The present peer‐reviewed veterinary literature contains conflicting information regarding the impact of surgical margin completeness on risk of local tumour recurrence in canine soft tissue sarcoma (STS). This systematic review and meta‐analysis was designed to answer the clinical question: “Does obtaining microscopically tumour‐free surgical margins reduce risk for local tumour recurrence in canine cutaneous and subcutaneous STS?” A total of 486 citations were screened, 66 of which underwent full‐text evaluation, with 10 studies representing 278 STS excisions ultimately included. Cumulatively, 16/164 (9.8%) of completely excised and 38/114 (33.3%) of incompletely excised STS recurred. Overall relative risk of 0.396 (95% confidence interval = 0.248‐0.632) was calculated for local recurrence in STS excised with complete margins as compared to STS excised with incomplete margins. Risk of bias was judged to be low for all studies in terms of selection bias and detection bias but high for all studies in terms of performance bias and exclusion bias. The results of the present meta‐analysis, coupled with the results of individual previous studies, strongly suggest that microscopically complete surgical margins confer a significantly reduced risk for local tumour recurrence in canine STS. Future studies ideally should adhere to standardized conducting and reporting guidelines to reduce systematic bias.  相似文献

Soft tissue sarcomas are derived from tissues of mesenchymal origin. Although local recurrence following surgical resection is the characteristic challenge in their management, 40% dogs with high‐grade tumours may also develop metastatic disease, despite successful local control. Soft tissue sarcoma is a complex disease and there are many uncertainties regarding the biology and optimal clinical management. There are currently no diagnostic tests that can reliably predict the amount of surgical margin required for a particular tumour, so there can be a mismatch between treatment and disease. Historically, the tendency has been to always recommend wide excision margins but this is not fully supported by recent evidence. A selection bias for less aggressive soft tissue sarcomas in primary care practice can account for good outcomes that are achieved despite narrow surgical excision margins. On the other hand, inappropriately conservative treatment will adversely affect outcomes for patients with more aggressive disease. This review provides an update on the current understanding of management of canine soft tissue sarcomas.  相似文献

Sixteen cases of malignant soft tissue sarcoma (STS; 10 canines and six felines) were treated with a novel triple therapy that combined photodynamic therapy, hyperthermia using indocyanine green with a broadband light source, and local chemotherapy after surgical tumor resection. This triple therapy was called photodynamic hyperthermal chemotherapy (PHCT). In all cases, the surgical margin was insufficient. In one feline case, PHCT was performed without surgical resection. PHCT was performed over an interval of 1 to 2 weeks and was repeated three to 21 times. No severe side effects, including severe skin burns, necrosis, or skin suture rupture, were observed in any of the animals. No disease recurrence was observed in seven out of 10 (70.0%) dogs and three out of six (50.0%) cats over the follow-up periods ranging from 238 to 1901 days. These results suggest that PHCT decreases the risk of STS recurrence. PHCT should therefore be considered an adjuvant therapy for treating companion animals with STS in veterinary medicine.  相似文献

Histologic grade is an important prognostic factor for both local recurrence and metastatic potential with canine soft tissue sarcoma (STS). Pre‐treatment biopsy with identification of tumour grade may aid in prognostication and determination of surgical margins necessary for local control. The purpose of this study was to evaluate the grading accuracy of various pre‐treatment biopsy techniques (wedge, punch, needle‐core) for STS in dogs. Medical records of 68 dogs diagnosed with a STS via pre‐treatment biopsy and confirmed by excisional biopsy were evaluated. The concordance in grade between excisional and pre‐treatment biopsies was 59%. Of the 41% that lacked concordance, 29% of pre‐treatment biopsies underestimated and 12% overestimated grade. The method of pre‐treatment biopsy did not significantly effect grade concordance. Based on these data, needle‐core biopsy appears to be similar in accuracy compared to open biopsy, however, grading determined by pre‐treatment biopsy in general should be interpreted with caution.  相似文献

