REASONS FOR PERFORMING STUDY: The reliability of diagnoses of obstructive conditions of the upper respiratory tract (URT) based on examinations performed at rest vs. at exercise is controversial. OBJECTIVE: To compare diagnosis of URT by endoscopy at rest with that achieved during high-speed treadmill exercise (HSTE). HYPOTHESIS: Endoscopy of URT at rest, when performed in isolation from other simpler techniques is unreliable in the prediction of dynamic respiratory obstructions. METHODS: Endoscopic findings of 600 Thoroughbred racehorses during quiet breathing were compared with findings during high-speed treadmill exercise. Other parameters were also assessed for their specificity in diagnosis. RESULTS: Endoscopy of the resting horse showed low sensitivity (0.15) in the diagnosis of dorsal displacement of the soft palate (DDSP) and palatal instability (PI). When endoscopy and reported noises were taken together there was still a 35% misdiagnosis rate. Although there was significant association between resting laryngeal function score (LFS) and dynamic vocal cord and/or arytenoid cartilage collapse at exercise, 19% of horses with a grade 4/5 LFS were able to attain and maintain full abduction during exercise and 7% of those with 'normal' grades 1 or 2 LFS at rest showed dynamic laryngeal collapse when exerted. Sensitivity of the diagnostic model was greatly increased (80%) when a history of inspiratory noise and palpable intrinsic muscle atrophy were included. CONCLUSIONS AND POTENTIAL RELEVANCE: Endoscopy of the upper respiratory tract of static horses is unreliable in the diagnosis of dynamic obstructions of the URT and should not be used in isolation in surgical decision-making or in the assessment of horses at the time of sale.  相似文献

Upper GI endoscopy is a frequently performed endoscopic procedure that is particularly suited for the diagnosis of upper GI diseases with a luminal or mucosal location. Contraindications are few and complications are rare. Procedures that can be performed during upper GI endoscopy include mucosal biopsy, brush cytology, aspiration of duodenal fluid for culture and cytology, foreign body retrieval, bougienage of strictures, and endoscopic placement of gastrostomy tubes. Unresolved issues that face veterinary endoscopists include the interrelationship between contrast radiology and endoscopy, the advisability of routine combination of upper and lower GI endoscopy, the reasons for discordance among clinical signs, endoscopic appearance, and biopsy findings, and the role of post-treatment biopsies.  相似文献

Gastrointestinal Lymphoma in 20 Dogs   总被引:1,自引:1,他引:0  
The records of 20 dogs with histopathologically diagnosed gastrointestinal (GI) lymphoma (LSA) evaluated between 1970 and 1984 were reviewed. Fifteen dogs were considered to have primary GI LSA, while five dogs had GI involvement in association with the multicentric form. Most clinical and laboratory findings were nonspecific, but positive-contrast upper GI radiography was suggestive of GI LSA in all of the dogs evaluated. Nine dogs had extensive lymphocytic-plasmacytic inflammatory infiltrates around the neoplastic foci, resulting in difficulty in obtaining a diagnosis of GI LSA when samples were obtained by endoscopy.  相似文献

The usefulness of endoscopic examination for the diagnosis and treatment of chronic enteritis in dogs was determined in this study. It was demonstrated, based on a clinical case, that endoscopy is essential for IBD diagnosis in dogs. Endoscopy also helps to estimate the efficiency of the treatment strategy applied. Nonspecific clinical examination results as well as a predominating lack of deviation in laboratory parameters make endoscopy the basic examination technique in mild and moderately advanced forms of chronic enteritis in the dog.  相似文献

Endoscopy in the field of buiatrics has gained growing importance in recent years. Parts of the upper respiratory and gastrointestinal tract, the urethra and urinary bladder, the abdomen and the teat are all accessible by means of endoscopy. The direct visualisation of inner or outer organ surfaces allows for rapid diagnosis and even prognosis of different pathological conditions. Flexible endoscopes of different dimensions are used for endoscopic examination of the upper respiratory tract. Examining this region, endoscopy is in some cases superior to other imaging techniques because of the direct visualization of surfaces and their pathological conditions and the possibility of collecting biopsies under visual control. The same is documented for diseases of the oesophagus. On the other hand, rumenoscopy is of experimental character so far. Urethrocystoscopy is mainly performed for diagnostic purposes with either flexible or rigid optics. Theloscopy represents the "gold standard" for surgical intervention in cases of disturbance of milk flow, since mucosal tissue can be removed under visual control. Therefore, this indication is one of the most important applications for endoscopy performed by practitioners. Another important field for the application of therapeutic endoscopy is the treatment of left displaced abomasum by repositioning and fixation to the abdominal wall under endoscopic control.  相似文献

