A 4-month-old domestic shorthair female cat weighing 1.3 kg was presented for evaluation of respiratory distress. The animal showed evident dyspnoea with exercise intolerance and a marked concave deformation of the sternum. After measurements of the fronto-sagittal and vertebral indexes, the pectus was classified as moderate and surgery was elected. Surgical correction was performed using an open approach to the sternum with osteotomy of the last sternebra and costochondral junctions of the eighth and ninth ribs bilaterally. A silicone based, U-shape external splint was manufactured and used to stabilise the sternum. Immediate and 5-week postsurgical radiographs revealed a decreased concavity of the sternum and an increase thoracic height at the level of the last sternebra. Postoperative results suggest that this technique could be an effective and economical option for cats with pectus excavatum with a non-compliant sternum.  相似文献

Two 4-week-old pups and a 7-week-old kitten with respiratory distress and pectus excavatum were treated by external application of a coaptation splint to the ventral aspect of the thorax. Sutures were placed percutaneously around the sternum and through predrilled holes in a piece of moldable splint material that had been contoured to fit a normal-shaped thorax. Correction of the sternal deformity and alleviation of respiratory distress were achieved in all 3 animals. Frontosagittal index and vertebral index were assessed before and after surgery. In all 3 animals, both indices were abnormal before surgery, but were within the normal range after surgery.  相似文献

Two sexually intact male Bengal cats, one a 4-month-old weighing 2.8 kg and the other, a 3-month-old weighing 2.0 kg, were presented to the University of Missouri-Columbia Veterinary Teaching Hospital for evaluation of respiratory distress. On initial presentation, both cats were dyspneic, exercise intolerant, and had marked concave deformation of the caudal sternum. Surgical correction of pectus excavatum was performed using a cylindrical external splint and U-shaped external splint. Post-operative thoracic radiography revealed that there was decreased concavity of the sternum and increased thoracic height at the level of the caudal sternebrae in both cats.  相似文献

A 13-year-old neutered female Labrador retriever had inspiratory dyspnea secondary to bilateral laryngeal paralysis. Radiographically, there was pectus excavatum with a mediastinal shift to the right. Arytenoid lateralization was performed, relieving the upper respiratory obstruction, and the sternal deformity also resolved. Chronic upper respiratory obstruction should be considered in dogs with pectus excavatum. Furthermore, correction of upper respiratory obstruction has the potential to result in resolution of pectus excavatum.  相似文献

Successful surgical repair of pectus excavatum deformities has so far only been reported in very young animals. This case report describes an alternative technique for repairing pectus excavatum in a young cat. The cat had shown moderate deformity with slight respiratory impairment when young and had experienced clinical problems with age. A sternum realignment technique involving a pin associated with an external splint was used. This alternative technique was a safe and efficient procedure in the five-month-old cat.  相似文献

An 11-year-old, spayed female American domestic shorthair cat was presented with open-mouth breathing and dyspnea due to respiratory tract disease. Radiographically, there was thickening of the retropharyngeal soft tissue and soft palate, focal pulmonary change, and severe pectus excavatum (PE), best seen on the left lateral view. Repeat thoracic radiographs 4 months later showed persistent PE, but only on the right lateral view; PE was absent on the left lateral view. The dynamic component of the observed PE was presumably acquired secondary to chronic upper respiratory disease and/or concurrent increased pliability of the thoracic wall from prolonged prednisolone treatment.  相似文献

This is the first published report of a dog with dynamic right ventricular outflow tract (infundibular) stenosis, right ventricular hypertrophy, and pectus excavatum. A juvenile dog presented with a grade V/VI left base systolic heart murmur, tachycardia, and pectus excavatum. Diagnosis of the aforementioned conditions was based on radiography, electrocardiography, and echocardiography. At 9 1/2 wk of age the heart murmur was no longer audible and the right ventricular stenosis and hypertrophy had dissipated and regressed, respectively. Resolution may be associated with growth of the dog. A good prognosis is foreseen.  相似文献

Clinical records made during routine vaccinations were compared between populations of domestic shorthair cats and Bengal kittens. An increased incidence (12/244) of thoracic wall deformity was detected amongst the Bengal kittens. Deformities detected were: pectus excavatum (five), unilateral thoracic wall concavity (six) and scoliosis (one). Five-generation pedigrees were analysed for the affected kittens that showed a high degree of common ancestry indicating the likelihood of a familial cause.  相似文献

