Intra‐abdominal hypertension (IAH) may lead to a multiple organ dysfunction syndrome associated with significant dysfunction of the cardiovascular, respiratory, renal, gastrointestinal and central nervous systems of human patients. A recent prospective multicentre epidemiological investigation in man concluded that IAH was associated with an increased risk of mortality in critically ill patients. In this review, we present current information pertaining to the potential clinical importance of IAH in the context of equine clinical practice. In conclusion, consideration of intra‐abdominal pressure should be a part of the clinical assessment of patient well‐being in critically ill equine patients.  相似文献

Reasons for performing study: Standing laparoscopic procedures, facilitated by abdominal insufflation with carbon dioxide, are being employed to an increasingly greater extent in horses. However, a sustained increase in abdominal pressure may be life‐threatening. A practical method for intra‐abdominal pressure (IAP) assessment is imperative. Although indirect methods for estimating IAP have been extensively studied in man, little work has been performed in veterinary medicine. Objectives: To investigate the utility of gastric manometry for purposes of evaluating IAP in horses. Methods: Gastric pressure (Pga) was estimated by balloon manometry in 8 healthy, mature horses, before and during a 30 min passive pneumoperitoneum induced by right paralumbar puncture. The balloon manometer was positioned within the gastric lumen and inflated using 2 separate volumes of air: 10 and 50 ml. Pga Gastric pressure was determined at baseline (0) and 5, 15 and 30 min after induction of passive pneumoperitoneum. Intra‐abdominal pressure was measured directly by right paralumbar puncture using an 8 gauge needle at baseline and immediately following establishment of passive pneumoperitoneum. Results: Baseline IAP values were negative and increased (P≤0.05) during development of passive pneumoperitoneum. However, recorded Pga measurements for both inflation volumes were positive before (baseline) and during the course of the passive pneumoperitoneum. Measured Pga values did not correlate with IAP at any time. Conclusions and potential relevance: Our results suggest that the indirect method used in human patients for estimating IAP by Pga is not applicable for horses.  相似文献

Intra‐abdominal haemorrhage in horses can be due to rupture of the spleen and may result in severe blood loss. The assessment and resuscitation of a foal that collapsed due to intra‐abdominal haemorrhage is presented. Diagnosis of the source of the blood loss required exploratory laparotomy and confirmed splenic capsular rupture and haemorrhage. Splenectomy using a 17th rib resection approach was used to control the haemorrhage. Complications included acute cardiac muscle damage, which was monitored using echocardiography and troponin I levels. The successful surgical management of the ongoing blood loss in this foal suggests that splenectomy should be considered an option to control severe blood loss due to splenic rupture.  相似文献

ObjectiveTo evaluate the influence of fentanyl on intra-abdominal pressures in spontaneously breathing dogs during capnoperitoneum.Study designProspective clinical study.AnimalsEleven healthy client-owned and five healthy experimental dogs undergoing laparoscopy.MethodsDogs were premedicated with acepromazine (0.03 mg kg?1 IV) and carprofen (4 mg kg?1 IV). Anaesthesia was induced with propofol and maintained with isoflurane in oxygen. The abdomen was insufflated with CO2 (11–16 cm H2O). Intra-abdominal pressures were measured with a transducer. Respiratory variables were measured with a spirometry sensor and side-stream capnography. Following preparation, fentanyl (1 μg kg?1) was injected over 30 seconds IV. Data were recorded 5 minutes before, during and 5 minutes after treatment. The following time points were selected for statistical analysis (anova, p < 0.05): ?160, ?140, ?120, ?100, ?80, ?60, ?40, ?20, 0, 30, 50, 70, 90, 110, 130 and 150 seconds after the start of fentanyl injection.ResultsIntra-abdominal pressure increased during inspiration in 15 dogs but decreased in one dog. Fentanyl treatment did not alter these patterns. Peak inspiratory and end-expiratory intra-abdominal pressures continuously decreased over time during the whole experiment and fentanyl exaggerated the decrease in inspiratory pressures but did not affect the rate of decrease in expiratory pressures. Differences between intra-abdominal pressures were stable before, but decreased after fentanyl administration from 4.1 ± 1.4 to 3.3 ± 1.2 cm H2O (at 0 and 150 seconds time points). End-tidal CO2 partial pressures increased from 6.0 ± 0.8 to 6.6 ± 0.9 kPa, respiratory rate decreased from 10.8 ± 2.6 to 7.8 ± 2.2 breaths per minute and tidal volume decreased from 13.7 ± 4.4 to 12.4 ± 2.9 mL kg?1 after fentanyl but these variables did not change before fentanyl treatment. Airway pressures did not change.Conclusions and clinical relevanceFentanyl did not increase intra-abdominal pressures in dogs.  相似文献

