An eight-year-old female Belgian shepherd dog was referred for investigation of chronic neck pain. The dog had sustained a pharyngeal injury 12 weeks previously while catching a stick. Radiographs of the cervical spine revealed signs consistent with a septic arthritis of the atlanto-occipital joint and osteomyelitis of both occipital condyles and the atlas. A foreign body was identified ultrasonographically in the retropharyngeal soft tissues, and a stick was surgically removed from a site ventral to the right side of the atlanto-occipital joint. The signs of neck pain started to resolve within a week of surgery.  相似文献

A 6-year-old Labrador retriever-cross was evaluated for an abnormal gait and head carriage 6 weeks after suffering trauma. The dog was presented with an ambulatory tetraparesis and was reluctant to move his head. Myelography and computed tomography demonstrated a subluxation of the atlanto-occipital joint with compression of the spinomedullary junction and the brain stem by the occipital bone. Removal of the compressive part of the occipital bone resulted in improvement of the clinical signs within 6 weeks, and resolution of clinical signs occurred 8 months after surgery.  相似文献

The purpose of this study was to evaluate ultrasonographic imaging as a means of easy and noninvasive diagnosis of articular and periarticular structures of the atlanto-occipital articulation in the horse; and to find a safe approach to the atlanto-occipital joint. Ultrasonographic investigations were performed on 6 healthy horses and 16 specimens from horses humanely destroyed age 2-17 years (mean 9.56 years). Preliminary examinations included anatomical studies and preparation of cross-sections. On 4 horse specimens, puncture of the atlanto-occipital joint under ultrasonographic guidance was performed and a new lateral approach undertaken. For ultrasonographic investigations, the neck was divided into 4 zones according to anatomical and clinical modalities: 1) insertion of the funiculus nuchae; 2) tuberculum dorsale of the atlas; 3) atlanto-occipital joint and 4) region above the foramen alare. Each zone was examined in transverse and longitudinal planes proceeding from cranial to caudal. Ultrasonography was found to be useful for soft tissue imaging of this region and was performed down to the atlanto-occipital joint of which the joint surfaces, joint capsule and collateral ligament could be delineated in both planes. We conclude that ultrasonography of the neck is a useful technique, representing an easy to use and safe method. However, further studies are indicated which should be performed on live subjects.  相似文献

Attempted stabilization of open, distal radius and ulnar fractures in a 3-year-old German shepherd dog using intramedullary pins and a Schroeder-Thomas splint resulted in malalignment of the limb and osteomyelitis. A double hook plate was used to rigidly stabilize the distal radial fracture after anatomical realignment. An autogenous cancellous bone graft was used where a lateral architectural defect remained after reduction. Culture of the fracture site showed Staphylococcus sp., which responded to chloramphenicol therapy. Fracture union and resolution of osteomyelitis occurred by 9 weeks after surgery, and the dog had no lameness and a normal muscle mass 22 weeks after surgery. The double hook plate provided rigid internal fixation of the radial fracture, allowed a rapid return to function during osteosynthesis, and minimal interference of antebrachiocarpal joint function occurred.  相似文献

Endotracheal tubes manufactured from red rubber and polyvinyl chloride (PVC) were compared, using radiographic techniques, in dog cadavers, under in vitro conditions and in anaesthetised dogs (in vivo study). All endotracheal tubes were radiographed in 'neutral' and flexed positions and the percentage reduction in the ventrodorsal radiographical diameter of the tube was calculated. The red rubber tubes kinked completely in 40 per cent of the cadavers, 75 per cent of the in vitro study and 40 per cent of anaesthetised dogs. The PVC tubes kinked completely in only one case (in vivo study). However, these tubes had a mean reduction in diameter of 15 per cent in the dog cadaver study, 19 per cent in the in vitro study and 26 per cent in the in vivo study. During procedures in which the trachea is intubated and the atlanto-occipital joint of the patient must be flexed, PVC endotracheal tubes may be less prone to kinking than red rubber tubes.  相似文献

This Case Report describes a periarticular abscess and a suppurative arthritis of the atlanto-occipital joint in an adult horse. The horse showed a painful swelling localised on the atlanto-occipital region and ataxia in all 4 limbs. During hospitalisation, the horse developed recumbency, dysphagia, facial paralysis and seizures. Ultrasonography of the atlanto-occipital region enabled a tentative diagnosis to be made, which was later confirmed at autopsy. Culture of the abscess revealed a Staphylococcus aureus. However, the horse had a history of recurrent fever, hypertrophy and abscessation of the submandibular lymph nodes, neck pain and dyspnoea for 2 months, which was suggestive of strangles.  相似文献

