OBJECTIVES: To evaluate the factors influencing the short-term (<14 days) outcome of thoracic surgery in dogs. METHODS: A retrospective review of 98 dogs undergoing thoracotomy over a five-year period was undertaken. RESULTS: A pre-operative diagnosis was achieved in 69 per cent of cases. Intrathoraic neoplasia had the lowest pre-operative diagnosis rate (5.5 per cent). Mortality rates of 21 per cent were recorded and were significantly higher for intrathoracic neoplasia (50 per cent) and significantly lower for persistent ductus arteriosus (7.4 per cent) and vascular ring anomaly (0 per cent). Median sternotomy was the preferred approach for pyothorax (85 per cent) and penetrating thoracic injuries (66 per cent). Intercostal thoracotomy was the preferred approach for all other diseases. Postoperative complications occurred in 39 per cent of cases. Wound complications were more common for pyothorax (45 per cent) and following median sternotomy (71 per cent). Thoracic drains were placed in 77 per cent of cases and complications were recorded in 23 per cent. Pyothorax and chylothorax had thoracic drains maintained for significantly longer periods of time. Longer thoracic drain duration was correlated significantly with increased complication rates. CLINICAL SIGNIFICANCE: The short-term outcome following thoracic surgery is influenced by diagnosis. The thoracic approach is determined by intrathoracic disease, but may influence outcome by affecting the incidence of postoperative wound complications. The risk of thoracic drain complications increases with drain duration, which is influenced by the underlying disease. Drains should be maintained for the minimal amount of time possible.  相似文献

Objectives : To assess complication rate, risk factors for complications and outcome in dogs with oesophageal and gastric endoscopic foreign body (FB) removal. Methods : Medical records of 102 dogs undergoing endoscopic removal of oesophageal and/or gastric FBs from March 2001 to November 2006 were retrospectively reviewed. All owners were contacted by telephone to provide follow‐up information. Results : West Highland white terriers, Yorkshire terriers and Bernese mountain dogs were over‐represented compared to the hospital population. Endoscopy alone was successful in 92/102 dogs (90·2 per cent), whereas gastrotomy (but no oesophagotomy) was required in 10 dogs (9·8 per cent). Complications in 13/102 dogs (12·7 per cent) were perforation (8), oesophageal stricture (1), oesophageal diverticula (1), perioesophageal abscess (1), pneumothorax and pleural effusion (1) and respiratory arrest (1). Six dogs (all weighing <10 kg) had complications resulting in death or euthanasia. Bone FBs, bodyweight of less than 10 kg, and oesophageal or gastric FB in place for more than three days were significant risk factors for complications. Of the dogs available for follow‐up (75/96), 92 per cent had no complications after discharge. Clinical Significance : Endoscopic FB removal is associated with a low overall complication rate with bone FBs and bodyweight of less than 10 kg as significant risk factors.  相似文献

Percutaneous sampling of one or more abdominal organs was performed under ultrasound guidance in 89 dogs and 16 cats. Tissue core samples were considered to be of diagnostic quality in 92 per cent of hepatic and 100 per cent of renal biopsies. Core biopsies were collected from the prostate in only three cases but each was adjudged of diagnostic quality. The technique allowed a definitive diagnosis or normal tissue to be confirmed in 65 per cent of animals undergoing hepatic biopsy, 83 per cent undergoing renal biopsy and 82 per cent undergoing prostatic biopsy or aspiration. Few complications occurred which could be ascribed with certainty to the procedure. One dog with extensive hepatic necrosis died one week after the biopsy, but necropsy examination was denied. One severely debilitated cat did not recover from general anaesthesia. One dog underwent aspiration of a sterile intraprostatic cyst, and developed prostatic abscessation one week later; prostatic carcinoma was subsequently found. Careful selection and preparation of patients for biopsy are essential. With this caveat, ultrasound-guided sampling of abdominal organs is a useful technique allowing a definitive diagnosis in a high proportion of cases. The technique is minimally invasive and the complication rate low.  相似文献