The findings of ultrasonography of the gastrointestinal (GI) tract of 265 dogs with GI disorders were analysed retrospectively. The sonographic changes associated with various inflammatory and neoplastic conditions and mechanical obstruction of the GI system were recorded and discussed. Sonographic alterations of the pancreas and the tissues adjacent to the GI tract were also included in the study. Ultrasonographic alterations of the GI tract were classified into three main categories: thickening of the GI wall, changes in peristalsis and dilation of the lumen. Localised thickening of the GI wall with disruption of its structure was caused by both neoplastic diseases and by inflammatory disorders. However, diffuse thickening with retained wall structure was generally associated with inflammatory diseases. The criteria previously established for the ultrasonographic diagnosis of intestinal obstruction were successfully applied to a large number of GI disorders. Pancreatitis was most often associted with hyperchoic mesentery and hypoechoic pancreas mass, but similar alterations were encountered in some cases of gastric or duodenal ulceration. Except in cases of invaginations and intestinal obstructions, the observed ultrasonographic changes were not specific enough for a definitive diagnosis. Nevertheless, ultrasonography proved to be a valuable technique in the diagnostic process of GI disorders of the dog.  相似文献

History and physical examination alone will most often provide conclusive evidence of urinary tract disorders in ruminants. Detailed examination of the urinary system is described, and examples of disease are presented. Ancillary testing may be indicated to confirm the diagnosis, direct treatment, or clarify the prognosis. PRACTICAL RELEVANCE: Feline idiopathic inflammatory bowel disease (IBD) denotes one form of chronic enteropathy that is immunologically mediated and characterized by persistent or recurrent gastrointestinal (GI) signs and histologic inflammation. Signs of vomiting, diarrhea and weight loss generally predominate, and mucosal inflammation may occur in any portion of the GI tract (especially the small intestine). Affected cats may also have concurrent inflammation in other organs, such as the pancreas and liver, which may impact clinical disease severity. CLINICAL CHALLENGES: The exact etiologies of this heterogeneous group of disorders have yet to be determined, though results from basic science and clinical studies suggest that interplay between genetic factors and enteric bacteria is crucial for disease development. The diagnosis is one of exclusion and requires intestinal mucosal biopsy to characterize the type and severity of the inflammatory infiltrate, and to differentiate IBD from other disorders, including alimentary lymphoma. Controversy exists concerning the relative diagnostic accuracy of endoscopic versus full-thickness specimens for the diagnosis of IBD and its differentiation from alimentary lymphoma. AUDIENCE: This article is intended to provide veterinary practitioners with a comprehensive clinical update on idiopathic IBD in cats. It reviews the current evidence-based data, the diagnostic approach, the evolving histologic criteria, and treatment options and outcome for feline patients with this syndrome.  相似文献

Intraoral endoscopy is used as a standard diagnostic technique at the reporting clinics in horses presented with stomatological problems. Endoscopy of the oral cavity was performed using a chip camera, camera control unit, xenon cold light source and a rigid endoscope. In a three year period (2002 to 2004), second instars of Gasterophilus spp. were diagnosed in 14 warmblood horses 2 to 17 years of age. All horses were presented with masticatory problems or problems accepting the bit. In one horse oesophageal obstruction occurred in the course of larval migration. Oral endoscopy can not only be used for the diagnosis of dental disease but also for detection of instars of Gasterophilus spp. in the interdental spaces of maxillary cheek teeth.  相似文献