Traumatic atlanto-occipital luxation was diagnosed in a cat that presented with right hemiparesis. The luxation was reduced by manipulation, and the head and neck immobilized for eight weeks with a splint formed from aluminum rod. Clinical neurologic recovery was complete.  相似文献

An 11-year-old male Pekingese was evaluated because of a history of head tilt. Neurologic examination revealed a right-sided head tilt, ataxia, scoliosis, and proprioceptive deficits. Diagnostic testing included magnetic resonance imaging (MRI) of the head and neck. After IV administration of gadopentetate dimeglumine, an extra-axial, highly contrast-enhanced mass in the brainstem, cerebellar herniation, and syringohydromyelia were detected via MRI. The dog was treated with corticosteroids and radiation therapy of the mass for 4 weeks (total dose, 42.5 Gy). Magnetic resonance imaging was repeated 9 weeks and 6 months after radiation therapy; compared with the initial findings, a reduction in the size of the brainstem mass was observed in both MRI scans. The third MRI scan also revealed a normal cerebellar shape, no evidence of herniation, and resolution of syringohydromyelia in the dog at that time. It is recommended that whenever syringohydromyelia is observed via MRI, a primary cause (cranial or caudal to the affected region) should be sought.  相似文献

A seven-month-old Rottweiler was referred for a diagnosed femoral neck fracture and a suspected vertebral fracture. The simplified neurological examination revealed an acute paraplegia. A discospondyilitis associated with a pathologic fracture of Th11 and a spinal compression, and a haematogenous epiphysitis of the femoral neck were diagnosed based on radiographic and computed tomographic examinations. The vertebral fracture was stabilised using screws and polymethylmetacrylate with gentamycin by a lateral intercostal approach. The dog was able to walk seven days later. A femoral head and neck ostectomy was performed two weeks later. Staphylococcus intermedius was isolated from both sites, which confirmed the diagnosis. The dog was treated with cephalexin (30 mg/kg/d) for six weeks. A telephone interview with owners indicated that no more clinical signs were present four months after the second surgery. Discospondylitis associated with haematogenous osteomyelitis has only been reported in two cases. Paraplegia is an unusual clinical presentation for discospondylitis. The lateral approach (which allowed a direct access to the infected site) and the technique (screws associated with antibiotic-impregnated cement) are unique for surgical treatment of discospondylitis. Although applying cement to an infected area may pose a risk, antibiotic-loaded cement is successfully used in the treatment of chronic osteomyelitis in humans.  相似文献

Six dogs were used for a radiological investigation of movement of the cervical vertebrae for comparison of range of motion, instantaneous centers of motion and patterns of flexion and extension with those determined for man. Five dogs were embalmed and one live dog was investigated under the influence of general anesthesia. All animals were adult with the exception of one of the embalmed animals. None of the animals exhibited any abnormalities of cervical vertebrae or intervertebral discs. It was demonstrated that range of motion of the dog cervical spine exceeds that of man, largely due to a greater flexion-extension range of the atlanto-occipital joint and of lateral flexion of the cranial and caudal cervical spine. Patterns of flexion and extension of the cranial and caudal cervical spine of the dog are similar to that of man with no differences in the location of centers for these movements. Other differences were primarily due to the organization of intervertebral articulations.
These relationships are discussed in regard to the significance of individual groups of cervical muscles, joint functions and influence of movement on the spinal cord.  相似文献

OBJECTIVE: To evaluate anatomic reduction and surgical stabilization of femoral capital physeal fractures in cats. DESIGN: Retrospective study. ANIMALS: 13 cats. PROCEDURE: Medical records of cats with unilateral or bilateral femoral capital physeal fractures evaluated from 1998 to 2002 were reviewed. Age and weight of cats at the time of surgery; breed; sex; concurrent injuries; severity of lameness before and 1, 2, 4, 6, and 8 weeks after surgery; the amount of fracture reduction achieved and number of Kirschner wires (K-wires) used; degree of degenerative joint disease of the hip joint and lysis of the femoral neck and head observed after surgery; whether K-wires were removed after surgery; and complications after surgery were evaluated. RESULTS: Thirteen cats with 16 capital physeal fractures were identified. There was significant improvement in the severity of clinical lameness in all cats from weeks 1 through 4 after surgery. There was no correlation between the scores of the individuals who evaluated radiographs for fracture reduction and placement of K-wires. CONCLUSIONS AND CLINICAL RELEVANCE: Results suggested that surgical stabilization and repair of femoral capital physeal fractures facilitate a short recovery period and a good prognosis for return to normal function in cats.  相似文献