Fifty cases (36 dogs and 14 cats) with a confirmed diagnosis of pyothorax were evaluated from five referral institutions in the UK and Ireland. Aetiology, clinical presentation, diagnosis, treatment and outcome of all cases were examined. The underlying cause of pyothorax was determined in 18 per cent of cases. Positive bacteriological cultures of the pleural fluid were obtained in 68.7 per cent of the animals. Treatment modalities included surgery and medical management, involving thoracostomy tube placement, thoracic aspiration, thoracic lavage and antimicrobial therapy. A successful outcome was achieved in 86 per cent of patients.  相似文献

OBJECTIVES: To evaluate indications for and outcome of perineal urethrostomy in cats. METHODS: The medical records of 59 cats that had undergone perineal urethrostomy were evaluated. Short-term follow up information (for a period of four weeks following surgery) was available for all of the cats. Long-term follow up information (for a period of at least four months) was available for 39 cats. RESULTS: Early complications occurred in 25.4 per cent of cats and late complications were observed in 28.2 per cent of cats. The most frequent late complication was recurring bacterial urinary tract Infection. CLINICAL SIGNIFICANCE: Despite frequent complications and recurring signs of lower urinary tract disease, 32.2 per cent of the cats had a disease-free long-term outcome (mean four years, median 3.9 years), and 88.6 per cent of clients interviewed thought that their cats had a good quality of life after surgery.  相似文献

Background: Parenteral nutrition (PN) is increasingly used to support hospitalized dogs and cats. Published assessments of outcome are limited. Objective: Evaluate type and prevalence of complications and risk factors for death and complications in dogs and cats receiving PN. Animals: Three hundred and nineteen dogs and 112 cats that received PN at a teaching hospital between 2000 and 2008. Methods: Retrospective case review. Diagnosis, duration of PN administration, concurrent enteral feeding, death, and mechanical, septic, and metabolic complications were abstracted from medical records. Association of each parameter with complications and death was analyzed by binary logistic regression. Results: Pancreatitis was the most common diagnosis (109/319 dogs, 34/112 cats), and 137/319 dogs and 51/112 cats died. Dogs and cats received 113 ± 40% and 103 ± 32% of resting energy requirement, respectively. Mechanical (81/319 dogs, 16/112 cats) and septic (20/319 dogs, 6/112 cats) complications were not associated with death (P > .05). Hyperglycemia was the most common metabolic complication (96/158 dogs, 31/37 cats). Hypercreatininemia in dogs (8/79) was the only complication associated with death (P < .01). Chronic kidney disease in dogs, hepatic lipidosis in cats, and longer duration of inadequate caloric intake before PN in both species were negatively associated with survival (P < .05). Factors positively associated with survival included longer duration of PN administration in both species, enteral feeding in cats with any disease, and enteral feeding in dogs with respiratory disease (P < .05). Conclusions and Clinical Importance: PN can be effectively used to provide the energy requirements of most critically ill dogs and cats. Most complications accompanying PN administration do not affect survival.  相似文献

A new automated affinity chromatographic method for determining glycated haemoglobin (GHb) in dogs and cats was tested. The method appeared to be practical, quick and accurate. The reference range, calculated on the basis of 50 healthy dogs and 43 healthy cats, lay between 2·4 and 3·4 per cent in dogs and 2·0 and 2·9 per cent in cats. Concentrations were not influenced by age or gender. GHb levels obtained for 21 dogs and 18 cats with newly diagnosed diabetes mellitus were significantly higher than those of the control animals, ranging from 4·5 to 8·6 per cent (median 6·1) in dogs and from 2·7 to 6·0 per cent (median 3.8 per cent) in cats. The GHb levels in 31 normoglycaemic dogs with anaemia ranged from 2·3 to 4·3 per cent (median 3·3 per cent), and those of 22 normogiycaemic cats with anaemia from 2·6 to 3·9 per cent (median 3·2 per cent); both sets of levels were significantly elevated compared to control group values. GHb concentrations in animals with polycythaemia, azotaemia or liver disease showed no significant deviations from the control group; in individual cases they were slightly elevated compared to the reference range. The automated measuring method employed can be used to determine GHb in dogs and cats. Anaemic animals should generally be excluded from the GHb determination.  相似文献

