Corneal diseases are very common in small animals. Corneal disease associated with bacterial agents is frequent in the dog and maybe less frequent in the cat. The medical history, important steps of the ophthalmic examination, and the ophthalmic diagnostic tests that are relevant in such corneal conditions are outlined. Bacterial corneal diseases in dogs and cats are most commonly considered in one of two categories--bacterial ulcerative keratitis and corneal abscesses. The clinical aspects of these two entities as well as the therapeutic strategies available for general practitioners and ophthalmologists are discussed. Ulcerative keratitis is frequent; it represents the most common ocular diseases in dogs and cats. Because some of these corneal ulcers can be very severe, progress rapidly, and therefore are sight threatening, the crucial steps of their diagnosis and management are stressed. The use of a magnification system, fluorescein dye, and corneal cytology and culture, if indicated, is necessary for diagnosis at an early stage of the disease. The treatment of bacterial ulcerative keratitis should eradicate the infection, reduce or stop the corneal destruction and support the corneal structures, control the uveal reaction and the pain associated with it, and minimize the scarring. The prognosis depends on the stage and the severity of the corneal ulceration, the etiology of the condition, and the therapeutic choice. A close follow-up of animals with corneal ulceration is highly recommended because corneal ulcers can progress rapidly.  相似文献

The cornea is naturally transparent. Anything that interferes with the cornea's stromal architecture, contributes to blood vessel migration, increases corneal pigmentation, or predisposes to corneal edema, disrupts the corneas transparency and indicates corneal disease. The color, location, and shape and pattern of a corneal lesion can help in determining the underlying cause for the disease. Corneal disease is typically divided into congenital or acquired disorders. Congenital disorders, such as corneal dermoids are rare in cats, whereas acquired corneal disease associated with nonulcerative or ulcerative keratitis is common. Primary ocular disease, such as tear film instability, adenexal disease (medial canthal entropion, lagophthalmus, eyelid agenesis), and herpes keratitis are associated with the majority of acquired corneal disease in cats. Proliferative/eosinophilic keratitis, acute bullous keratopathy, and Florida keratopathy are common feline nonulcerative disorders. Nonprogressive ulcerative disease in cats, such as chronic corneal epithelial defects and corneal sequestration are more common than progressive corneal ulcerations.  相似文献

Corneal disease is common in equine ophthalmology and requires vigilant monitoring and appropriate therapy to optimize the outcome.Many equine corneal diseases, particularly those that progress rapidly, may benefit from surgical intervention. These include descemetoceles, deep corneal lacerations and ulcers, corneal perforation/iris prolapse, ulcerative keratitis, corneal stromal abscesses, and corneoscleral neoplasia. Indications for corneal transplantation include optical, tectonic, therapeutic, and cosmetic purposes. Corneal transplantation is most often implemented in equine patients for tectonic and therapeutic reasons when a cornea is compromised by corneal stromal abscess, iris prolapse, or neoplasia.This article provides an outline of when to consider surgical intervention for corneal disease, the procedures available and expected outcomes, and how appropriate early surgical intervention can dramatically improve the end result.  相似文献

Superficial stromal keratitis or pannus is a syndrome of corneal, conjunctival and third eyelid inflammation. Superficial stromal keratitis mainly presents as a subepithelial corneal infiltration of vascular connective tissue, and usually arises from the lateral (temporal) limbal area. In some dogs perilimbal hyperaemia and third eyelid blepharitis can be present without corneal involvement. The most commonly affected breed of dog is the German Shepherd. Most cases of superficial stromal keratitis can be controlled with topical corticosteroids, and only rarely is cryosurgery or superficial keratectomy required to remove excessive pigment and or granulation tissue. The precise aetiology of SSK is unknown, but is likely to be multifactorial, with sunlight being a significant factor. Corneal lipidosis and keratoconjunctivitis sicca can occur secondary to superficial stromal keratitis.  相似文献

This study was performed to evaluate the effects of conditioned media (CM) from human amniotic epithelial cells (HAECs) on the corneal wound healing process. Eighteen rabbits (36 eyes) were used and randomly assigned to three groups according treatment: CM from HAECs (group 1), vehicle alone (group 2), and saline (group 3). Corneal alkali injuries were induced with 1 N sodium hydroxide. Each reagent used for treatment evaluation was injected into the dorsal bulbar subconjunctiva and the area of the corneal epithelial defect was measured every other day. Two animals from each group were euthanized at a time on days 3, 7, and 15, and the cornea was removed for histological examination. The sum of the epithelial defect areas measured on day 0 to day 6 as well as day 0 to day 14 in group 1 was significantly smaller than those of other groups. Histological examination revealed that the group 1 corneas had less inflammatory cell infiltration and showed more intact epithelial features compared to the other groups. These results suggest that CM from HAECs promote corneal wound healing in rabbits.  相似文献