Six cases of mechanical lameness involving the coxofemoral joint are presented. All dogs had a history of chronic rear leg pain and lameness, and decreased range of motion of the hip. On physical examination, ventral subluxation of the coxofemoral joint during extension was present in three dogs. A radiographic diagnosis of subluxation was made in five dogs. In each dog, surgical exploration of the area caudal to the coxofemoral joint revealed a fibrous mass between the lesser or third trochanters and the ischium, diagnosed as a nonneoplastic fibrous reaction by microscopic examination. The clinical, radiographic, and histopathologic signs were similar to previously described cases of myositis ossificans. Five dogs returned to normal function after excision of the soft tissue mass, and one dog was normal when lost to follow-up 4 weeks postoperatively.  相似文献

A 17-year-old mixed breed pony was presented with intermittent neck stiffness during regular training rides in the six months preceding its admission. All parameters were within the normal range, except for an increase in heart rate (48 bpm) during the examination. Concave areas were observed at the level between the atlas and axis vertebrae (C1-C2) on both the left and right sides of the neck. Radiographs were taken of the laterolateral, left ventrodorsal oblique, and right ventrodorsal oblique projections in the cranial cervical region. The images revealed severe structural disorders, including trauma with luxation of the atlantoaxial joint, a fusion of the atlas and axis vertebrae, and evidence of a false joint in the axis bone. Subsequent ultrasonography showed hyperechoic areas in the rectus capitis caudalis and obliquus capitis caudalis muscles. Neck mobility was improved after two weeks of stall rest, administration of oral phenylbutazone (4.4 mg/kg), and topical cold gel treatment. The overstretching of surrounding soft tissues accompanying chronic injury of cranial cervical area may result in neck stiffness in the aging pony. Radiographic and ultrasonographic findings were consistent with the presence of cranial cervical trauma leading to neck stiffness. These findings emphasized the essential role of imaging diagnostics in treating a pony with intermittent neck stiffness.  相似文献

A 6 yr old pregnant Yorkshire terrier bitch presented 62 days after mating with an acute history of vomiting and coughing. The owners also reported that the dog was polyuric and polydypsic for the last 2 weeks. Complete blood count, serum biochemistry, and urinalysis revealed hyperglycemia, ketonemia, ketonuria, and metabolic acidosis. Diabetic ketoacidosis was diagnosed and after emergency treatment, including fluid therapy, prophylactic antibiotics, and regular insulin, the bitch whelped six healthy normal puppies. Two weeks after treatment, the bitch was clinically normal with normal fructosamine levels. To the authors' knowledge, this is the first reported case of gestational diabetes mellitus in a small breed dog.  相似文献

A five-year old, spayed female, Bearded Collie was presented with a 24-hour history of non-weight-bearing lameness of the right thoracic limb after sustaining vehicular trauma. Radiographs revealed a craniolateral scapulohumeral luxation and a distally and medially displaced fracture of the lesser tubercle of the humerus. Open reduction and internal fixation of the fracture was achieved with lag screw fixation and an anti-rotational Kirschner wire. Surgical repair resulted in compression across the fracture line, anatomic reduction of the articular surface, and a stable scapulohumeral joint following reduction of the humeral head in the glenoid. Six weeks postoperatively, the patient exhibited no evidence of pain or lameness on the right thoracic limb and radiographs revealed complete healing of the fracture and normal articulation of the scapulohumeral joint. This is the first report of a lesser tubercle fracture associated with a craniolateral shoulder luxation. Surgical intervention resulted in the return of full shoulder joint function in this dog.  相似文献

