) for enteral feeding in dogs. Medical records of dogs that had placement of a percutaneous endoscopic gastrostomy tube followed by a LPGD between 1995 and 2003 were reviewed. Fifteen dogs had 29 devices placed for a variety of disease states in which enteral nutritional support was indicated. The individual tubes were placed for a median duration of 212 d. Most complications occurred more than 15 d after placement and were of no health risk to the animals. The most common reason for LPGD replacement was leakage through the 1-way antireflux valve. Fourteen of 15 owners that were contacted were supportive of the device. Based on its durability, ease of placement, minor complications, and positive owner impressions, we conclude that the LPGD is a valuable device for long-term enteral nutritional support of dogs.  相似文献

An indwelling stomach tube was placed through a cervical esophagostomy in 5 clinically normal horses and in 3 horses requiring artificial alimentation. In all but one case, surgery was performed on conscious horses following tranquilization and infiltration of the surgical sites with a local anesthetic. Six horses were fed liquid rations through the indwelling tubes, which were left in place from 3 to 26 days. After the tubes were removed, the stomas healed rapidly and with minimal complications. Cervical esophagostomy proved to be a safe, easy, and effective method for the provision of extraoral feeding of the horse.  相似文献

Percutaneous gastrostomy tubes were placed non-endoscopically in 31 cats and 10 dogs using either a rigid insertion tube (n=13) or an Eld gastrostomy tube applicator (n=28). Tubes were placed successfully in 38 of the 41 animals and the consequent feeding was of therapeutic benefit to 31 of the animals. Six of 41 died or were euthanased for reasons unrelated to gastrostomy. In four cases (10 per cent), gastrostomy failed with respect to correct tube placement or tube feeding. Overall complications occurred in 18 of 41 animals. Severe procedural complications occurred in two cats; a cardiorespiratory arrest during pharyngeal manipulation and erroneous tube placement through the distal oesophagus. Moderate late complications included peristomal food leakage (n=2), peristomal abscess (n=2) and pyloric outflow obstruction by a migrated tube (n=1). Complications associated with the feeding procedure, nausea and vomiting (n=3), led to aspiration pneumonia in one case.  相似文献

Cervical esophagostomy for tube feeding was evaluated in 11 ponies. Minor complications responded to supportive therapy in 8 ponies. Two died of complications, and 1 pony had a permanent fistula because of persistent infection. There was a positive correlation between the duration of tube feeding and the event of closure of the esophageal stoma after the tube was removed. There was no difference in the frequency of complications related to duration of tube feeding. When the distal end of the feeding tube was located in the thoracic portion of the esophagus, instead of in the stomach, tubes were more readily dislodged. Reinsertion of some tubes was difficult or impossible to do and resulted in false passage of the tube into the mediastinal space, dissecting infections into the thorax, or both. Reflux of food around the tube occurred in ponies with the distal end of the feeding tube located in the stomach, but this was not associated with other complications.  相似文献

The medical records of 39 dogs and 8 cats which had jejunostomy feeding tubes placed using modifications of a previously described surgical technique were retrospectively reviewed. Modifications included the addition of a suture to anchor the feeding tube to the jejunum, the use of a continuous circumferential suture to pexy the jejunum to the abdominal wall, and the placement of a skin suture that penetrated deep into the abdominal wall fascia. All tubes were placed during surgical treatment of a primary intraabdominal disease. Tubes were used for feeding from 1 to 41 days (average 9.7 days). Sixteen patients (34%) developed mild to moderate complications such as chewing at the tube, inadvertent tube removal, tube obstruction, and cellulitis around the ostomy site. Three patients (6%) developed severe complications associated with breakdown of the surgical site. The modified jejunostomy technique was effective for placement of jejunostomy tubes as a complementary procedure to exploratory celiotomy.  相似文献