Objective – To establish a reference interval for intra‐abdominal pressure (IAP) measured by urinary bladder catheterization in normal cats and determine if IAP is affected by observer variation, volume of saline instillation before measurement, or subject variables of gender, positioning, body condition score, and sedation. Design – Prospective experimental study. Setting – Private referral center. Animals – Twenty healthy adult cats. Interventions – Sedation with butorphanol, midazolam, and propofol for catheterization of the urinary bladder and measurement of IAP. Measurements and Main Results – A 5‐Fr red rubber urinary catheter was placed under sedation, and IAP was determined using a water manometer with the cats in right lateral and sternal recumbency. Three readings were taken in each position by 2 observers. The cats were allowed to recover with the urinary catheter in place, and IAP was measured in each cat while they were awake in right lateral and sternal recumbency. Conclusions – In this population of clinically healthy cats, median (interquartile range) IAP taken over all measurements was 7.00 cm H2O (5.23–8.83 cm H2O). There was no statistical difference between observers or subject gender. Factors associated with a statistically significant increase in IAP were right lateral compared with sternal recumbency (P=0.002), being awake compared with sedated (P<0.001), having a higher body condition score (P=0.01 and 0.001), instillation of a higher volume of saline into the bladder for measurement (P<0.001), and struggling during awake measurements (P<0.001).  相似文献

ObjectiveTo measure trans-diaphragmatic pressures, as an indication of diaphragmatic contractility, in anaesthetized dogs breathing normally, or during inspiratory obstruction (Mueller’s manoeuvre) in order to assess if the method is practicable for use in clinical circumstances.Study designPilot study.AnimalsTwenty eight client-owned dogs, ASA I or II, 1–10 years old, 5–30 kg bodyweight, which required anaesthesia for surgery, and were to be positioned in lateral recumbency.MethodsFollowing a standardized regimen of premedication and anaesthetic induction, anaesthesia was stabilized and maintained with isoflurane. Two commercially available balloon catheters were introduced orally, and advanced, one into the stomach and one into the mid-third of the oesophagus. Oesophageal and gastric pressures were measured from these catheters, and trans-diaphragmatic pressure (Pdi) calculated and recorded continuously. At three separate time points during anaesthesia, for one breath, inspiration was obstructed (Mueller’s manoeuvre) and Pdi was measured.ResultsPlacement of the catheters in the stomach was not easy, and failed in three cases. In five dogs, their size resulted in failure of correct placement of catheters. Good traces of all three pressures reflecting respiratory cycles were obtained from 20 dogs. During normal spontaneous breathing (mean ± SD [range]) Pdi was 5 ± 2.1 (3–10) mmHg. During Mueller’s manoeuvre, Pdi was 14.6 ± 4.5 (9–21) mmHg. Abnormal waveforms were seen included cardiac oscillations (five dogs), inadequate intra-gastric pressure tracing (one dog), deflections with a double peak (one dog), and multiple artifacts when there was increased heart rate and tachypnoea (two dogs in response to surgery).Conclusions and clinical relevanceMeasurement of trans-diaphragmatic pressure with balloon catheters was practicable in suitably sized dogs anaesthetized for clinical purposes and might be a useful tool in the assessment of diaphragmatic function. A range of catheters are required if the technique is to work in all dogs.  相似文献