Endoscopic examination of the upper gastrointestinal (GI) tract in dogs and cats is a relatively recent development. It was just over 20 years ago, in 1978, that the first series report of GI endoscopy in dogs and cats was presented. In those initial days, endoscopy was truly a novelty, and the primary uses for endoscopes were to retrieve foreign objects or visualize the esophagus and stomach. It was not until the mid-1980s that technology and training made gastroduodenoscopy standard practice. The evolution in endoscopic technology has also mirrored the evolution in computers and imaging modalities. The original fiberoptic endoscopes, although still available, are giving way to video endoscopy and digital imaging capabilities that provide spectacular images and an ability to generate hard copy images for medical records, teaching purposes, and research data that was previously not possible.  相似文献

SUMMARY: Endoscopy of the upper respiratory tract was performed in 100 horses during high speed treadmill exercise. Reasons for endoscopy were a history of an abnormal noise during exercise in 75 horses, poor performance in 17 horses and to evaluate the results of upper respiratory tract surgery in 8 horses. Of the 75 horses with a history of an abnormal noise during exercise the cause was determined in 67 (89%). Endoscopic abnormalities were detected at rest in 40 of these 75 horses (53%). In these 40 horses, a similar diagnosis as to the cause of the abnormal noise was made at rest and during exercise on the treadmill in 19 cases, while in the remaining 21 the endoscopic findings during exercise varied from that seen at rest. This included 3 horses in which a diagnosis was made at rest but no abnormalities were detected during exercise. Some of the findings during treadmill endoscopy included laryngeal dysfunction, grades 3, 4 and 5 (22 cases), dorsal displacement of the soft palate (20), epiglottic entrapment (8), epiglottic flutter (4), aryepiglottic fold flutter (4), pharyngeal collapse (3), arytenoiditis (3), vocal cord flutter (3), false nostril noise (2), pharyngeal lymphoid hyperplasia (2), soft palate haemorrhage (1) and positional arytenoid collapse (1). More than one abnormality was observed during exercise in 7 horses. A complete and correct diagnosis based on the resting endoscopy findings alone was made in 19 (25%) of these 75 cases. In the 17 horses examined because of poor performance, no abnormalities were detected during treadmill endoscopy that were not evident at rest. None of these 17 horses presented with a history of an abnormal respiratory noise, although one, diagnosed as having grade 4 laryngeal function at rest and exercise, did make a characteristic inspiratory noise during treadmill exercise. Eight horses were evaluated after surgery for correction of laryngeal hemiplegia, as the post-operative performance or the amount of respiratory noise present was considered unsatisfactory. Of these, 3 were found to have a satisfactory airway during exercise and other reasons for poor performance were detected; 3 had insufficient abduction; and 2 had intermittent dorsal displacement of the soft palate. Endoscopy of the upper respiratory tract was found to be a useful technique for evaluating the cause of abnormal respiratory noise in most cases. We concluded that treadmill endoscopy in horses presented for poor performance, without a history of an abnormal respiratory noise, was of little value. The technique, in conjunction with arterial blood gas measurements, was useful in determining the efficacy of surgical treatment of laryngeal hemiplegia.  相似文献

Endoscopy of the caudal reproductive tract, or post-uterine endoscopy, is a method of examining the mucosal aspect of the vagina and vestibule/vulva of the live bitch. In this study the post-uterine, or caudal, reproductive tracts of forty-five bitches of pedigree and mixed ancestry were examined endoscopically. The instrumentation and method of endoscopic examination are described. The endoscopic appearance of the mucosal aspect of the vagina, vestibule and vulva was observed during anoestrus and several phases of the sexual cycle, and in the spayed bitch. Visual records made during endoscopy were compared with the exfoliative vaginal cytology and behavioural responses. The results obtained indicate that post-uterine endoscopy is a relatively rapid and accurate method of detecting the sexual status of the bitch, and compared well with the other methods used in this study. It is suggested post-uterine endoscopy also has a value in allied investigative procedures or reproductive processes.  相似文献