This article describes the clinical management of potential ibuprofen toxicosis in South American red-footed tortoise (Geochelone carbonaria). A 2.5-year-old, 0.78-kg Geochelone carbonaria tortoise was presented to the emergency clinic after ingesting solubilized ibuprofen (200 mg) in a gelatin capsule. Treatment on initial presentation consisted of esophagostomy tube placement for gastric lavage and activated charcoal administration, intravenous and intraosseous fluid therapy, and administration of gastrointestinal protectants (sucralfate and famotidine). The tortoise was discharged to the owners. Although follow-up diagnostic monitoring was minimal because of owner compliance, the patient was noted to be alive and in reasonable health 1 year after initial presentation. This is the first report on the management of potential nonsteroidal anti-inflammatory drug toxicosis in any chelonian species.  相似文献

Feeding guidelines on commercial dog food packages provide a suggested starting point for food provision for dogs. The equations used to develop commercial dog food feeding guidelines and the owner feeding practices surveyed were examined in this study. The equations used to develop feeding guidelines and the energy density calculation method (Traditional Atwater or Modified Atwater equation) were investigated for 200 dry dog foods sold in Canada. Not all energy densities of products were calculated using the modified Atwater equation, a requirement if claiming compliance with the Association of American Feed Control Officials (AAFCO). Commercial feeding guidelines provide conservative estimates of pet dog energy needs. A survey of dog owners’ feeding practices was conducted, with 739 responses analyzed. Respondents generally took appropriate action to manage the weight of their dogs through both exercise and dietary management. Further investigation should explore how owners may be successfully managing their dogs’ weight without veterinary supervision.  相似文献

Objectives : To examine the opinions among small animal owners regarding the management of pain and surgery in small animals. Methods : A questionnaire was presented to 800 owners of dogs or cats who visited one of the four participating clinics in Finland in February 2006. Results : A total of 482 owners completed the questionnaire (60·3 per cent response rate); 90 per cent of the respondents were female. Owners classified surgical procedures (for example, fracture repair, skin tumour removal and neutering) as more painful than medical conditions (otitis externa and lameness). In addition, owners disagreed most with statements that they had received sufficient information on the appropriate methods of management of animal pain and that the recognition of animal pain is easy. With respect to surgical procedures, owners expressed greatest concern in relation to the animal experiencing fear or anxiety during hospitalisation and the presence of postoperative pain. Clinical Significance : The animal owners had concerns about the presence and management of animal pain, fear and anxiety. Knowledge of these animal owner opinions could aid veterinary practitioners when communicating with their clients on medical and surgical patient management. Furthermore, studies are merited to clearly define how best to successfully respond to these owner attitudes.  相似文献

OBJECTIVE: To determine indications for cystostomy tube use in dogs and cats, complications associated with their use, and outcome of dogs and cats in which cystostomy tubes had been inserted. DESIGN: Retrospective case series. ANIMALS: 37 dogs and 39 cats. PROCEDURES: Information was obtained from medical records. Long-term follow-up information was obtained by use of a client questionnaire. RESULTS: Indications for cystostomy tube placement were bladder dysfunction, urinary tract rupture, obstructive urinary tract neoplasia, urinary diversion following urogenital surgery, obstructive urolithiasis, and feline lower urinary tract disease. Median time tubes were in place was 11 days, but duration of tube use was significantly longer for animals with bladder dysfunction than for animals with urinary tract trauma, urinary diversion, or urinary tract obstruction. Thirty-seven (49%) animals had tube complications. Development of complications was not significantly associated with species, age, body weight, duration of tube use, or tube type, except that animals were significantly more likely to develop complications following long-rather than short-term use. In 42 animals, the underlying condition resolved and the tube was removed; 22 animals died or were euthanatized with the tube in place. CONCLUSIONS AND CLINICAL RELEVANCE: Results suggested that cystostomy tubes may be used for animals with various conditions related to problems with urine outflow. Nearly half the animals in the study developed complications related to the cystostomy tube, suggesting that potential complications should be discussed with owners prior to tube placement. However, most complications were easily resolved.  相似文献

