A male, 10 month old llama with malformation of both front limbs was presented. Both front limbs had one more digit located medially. The distal phalanx of this additional digit at the left front limb reached the ground by the tip of the keratinzed pad and the toenail. The accessory digit at the right front limb was bent in a 90 degree angle caudolateraly. Beside the digital bones of the accessory digits the second metacarpal bone and the first carpal bone could be detected in both front limbs by radiological examination. Secondary a bilateral slight carpal valgus deformity could be seen.  相似文献

A 1-month-old alpaca cria presented with a 13 degree valgus deformity of the left metacarpophalangeal joint. The angular limb deformity was centered on the distal metacarpal physes. Transphyseal bridging of the physes was recommended. Two 2.7 mm cortical bone screws were placed either side of the distal metacarpal physes and a figure of eight wire was placed medially around the screw heads. The screws extended through the medial metacarpus into the axial cortex of the lateral metacarpus. Seven weeks after surgery the limb was straight and the screws and wire were removed. Transphyseal bridging of the distal metacarpal physes can be effectively used for the treatment of metacarpophalangeal valgus in crias with open physes.  相似文献

A 2-mo-old, 90-kg, intact male camel (Camelus dromedarius) was admitted for surgical removal of a supernumerary digit associated with the medial aspect of the right carpus and metacarpus. Radiographic views of the carpus and metacarpus revealed the supernumerary digit articulating with the middle carpal joint via an extra carpal bone. The supernumerary digit consisted of three bones. Surgical removal of the supernumerary digit was recommended to restore normal limb conformation, limit the possibility of future lameness, and improve the cosmetic appearance of the limb. Complete surgical removal of the digit was performed by disarticulating the extra carpal bone from its attachments to the second and radial carpal bones. One year after surgery, the leg appeared grossly normal and the camel showed no sign of lameness.  相似文献

Congenital malformation of the carpal joint in a young dog resulted in a progressive lameness. Traumatic disruption of the carpus in another dog resulted in carpal bone necrosis, infection, and chronic instability. Radial metacarpal arthrodesis was performed in both dogs. The diseased carpal bones were excised, sparing the accessory carpal bone in both dogs. Radial metacarpal arthrodeses were then performed, using bone plate fixation. In the dog with the malformed limb, the gait had improved, but intermittent lameness was still present 8 months later because of an associated malarticulation of the cubital joint. The dog with the traumatized carpus had little or no lameness associated with normal activity for 32 months.  相似文献

OBJECTIVE: To describe a technique for carpal panarthrodesis using a medially applied dynamic compression plate (DCP) and to evaluate outcome. STUDY DESIGN: Retrospective study. ANIMALS: Nine dogs with 10 carpal joint injuries. METHODS: Medical records of dogs that had carpal panarthrodesis by medial application of a DCP were reviewed. Signalment, cause and type of injury, preoperative treatment, operative technique, and postoperative clinical and radiographic outcome were retrieved. RESULTS: Screw loosening in the metacarpal bones required surgical revision in 3 dogs. The plate was removed because of lick dermatitis in another dog. No complications were observed in 6 arthrodeses. All dogs were subsequently sound. CONCLUSIONS: Medial application of a DCP is a reliable, reproducible method for carpal panarthrodesis. The load on the edge of the plate provides an increased area moment of inertia of the plate, enhancing it resistance to bending forces. Because of the valgus standing position of the canine carpus, dynamic compression is achieved. The DCP is secured to 1-3 metacarpal bones and loosening of the implant and bone fractures are expected to occur less frequently. CLINICAL RELEVANCE: Medial plating for carpal panarthrodesis is a valid alternative method that can be used in dogs with injuries to the carpus for which panarthrodesis of the carpal joint is indicated.  相似文献