The records of 25 dogs and 2 cats treated with peritoneal dialysis during an 11-year period were evaluated. The indications for peritoneal dialysis were acute renal failure in 21 animals, chronic renal failure in 5 animals, and azotemia of undetermined cause in 1 animal. Peritoneal dialysis resulted in a significant (P less than 0.05) decrease in serum urea nitrogen concentration in 19 of the dogs and a significant (P less than 0.05) decrease in serum creatinine in 20 dogs. The most common complication of peritoneal dialysis was hypoalbuminemia (11 animals affected). Other common complications were dialysate retention/catheter obstruction (8 animals), peritonitis (6 animals), hypochloremia (6 animals), and subcutaneous leakage of dialysate (6 animals). Twelve dogs and 2 cats died during treatment, 6 dogs were euthanatized, and 1 dog was lost to follow-up evaluation. The remaining 6 dogs survived and were discharged from the hospital after successful peritoneal dialysis. On the basis of the results of this study, the authors concluded that peritoneal dialysis, although associated with a high complication rate, was a successful technique for reducing azotemia in dogs with acute and chronic renal failure. Survival rates were poor because of the severity of the underlying renal diseases.  相似文献

O bjectives : The objective of this study was to evaluate the relationship between the volume of fluid being produced at the time of thoracostomy drain removal and the time to hospital discharge in dogs and cats.
M ethods : Records of 101 dogs and 26 cats with thoracostomy drains were reviewed. Three subgroups were created according to the reason for thoracostomy drain placement: P (postsurgical), A (air) and F (fluid). A generalised linear model with Poisson Errors was performed to test the relationship between the volume of fluid produced at the time of thoracostomy drain removal and the time to discharge. The volume of fluid produced and the time to discharge were compared between species and subgroups.
R esults : No significant relationship was found between the volume of fluid produced at the time of thoracostomy drain removal and the time to discharge in either species or between the time to discharge and the reason for thoracostomy drain placement. Animals with a volume of fluid higher than 2 ml/kg/day at the time of thoracostomy drain removal did not have increased hospitalisation times.
C linical S ignificance : Thoracostomy drain can be removed, without clinical compromise, when the volume of fluid produced exceeds 2 ml/kg/day. However, other clinical parameters must be taken into consideration.  相似文献

Three separate studies that collectively represent 107 dogs and 52 cats with supracondylar femoral fractures are presented. The animals were treated using a normograde intramedullary pinning technique at the Norwegian School of Veterinary Science during the period 1964 to 1993. The surgical technique involves insertion of one or two intramedullary pins normograde from the stifle joint without passing their proximal end proximal to the level of the femoral neck. The results of treatment were obtained for 73 dogs and 39 cats. Of these cases, 58 dogs (79.5 per cent) and 32 cats (82.1 per cent) were found to be free of lameness after the completion of treatment. Differences in the results of treatment, based on species or size, were not found. Of all cases in which the results of treatment were known (n = 112), complications were recorded in nine animals (8.0 per cent). Infection and instability were the most common complications, affecting four and three cases, respectively.  相似文献

Rectal swabs or faecal samples for the isolation of Campylobacter species were taken from 120 dogs and cats in an animal shelter in which only one kitten showed signs of gastrointestinal disease, and rectal swabs were taken from 46 dogs, 22 of which showed signs of gastrointestinal disease, in another shelter. At the first shelter, the swabs from 24 of 47 dogs (51.1 per cent) and 36 of 48 cats (75 per cent) yielded a Campylobacter species. The rate of isolation was significantly higher from dogs and cats less than six months old, and significantly higher from cats than from dogs (P< or =0.05). At the second shelter Campylobacter species were isolated from 40 of 46 dogs (87 per cent), but there was no significant difference between the age groups. Campylobacter species were isolated from 19 (86.4 per cent) of the 22 dogs with signs of gastrointestinal disease and from 21 (87.5 per cent) of the 24 unaffected dogs. Several culture methods were applied to the samples collected from both shelters, and the combination significantly increased the recovery of Campylobacter species.  相似文献

