REASONS FOR PERFORMING STUDY: Previous studies have shown that in man ultrasonography is more accurate than radiography for detecting rib fractures. OBJECTIVES: To describe clinical, radiographic and ultrasonographic findings related with rib fractures in newborn foals in an equine critical care unit; and to compare diagnostic accuracy of ultrasonography to radiography. METHODS: A prospective ultrasonographic study was performed on 29 foals presented to the emergency unit. This study was performed at the Centre Hospitalier Universitaire Vétérinaire (CHUV), University of Montreal. Physical examination as well as radiographic and ultrasonographic examinations were performed. RESULTS: Thoracic radiographs revealed 10 rib fractures in 5 of 26 (19%) foals. Ultrasonography revealed 49 fractures in 19 of 29 (65%) foals of which fillies (n = 13; 68%) were significantly over represented as were fractures to the left thorax (n = 15; 78%). Seventeen of 19 foals (90%) had rib fractures located 3 cm or less from the costochondral junction, the distal part of the rib being displaced laterally in all cases. In 2 foals, where both thoracic radiographs and ultrasonography detected rib fractures, the site of fractures was located on the mid portion of the rib. Rib fractures were detected only by thoracic radiographs in one foal. Sixty-five percent (32/49) of fractured ribs had a moderate displacement (1-4 mm). CONCLUSIONS: Rib fractures are seen frequently in newborn foals in equine critical care units. Ultrasonography is more accurate than radiography and reveals fractures in most patients presented in emergency. The position (costochondral junction) of rib fractures and of the fragments suggest that most thoracic trauma probably occurs during parturition. POTENTIAL RELEVANCE: Ultrasound imaging increases awareness and improves the diagnosis of rib fractures in newborn foals.  相似文献

Reasons for performing study: Fractured ribs are encountered quite frequently in newborn Thoroughbred foals, often with fatal outcome. Surgical repair of fractures therefore requires consideration as a means of reducing mortality. Objectives: To evaluate the repair of rib fractures using internal fixation techniques in foals at 2 different equine hospitals following similar diagnostics and case selection. Methods: The records of 14 foals that underwent internal fixation of fracture ribs were reviewed. Subject details, clinical presentation, diagnosis, surgical technique, post operative care and complications were recorded. Follow-up information was obtained in 7 foals. Results: The fractured ribs were reduced and stabilised using reconstruction plate(s), self-tapping cortical screws and cerclage wire in 12 cases, Steinmann pins and cerclage wires in 1 case and both techniques in 1 case. Not every rib was reduced on each case. Surgical reduction was performed on an average of 2 ribs, range 1-3 ribs in each foal. At the time of writing, 4 foals had been sold, one age 2 years was in training and 2 others died from unrelated causes. Conclusions: Our data support the use of surgical stabilisation utilising reconstruction plates, self-tapping cortical screws and cerclage wire for selected cases of thoracic trauma in neonatal foals. The use of Steinmann pins may be suboptimal due to cyclic failure, implant migration and the potential for iatrogenic internal thoracic trauma. Potential relevance: Foals with existing extensive internal thoracic trauma resulting from rib fracture(s), or the potential for such trauma, previously considered to have a guarded to poor prognosis for survival, may be successfully managed with internal fixation of selected fracture sites.  相似文献

Thoracic radiographs from 22 neonatal foals were reviewed to investigate the radiographic appearance of the thorax in normal, immature, and septicemic foals, and in foals with neonatal respiratory distress syndrome. The size and radiographic appearance of intrathoracic structures and abnormal lung opacities were evaluated. The craniocaudal and apicobasilar dimensions of the heart were 5.6–6.3 and 6.7–7.8 times the length of a midthoracic vetebral body, respectively, in normal, immature and septicemic foals. Apicobasilar measurements were greater (8.0–8.7) in the foals with respiratory distress syndrome. Normal foals had clear lung fields within 12 hours of birth. A more marked interstitial pattern was observed in immature and septicemic foals compared to normals. Diffuse air–space (alveolar) pattern with air bronchograms was seen in foals with respiratory distress syndrome. It was concluded from this series that thoracic radiographs taken 24–48 hours after birth may aid differentiation of normal foals, septicemic or immature foals, and foals with respiratory distress syndrome.  相似文献