Subluxation of the second carpal bone (C2) in two racing greyhounds, and luxation associated with other carpal injuries in a Staffordshire bull terrier, were diagnosed as causes of thoracic limb lameness. The clinical signs of subluxation were subtle, with local soft tissue swelling and a pain reaction on digital pressure over the dorsal aspect of C2. In contrast, the dog with a complete luxation was very lame, having marked soft tissue swelling with carpal hyperextension and valgus. Diagnosis was made by radiography. Subluxation of C2 was seen as a small, dorsally displaced opacity between the proximal and distal rows of carpal bones on the mediolateral view and, on the oblique view taken in only one case, as a marked dorsal displacement of one-third of the proximal joint surface of C2. In the case of luxation, C2 appeared on the mediolateral view lying dorsal to the radial carpal bone. There was also fracture of C4. Both greyhounds were treated by internal fixation and returned to racing. The Staffordshire bull terrier was treated by pancarpal arthrodesis with a successful outcome.  相似文献

Congenital limb deformities are rarely reported in the cat. The macroscopic and radiographic features of aphalangia are described in a 2-month-old male kitten showing a shortened limb that ended, at the level of the carpus, in a stump without digits. A nail was present at the level of the first phalanx and on the palmar surface only two footpads were present. The radiographs showed an absence of phalanges. The first metacarpal and the proximal and distal phalanges of digit 1 were present. The deformed metacarpal bones were reduced in length; the carpal bones were incompletely ossified. This defect is a rare condition in many animal species. To the author's knowledge, the congenital fore limb deformity described here is the first documented case in a cat.  相似文献

In a male Korean native calf 14 days of age, polymelia (notomelia) was observed macroscopically and radiographically. External features included two normal forelimbs, two normal hindlimbs and two undeveloped extra forelimbs. The extra forelimbs were attached to the caudal regions of the right scapula and devoid of muscular tissues. In the extra forelimbs, a scapula-like bone formed a joint with the incompletely duplicated humerus. The humerus fused with the incompletely duplicated radius. The ulna, carpal bones, metacarpal bones and phalanges were completely duplicated. But one set of the duplicated carpal bones consisted of five bones: radial, accessory, fourth carpal, fused second and third carpal, and fused ulnar and intermediate carpal bones. The hoof and the rudimentary hoof of accessory digit were duplicated.  相似文献

Twenty-eight llamas with a mean age of 6.2 months had bilateral forelimb valgus deformities. The range of valgus deformity, as measured on a craniocaudal radiographic view, was 6 to 25 degrees, with a similar degree of deviation in each limb. Morphologic abnormalities in the distal portion of the radius and ulna were more prominent in the ulna and included metaphyseal and epiphyseal flaring, physeal ectasia, and metaphyseal sclerosis. Distal radial epiphyseal ectasia, and metaphyseal sclerosis. Distal radial epiphyseal wedging was believed to be secondary to the ulnar deformity. Radial bowing with lateral and caudal concavity and unequal cortical thickness was evident. Epiphyseal and metaphyseal flaring with physeal ectasia was noticed in the distal portion of the third and fourth metacarpal bones. Carpal bones were unaffected.  相似文献

Three horses with carpal instability due to comminuted second carpal bone fractures (Cases 1 and 3), fracture of the head of the second metacarpal bone (Case 1) or comminuted fractures of the fourth carpal bone, ulnar and intermediate carpal bones (Case 2) were treated by minimally invasive approach for partial (Cases 1 and 3) or pancarpal (Case 2) joint arthrodesis, using locking compression plates. The joint cartilage was removed by either an arthroscopic approach (middle carpal joint and antebrachiocarpal joint) or a percutaneous drilling technique (carpometacarpal joint). Two or 3 locking compression plates were contoured to the dorsolateral, dorsomedial and dorsoaxial aspects of the carpal joints using a custom‐made tunnelling tool and a minimally invasive tunnelling technique, and the screws were positioned through stab incisions. All cases recovered well, were lame free at the walk, were able to trot and gallop and could be used for leisure and pasture activities (partial carpal arthrodesis) and breeding (pancarpal arthrodesis). Post‐operative x‐rays showed progressive joint fusion after 12 months (Case 1), 5 months (Case 2) and 10 months (Case 3). Case 2 with a pancarpal arthrodesis showed a mechanical lameness at the walk due to the inability to flex the carpus. Carpal flexion after carpometacarpal and middle carpal arthrodesis in Case 1 was calculated to be 42.6° and 44° in Case 3.  相似文献