The purpose of this retrospective study was to evaluate the use of partial parenteral nutrition (PPN) in dogs and cats. The medical records of all dogs and cats receiving PPN between 1994 and 1999 were reviewed to determine signalment, reasons for use of PPN, duration of PPN administration, duration of hospitalization, complications, and mortality. Complications were classified as metabolic, mechanical, or septic. One hundred twenty-seven animals (80 dogs and 47 cats) were included in the study, accounting for 443 patient days of PPN. The most common underlying diseases were pancreatitis (n = 41), gastrointestinal disease (n = 33), and hepatic disease (n = 23). Median time of hospitalization before initiation of PPN was 2.8 days (range, 0.2-10.7 days). Median duration of PPN administration was 3.0 days (range, 0.3-8.8 days). Median duration of hospitalization was 7 days (range, 2-20 days). In the 127 animals receiving PPN, 72 complications occurred. These included metabolic (n = 43), mechanical (n = 25), and septic (n = 4) complications. The most common metabolic complication was hyperglycemia (n = 19), followed by lipemia (n = 17) and hyperbilirubinemia (n = 6). Most complications were mild and did not require discontinuation of PPN. Ninety-three (73.2%) of the 127 patients were discharged. All 4 animals with septic complications were discharged from the hospital. The presence, type, and number of complications did not impact the duration of hospitalization or outcome. However, animals that received supplemental enteral nutrition survived more often than those receiving PPN exclusively. Although PPN seems to be a relatively safe method of providing nutritional support, future studies are warranted to determine its efficacy.  相似文献

Objective: To determine reasons for conversion from diagnostic laparoscopic procedures to celiotomy in dogs and cats. Study Design: Case series. Animals: Dogs (n=54), cats (40). Methods: Medical records (2004–2008) were reviewed for dogs and cats that had diagnostic laparoscopic or laparoscopic‐assisted biopsy. Numbers of conversions to laparotomy were recorded, including cause and type (elective versus emergent), postoperative complications, and short‐term outcome. Specific risk factors for conversion, including signalment, preoperative diagnostics, and surgical findings were assessed; categorical variables were tested by χ2 and Fisher's exact tests; continuous variables by Student's t‐test and Wilcoxon's rank‐sum tests; multivariate logistic regression models were created. Results: Twenty animals (21%) had laparoscopic conversion; 13 (65%) were considered elective and 7 (35%) emergent conversions. There was no significant difference between animals requiring and those not requiring conversion for age, weight, sex, body condition score, clinical signs, previous abdominal surgery, or surgeon experience. Significant risk factors for conversion included low total solids (P=.03), presence of a solitary liver tumor (P<.01), and diagnosis of neoplasia (P<.01). Conclusions: A conversion rate of 21% was found in this population of dogs and cats undergoing laparoscopic diagnostic procedures. A preoperative finding of a solitary liver tumor, low total solids, and diagnosis of malignancy were all significant risk factors for conversion.  相似文献

classification scheme for sacral fractures was based on a review of sacral fractures in 34 dogs and 17 cats. They were classified into five categories based on their radiographic appearance on standard lateral and ventrodorsal radiographs of the pelvis. Concomitant pelvic injuries were commonly observed: 32 per cent of dogs had a fracture of an ilium and 65 per cent of cats had unior bilateral sacroiliac subluxation. Neurological deficits were common where the fractures traversed the spinal canal or sacral foraminae. Comparison is made between sacral fractures in dogs and cats, and humans.  相似文献

Objective: To identify preoperative risk factors associated with mortality before discharge in cats having a single or multiple ureterotomy procedures to treat a ureteral obstruction. Study Design: Case series. Animals: Cats (n=47). Methods: Data were obtained from the medical records (2002–2009) of cats that had undergone ureterolithotomy procedures. Multiple preoperative factors were evaluated for association of survival to discharge. Result: Survival to discharge after ureterolithotomy was 79% (37/47). Over 79% of cats were azotemic before surgery and 94% had chronic kidney disease changes at the time of ultrasonographic diagnosis. Six cats required an additional surgical procedure because of complications with ureterolithotomy. Overall prevalence of postoperative uroabdomen was 6% (3/47). On multivariate analysis, there were no preoperative variables significantly associated with survival to discharge. Conclusions: Ureterolithotomy in cats was associated with a 21% mortality rate before hospital discharge. No preoperative variables associated with mortality were identified; therefore, further studies are needed to identify more discriminating preoperative characteristics for mortality after ureterolithotomy in this population of cats.  相似文献

