A 7-year-old Holstein cow was presented for reduced appetite and decreased milk production. Based on physical examination, cecal dilatation was the primary differential diagnosis and was confirmed at surgery. However, in addition to the dilated cecum, 2 large cystic masses were found firmly attached to the proximal loop of the ascending colon.  相似文献

Cecal volvulus in a ewe was diagnosed and surgically treated. Clinical signs were similar to those observed in cattle with cecal volvulus. Rectal palpation, which is an important diagnostic aid in cattle, was not possible in the ewe. Thus, the decision for surgery was based on clinical signs indicative of gastrointestinal obstruction, with the diagnosis of cecal volvulus being made at the time of surgery. Recovery after surgery was rapid, and follow-up evaluation at 13 months indicated no recurrence of the problem.  相似文献

OBJECTIVE: To compare concentrations of acetic, propionic, butyric, and i- and n-valerianic acids in digesta samples obtained from the rumen, cecum, proximal loop of the ascending colon (PLAC), and rectum of healthy cows and cows with cecal dilatation or dislocation (CDD). ANIMALS: 20 cows with CDD and 20 healthy cows. PROCEDURE: Samples were collected from all sites during surgical correction of CDD and also from the rectum 1, 2, and 3 days after surgery (group CDD). Samples from healthy (control) cows, matched on the basis of diet and milk yield, were obtained at a slaughterhouse. Concentrations of volatile fatty acids (VFA) were analyzed by use of gas chromatography. Absolute concentration of each VFA was additionally corrected for pH to allow calculation of the concentration of undissociated VFA. RESULTS: Absolute concentration and concentration of the undissociated form of all analyzed VFA were significantly increased in samples collected from the cecum and PLAC of cows in group CDD, compared with concentrations for control cows. Within 3 days after surgery, significant decreases of the absolute concentration of butyric, i- and n-valerianic acids, and undissociated i- and n-valerianic acids were evident in samples obtained from the rectum of group-CDD cows. Concentrations of VFA in samples obtained from the rectum during surgery correlated with corresponding VFA concentrations in samples obtained from the PLAC. CONCLUSIONS: Concentrations of VFA are increased in the cecum and PLAC of cows with CDD. However, the role of increased concentrations of VFA in the etiopathogenesis of CDD is unknown.  相似文献

Cecal dilatation and volvulus are common entities in dairy cattle, whereas cecal torsion is not often encountered. These diseases have been related to excessive volatile fatty acid production. Obstruction of the remainder of the large colon may be simple or strangulating in nature. These obstructions are relatively rare. Clinical signs, surgical management, and surgical complications of diseases of the cecum and large intestine are discussed.  相似文献

A 4.5-year-old female spayed Rex rabbit (Oryctolagus cuniculus) was presented as an emergency case for hyporexia, pigmenturia, and abnormal feces. The rabbit had been treated for intermittent hyporexia with outpatient medical management and over the next 2 months demonstrated intermittent signs of gastrointestinal stasis, which were only partially responsive to aggressive medical management. Serial radiographs demonstrated persistent gas within the cecum as well as left-sided, cranial displacement of the cecum. A partial cecal obstruction or volvulus was suspected but could not be confirmed, as advanced imaging and exploratory laparotomy were declined. Two months following its initial presentation, the rabbit developed acute signs of shock and colic, but arrested prior to emergency evaluation. Postmortem radiographs showed marked generalized gas dilation of the small intestines and cecum. Necropsy revealed a 360° mesenteric root torsion with devitalization of much of the small intestines, with an additional 180° rotation of the cecum. This case demonstrates that mesenteric root torsion occurs in rabbits and is an important consideration in rabbits presenting with acute shock or chronic, intractable gastrointestinal signs.  相似文献

A surgical procedure was developed for the simultaneous cannulation of the ileum and fistulation of the cecum in breeding-age gilts to study the effects of enteric microflora on the nutritional requirements and/or status of the pig. Flexible T-shaped cannulae were made from plastic tubing, placed surgically in the caudal part of the ileum, and exteriorized in the right paralumbar fossa. After the exteriorization of the cecal apex in the right flank area, a cecal fistual was made caudoventral to the ileal cannula. Prececocolonic contents could be collected from the ileal cannulae and/or substances could be infused into the cecum. The flexible cannulae were light weight (35 g), were easily replaced when dislodged with minimal tissue trauma and minimal discomfort to the animal, and had good animal tolerance and patency in long-term experiments. The cecal fistulae allowed the direct introduction of various substances into the cecum. There was little external projection from the abdominal wall, and the fistulae were subject to minimal leakage. The procedure did not adversely affect the subsequent health or performance of the 43 gilts used in the experiment. Postmortem examination of gilts euthanatized 12 to 26 weeks postoperatively revealed normal tissue adhesions around the surgical sites with no detrimental effects on the gastrointestinal tract.  相似文献

A 12-month prospective study was performed to determine the predictive values for clinical signs of abomasal ulcer disease in a hospital population of 296 adult dairy cows which were believed to have gastrointestinal dysfunction. There were 26 ulcer-positive cows (prevalence 8.8%). The occult blood test was a more reliable indicator of abomasal ulcer disease than were the abdominal pain or anemia tests. Only three ulcer-positive cows were negative for all three tests.