The integrity and transparency of the cornea plays a key role in preserving vision. This paper reports a procedure to create an artificial sheet of corneal epithelium from cryopreserved limbal stem cells (LSCs) and to use this for corneal transplantation. Corneal LSCs were isolated from biopsy specimens of rabbit limbal lamellar and cryopreserved in liquid nitrogen at 2–4 passages. The cells were grown in culture medium for 12–14 days on top of a cell-free human amniotic membrane framed on a nitrocellulose sheet. The corneal epithelium generated was transplanted into the right eyes of 14 LSC deficient (LSCD) rabbits (seven experimental animals, seven controls) with corneal damage. The seven LSCD rabbits in the experimental group were transplanted with a corneal epithelial sheet generated from the cryopreserved corneal LSCs. Four LSCD rabbits were used as the vehicle control and were transplanted with a cell-free amniotic membrane, and the remaining three LSCD rabbits were negative controls without transplantation. Over a 2-month recovery period, 2/7 animals in the experimental group recovered completely, four recovered partially and one did not respond. In the control groups, three negative controls and three vehicle controls lost their vision completely, and one of the vehicle controls partially recovered transparency of the cornea Following treatment, corneal transparency of the experimental rabbits was significantly improved compared to controls (P < 0.05). The results indicated that cryopreserved corneal LSCs can repair damaged rabbit cornea, suggesting a possible new clinical approach to reconstruction of corneal epithelium.  相似文献

The purpose of this case report is to describe the clinical course and cytologic findings, treatment, and outcome of eosinophilic keratoconjunctivitis in two rabbits. Ophthalmic examination revealed ocular discharge, dacryocystitis, blepharitis, conjunctivitis, white conjunctival and corneal plaques, corneal vascularization, and stromal infiltration with different degrees of severity in each case. In case 2 there was also ulcerative disease of the cornea. Computerized tomography scan of the head, corneal biopsy for histopathologic examination with additional Luna and Giemsa stain were performed in case 2 and conjunctival as well as corneal specimens were obtained for bacteriologic culture and cytologic examination in case 1. Based on test results, a diagnosis of eosinophilic keratoconjunctivitis was made in case 2 and a tentative diagnosis of eosinophilic keratoconjunctivitis was made in case 1. Response to treatment with a topical steroid and topical cyclosporin was supportive of the diagnosis in both cases and shared many similarities with the response to treatment previously described in cats. Eosinophilic keratitis should be considered as part of a differential diagnosis list in rabbits with a history of keratitis.  相似文献

Corneal degeneration may occur with a deposition of lipids or calcium, or both. Calcareous and lipid degeneration may be either primary or secondary, associated with systemic diseases such as primary hyperlipidemia, hyperlipidemia associated with hyperadrenocorticism, and hypothyroidism. The authors report a case of bilateral corneal lipid and calcium degeneration in a 7-year-old female Poodle with hyperadrenocorticism. The condition worsened with Lysodren therapy but responded to surgical excision.  相似文献

Corneal fungal diseases, including fungal keratitis and stromal abscess, are uncommon in small animals. Ocular infection secondary to systemic mycosis is reported far more frequently. Suspicion of a fungal corneal ulcer should be raised based on a history of underlying trauma, especially with plant material, geographic location, chronic use of topical antibiotics or corticosteroids, or an extremely prolonged course of disease despite appropriate treatment. Clinical signs observed with fungal keratitis may include blepharospasm, epiphora, miosis, corneal opacity, and vascularization. Unfortunately, none of these signs is specific to fungal infection. If fungal keratitis is suspected or confirmed, then aggressive medical therapy should be instituted. Medications used include topical antifungals, parasympatholytics, anticollagenases, and antibacterials as well as systemic anti-inflammatory drugs. Because there are very few fungicidal medications, the course of medical treatment for fungal corneal disease requires a prolonged duration with frequent re-examination and assessment. Surgical treatment is sometimes required to save the eye and vision. Surgeries to be considered include debridement, conjunctival graft placement, and corneal transplantation.  相似文献