This study involves the evaluation of pre-purchase radiographic studies of South African Thoroughbred yearlings. Radiographic changes were recorded and compared with similar international studies. The study differs from other studies in that a lower prevalence of pedal osteitis (1.26%), dorsal osteochondral fragmentation of the metatarsophalangeal joint (1.60%), distal metacarpal sagittal ridge changes (15.7%), ulnar carpal bone lucencies (8.33%), carpal osteophytes (1.19%), distal intertarsal and tarsometatarsal joint radiographic changes (9.92%), tarsal osteochondrosis lesions (4.40%) and stifle osteochondrosis lesions (0.4%) was found. The prevalence of dorsal osteochondral fragments in the metacarpophalangeal joint was similar to other studies (1.60%). A higher prevalence of vascular channels as well as irregular borders and lucencies was evident in the proximal sesamoid bones. There was a higher prevalence of palmar metacarpophalangeal and plantar metatarsophalangeal osteochondral fragments (2% and 7.10% respectively). Palmar metacarpal disease, metacarpal supracondylar lysis, proximal sesamoid bone fractures and carpal osteochondral fragmentation were absent in the current study. Additional findings recorded in the current study were proximal interphalangeal joint hyperextension (left front 15.13%, right front 18.91%), the solar angle (right front 2.38 degrees, left front 2.79 degrees), the prevalence of carpal bone 1 (30.95%) and carpal bone 5 (1.59%). Management, nutrition and genetics in the various groups of Thoroughbred yearlings should be further investigated in order to explain the reasons for the differences recorded in the current study.  相似文献

The following case report describes the diagnosis and surgery of bilateral polydactyly of unknown origin in a colt. A 7-month-old Berber colt was referred for cosmetic and curative excision of supernumerary digits. Radiographic examination revealed bilateral polydactyly and well-developed first carpal bones. Surgery consisted of an osteotomy of both second metacarpal bones combined with an amputation of the supernumerary digits. The follow-up at 18 months after surgery revealed a sound horse with an excellent cosmetic outcome.  相似文献

Three techniques for inducing analgesia of the proximal metacarpal region were evaluated for the frequency of inadvertent injection into the middle carpal and carpometacarpal joints. Using methylene blue solution as a marker dye and 30 fresh cadaver specimens each, three clinicians performed either 30 infiltrations at the origin of the suspensory ligament (method A), 30 palmar and palmar metacarpal nerve blocks at the proximal end of the metacarpus (method B), or 30 palmar and palmar metacarpal nerve blocks at the distal aspect of the accessory carpal bone (method C). The frequency of inadvertent injection into the distal carpal joints was 37, 17, and 0% for methods A, B, and C, respectively. The association between method and injection into the joints was significant (p less than 0.01). Infiltration of the distal carpal joints occurred with injection distances from the carpometacarpal joint of 1.5 to 4.5 cm. Although there was no joint injection with method C, the carpal synovial sheath was inadvertently infiltrated in 68% of the specimens. Injection into the distal carpal joints can occur when deep injections are made into the proximal palmar aspect of the metacarpus because of the distopalmar outpouchings of the carpometacarpal joint between the axial surfaces of the second and fourth metacarpal bones and the abaxial surface of the suspensory ligament.  相似文献