Interest in noninvasive feeding tube placement in companion animals led to the adaption of a human technique utilizing endoscopy to place nasojejunal feeding tubes. Data from medical records in which nasojejunal feeding tubes were attempted were reviewed. Feeding tubes were attempted and successfully placed in five dogs within a median of 35 min. Feeding tubes remained in place for approximately 7 days. Complications included facial irritation (5/5), sneezing (5/5), fractured facial sutures (4/5), vomiting (3/5), diarrhea (3/5), crimping of feeding tube (3/5), regurgitation (1/5), epistaxis (1/5), clogging of the feeding tube (2/5), and oral migration with premature removal of the feeding tube (1/5). The deployment technique used in this study was found to be cumbersome. Despite minor complications, endoscopy can be used to rapidly and accurately place nasoenteric feeding devices.  相似文献

A free-ranging California condor (Gymnogyps californianus) presented severely dehydrated, unable to stand, very weak, and with a distended crop. Toxicologic analysis revealed an elevated blood lead level of 291.4 microg/ ml. There was no evidence of lead exposure on radiographs. The condor was treated with chelating agents and intensive supportive care. Over a period of 10 days, a majority of its clinical signs improved, and lead levels dropped to near normal. However, the condor was left with a severe, persistent crop stasis and malnutrition. An esophagostomy tube, and later an ingluviostomy tube, were placed and maintained for a period of 1 mo. The condor was managed with continuous infusion feeding. After the feeding tube was removed, the condor was managed with small amounts of soft food orally. After 10 wk of treatment, the crop returned to normal function.  相似文献

The importance of owner compliance in following treatment is recognized in all aspects of veterinary and human medicine. However, in veterinary behavioral medicine, where the majority of the ‘treatment’ is conducted by the owners themselves after the consultation, compliance is fundamental to the successful outcome of the case. The owners of 85 cats with a range of different ‘behavior problems’ were recruited to participate in this study. Each owner had 2 visits from a clinician: at the first background information was gathered, the owner and clinician assigned a subjective score of ‘severity’ and a full behavior consultation was conducted. At a second visit 8 weeks later, owner and clinician rated the severity of the problem again, and the owner was asked questions that were designed to check their compliance to various aspects of the treatment program. The owners reported a decrease in severity after treatment in 92% of cases, and the clinician in 78% of cases. The average compliance of owners to programs was 61%. A strong positive correlation was found between change in severity score rated by the clinician, and compliance score (Spearman's ρ = 0.718, P < 0.01), a weaker positive correlation was found between compliance and difference in owner severity scores between visits (Spearman's ρ = 0.402, P < 0.01). There was no significant correlation found between compliance score and the severity score on visit one, rated either by owner or clinician. The results of this study confirm that owner compliance is an important factor in treatment outcomes for behavior problems in domestic cats.  相似文献

Critical care veterinarians observe a wide variety of patients who easily become malnourished and display negative nitrogen balance due to inappetance, complete anorexia, vomiting, or inability to normally prehend food, as in patients with head and facial trauma (Tables 1 and 2). Negative nitrogen balance is associated with delayed wound healing and increased incidences of gastroinstestinal mucosal atrophy, bacterial translocation, sepsis, and mortality. Early interventional nutrition is necessary for maintenance of normal function and for proper healing to occur. While total parenteral nutrition (TNP) is an option, enteral nutrition is preferred whenever possible. Enterocytes undergo atrophy within several days in the absence of direct trophic stimuli Within the intestinal lumen.
"If the gut works, use it" has gained widespread Popularity in early interventional nutrition in the critically ill veterinary patient. Force-Feeding is often stressful for both patient and caretaker alike. Instead, tube feeding is often well tolerated and an easier route for enteral nutrition. The following methods of tube feeding have been described: nasoesophageal, nasogastic, nasojejunostomy, esophagostomy, pharyngostomy, gastrostomy, and jejunostomy.
Esophagostomy tubes were first described for use in human patients with maxillofacial and cervical neoplasta. Later, Dr. Crowe described their use for veterinary patients (1989). Since that time, several methods for esophagostomy tube placement have been proposed. The modified percutaneous method is one of the easiest to perform, and requires minimal equipement.  相似文献