OBJECTIVE: Short-term, in vivo evaluation of a total-elbow arthroplasty (TEA) system in normal dogs. STUDY DESIGN: Prospective evaluation comparing pre- and postoperative findings. ANIMALS: Six normal, skeletally mature, large-breed dogs. METHODS: Physical, radiographic, and force-plate gait examinations were performed on all dogs before surgery. TEA was performed in the dogs using a canine TEA system. Examinations were repeated every 8 weeks for 24 weeks, with an additional examination at 52 weeks. Pre- and postoperative findings were compared. RESULTS: The TEA led to an excellent outcome in 3 of 6 dogs. Force-plate gait examination found that the dogs continued to improve over time and had a peak vertical force (PVF) in the surgical limb that was 99.6% of normal (range, 95.8% to 106.4%) 52 weeks after surgery. Major problems encountered during the postoperative time period were non-weight-bearing lameness (1 dog), osteomyelitis (1 dog), and fracture of the ulna (1 dog). CONCLUSIONS: TEA can be successfully performed in dogs. CLINICAL RELEVANCE: Based on 1-year data, TEA can be successfully performed in dogs and could be considered as a treatment alternative for adult dogs with severe osteoarthritis and lameness of the elbow joint.  相似文献

An external fixator consisting of two Ellis pins connected by a flexible band was developed and evaluated as a treatment for craniodorsal coxofemoral luxations in dogs. The technique for closed application of the fixator without injury to the coxofemoral joint or sciatic nerve was developed in six dog cadavers. The coxofemoral joints were then surgically destabilized and the limbs were manipulated through a full range of motion to assess the efficacy of the fixator in maintaining joint reduction. The fixator maintained joint reduction and stability after a surgically created craniodorsal luxation except when the femur was externally rotated 90d?. A flexible external fixator was then applied unilaterally in four healthy dogs. The dogs tolerated the fixator well and were bearing weight on the limb within 2 days after surgery; the range of motion was not limited by the fixator. The efficacy of a flexible external fixator in maintaining joint reduction after craniodorsal coxofemoral luxation was then evaluated in eight large dogs. The right coxofemoral joint in each dog was luxated surgically by removal of the dorsal joint capsule and transection of the ligament of the head of the femur and deep gluteal muscle. The joint was reduced and the fixator pins were applied in a closed fashion. In four dogs, a flexible external band was applied to the pins. Luxation did not reccur in these four dogs. The bands were not applied initially in four control dogs. Luxation occurred in three of the four control dogs within 24 hours of surgery. The joints that luxated were reduced and the flexible bands applied. Luxation did not recur after the bands were in place. The dogs tolerated the external fixators well, were bearing weight within 2 days of surgery, and walking with only minimal lameness 5 days after surgery. Luxation of the coxofemoral joints did not occur during the 2-week period in which the fixators were in place. The joints remained stable 1 week after removal of the fixators, at which time the dogs were euthanatized. Necropsy evaluation identified inflammation surrounding the pins and fibrous thickening of the dorsal joint capsule. The flexible external fixators were applied closed, maintained reduction of the coxofemoral joint after replacement of a craniodorsal luxation, and allowed weight bearing and limb usage soon after surgery. The flexible external fixator has several advantages over other methods of treating craniodorsal coxofemoral luxations. Complications noted in this study included pin tract drainage, pin loosening, and disruption of the flexible bands.  相似文献

A two‐year‐old Rottweiler presented for acute onset of a right hindlimb lameness 20 weeks after a cementless total hip replacement (THR) and 16 weeks after open reduction to address luxation of the THR. Radiographs revealed periosteal proliferation of the medial acetabulum and a stable implant. Synovial fluid cytology was consistent with inflammatory joint fluid. Treatment consisted of surgical debridement and intravenous and oral antibiotics. THR implants were not removed. Culture of tissue removed from the THR site yielded growth of Pseudomonas and Staphylococcus species. Lameness resolved 2 months after surgery. Twenty months after surgery, the dog was exercising normally with no clinical lameness and pelvic radiographs revealed no evidence of implant loosening and markedly decreased periosteal reaction. To the authors’ knowledge, this is the first report of an infected THR site successfully treated without prosthesis explantation in the dog.  相似文献

A biomechanical analysis of the results of an over-the-top procedure for replacement of the cranial cruciate ligament (CCL) in the dog is presented. Using 15 adult mongrel dogs, the CCL in one stifle joint was reconstructed using fascia lata and the lateral one-third of the patellar ligament. The opposite CCL served as the control. Animals were sacrificed at 0, 4, 12 and 26 weeks postoperation and axial failure tests were performed. Stiffness, maximum load, and elastic modulus of the replacement increased over time, while elongation to maximum load continually decreased as compared to controls. Other parameters showed less consistent trends.
The results are encouraging given the reduction in joint laxity and the increases in tissue stiffness and strength. However, the ligament substitute was still unable to replicate the mechanical properties of the normal cruciate ligament. Longer studies therefore are required to determine if this replacement is capable of completely restoring joint stability and normal function.  相似文献