Hypertrophic osteopathy (HO) is a rare condition in the horse characterised by symmetrical proliferation of connective tissue and subperiosteal bone, along the diaphyses of limb bones. In this case, a 10‐year‐old Irish Draught cross Thoroughbred mare was presented for investigation of weight loss, lethargy and firm swelling of all 4 distal limbs. Radiographs of the distal limbs revealed palisade‐like periosteal new bone formation perpendicular to the cortex of the distal third metacarpal and metatarsal bones, proximal (P1) and mid (P2) phalanges. Rectal examination revealed a mass of approximately 20 x 30 cm in the right caudoventral abdomen. The mare was diagnosed with HO due to a granulosa thecal cell (GTC) tumour. The GTC tumour was removed and the diagnosis confirmed on histological examination. Clinical examination and radiographs of the distal limbs after surgery demonstrated reduction in size of the limb swellings and new bone formation. To the authors' knowledge this is the first report of HO due to a GTC tumour in a mare.  相似文献

Vascular alterations play important roles in many orthopedic diseases such as osteoarthritis, tendonitis, and synovitis in both human and equine athletes. Understanding these alterations could enhance diagnosis, prognosis, and treatment. Contrast‐enhanced ultrasound (CEUS) could be a valuable method for evaluation of blood flow and perfusion of these processes in the equine distal limb, however no reports were found describing feasibility or safety of the technique. The goal of this prospective, experimental study was to describe the feasibility and safety of distal limb CEUS in a sample of six horses. For each horse, CEUS of the distal limb was performed after intravenous injections of 5 and 10 ml, as well as intra‐arterial injections of 0.5 and 1 ml contrast medium. Vital parameters were monitored and CEUS images were assessed qualitatively and quantitatively for degree of contrast enhancement. None of the horses had clinically significant changes in their vital parameters after contrast medium injection. One horse had a transient increase in respiratory rate, and several horses had mild increases of systolic blood pressure of short duration after intravenous, but not after intra‐arterial injections. Intra‐arterial injection was possible in all horses and resulted in significantly improved contrast enhancement both quantitatively (P = 0.027) and qualitatively (P = 0.019). Findings from this study indicated that CEUS is a feasible and safe diagnostic test for evaluation of the equine distal limb. Future studies are needed to assess the clinical utility of this test for horses with musculoskeletal diseases.  相似文献

ObjectiveTo evaluate the peri-operative analgesic efficacy of intra-articular bupivacaine administered before or after stifle arthrotomy.Study designProspective, randomized, blind, placebo-controlled experimental trial.AnimalsThirty-nine healthy goats.MethodsThe goats were allocated randomly to one of three intra-articular treatment groups: group PRE (bupivacaine before and saline after surgery), group POST (saline before and bupivacaine after surgery) and group CON (saline before and after surgery). Anaesthesia was maintained with a constant end-tidal sevoflurane of 2.5%. Intra-operatively heart rate (HR), respiratory rate and mean arterial blood pressure (MAP) after critical surgical events (CSE) were recorded and compared with pre-incision values. Propofol requirements to maintain surgical anaesthesia were recorded. Flunixin was administered for 5 days. Post-operative pain assessment at 20 minutes, 2 hours, 4 hours after recovery and on day 2 and 3 included a multidimensional pain score (MPS), a lameness score and mechanical nociceptive threshold (MNT) testing. Rescue analgesia consisted of systemic opioids. Data were analysed using Kruskal–Wallis, Mann–Whitney, Friedman or chi-square tests as appropriate.ResultsIntra-operatively, group PRE had lower HR and MAP at several CSEs than groups POST/CON and required less propofol [0 mg kg?1 (0–0 mg kg?1)] than group POST/CON [0.3 mg kg?1 (0–0.6 mg kg?1)]. Post-operatively, group POST had significantly higher peri-articular MNTs than groups PRE and CON up to 4 hours after recovery. No treatment effect was detected for MPS, lameness scores and rescue analgesic consumption at any time point.Conclusions and clinical relevancePre-operative intra-articular bupivacaine provided notable intra-operative analgesia in goats undergoing stifle arthrotomy but did not reduce post-operative pain. Post-operative intra-articular bupivacaine provided a short lasting reduction of peri-articular hyperalgesia without affecting the requirements for systemic analgesia. Multimodal perioperative pain therapy is recommended to provide adequate analgesia for stifle arthrotomy in goats.  相似文献