The surgical and medical records of 67 dogs and nine cats which underwent median sternotomy over a five-year period were reviewed. The indication for median sternotomy and the short and longer term complications were recorded. Twenty-six of the dogs died or were euthanased within 48 hours of the surgery as a consequence of the pre-existing disease or complications of the intrathoracic surgical procedure. A further four dogs were enthanased between 48 hours and 14 days following confirmation of neoplastic processes. Thirty-seven dogs were alive at 14 days; of these, seven dogs (19 per cent) experienced short-term wound complications, including haemorrhage, wound infection, thoracic limb neurological deficits and excessive postoperative discomfort. Of the 37 dogs alive for longer term follow-up, eight dogs (22 per cent) experienced wound complications, including haemorrhage, sternal fracture, sternal osteomyelitis and delayed wound healing. No complications were noted in the cats.  相似文献

Ringworm is an uncommon disease of dogs, but cats, especially the longhaired breeds, are more frequently infected. Of the several dermatophytes involved, Microsporum canis causes 94 per cent of ringworm in cats and 65 per cent in dogs. This species is highly contagious for man but the true incidence of human infection is unknown. Ringworm caused by M canis usually responds readily to treatment, but when the infection establishes in a colony of cats, eradication can be difficult and expensive. Other forms of ringworm in dogs and cats are caused by dermatophytes acquired from wild animals, mainly small rodents such as mice and voles. This is an uncommon and trivial disease of cats, but some infections in dogs can be remarkably persistent and difficult to resolve.  相似文献

Sixty-two dogs with laryngeal paralysis were presented over a three year period. Presenting clinical signs included inspiratory stridor (100 per cent), exercise intolerance or syncopal episodes (81 per cent), loss or alteration in phonation (48 per cent) and coughing or gagging when eating (29 per cent). Causes of the paralysis were determined as traumatic (two dogs), neoplastic (two dogs), iatrogenic (two dogs), congenital (one dog) and idiopathic (55 dogs). Unilateral arytenoid lateralisation was performed in all dogs. The perioperative complication rate was approximately 10 per cent, while the success rate as judged by owners one year postoperatively was greater than 90 per cent. The technique avoided many of the recorded complications of intralaryngeal surgery. Operative times were short and the requirement for postoperative monitoring was minimal. Increasing familiarity with the technique favourably influenced the incidence of complications and the success rate.  相似文献

OBJECTIVE: To assess use of thoracoscopy to determine causes of pleural effusion in dogs and cats. DESIGN: Retrospective study. ANIMALS: 15 dogs and 3 cats with pleural effusion. PROCEDURE: Medical records were reviewed from 1998 to 2001 for dogs and cats that had exploratory thoracoscopy, biopsy, and histologic analysis to determine the etiology of pleural effusion. Intraoperative and postoperative complications were recorded. Surgical biopsy specimens were evaluated for quantity and quality for providing a histologic diagnosis. RESULTS: Biopsy specimens were deemed adequate in quantity and quality to render a histologic diagnosis in all animals. Etiology of the effusion was neoplasia in 8 animals and non-neoplastic pleuritis in 10 animals. Median survival time of animals with neoplasia was 15 days, whereas those with inflammatory diseases had median survival time of > 785 days. Postoperative pneumothorax was encountered in 2 animals subsequent to pulmonary biopsy. No other major complications were recorded. CONCLUSIONS AND CLINICAL RELEVANCE: Thoracoscopy is a diagnostic option that provides excellent viewing of intrathoracic structures and adequate biopsy specimens with minimal complications. This technique provides a less invasive alternative to thoracotomy for evaluating the etiology of pleural effusion.  相似文献