Spatial compound sonography improves visualization of tissue details and allows clearer delineation of structural margins. Improved image clarity is due to reduced speckling artifact; however, other types of acoustic shadowing artifacts may be unchanged or variably altered when conventional and spatial compound sonographic images are compared. Because intrarenal distal shadowing artifacts in conventional sonographic images are oftentimes the first or only evidence that a nephroliths is present, we compared the appearance and associated artifacts of nephroliths examined with both imaging modes. Consensus evaluation by two evaluators confirmed differences in appearance of nephroliths based on imaging mode. Nephroliths with conventional imaging mode were less hyperechoic and had better margin delineation while nephroliths were more hyperechoic and had less distinct margins with spatial compound imaging mode. Distal acoustic shadowing artifacts were present in 43% of spatial compound imaging mode vs. 86% of conventional imaging mode. When present in both imaging modes, intensity of these artifacts was weaker and the distance traveled was shorter in spatial compound imaging mode. Multiple diverging acoustic shadowing artifacts originating from a single source, the nephroliths were occasionally noted in spatial compound but not conventional imaging mode. These results demonstrate that the absence of distal acoustic shadowing cannot be used to exclude the presence of a nephrolith in dogs and cats. Optimal diagnosis of nephroliths, margin delineation, and visualization of the distal renal parenchyma requires paired radiography and sonography, and alternating between sonographic imaging modes is therefore suggested.  相似文献

Soft tissue sarcoma constitutes a heterogeneous group of mesenchymal tumours. Although they are common in the dog, many uncertainties surround the best options for clinical management. Despite recent improvements in outcome, approximately one in five patients may still die as a result of their disease. There is some evidence that wide surgical excision may not be required for every soft tissue sarcoma but, conversely, complacency in treatment may adversely affect outcomes for patients with aggressive disease. The purpose of this review is to examine the issues affecting the management of canine soft tissue sarcoma, and to evaluate the human literature for lessons that may guide future treatment directions for dogs. Comparative lessons from human soft tissue sarcoma that may be important for the canine patient in the future include (1) understanding the oncogenic potential of the pseudocapsule to better predict tumour behaviour and optimal surgical margins, (2) recognising the importance of planned multi‐modality therapy for improving tumour control, (3) considering a role for compartmental resection strategies and (4) improving the accuracy of pretreatment analysis of the tumour to better predict behaviour and optimal treatment options.  相似文献

Early postoperative neuroimaging has been performed in people for over 20 years to detect residual brain tumor tissue and surgical complications. The purpose of this retrospective study was to describe characteristics observed using early postoperative magnetic resonance imaging in a group of dogs undergoing craniotomy for brain tumor removal. Two independent observers came to a consensus opinion for presence/absence of the following MRI characteristics: residual tumor tissue; hemorrhage and ischemic lesions; abnormal enhancement (including the margins of the resection cavity, choroid plexus, meninges) and signal intensity changes on diffusion‐weighted imaging. Five dogs were included in the study, having had preoperative and early postoperative MRI acquired within four days after surgery. The most commonly observed characteristics were abnormal meningeal enhancement, linear enhancement at margins of the resection cavity, hemorrhage, and a thin rim of hyperintensity surrounding the resection cavity on diffusion‐weighted imaging. Residual tumor tissue was detected in one case of an enhancing tumor and in one case of a tumor containing areas of hemorrhage preoperatively. Residual tumor tissue was suspected but could not be confirmed when tumors were nonenhancing. Findings supported the use of early postoperative MRI as a method for detecting residual brain tumor tissue in dogs.  相似文献

Magnetic resonance imaging (MRI) has been recommended for staging and surgical planning in cats with injection site sarcomas (ISS). The purpose of this retrospective study was to describe low‐field MRI characteristics of confirmed injection site sarcomas in a group of cats. Low‐field MR images, thoracic radiographs, histopathology findings, and medical records of cats that fulfilled histological criteria of injection site sarcoma were retrieved and reviewed retrospectively. Presence or absence of tumor mineralization and pulmonary metastases were recorded from thoracic radiographs. Characteristics recorded from low‐field MRI studies included tumor number, volume (ellipsoid method), intensity relative to surrounding musculature, homogeneity, regions of signal void (mineralization) or cavitation, degree and pattern of contrast enhancement, tumor margination, presence of a peripheral T2W hyperintense zone, and bone contact. A total of 19 cats met inclusion criteria. Cats with multiple tumors were more likely to have had previous excisional biopsy, and were less likely to undergo definitive surgery. All tumors were hyperintense relative to surrounding musculature on T1W and T2W images. Larger tumors were more likely to exhibit mineralization (P < 0.05). Tumor volume could not predict tumor‐free margins at definitive surgery. The majority of tumors showed moderate to marked heterogeneous contrast enhancement. Infiltrative margins and the presence of a peripheral T2W hyperintense zone were more prevalent following excisional biopsy, while cavitation was more prevalent following incisional biopsy. Findings indicated that low‐field MRI characteristics of injection site sarcoma may vary widely and may be affected by prior incisional or excisional biopsy.  相似文献