Forty-five cows were positive for at least one of the three clinical tests. The principal diseases present in the 22 cows that had false-positive results were traumatic reticuloperitonitis, abomasal displacement, liver disease, cecal volvulus and pneumonia or pleuritis.  相似文献

Fourteen horses with cecal impaction were treated by cecocolic anastomosis. In nine horses, the anastomosis followed enterotomy and evacuation of the cecal contents, and in two horses it followed saline infusion and massage of the cecum. In three horses, the anastomosis was the only surgical procedure performed. Cecocolic anastomosis was performed between the lateral and dorsal cecal teniae, and the lateral and medial free teniae of the right ventral colon. The anastomosis was hand sutured in five horses. In all other horses, the GIA surgical stapling instrument was used in combination with hand sutured seromuscular layer closures and became the preferred surgical technique. The anastomosis allowed an alternative route for the transit of ingesta from the cecum to the right ventral colon.
Twelve of the 14 horses survived 2 months or longer after surgery (short-term survival rate, 86%) and 10 horses survived 12 months or longer after surgery (long-term survival rate, 71%). Early postoperative complications included mild abdominal pain (6 horses), wound infections (3 horses), fatal peritonitis (2 horses), and large colon distention necessitating reoperation (2 horses).  相似文献

The case records of 102 cows with abomasal volvulus and 71 cows with right-side displacement of the abomasum (RDA) were examined to determine whether the preoperative base-excess concentration of the extracellular fluid could be used both as a prognostic indicator for post-operative recovery in cattle with abomasal volvulus and as an aid in differentiating between abomasal volvulus and RDA. The survival rate of cows with abomasal volvulus decreased as the base excess concentration decreased (P = 0.08); the lowest survival rate was observed in cows with base excess less than or equal to -0.1 mEq/L. There was a significant difference (0.01 less than P less than 0.025) among base excess ranges between abomasal volvulus and RDA. All cows with preoperative base-excess concentrations less than or equal to -5.0 mEq/L had abomasal volvulus, rather than RDA.  相似文献

Data at admission and at surgery were collected on 458 cows with right displacement of the abomasum or abomasal volvulus, to derive multiple logistic regression models for predicting postsurgical outcome (productive, salvaged, or terminal). The derived models contained few and easily obtained variables. The weight associated with each variable was determined objectively. Three admission variables (heart rate, base excess, and plasma chloride concentration), and 5 surgical variables (heart rate, base excess, diagnosis, method of decompression used, and appearance of abomasal serosa) were used in the final models. Predicted outcomes that used the admission and surgical models were closely related with actual outcomes. Total correct classification for satisfactory (productive) versus unsatisfactory outcome (salvaged and terminal) was 78.2% for the admission model and 82.7% for the surgical model. Combining data on cows with productive and salvaged outcomes as satisfactory outcome, and terminal as unsatisfactory outcome, total correct classification was 90.7% for the admission model and 93.2% for the surgical model. Using predicted probabilities, the market value of productive and salvaged cows, and the medical and surgical costs, one can calculate the expected economic value of each outcome. Treatment can be justified if the sum of the expected value of productive and salvaged outcome exceeds the sum of the medical and surgical costs and the expected salvaged value of the cow that was not treated surgically.  相似文献

Thirty cows with caecal dilatation underwent clinical and ultrasonographic examinations, followed by a right flank laparotomy and surgical correction. The intraoperative findings were compared with the results of the ultrasonographic examination. The appearance, position, dimensions, diameter and nature of the contents of the caecum and proximal and spiral ansa of the colon were determined with a 3.5 MHz linear transducer. The wall of the proximal ansa of the colon and of the dilated caecum closest to the abdominal wall was visible in all the cows and appeared as an echogenic semicircular line immediately adjacent to the peritoneum. The contents of the caecum and of the proximal and spiral ansa of the colon were not visible in 21, 25 and 25 cows, respectively, owing to gas. In the remaining cows, the contents were hypoechogenic to echogenic in appearance. In all of the cows, the dilated caecum was imaged from the right abdominal wall at the level of the tuber coxae. The caecum was imaged from the 12th, 11th and 10th intercostal spaces in 11, five and three cows, respectively. The caecum and proximal ansa of the colon were situated immediately adjacent to the right abdominal wall in 28 cows, but in the other two cows parts of these structures were pushed away from the abdominal wall by the liver or gall bladder. The diameter of the caecum, measured at various sites varied from 7.0 to 25.0 cm. Caecal dilatation was diagnosed on the basis of the results of rectal examinations in 28 of the cows, but in all 30 cows on the basis of the results of the ultrasonographic examinations. Dilatation and caudal displacement of the caecum were diagnosed in 18 cows; dilatation and cranioventral retroflexion of the caecum were diagnosed in six cows, and dilatation and craniodorsal retroflexion of the caecum were diagnosed in two cows. In the four other cows, the direction of the retroflexed caecum could not be determined. The diagnosis of caecal dilatation based on the ultrasonographic findings was confirmed in all the cows during exploratory laparotomy. The results of ultrasonography and exploratory surgery with regard to the position of the dilated and sometimes retroflexed or twisted caecum were in complete agreement in 18 cases, in partial agreement in eight cases, but in four cases did not agree.  相似文献