OBJECTIVE: To establish an objective method of determining proventricular diameter in psittacine birds by assessment of lateral whole-body radiographic views. DESIGN: Retrospective case-control study. ANIMALS: 100 parrots with no signs of gastric disease and 19 parrots with signs of gastric disease. PROCEDURES: Measurements were obtained for the following variables: proventricular diameter at the level of the junction between the last thoracic vertebra and synsacrum, maximum distance between the dorsal serosa of the proximal aspect of the proventriculus and dorsal border of the sternum, maximum coelomic cavity height at the level of the proximal aspect of the proventriculus, and maximum dorsoventral height of the keel of the sternum. The ratio of proventricular diameter to each of those measurements was calculated and compared among species within the group without signs of gastric disease and between the gastric and nongastric disease groups. RESULTS: No significant differences were seen among species of parrots without signs of gastric disease for any ratio, but there were significant differences between parrots with gastric signs and those without gastric signs for all ratios. Only the proventricular diameterto-maximum dorsoventral height of the keel of the sternum ratio had no numeric overlap between groups. Sensitivity and specificity of the ratio for detection of proventricular enlargement were both 100%. Six causes associated with proventricular enlargement were identified. CONCLUSIONS AND CLINICAL RELEVANCE: Evaluation of the proventricular diameter-to-keel height ratio is a new method for evaluating proventricular size in psittacines. Ratio values < 0.48 indicate normal proventricular diameter and the absence of proventricular disease.  相似文献

The radiographic skeletal form and structure of all cats with mucopolysaccharidosis VI is described. Common manifestations included epiphyseal dysplasia, generalized osteoporosis, abnormal nasal turbinate development, his subluxation, impaired development of skeletal growth, pectus excavatum, hyoid hypoplasia, aplasia, hypoplasia and fragmentation or abnormal ossification of the dens, and aplasia or hypoplasia of frontal and sphenoid sinuses. The skeletal measurements of two affected cats were compared with those of normal, sex-matched littermates, and the measurements of two affected female cats were compared with those of a normal male littermate.  相似文献

A six-week-old female kitten presented for severe pectus excavatum. Surgical correction was performed. One week later the kitten developed dyspnoea and a systolic murmur was detected on thoracic auscultation. Pulmonary oedema and cardiomegaly were identified radiographically, and a left to right intracardiac shunt was visualized using non-selective angiocardiography. Clinical signs resolved spontaneously one month later. The kitten has been asymptomatic for 24 months.  相似文献

OBJECTIVE: To determine clinical signs, physical examination findings, radiographic features, and concurrent diseases in cats with laryngeal paralysis, as well as evaluate the outcome of medical or surgical management. DESIGN: Retrospective study. ANIMALS: 16 cats. PROCEDURE: Medical records from January 1990 to April 1999 were examined for cats with laryngeal paralysis. Signalment, clinical signs, physical examination findings, cervical and thoracic radiographic findings, laryngeal examination results, and clinical outcome were reviewed. RESULTS: No breed or sex predilection was identified in 16 cats with laryngeal paralysis. The most common clinical signs included tachypnea or dyspnea, dysphagia, weight loss, change in vocalization, coughing, and lethargy. Clinical signs were evident for a median of 245 days. Airway obstruction was apparent on cervical and thoracic radiographic views in 9 cats. Examination of the larynx revealed bilateral laryngeal paralysis in 12 cats and unilateral laryngeal paralysis in 4 cats. The 4 cats with unilateral disease were managed with medical treatment, and 3 of these had acceptable long-term outcomes. Seven of 12 cats with bilateral paralysis underwent surgery; procedures performed included left arytenoid tie back, bilateral arytenoid tie back and ventriculo-cordectomy, and partial left arytenoidectomy. One cat was euthanatized as a result of complications from surgery. CONCLUSIONS AND CLINICAL RELEVANCE: Laryngeal paralysis is an uncommon cause of airway obstruction in cats. Cats with less severe clinical signs (often with unilateral paralysis) may be successfully managed with medical treatment, whereas cats with severe airway obstruction (often with bilateral paralysis) may benefit from surgical intervention.  相似文献