Purpose To study the effects of topical administration of 1% morphine on corneal analgesia in rabbits submitted to lamellar keratectomy and to assess the expression of matrix metalloproteinase‐1, metalloproteinase‐2, metalloproteinase‐9 (MMPs), type IV collagen, and interleukin‐10 (IL‐10) during the treatment. Methods Morphine group (MG) received 50 μL of topical 1% morphine four times daily, while the control group received saline instead. Corneal touch threshold (CTT) and the wound area were assessed until corneal healing. Corneal samples were processed for routine histology, immunohistochemistry, zymography, and ELISA. Results Following keratectomy, CTT increased significantly from 6 to 96 h time points. Mean corneal re‐epithelization rate and scores of leukocyte infiltration did not differ significantly between treatment groups. Immunolabeling pattern for MMP‐1, MMP‐9, and type IV collagen was similar in both treatment groups. In the MG, zymography indicated significantly higher levels of active MMP‐2 on days 6 and 12; and in the latent MMP‐9, on days 3 and 6, and in the active MMP‐9, on day 6. Latent MMP‐2 and MMP‐9, and active MMP‐9 decreased to values close to those of healthy corneas on day 12, but levels of active MMP‐2 remained significantly elevated in the MG. IL‐10 levels measured on days 1–6 were reduced as compared to those of healthy corneal tissue and returned to levels close to those of healthy corneas on day 12. Conclusion Topical morphine promoted corneal analgesia for up to 4 days and did not delay corneal re‐epithelization. The re‐establishment of MMPs and IL‐10 to levels close to baseline values at the end of the study and the expression of type IV collagen in both groups reinforce that, with caution, 1% morphine can be used after lamellar keratectomy in rabbits.  相似文献

A 10‐year‐old intact male Yorkshire Terrier was referred for investigation of a large raised and nonpainful corneal lesion oculus dexter. Clinical examination revealed a pale, translucent corneal mass, which occupied half of the corneal surface and measured 11 mm × 11 mm × 13 mm. The mass was removed by superficial keratectomy and equine amniotic membrane (AM) was transplanted into the large corneal defect to cover the wound and provide tectonic support for the remaining cornea. The mass was histologically confirmed as a corneal epithelial inclusion cyst. There was no evidence of recurrence or complication at the surgical site 100 days postoperatively. Corneal epithelial inclusion cysts are uncommon in dogs. Although superficial keratectomy is the recommended treatment for corneal inclusion cyst, the combination of superficial keratectomy and AM transplantation had to be considered as an alternative for repair of large corneal defects. This is the first case report of the combined application of AM and superficial keratectomy to successfully treat a corneal inclusion cyst in a dog.  相似文献

This article reviews conditions that primarily affect the corneal epithelium as distinct from corneal stromal diseases. Corneal ulceration is discussed elsewhere in this issue. The other corneal conditions include a variety of more subtle epithelial disease,which might colloquially be termed epitheliopathies, as well as uninfected indolent superficial ulcerations, corneal neoplasia, and eosinophilic keratitis. The fungal plaque is an unusual and somewhat chronic form of epithelial infection. Although less common than corneal ulcers, this collection of lesions may present uniquely challenging obstacles to diagnosis and identification of a cure. Newer therapies and surgical strategies are discussed.  相似文献

Dental disease is one of the most common reasons for pet rabbits to need veterinary treatment. The continual eruption and growth of the teeth predispose rabbits to dental problems. Any abnormality in the shape, position, or structure of the teeth interferes with normal wear and can lead to malocclusion and crowns that are no longer functional, and may grow into surrounding soft tissue causing pain and eating or grooming difficulties. Root elongation is a feature of dental disease in rabbits and can lead to a number of clinical problems such as epiphora, dacrocystitis, or abscesses. Repeated examination of rabbits with dental problems alongside skull radiography and visual examination of prepared skulls from affected cases has shown that the majority of rabbits with dental problems are suffering from progressive changes in the shape, structure, and position of the teeth. The nature of the abnormal dental changes suggests that underlying metabolic bone disease is a possible cause. This progressive syndrome of acquired dental disease can be staged. This article describes the clinical and pathological features of each stage of progressive syndrome of acquired dental disease and provides recommendations for the treatment of cheek teeth malocclusion.  相似文献