Objective— To evaluate recurrence rate and disease-free interval (DFI) of dogs with low-grade soft tissue spindle cell sarcoma of the extremities treated by marginal excision.
Study Design— Retrospective study.
Animals— Dogs (n=35) with soft tissue low-grade spindle cell sarcoma.
Methods— Medical records were reviewed and dogs that had marginal surgical resection of low-grade soft tissue spindle cell sarcoma at or distal to elbow and stifle were included.
Results— Histopathologic margins were dirty (12 dogs), clean but close (12), and clean (11). Follow-up after surgery occurred from 210 to 2202 days (minimum, 180 days). Local recurrence and metastatic rates were 10.8% and 0%, respectively. Median DFI and survival time were not reached, because <50% of dogs died of disease-related events. Mean DFI and mean survival time were 697.8 days (95% CI: 559.7–836 days) and 703.5 days (95% CI: 566.6–840.5 days), respectively. There were no significant differences among survival functions stratified by histologic margins.
Conclusion— Marginal surgical excision without adjuvant treatment of low-grade soft tissue spindle cell sarcoma of the extremities results in a low local recurrence rate.
Clinical Relevance— Low-grade spindle cell sarcomas located at or distal to the elbow and stifle joints can be excised without need for wide or radical surgery.  相似文献

Soft tissue sarcomas (STSs) develop from mesenchymal cells of soft tissues, and they commonly occur in the skin and subcutis of the dog. Although phenotypically diverse with frequently controversial histogenesis, STSs are considered as a group because they have similar features microscopically and clinically. Following resection, local recurrence rates are low in general but vary according to histologic grade and completeness of surgical margins. Complete margins predict nonrecurrence. Even most grade I STSs with "close" margins will not recur, but propensity for recurrence increases with grade. The frequency of metastasis has not been accurately estimated, but it is believed to be rare for grade I STSs and most likely to occur with grade III STSs. However, metastasis does not necessarily equate with poor survival. High mitotic index is prognostic for reduced survival time. Further research is needed to determine more precise estimates for recurrence rates and survival as related to completeness of surgical margins and to delineate potential differences in metastatic rate and median survival time between grades. Other potential indicators of prognosis that presently require further investigation include histologic type, tumor dimension, location, invasiveness, stage, markers of cellular proliferation, and cytogenetic profiles. Common issues limiting prognostic factor evaluation include biases from retrospective studies, small sample sizes, poor verification of metastasis, inconsistent STS classification and use of nomenclature, difficulties in differentiating STS phenotype, and diversity of the study population (stage of disease and treatment status).  相似文献

Increased numbers of tumour‐associated macrophages correlate with rapid tumour growth and metastasis in tumours. Thus, macrophage depletion has potential as a novel cancer therapy and positive responses have been reported in rodent tumour models. To investigate the effectiveness of this approach in dogs with cancer, we evaluated the effects of the macrophage‐depleting agent liposomal clodronate (LC) in dogs with soft‐tissue sarcoma (STS). To this end, we conducted a clinical trial of LC therapy in 13 dogs with STS. Repeated LC administration was well tolerated clinically. Preliminary examination of tumour biopsy sets from 5 of the 13 dogs demonstrated that the density of CD11b+ macrophages was significantly decreased after LC treatment. Circulating concentrations of interleukin‐8 were also significantly reduced. These preliminary studies are the first to suggest that LC can be used as a systemic macrophage‐depleting agent in dogs to reduce numbers of tumour‐associated macrophages.  相似文献

Mohs micrographic surgery (MMS) is the gold standard for the excision of locally invasive cutaneous malignancies in human dermatological surgery. Using a unique horizontal sectioning technique, MMS enables 100% surgical margin assessment and provides the lowest recurrence rates for locally invasive tumours. The purposes of this preliminary study were to explore the feasibility of application of MMS in the veterinary setting and to establish practical advantages and limitations of its use in a pilot programme. It was hypothesized that MMS technique could provide 100% tumour margin assessment using frozen and/or formalin‐fixed horizontal histopathologic sections. Tumour excision and colour‐coded mapping were performed, and specimen tissue was fixed using either frozen sections or formalin‐fixed sections. Horizontal sections were assessed for quality and presence and location of neoplastic cells based on the mapped orientation. The MMS technique was used in the excision of six squamous cell carcinomas and five mast cell tumours. In all cases, the MMS permitted 100% tumour margins examination.  相似文献