OBJECTIVE: To determine those bones in the distal aspect of the limbs of Greyhounds with fatigue fractures that have the greatest left-to-right differences in bone-mineral density (BMD). SAMPLE POPULATION: Limbs obtained from 20 Greyhounds. PROCEDURE: Dual-energy x-ray absorptiometry (DXA) of the distal aspect of each limb and isolated bones from 10 dogs with a fracture of the central tarsal bone (CTB) of the right pelvic limb was performed. High-resolution scanning was performed on excised bones, and BMD measurements of CTB also were obtained from limbs of dogs without a CTB fracture. RESULTS: The BMD of the accessory carpal bone and calcaneus was not significantly different from the BMD of those bones in the contralateral limb. Although BMD of the CTB of the entire right pelvic limb and isolated bones from dogs with a CTB fracture was lower, compared with values for the entire left pelvic limb, values for isolated CTB from dogs without a CTB fracture were not significantly different. Metacarpal or metatarsal and thoracic or pelvic limb significantly affected BMD for measurements of the entire limb and isolated bones. Left-to-right differences in BMD were greatest for metacarpal 5. CONCLUSIONS AND CLINICAL RELEVANCE: Asymmetric adaptive remodeling of metacarpal 5 can be detected by DXA. The potentially confounding effects of CTB fracture and unknown racing history made it difficult to interpret BMD changes in the CTB of these specimens. Densitometry could be developed as an in vivo assessment for risk of fractures in racing Greyhounds.  相似文献

Lameness associated with carpal varus deformity was recognised in 10 dogs, eight of which were Dobermann Pinschers. The dogs had a mean age of seven years and nine months. Carpal varus was usually bilateral with concurrent carpal hyperflexion and pronation. Carpal varus was present prior to the onset of lameness in four cases. With stress radiography the median angle of varus deviation in all carpi was 14.6 degrees , and an increase in the size of the ulnar /ulnar carpal bone joint space was seen in six of the seven dogs. Radiographic changes included: enthesophyte formation at the proximolateral aspect of metacarpal V, periarticular soft tissue swelling, especially lateral, bone proliferation at the carpometacarpal joint I and enthesophyte formation at metacarpophalangeal joint V. Four dogs were admitted for problems other than carpal varus and lameness was not treated. Five dogs were treated with nonsteroidal anti-inflammatory drugs, but lameness was not completely resolved and became exacerbated with exercise. One dog was successfully treated by pancarpal arthrodesis.  相似文献

Bilateral carpus valgus with concomitant outward rotation and cranial bowing of the distal radii was diagnosed in a crossbred foal. The foal was not lame on admission and showed no radiographic evidence of carpal bone abnormalities. Surgery was limited to the most severely affected leg, and consisted of a combination of growth promotion (periosteal transection and stripping) and temporary physeal retardation (transphyseal bridging) procedures. Correction of the valgus deformity was nearly complete in the operated limb and substantial improvement was observed in the cranial bowing and outward rotation in both limbs, five months postoperatively.  相似文献

A 2‐day‐old Warmblood filly was presented for examination of an angular limb deformity of the left front limb and an upright conformation of both metacarpophalangeal joints. Radiological examination revealed bilateral absence of the metacarpophalangeal joint space with fusion of the third metacarpal bone and first phalanx (synostosis). No treatment was undertaken. The filly was readmitted to the clinic 10 weeks later for bilateral front limb lameness. On radiological examination, the synostosis of the front metacarpophalangeal joints was still present. Physitis of the distal growth plate of the right third metacarpal bone and proximal growth plate of the right proximal phalanx, and an avulsion fracture of the palmaromedial and proximal aspect of the left middle phalanx, with a cystic like lesion on the medial aspect of distal first phalanx and proximal middle phalanx were diagnosed. Given the poor prognosis, the foal was subjected to euthanasia. Post mortem examination confirmed the absence of the metacarpophalangeal joint space with a trabecular bony union between the third metacarpal bone and the first phalanx. A rudimentary joint capsule was present at the level of the absent joints as well as a small zone of articular cartilage, which invaginated over a short distance into the dorsal trabecular bone on the right front limb. On the medial aspect of the left proximal interphalangeal joint, a focal defect of articular cartilage with exposure of subchondral bone was observed. This is the first case report of a foal born with congenital aplasia of both metacarpophalangeal joints. Congenital malformations should be considered as differential diagnosis in lame foals or foals born with angular or flexural limb deformities.  相似文献