OBJECTIVES: To evaluate clinical effects of immobilization followed by remobilization and exercise on the metacarpophalangeal joint (MPJ) in horses. ANIMALS: 5 healthy horses. PROCEDURE: After lameness, radiographic, and force plate examinations to determine musculoskeletal health, 1 forelimb of each horse was immobilized in a fiberglass cast for 7 weeks, followed by cast removal and increasing amounts of exercise, beginning with hand-walking and ending with treadmill exercise. Lameness examination, arthrocentesis of both MPJ, single-emulsion radiographic examination, nuclear scintigraphic examination, ground-reaction force-plate analysis, and computed tomographic examination were done at various times during the study. RESULTS: All horses were lame in the immobilized MPJ after cast removal; lameness improved slightly with exercise. Force plate analysis revealed a significant difference in peak forces between immobilized and contralateral limbs 2 weeks after cast removal. Range of motion of the immobilized MPJ was significantly decreased, and joint circumference was significantly increased, compared with baseline values, during the exercise period. Osteopenia was subjectively detected in the immobilized limbs. Significant increase in the uptake of radionucleotide within bones of the immobilized MPJ after cast removal and at the end of the study were detected. Loss of mineral opacity, increased vascular channels in the subchondral bone, and thickening within the soft tissues of the immobilized MPJ were detected. CONCLUSIONS AND CLINICAL RELEVANCE: Results indicate that 8 weeks of enforced exercise after 7 weeks of joint immobilization did not restore joint function or values for various joint measurements determined prior to immobilization.  相似文献

Severe pectus excavatum sternal deformity in a 3-month-old kitten was repaired by use of percutaneous circumcostal and circumsternal sutures to coapt the sternal deformity to an external splint made from thermoplastic material. One year after treatment, the kitten remained free of clinical signs of the disease or complications from the repair. The technique represents a conservative method of repair of pectus excavatum deformities in immature animals.  相似文献

OBJECTIVE: To compare the use of radiography and fluoroscopy for detection and grading of tracheal collapse in dogs. DESIGN: Retrospective case series. Animals-62 dogs with tracheal collapse. PROCEDURES: For each dog, tracheal collapse was confirmed fluoroscopically and lateral cervical and thoracic radiographic views were reviewed. A board-certified radiologist (who was unaware of the dogs' clinical history) evaluated the cervical, thoracic inlet, thoracic, carinal, and main stem bronchial regions in all fluoroscopic videos and radiographic images for evidence of collapse. Cervical, thoracic inlet, thoracic, and carinal regions in both radio-graphic and fluoroscopic studies were graded for collapse (0%, 25%, 50%, 75%, or 100% decrease in diameter). RESULTS: Lateral cervical and thoracic radiographic images were available for 54 dogs, and inspiratory and expiratory lateral cervical and thoracic radiographic images were available for 8 dogs. For detection of tracheal collapse, assessment of radiographic views was sensitive and had the best negative predictive value in the cervical and thoracic inlet regions. Assessment of radiographic views was most specific and had the best positive predictive value in the thoracic inlet, thoracic, carina, and main stem bronchial regions. Radiography underestimated the degree of collapse in all areas. Review of inspiratory and expiratory views improved the accuracy of radiography for tracheal collapse diagnosis only slightly. CONCLUSIONS AND CLINICAL RELEVANCE: Compared with fluoroscopy, radiography underestimated the frequency and degree of tracheal collapse. However, radiography appears to be useful for screening dogs with potential tracheal collapse.  相似文献

OBJECTIVES: To establish reference mechanical nociceptive threshold (MNT) values of the equine thoracic limb and to assess the use of MNT values to detect pain associated with induced osteoarthritis in the middle carpal joint. ANIMALS: 24 adult horses. PROCEDURES: MNT values were evoked by a pressure algometer at 17 sites within each thoracic limb during 2 baseline sessions conducted an average of 5 days apart. Effects of age, sex, weight, and wither height on MNT values were assessed separately for each site. Tolerance of horses to the procedure was graded subjectively and correlated with MNT values. Synovitis and osteoarthritis were induced arthroscopically in the middle carpal joint of 1 randomly selected thoracic limb. The opposite limb served as a sham-operated control limb. Mechanical nociceptive threshold values were recorded weekly and correlated with clinical, radiographic, and necropsy scores measured over 10 weeks. Lower MNT values corresponded with increased pain, whereas higher MNT values indicated reduced pain. RESULTS: A gradual increase in MNT values was detected from proximal-to-distal sites of the thoracic limbs. High MNT values were recorded for geldings and tall horses. In general, tolerance to procedure scores was positively correlated with overall pooled MNT values within each thoracic limb. From 2 to 6 weeks after surgery, the osteoarthritic limb had significantly reduced MNT values within the carpal region. The osteoarthritic limb also had significant changes in clinical examination, radiographic, and necropsy scores, which were poorly correlated with MNT values. CONCLUSIONS AND CLINICAL RELEVANCE: Pressure algometry provided objective assessment of nociception of the thoracic limb; however, MNT values were poorly correlated with clinical variables used to assess osteoarthritis.  相似文献