A 510 kg, 18‐year‐old Warmblood gelding used for high level dressage was presented with a 6 week history of a growing mass at the rostral aspect of the incisive bone. Radiographic examination of the maxillae showed an osteolytic lesion between the left maxillary central and intermediate teeth spreading caudally into the hard palate. Biopsy of the mass was consistent with an aggressive oral fibrosarcoma. Diagnostic work‐up ruled out metastatic lesions. An amputation of the incisive bone and part of the rostral maxillae was performed with the intent of removing the mass in healthy margins. However, the neoplastic tissue extended to the rostral aspect of the ventral nasal conchae and a resection in healthy margins was not performed to avoid surgical complications due to communication with the nasal conchae. After surgery, the horse's appetite and food uptake were not impaired and cosmetic appearance was excellent. Active drains were used in order to prevent any submucosal collection of fluids. Histopathological examination of the surgical margins of the removed maxillae confirmed the presence of tumoural tissue. Even though excision of the tumour was not complete, no macroscopic local recurrence was reported post operatively at 36 months' follow‐up. Based on histological re‐evaluation of the removed tissue and the absence of recurrence a definitive diagnosis of ossifying fibroma was made.  相似文献

Spontaneously occurring soft tissue sarcoma (STS) is relatively common in canine cancer patients. Because of the similarities to human disease, canine STSs are a valuable and readily available resource for the study of new therapeutics. In this study, a canine patient‐derived xenograft (PDX) model, CDX‐STS2, was established. The CDX‐STS2 model was engrafted and expanded for systemic administration studies with chemotherapeutic agents commonly used to treat STS, including doxorubicin, docetaxel and gemcitabine. Immunohistochemistry for drug‐specific biomarkers and tumour growth measurement revealed tumour sensitivity to doxorubicin and docetaxel, whereas gemcitabine had no effect on tumour growth. Although many human PDX tumour models have been established, relatively few canine PDX models have been reported to date. CDX‐STS2 represents a new STS PDX research model of canine origin that will be useful in bridging preclinical research with clinical studies of STS in pet dogs.  相似文献

A study of 26 dogs (examined consecutively) with infiltrative subcutaneous neoplasms (mastocytoma, n = 11; soft tissue sarcoma, n = 13; and adenocarcinoma, n = 2) was conducted. Dogs were evaluated by physical examination, survey radiography, ultrasonography (US), and x-ray computed tomography (CT) prior to surgical excision of the tumor. The purpose of the evaluation was to accurately define gross neoplastic margins before surgical excision and to determine whether a difference could be observed between routine clinical staging (physical examination and survey radiography) and more detailed clinical staging (US and CT imaging). The clinical stage of 5 of 26 neoplasms assessed by US and of 17 of 26 neoplasms assessed by CT was determined to be more advanced because of previously undetected neoplasia, greater neoplastic size, or greater tissue invasiveness. Preoperative imaging of infiltrative subcutaneous neoplasms, using US and CT, is highly recommended to accurately determine gross neoplastic margins.  相似文献

Reasons for performing study: Detailed magnetic resonance imaging (MRI) and histological appearances of the proximal aspect of the suspensory ligament (PSL) in the forelimb of nonlame horses have not been previously documented. Objectives: 1) to describe detailed anatomy of the PSL, 2) describe high‐ and low‐field MRI and histological appearances of the PSL and surrounding structures in the forelimb of horses with no carpal or proximal metacarpal pain, 3) assess the relationship between age, breed, gender, height, bodyweight and MRI findings and 4) describe the histological appearance of the PSL and compare this with MRI findings. Methods: High‐ and low‐field MR images of the PSL and related structures from 30 cadaver limbs of nonlame horses were analysed subjectively and objectively. Univariable and multivariable linear regression analyses were used to assess the association of age, breed, gender, height and bodyweight with MRI findings. Histological and MRI findings of the PSL of 9 limbs were compared subjectively. Results: The collagenous tissue of the PSL had low to intermediate signal intensity depending on the pulse sequence. There was a large variation among horses in the amount, shape and signal intensity of the muscle and adipose tissue within the PSL. Comparison of MR images with histological slides revealed that the high signal intensity areas corresponded to adipose tissue and intermediate signal intensity areas to muscle tissue. The medial lobe of the PSL had a smaller cross sectional area (CSA) than the lateral lobe; there was a positive association between CSA of the PSL and both horse height and bodyweight (P<0.001). Conclusions and potential relevance: The large variability in the MRI appearance of the PSL in nonlame horses should be borne in mind when interpreting MR images of lame horses.  相似文献