Objective— To report a technique for tube cystostomy placement via a minimally invasive inguinal approach and outcome in 9 dogs and 6 cats with urinary tract obstruction or detrusor atony.
Study Design— Case series.
Animals— Dogs (n=9) and cats (6).
Methods— Medical records (January 2004–January 2008) of dogs and cats that had tube cystostomy via an inguinal approach were reviewed. Retrieved data included signalment, diagnosis, surgical technique, and complications. Access to the bladder was through a muscle splitting approach in the inguinal region with the cystostomy tube placed through a skin incision made several centimeters proximal to this incision and secured in the bladder by a purse string suture. Cystopexy during closure of the muscle layers ensured secure closure and minimized the likelihood of uroabdomen if tube dislodgment occurred.
Results— Cystostomy tubes were placed in 5 cats as an emergency procedure for treatment of acute urinary tract obstruction or urethral rupture, and as an elective procedure in 9 dogs and 1 cat. No complications occurred during cystostomy tube placement. Postprocedural complications were minor (peristomal irritation in 2 dogs with latex catheters, catheter laceration, premature removal) and only occurred when tubes were retained for >4 weeks. Urinary tract infection at catheter removal in 6 dogs resolved with antibiotic administration.
Conclusions— An inguinal approach for cystostomy tube placement facilitated rapid catheter placement into the bladder with minimal soft tissue dissection. Cystopexy during abdominal wall closure provided peritoneal protection should premature dislodgement of the cystostomy tube occur.
Clinical Relevance— An inguinal approach should be considered for rapid tube cystostomy particularly in metabolically compromised animals.  相似文献

OBJECTIVES: Lateral thoracotomy is widely used for surgical management of thoracic diseases in small animals. The purpose of this paper is to describe the indications for lateral thoracotomy in dogs and cats and the associated outcomes and complications. METHODS: Medical records of animals undergoing lateral thoracotomy were reviewed and owners contacted regarding complications and survival. Relationships between signalment and treatment variables and outcome variables were investigated. RESULTS: Seventy dogs and 13 cats underwent lateral thoracotomy. Sixty-two per cent of cats and 91 per cent of dogs survived to discharge. Survival to discharge was significantly lower in cats than dogs, for neoplastic than non-neoplastic disease and in older animals. Survival to discharge was higher in animals undergoing patent ductus arteriosus ligation than in those undergoing lung lobectomy or oesophageal surgery. Survival to discharge was not related to surgeon experience. The incidence of complications was not related to species, age, disease, duration of surgery, surgeon experience or duration of thoracostomy tube placement. A low complication rate (5 per cent) was associated with thoracostomy tubes. CLINICAL SIGNIFICANCE: The approach of lateral thoracotomy has a minimal complication rate and animals with a disease requiring this approach have a high survival rate.  相似文献

Tube gastrostomy was performed in 19 dogs over a period of 36 months. Indications for tube gastrostomy included non-oral feeding for the management of oesophageal injuries (nine cases) and dysphagia resulting from oral or pharyngeal trauma (two cases) and decompression and, or, gastropexy for gastric diseases including gastric dilatation volvulus (four cases), gastric outflow diseases (three cases) and gastric neoplasia (one case). All the tubes were placed surgically and left in situ for periods of up to 14 days. Feeding was by frequent bolus administration of a proprietary enteral food initially followed by homogenised commercial dog food. The tubes were readily managed and patency maintained in all cases. Gastric stomata granulated within three days of tube removal. Fourteen dogs recovered and returned to normal oral feeding while three died as the result of their presenting condition. A fourth was euthanased four months after surgery because of recurrence of a gastric malignancy. This review indicates that surgically placed tube gastrostomy is readily performed and is an effective means of providing non-oral alimentation and, or, gastric decompression.  相似文献