Two six-week-old intact Welsh terrier littermates, a male and a female, were presented for congenital ventral thoracic wall deformities characterised by noticeable funnel-like depressions of the cranial sternum associated with inversion of the rib cage. No exercise intolerance or cardiac murmurs were noted. Thoracic radiographic examination revealed a significant dorsal deviation of the first to the fifth sternebrae. At 12 weeks of age, the thoracic depressions had improved markedly in both puppies. Thoracic radiography to reassess the sternal deviation was at this stage within normal limits, demonstrating complete radiographic resolution of the sternal deformity.  相似文献

OBJECTIVE: To document pulmonary function, ventilator management, and outcome of dogs with thoracic trauma that required mechanical ventilation because of severe pulmonary contusions. DESIGN: Retrospective study. ANIMALS: 10 dogs that required mechanical ventilation because of severe pulmonary contusions caused by blunt thoracic trauma. PROCEDURE: Signalment, historical data, arterial blood gas values, oxygen tension-based indices, ventilator settings, peak inspiratory pressure, positive end-expiratory pressure, tidal volume, and minute ventilation values were retrieved from medical records. RESULTS: All 10 dogs required positive-pressure ventilation because of dyspnea following trauma and had severely abnormal pulmonary function. Survival rate to discharge was 30%. Dogs were categorized into 2 groups; group A included 5 dogs in which pulmonary function improved during ventilation, whereas group B included 5 dogs that were euthanatized because of progressive lung dysfunction (n = 4) or cardiac arrest (1). Mean +/- SD body weight of group-A dogs (30.9 +/- 15.9 kg [68 +/- 35 lb]) was significantly greater than that of group-B dogs (7.6 +/- 1.8 kg [16.7 +/- 4 lb]). Dogs with improved lung function had peak inspiratory pressure that decreased progressively, whereas lung compliance deteriorated in dogs in group B. CONCLUSIONS AND CLINICAL RELEVANCE: Dyspneic dogs with severe pulmonary contusions may require and benefit from positive-pressure ventilation Prognosis is better for dogs that weigh > 25 kg (55 lb).  相似文献

OBJECTIVE: To evaluate the effects of one-lung ventilation (OLV) on oxygen delivery (DO2) in anesthetized dogs with a closed thoracic cavity. ANIMALS: 7 clinically normal adult Walker Hound dogs. PROCEDURE: Dogs were anesthetized. Catheters were inserted in a dorsal pedal artery and the pulmonary artery. Dogs were positioned in right lateral recumbency. Data were collected at baseline (Paco2 of 35 to 45 mm Hg), during two-lung ventilation, and 15 minutes after creating OLV. Hemodynamic and respiratory variables were analyzed and calculations performed to obtain DO2, and values were compared among the various time points by use of an ANOVA for repeated measures. RESULTS: OLV induced a significant augmentation of shunt fraction that resulted in a significant reduction in Pao2, arterial oxygen saturation, and arterial oxygen content. Cardiac index was not significantly changed. The net result was that DO2 was not significantly affected by OLV. CONCLUSIONS AND CLINICAL RELEVANCE: Use of OLV in healthy dogs does not induce significant changes in DO2, which is the ultimate variable to use when evaluating tissue oxygenation. One-lung ventilation can be initiated safely in dogs before entering the thoracic cavity during surgery. Additional studies are necessary to evaluate OLV in clinically affected patients and variations in age, body position, and type of anesthetic protocol.  相似文献

CASE DESCRIPTION: A 5-year-old sexually intact male cockatiel was examined because of progressive dyspnea of 1 week's duration. CLINICAL FINDINGS: On auscultation of the lungs and air sacs, crackles were detected; the abdomen was distended and fluctuant on palpation. Eleven milliliters of clear yellow fluid was collected via abdominocentesis. Radiography (with and without contrast medium) and ultrasonography revealed a soft tissue mass in the caudoventral portion of the coelom. TREATMENT AND OUTCOME: Exploratory surgery of the coelomic cavity was performed and the neoplasm was excised. Histologic examination of the neoplasm was consistent with a high-grade pancreatic exocrine adenocarcinoma. Celecoxib, a cyclooxygenase (COX)-2 inhibitor, was administered for pain management and for potential antineoplastic activity. For 4.5 months after surgery, the bird had no recurrence of clinical signs; however, dyspnea recurred and during evaluation, the bird died. Necropsy findings indicated that the pancreatic adenocarcinoma had metastasized to surrounding tissues and vessels, which was not unexpected given the high grade assigned to the neoplasm during histologic analysis. CLINICAL RELEVANCE: Pancreatic neoplasms are associated with a poor prognosis, regardless of treatment modality. Celecoxib can be administered as palliative treatment to affected birds, but as with any nonsteroidal anti-inflammatory drug, COX-2 inhibitors should be used cautiously because they can adversely affect renal function by decreasing renal prostaglandin synthesis.  相似文献