The aim of our retrospective study was to produce and analyse epidemiological data on gastrointestinal (GI) disorders of Austrian leisure horses. Case records of horses referred to the Equine Clinic of the Veterinary University of Vienna were collected from an electronic hospital information system using its search options. All cases reported to have suffered a GI disorder between March 2003 and February 2015 were selected. In this period of 4314 days, data of 1833 GI cases, mostly accompanied by colic signs, were analysed. The vast majority of our GI cases appeared in the abdominal parts of the GI tract 96% (1760/1833) while just 1.3% (24/1833) could be definitively attributed to oesophageal disorders, whereas in 2.7% (49/1833) of the cases a definitive diagnosis could not be made. The highest prevalence of GI disorders was seen in July and October/November. A breakdown of figures for cases clustered under the colic syndrome showed that colon impactions in left ventral large and/or pelvic flexure (35%) were the major problem followed by gastric disorders (18%), spasmodic colic (9%), enteritis (7%), caecal tympany (6%); caecal impaction (4%), colitis (3%), abnormal anatomical positions of large colon including complete and partial strangulation/volvulus, nephrosplenic entrapment of the left ventral and dorsal colon and right dorsal displacement of the large colon (3%) and small intestinal volvulus (2.7%). In 12% of the cases, the cause of the abdominal discomfort was likely idiopathic paralytic ileus. The searching tool of the electronic patient data also brought up cases with malabsorbtion/IBD syndrome and GI ruptures, both these contributed less than 1% of the referred GI cases. Results suggested that the causes of colic are diverse, but the major reasons for referred cases of colic in the east Austrian horse population are large colon impaction or displacements. Seasonal distribution and specific causes of gastrointestinal disorders should be considered essential components of the initial clinical assessment of horses presenting with colic.  相似文献

Exotic small mammal medicine is a relatively new specialty area within veterinary medicine. Ferrets, rabbits, and rodents have long been used as animal models in human medical research investigations, resulting in a body of basic anatomic and physiologic information that can be used by veterinarians treating these species. Unfortunately, there is a paucity of veterinary articles that describe clinical presentation, diagnosis, and treatment options of gastrointestinal (GI) disease as it affects exotic small mammals. Although there is little reference material relating to exotic small mammal GI disease, patients are commonly presented to veterinary hospitals with digestive tract disorders. This article provides the latest information available for GI disease in ferrets (Helicobacter mustelae gastritis, inflammatory bowel disease [IBD], GI lymphoma, systemic coronavirus, coccidiosis, and liver disease), rabbits (GI motility disorders, liver lobe torsion, astrovirus, and coccidiosis), guinea pigs (gastric dilatation volvulus [GDV]), rats (Taenia taeniaeformis), and hamsters (Clostridium difficile). Both noninfectious diseases and emerging infectious diseases are reviewed as well as the most up-to-date diagnostics and treatment options.  相似文献

Gastrointestinal (GI) disorders including a wide range of infectious, inflammatory, autoimmune, etc. disorders. Inflammatory bowel and celiac disease are non-fatal but overwhelming GI associated disorders. IBD and celiac’s complications, besides the great suffering, disturb the normal life of the patients and make them involved in mental and physical problems. The emerging role of genetic content is deniable for GI inflammatory disorders incidence, and long non-coding RNAs (lncRNAs) function is the recent topic for its association. Analyzing of absolute lncRNAs interference in GI inflammatory appearance remains in infancy, and more studies are requested. Here, we concisely performed a systematic review in the last knowledge up to 2020 to identify all of the significant lncRNAs associated with the initiation and progression of GI inflammatory diseases. Accordingly, this assay attempted to refer to the expression of lncRNAs changing from the normal state, discovery of genetic mechanisms, and main effectors that would trigger associated IBD and celiac expression and immune responses would be effective for therapeutic approaches. It could be useful for prognostic and diagnostic purposes of GI associated inflammatory disorders.  相似文献

Sinus disease.     
The diagnosis and treatment of diseases of the paranasal sinuses and conchae of horses are complicated by the large size of these structures, their complex anatomy, difficulties of access to them, and the advanced state of many diseases before diagnosis is made. Improved diagnostic methods include nuclear scintigraphy, computed tomography, and direct endoscopy of the sinuses. Treatment of some sinus diseases can be enhanced by access through direct sinus endoscopy for focal lesions, use of large frontal bone flaps for diffuse sinus lesions, standing surgery through a frontal flap for a variety of sinus disorders, and intralesional formalin for progressive ethmoidal hematomas.  相似文献

This is understandable, because it is a relatively noninvasive procedure in which a tremendous amount of diagnostic data may be obtained.