OBJECTIVE: To determine effects of treadmill exercise on subchondral bone of carpal and metacarpophalangeal joints of 2-year-old horses. ANIMALS: 12 healthy 2-year-old horses. PROCEDURE: Horses were randomly assigned to the control (n = 6) or exercised (6) groups. Horses in the exercised group ran on a high-speed treadmill 5 d/wk for 6 months. Horses in the control group were hand walked for the same amount of time. Results of clinical, radiographic, nuclear scintigraphic, and computed tomographic examinations, and serum and synovial concentrations of biochemical markers of bone metabolism were compared between groups. RESULTS: Exercised horses were significantly lamer at the end of the study than control horses. Radionuclide uptake in the metacarpal condyles, but not in the carpal joints, was greater in exercised horses, compared with control horses. Exercised horses also had a higher subchondral bone density in the metacarpal condyles than control horses, but such differences were not detected in the carpal bones. CONCLUSIONS AND CLINICAL RELEVANCE: None of the diagnostic techniques evaluated was sufficiently sensitive to detect all osteochondral damage. Computed tomography and computed tomographic osteoabsorptiometry were superior to conventional radiography for detecting small osteochondral fragments. Nuclear scintigraphy was a sensitive indicator of subchondral bone change but lacked specificity for describing lesions and discerning normal bone remodeling from damage. Newer techniques such as computed tomography may help clinicians better diagnose early and subtle joint lesions in horses prior to development of gross joint damage.  相似文献

A 9‐year‐old Warmblood gelding underwent magnetic resonance (MR) imaging of the carpal and proximal metacarpal regions for evaluation of prolonged right forelimb lameness. Magnetic resonance findings were indicative of desmopathy of the right lateral carpometacarpal ligament and the interosseous ligament between the third and fourth metacarpal bones. Based on the MR findings and lack of response to conservative therapy, surgery using a drilling technique was performed to facilitate fusion of the right carpometacarpal joint. After surgery and a 6‐month rest and rehabilitation programme, the horse returned to soundness. This case report describes a unique combination of lateral carpal/proximal metacarpal soft tissue injuries that resulted in prolonged lameness and were treated with facilitated arthrodesis of the carpometacarpal joint. These soft tissue injuries should be considered as a differential diagnosis in horses with lameness localised to the proximal metacarpal/distal carpal region.  相似文献

Reasons for performing study: There is limited information on magnetic resonance imaging (MRI) findings in the carpus and proximal metacarpal region of lame horses. Objectives: To document MRI findings in horses with lameness localised to the carpus and/or proximal metacarpal region. Methods: Clinical records of horses that underwent MRI of the carpus and/or proximal metacarpal region at the Animal Health Trust between January 2003 and September 2010 were reviewed. Magnetic resonance images of all horses and available radiographs, ultrasonographic and scintigraphic images were assessed. When possible, MRI findings were related to the results of other diagnostic imaging techniques. Results: Seventy‐two MR studies of 58 lame limbs in 50 horses from a broad range of work disciplines and ages were reviewed. The most commonly detected primary abnormality was decreased signal intensity in T1‐ and T2‐weighted images in the medial aspect of the carpal bones and/or the proximomedial aspect of the metacarpal bones (n = 29). Nine horses had syndesmopathy between the second and third metacarpal bones. In 6 horses the primary abnormalities were identified in the palmar cortex of the third metacarpal bone (McIII). Significant abnormalities of the suspensory ligament (SL) with associated lesions in the adjacent palmar cortex of the McIII were seen in 4 limbs. Ligament and associated osseous abnormalities between the second and third carpal bones and second and third metacarpal bones were detected in 4 limbs. Conclusions and potential relevance: Magnetic resonance imaging enabled diagnosis of a variety of lesions not detected by conventional imaging in horses from a wide range of work disciplines. The distribution of injury types differed considerably from previous studies.  相似文献