Corneal sensitivity in dogs with diabetes mellitus   总被引:5,自引:0,他引:5  
OBJECTIVE: To compare aesthesiometer-determined corneal sensitivity between diabetic and nondiabetic dogs and to investigate the correlation between corneal sensitivity and duration of diabetes or status of glycemic control, as estimated by use of glycated blood protein concentrations. ANIMALS: 23 diabetic and 29 nondiabetic normoglycemic dogs. PROCEDURE: A Cochet-Bonnet aesthesiometer was used to measure corneal touch threshold (CTT) in 5 corneal regions of each dog. At the time of ocular examination, duration of diabetes mellitus was estimated from the history, and blood was drawn for assessment of blood glycosylated hemoglobin and serum fructosamine concentrations. RESULTS: Median CTT for central, nasal, dorsal, temporal, and ventral corneal regions in nondiabetic dogs (1.6, 2.3, 2.8, 2.8, and 5.1 g/mm2, respectively) was significantly lower than in diabetic dogs (2.8, 4.0, 5.1, 5.1, and 6.6 g/mm2, respectively). Median regional CTT in diabetic dogs was not significantly correlated with estimated duration of diabetes mellitus or blood glycated protein concentrations. No significant difference was found in regional CTT between eyes of normoglycemic dogs with unilateral cataracts. CONCLUSIONS AND CLINICAL RELEVANCE: Diabetic dogs have significantly reduced corneal sensitivity in all regions, compared with nondiabetic normoglycemic dogs. Regional variation in corneal sensitivity is similar in diabetic and normoglycemic dogs. Neither glycemic control nor duration of diabetes, as estimated, is significantly correlated with corneal hyposensitivity. Corneal nerve dysfunction may be associated with recurrent or nonhealing ulcers in diabetic dogs for which no other underlying cause can be found.  相似文献

Corneal vascular neoplasms (hemangioma and hemangiosarcoma) are rare in all species. Reported cases are single case reports in a single species. Archived cases of corneal hemangioma and hemangiosarcoma from dogs, cats, and horses were obtained from the Comparative Ocular Pathology Lab of Wisconsin (COPLOW, Madison, WI), tabulated, and examined. This retrospective study describes the breeds, ages, tumor types, and characteristics of vascular neoplasms that appeared to be primarily corneal in location, in feline, canine, and equine patients, with gross and histologic images. There is a discussion of predisposing factors and speculated association with chronic ocular surface disease.  相似文献

As rabbits gain increasing popularity as house pets, more rabbit owners are seeking basic preventive veterinary care. Although a knowledge of the unique anatomic and physiologic characteristics of pet rabbits is essential, annual examination and review of diet and husbandry are as important as with any other species. Many disease states can be detected or prevented with routine veterinary care. Despite all the advances in diagnostic testing in veterinary medicine, there will never be a substitute for a complete physical examination.  相似文献

Ocular emergencies can be very intimidating for veterinarians. Most ocular emergencies can be stabilized by the veterinarian until an ophthalmologist can be consulted if necessary. Proptosis, or forward displacement of the globe, can occur secondary to any blunt trauma to the head. The two options for a proptosed eye are enucleation or replacement with tarsorrhaphy depending on the viability of the extraocular tissues and eye. Glaucoma, or increased intraocular pressure (above 35 mm Hg), is diagnosed by measuring the intraocular pressure using a Shiotz or electronic tonometer. Emergency treatment includes intravenous mannitol and topical pilocarpine. Uveitis, or inflammation of the iris, ciliary body and/or choroid, can have many underlying causes such as neoplasia, infection, lens induced, and trauma. The treatment consists of treating the underlying cause, and administering topical atropine to alleviate pain and topical corticosteroids to decrease inflammation. Corneal ulcers, or defects in the corneal epithelium, are classified according to the depth of the defect. The treatment consists of topical antibiotics and topical atropine. If the ulcer is deep, is melting, or is a descemetocele, then a third eyelid or conjunctival flap is usually necessary. The cornea should be sutured if it is perforated or lacerated. Hyphema, or blood in the anterior chamber, can occur secondary to trauma, neoplasia, infection, or a coagulapathy. The treatment consists of treating the underlying cause and topical corticosteroids with or without antibiotics.  相似文献

A four-year-old, 1·3-kg entire male Holland Lop pet rabbit (Oryctolagus cuniculus) was presented with a two-day history of bilateral epiphora and blepharospasm. Fluorescein staining revealed bilateral medio-ventral corneal ulceration. Initial topical treatment included ophthalmic broad-spectrum antibiotics and artificial tear solutions. Over a six-month period, antibiotics were changed based on corneal culture and sensitivity test results, and periodic corneal debridement was performed. With little to no improvement, and recurrence of the previous clinical signs, surgical intervention was considered but withheld because of individual pet consideration and owner's reluctance. Hydrophilic contact lenses were placed for corneal support but failed and caused a severe ocular response. Corneal collagen shields were placed bilaterally in order to promote corneal healing. Recheck examination performed seven days after placement revealed complete resolution of the ulcer in one eye and major reduction of the corneal ulcer in the contra-lateral eye.  相似文献