A four-year-old Appaloosa brood mare was referred for evaluation of a colic episode of 24 hours' duration. Physical examination revealed a distended cecum filled predominantly with fluid. A tentative diagnosis of cecal dysfunction was determined, and surgical intervention was performed by means of typhlotomy and ileocolostomy techniques through a complete bypass of the cecum. Postanaesthetic myopathy and incisional infection developed as postoperative complications. The mare was discharged 13 days after surgery. Follow-up information obtained from the owner and referring veterinarian 4 months after discharge revealed full recovery of the mare.  相似文献

The microvascular circulation of the cecum was studied in 15 adult horses, using microangiography and light microscopy combined with gross studies and scanning electron microscopy of vascular replicas. After heparinization, the horses were euthanatized and the cecum was transected at the cecocolic junction. Blood was flushed free of the circulation with isotonic NaCl and the cecal lumen was slightly distended. In 6 horses, the vascular system was injected with a modified radiopaque medium and evaluated radiographically. Sections evaluated radiographically were also prepared for histologic examination by standard methods. Eight horses were injected with 1 of 2 types of plastics and studied grossly or by scanning electron microscopy. In 1 additional horse, the lateral and medial arteries were injected with different colored plastic for gross studies. The lateral and medial cecal arteries appear to supply the respective areas of the cecum with minimal mixing of the 2 circulations. The major vascular supply to the cecal apex appeared to be through the medial cecal artery. Both the lateral and medial cecal arteries gave rise to cecal retia, which formed a mesh-like network around the respective veins. Vessels from these retia supplied the cecal tissue and the cecal lymph nodes. The continuation of the retia was through long terminal arteries that coursed around the circumference in the submucosa, forming an extensive submucosal plexus. This plexus supplied both the mucosa, and the tunica muscularis and serosa. Vessels within the longitudinal and circular muscle layers of the muscularis externa ran parallel to the muscle fibers, and consequently, perpendicular to each other.(ABSTRACT TRUNCATED AT 250 WORDS)  相似文献

Four cows were diagnosed as having volvulus of the distal jejunoileum. Affected cattle became acutely depressed, dehydrated, and tachycardic, all developed abdominal distention, and three of four were in severe pain at the time of admission. Feces were scant or absent. The volvulus was corrected by exteriorizing the small intestine and replacing it in correct position. All four cows recovered without complications and were well 6 months or more following surgery.  相似文献

The objective of this study was to identify risk factors and describe clinical signs in 7 foals with cecal rupture; none of the foals survived. Six foals had undergone general anesthesia; 5 for orthopedic procedures. Six of the foals were receiving nonsteriod anti-inflammatory drugs. Most foals started showing colic signs on day 2 after surgery, preceded in 3 cases by dullness. Cecal rupture occurred between 4 hours and 2 days after the first signs of colic were noticed. Intestinal motility was decreased or absent in all foals for which it was recorded.Foals undergoing general anesthesia should be closely monitored for any sign of dullness, prolonged recumbency, reduced fecal output, and signs of abdominal discomfort for 3 days postoperatively, especially in cases following orthopedic surgery. If any of the above occurs, cecal impaction should be considered as a differential diagnosis. A prompt exploratory laparotomy may be a reasonable diagnostic option before the cecum ruptures with fatal consequences.  相似文献