Surgical excision is the foundation of treatment for early-stage solid tumors in man and companion animals. Complete excision with appropriate margins of surrounding tumor-free tissue is crucial to survival. Intraoperative imaging allows real-time visualization of tumors, assessment of surgical margins, and, potentially, lymph nodes and satellite metastatic lesions, allowing surgeons to perform complete tumor resections while sparing surrounding vital anatomic structures. This Review will focus on the use of near-infrared imaging and optical coherence tomography for intraoperative tumor visualization.  相似文献

With the recent advances in diagnostic imaging technology, cancer imaging in veterinary medicine has become more specific for disease diagnosis, more accurate for determining tumour margins and more sensitive for detecting metastatic disease. Ultrasound provides highly detailed images of parenchymal masses and infiltrative lesions while providing a means for aspiration or biopsy using real‐time image guidance. Computed tomography and magnetic resonance‐imaging techniques provide exquisite anatomical resolution that improves diagnostic accuracy, provides an accurate means of radiation or surgical treatment planning and a quantitative means for monitoring response to therapy. In addition to traditional anatomic imaging, new techniques are being developed for estimating functional parameters such as tumour perfusion, cell metabolism and gene expression. While conventional planar scintigraphy has been available for some time, newer nuclear imaging modalities such as positron emission tomography promise to further improve the accuracy of initial tumour diagnosis and staging and determination of response to therapy. Although many of these functional techniques are not yet clinically available, it is highly likely that some will be integrated into routine clinical practice in the near future.  相似文献

Although MRI has become widely used in small animal practice, little is known about the validity of advanced MRI techniques such as diffusion‐weighted imaging and diffusion tensor imaging. The aim of this retrospective analytical observational study was to investigate the characteristics of diffusion parameters, that is the apparent diffusion coefficient and fractional anisotropy, in dogs with a solitary intracranial meningioma or histiocytic sarcoma. Dogs were included based on the performance of diffusion MRI and histological confirmation. Statistical analyses were performed to compare apparent diffusion coefficient and fractional anisotropy for the two types of tumor in the intra‐ and peritumoral regions. Eleven cases with meningioma and six with histiocytic sarcoma satisfied the inclusion criteria. Significant differences in apparent diffusion coefficient value (× 10?3 mm2/s) between meningioma vs. histiocytic sarcoma were recognized in intratumoral small (1.07 vs. 0.76) and large (1.04 vs. 0.77) regions of interest, in the peritumoral margin (0.93 vs. 1.08), and in the T2 high region (1.21 vs. 1.41). Significant differences in fractional anisotropy values were found in the peritumoral margin (0.29 vs. 0.24) and the T2 high region (0.24 vs. 0.17). The current study identified differences in measurements of apparent diffusion coefficient and fractional anisotropy for meningioma and histiocytic sarcoma in a small sample of dogs. In addition, we observed that all cases of intracranial histiocytic sarcoma showed leptomeningeal enhancement and/or mass formation invading into the sulci in the contrast study. Future studies are needed to determine the sensitivity of these imaging characteristics for differentiating between these tumor types.  相似文献

The purpose of this study was to develop an optimized imaging protocol for canine dental imaging using single‐detector row CT and multidetector row CT. Two medium‐sized mesaticephalic cadaver dog heads were scanned using varying slice thickness, sequential vs. helical scanning mode, sequential CT table increment, pitch, image reconstruction algorithm, and an additional moderate edge enhancement filter. For each series, two reviewers scored dental roots as not visible (score: 0), partially visible (score: 1) or completely visible (score: 2). Sharpness of the dento‐alveolar margin was scored as blurry (score: 0), sharp (score: 1) or very sharp (score: 2). Consensus reader scores were compared among CT protocols. For single‐detector row CT, complete visualization of all tooth roots was only achieved with 1 mm slice thickness and image interval sequential or helical series (pitch of 1 or 2) using a high‐frequency image reconstruction algorithm with or without additional edge enhancement. For four‐detector row CT, complete visibility of all tooth roots was recorded in all series. For single‐detector row CT, all medium frequency algorithm series yielded blurry margin scores and all other series yielded sharp margin scores. For four‐detector row CT, sequential and helical 0.5 mm thickness images yielded very sharp dento‐alveolar margin scores, whereas the 1 mm series yielded only sharp margin scores. Authors concluded that the optimal dental imaging protocol was a sequential mode 1 mm slice thickness and interval with a high‐frequency image reconstruction algorithm and an additional moderate edge enhancement filter.  相似文献