Subject-based reference values have been largely overlooked in veterinary medicine. These values represent longitudinal data rather than the cross-sectional data represented by standard population-based reference values. As such they provide information about biological and analytical variation. Inherent random variation of analytes around a homeostatic set point is referred to as biological variation; data on biological variation are underutilized in veterinary medicine and have multiple applications that include setting analytical goals, predicting the utility of population-based reference intervals (RIs), assessing the value of partitioning reference values, and evaluating the significance of changes in serial results. To generate these data, relatively few individuals are sampled for a short period of time. Given the difficulty of obtaining specimens from large number of healthy individuals to establish a cross-sectional RI for many veterinary species, especially exotic species, use of subject-based RIs is a practical alternative approach for the veterinary diagnostician. Furthermore, for the majority of biochemical analytes and even many hemostatic variables, population-based reference values are less sensitive than subject-based reference values for detecting pathologic changes in an individual. The focus of this review is the clinical usefulness of subject-based reference values and diagnostic implications for their use. Implementation of the concepts of biological variation, individuality, and reference change value (RCV) may allow large diagnostic laboratories to offer more sensitive reference values to assess health and detect disease.  相似文献

Over the last few years there has been a nationwide cooperative effort to establish threshold concentrations and withdrawal time guidelines for corticosteroid use in racehorses. As dosing regimens are specific to individual horses and highly variable, it is not possible to establish regulatory guidelines for every dosing scenario and therefore they are often based on single dose administration studies. The goal of the study described here was to assess the applicability of current regulatory recommendations for intra‐articular corticosteroids based on clinical protocols used by practitioners. A total of 58 Thoroughbred and 82 Quarter Horse racehorses received varying doses of triamcinolone acetonide, methylprednisolone acetate, isoflupredone or betamethasone intra‐articularly in various joints by the treating practitioner. Blood samples were collected at 0, 7, 10, 14, 21, 28 and 35 days post drug administration and serum samples analysed by liquid chromatography mass spectrometry for quantitation of drug concentrations. Serum elimination varied depending upon the dose and the number and specific joints treated. Serum concentrations fell below the ARCI threshold guidance by Day 7 (100 pg/ml) for both triamcinolone acetonide (2–40 mg dose) and isoflupredone acetate (4–30 mg dose) and Day 21 (100 pg/ml) for methylprednisolone acetate (20–600 mg dose). Betamethasone fell below the regulatory threshold (10 pg/ml) by 7 days for all Quarter Horses and for 7/10 Thoroughbreds studied. Drug concentrations were below the regulatory threshold by Day 10 in the remaining 3 horses receiving betamethasone.  相似文献

ObjectiveTo describe the pharmacokinetics of intra-articularly (IA) administered morphine.Study designExperimental randomized, cross-over study.AnimalsEight adult healthy mixed breed horses aged 6.5 ± 2.3 (mean ± SD) years and weighing 535 ± 86 kg.MethodsUnilateral radiocarpal synovitis was induced by IA injection of 3 μg lipopolysaccharide (LPS) on two occasions (right and left radiocarpal joint, respectively) separated by a 3-week wash-out period. Treatments were administered 4 hours post-LPS-injection: Treatment IA; preservative free morphine IA (0.05 mg kg?1) plus saline intravenous (IV) and treatment IV; saline IA plus preservative free morphine IV (0.05 mg kg?1). Concentrations of morphine, morphine-3-glucuronide and morphine-6-glucuronide (M6G) were determined repeatedly in serum and synovial fluid (SF) by high-performance liquid chromatography mass spectrometry, at 2 and 4 hours and then at 4 hours intervals until 28 hours post-treatment.ResultsInjection of LPS elicited a marked and comparable synovitis in all LPS-injected radiocarpal joints. IA administered morphine was detectable in SF of all eight joints 24 hours post-treatment and in 6/8 joints 28 hours post-treatment. The terminal half-life of morphine in SF was estimated to be 2.6 hours. IA administration of morphine resulted in mean serum concentrations of morphine below 5 ng mL?1 from 2 to 28 hours after treatment.Conclusions and clinical relevanceIntra-articularly administered morphine remained within the joint for at least 24 hours. At the same time only very low serum concentrations of morphine and M6G were detected. The present results suggest that IA morphine at 0.05 mg kg?1 may be used for IA analgesia lasting at least 24 hours and give strong support to the theory that previously observed analgesic and anti-inflammatory effects of IA morphine in horses are most likely to be mediated peripherally.  相似文献