Cecal impaction and cecal perforation, the two most common equine cecal diseases, are thought to develop after slowing or interruption of a single progressive motility pattern, which begins in a pacemaker area near the apex, occurs once every 3 minutes, and propels ingesta from the cecum to the right ventral colon. Rectal examination in horses with cecal impaction is the most useful technique to grade the severity of the condition. Medical treatment is undertaken if the impaction is judged to be mild to moderate. Surgical correction of cecal impaction in severe cases requires a ventral midline celiotomy, and exploration reveals a large ingesta-filled cecum and relatively empty large colon. Currently, the techniques of typhlotomy with manual evacuation of ingesta, combined with a complete bypass of the cecum by use of a jejunocolostomy, is the preferred method of surgical management. The use of a cecocolic anastomosis remains a viable alternative surgical procedure. Cecal perforation (CP), a uniformly fatal disease of horses, most often develops when the subtle signs of cecal impaction are missed or are masked by the administration of nonsteroidal antiinflammatory agents. CP can occur in mares around the time of foaling and, in this form, is not associated with cecal outflow dysfunction. Surgical management of cecocecal or cecocolic intussusception is required and involves resection of the diseased portion of cecum, either with extra- or intraluminal techniques. Both the side-to-side and end-to-side jejunocecal anastomoses are useful and successful techniques for bypass of simple or strangulating lesions of the ileum.  相似文献

The objective of this study was to evaluate serum mineral and electrolyte concentrations at the time of on-farm diagnosis of left displaced abomasum, right displaced abomasum, or abomasal volvulus in dairy cows. Data were collected from 104 affected cows and 96 control cows matched with cases, based on herd, parity, and stage of lactation. Cows with abomasal displacement or volvulus had significantly lower calcium, phosphorous, magnesium, potassium, and chloride concentrations and increased anion gap at the time of diagnosis compared with control cows from the same herds. The percentages of cases and controls with total serum calcium concentrations below the lower limit of the laboratory reference range (2.08 mmol/L [8.3 mg/dL]) were 70% and 23%, respectively. Based on the large percentage of cases with hypocalcemia, administering calcium salts at the time of treatment of field cases of abomasal displacement or volvulus may be beneficial.  相似文献

Several surgical alternatives have been described for the management of cecal impaction in the horse, but none has met with consistently successful results. This study was done to evaluate a surgical bypass of the cecum by anastomosis of the ileum to the right ventral colon (ileocolostomy). A ventral midline celiotomy was performed on nine adult ponies (155-350 kg) and a mechanically stapled 10 cm side-to-side ileocolostomy was created. In five ponies a complete cecal bypass (CCB) was created by transecting the ileum distal to the anastomosis. In the other four, an incomplete cecal bypass (ICB) was created with no interruption of the ileum. Six horses with clinical cecal impaction also underwent cecal bypass procedures. Five had a CCB and one had an ICB. All the ponies maintained body weight, had no change in consistency of the feces and had no abdominal pain during the 6 month observation period. At necropsy, the lengths of the lateral cecal band, lateral free band of the colon, and the diameter of the anastomotic stoma were compared to measurements made at surgery. The lateral cecal band length decreased significantly more in the CCB ponies than in the ICB ponies (p = 0.008). The anastomotic stoma diameter was significantly larger in the ICB group than in the CCB group (p = 0.032). Five of the six clinical cases recovered and returned to their previous activity. CCB by an ileocolostomy resulted in removal of the cecum from the functional flow of ingesta without complication in the ponies, and was successful in five clinical cases of cecal impaction.  相似文献

This paper describes the latest results of research in the areas of etiopathogenesis and treatment of cecal dilatation/-dislocation (CDD) in cows. Similar etiopathogenic mechanisms of CDD and displacement of the abomasum, as has been hypothesized in the literature, were not confirmed. Spontaneous CDD is most likely the consequence of a dysfunction of the spiral colon. Bethanechol is an appropriate motility-modulating drug for medical and postsurgical treatment of spontaneous CDD.  相似文献

We determined the abomasal emptying rates of Holstein-Friesian cows at different stages of lactation, with left displaced abomasum (LDA), or immediately after surgical correction of LDA or abomasal volvulus (AV). D-xylose (0.5 g/kg body weight [BW], 50% solution) was injected into the abomasum in healthy cows (group 1, 4-7 days in milk [DIM], n = 7; group 2, 90-120 DIM, n = 7; group 3, > 300 DIM, n = 7) and in cows with LDA (n = 10; group 4). D-xylose was injected into the abomasum during right flank laparotomy in cows with LDA (n = 22; group 5) and cows with AV (n = 15; group 6). The time to maximal serum D-xylose concentration was used as an index of emptying rate. The abomasal emptying rates for cows in groups 1, 2, and 3 were similar, whereas emptying was slower in cows with LDA and in cows after surgical correction of LDA or AV. The abomasal emptying rate of cows with LDA was slowed to a greater extent immediately after surgery, when compared to the rate obtained before surgery. There was no difference in abomasal emptying rate immediately after surgical correction between cows with LDA or AV. The results indicate that the increased incidence of LDA in the first month of lactation is not associated with an intrinsic decrease in abomasal emptying rate in healthy cows. Our findings also demonstrate that surgical correction further slows the emptying rate in cows with LDA.  相似文献