A skeletal ossification index (SOI) was devised to standardize the radiographic evaluation of the carpal and tarsal regions of newborn foals (less than two weeks of age). This index was based on two radiographic views of a carpus and a tarsus. Four grades of cuboidal bone ossification were discerned: Grade 1 foals had some cuboidal bones with no ossification; Grade 2 was assigned when all cuboidal bones had radiographic evidence of some ossification; in Grade 3 the bones were small and round; in Grade 4 the cuboidal bones were shaped like the adult counterparts. Radiographs were taken of 24 neonatal foals of various gestational ages (range, 297–374 days). The relationships between the SOI and two measures of newborn foal developmental maturity, gestationai age and body weight, were assessed. There was no relationship between the sex of the foal and the SOI, but there was a relationship between the SOI and the gestational age (one-tailed Spearman's rank correlation coefficient, r = 0.748) and between the SOI and body weight (r = 0.77) for the 24 foals. For the ten foals with gestational ages less than 320 days, the relationship between body weight and the SOI was significant (r = 0.953), but the relationship between gestational age and the SOI was not significant (r = 0.605). The degree of ossification of the bones of neonatal foals that are born prematurely (< 320 days of gestation) or are "small-for-gestational age" at birth may be incomplete, and we suggest that radiographs of these regions should be made to assess ossification in foals in these two high risk groups.  相似文献

【目的】 新生犊牛被动免疫转移失败(failure of passive transfer,FPT)会导致犊牛发病率和死亡率显著增高,影响后期生长发育。本试验针对国外引进的安格斯母牛初乳产量和质量进行测定,并对其新生犊牛被动免疫效果进行评估,以期为新生犊牛初乳管理措施制定提供科学依据。【方法】 随机选择健康安格斯初产母牛(24月龄)和经产母牛(36~48月龄)各15头,待母牛产犊后立即进行人工挤奶测定初乳产量、营养成分及免疫球蛋白浓度。选取初产和经产新生犊牛各15头,测定其初生重,分别采集犊牛出生后未食初乳(0 h)和食初乳后(24~36 h)血样,测定其血清总蛋白含量及各生理生化指标。【结果】 初产母牛平均初乳产量极显著低于经产母牛,但其初乳中乳蛋白、非脂乳固体、乳糖、灰分含量,以及密度和电导率均极显著高于经产母牛(P<0.01)。初产母牛和经产母牛初乳中免疫球蛋白浓度分别为28.72%和26.24%,合格率分别为93.33%和91.67%,差异均不显著(P>0.05);初产和经产新生犊牛被动免疫失败率分别为33.3%和25.0%;初产新生犊牛平均初生重极显著低于经产新生犊牛(P<0.01);被动免疫成功犊牛平均初生重和球蛋白含量均极显著高于被动免疫失败犊牛(P<0.01)。【结论】 新疆安格斯犊牛FPT的主要原因可能是母牛初乳产量低导致新生犊牛初乳摄入不足,初生重较低的犊牛FPT的风险性较高。本研究为制定切实可行的新生安格斯犊牛初乳补饲计划提供了科学基础。  相似文献

Thoracic injuries caused by blunt trauma are commonly encountered emergencies in veterinary medicine. However, published studies are lacking that compare radiology to CT in blunt trauma caused by motor vehicle accidents in canine patients. The aim of this prospective diagnostic accuracy, methods comparison study were to estimate the sensitivity (Se) and specificity (Sp) of thoracic radiology relative to CT for detecting lung contusions, pneumothorax, pleural effusion, and rib fractures. The study further aimed to develop a severity scoring system for radiology and CT and to compare the findings between the two modalities. The hypothesis was that radiology would be less sensitive than CT at detecting these injuries and that radiology would underestimate the severity of lung contusions. Fifty‐nine patients met the inclusion criteria. Radiology underestimated the presence of lung contusions (Se = 69%, 95% confidence interval) and overestimated the severity of the contusions relative to CT. There was high interobserver variability in evaluating lung contusion severity (coefficient of variation = 91%). Both the three‐view thoracic and horizontal beam radiography had poor sensitivities for the detecting pneumothorax (Se = 19% and 63%, respectively) and pleural effusions (Se = 43% and 71%, respectively). Similarly, the sensitivity (56%) of radiographs for the detection of rib fractures was poor relative to CT. Findings from the current study indicated that thoracic radiography had low sensitivity for detecting lesions related to blunt thoracic trauma caused by motor vehicle accidents and supported the use of CT as an additional diagnostic imaging modality in these patients.  相似文献

To test the hypothesis that changing the orientation of a thoracic radiograph aids diagnosis of rib fractures, a case–control study was carried out using 120 radiographs of small animal patients with recent thoracic trauma. Six independent observers representing three levels of experience viewed the radiographs in randomized order in conventional and unconventional orientations at 15‐s intervals in a PowerPoint® presentation. Sensitivity for rib fractures was 53–69% and specificity was 74–97%, depending on the observer. Radiologists had higher specificity than less experienced observers (P=0.0001), but comparable sensitivity for rib fractures. Radiologists had significantly higher accuracy than residents (P=0.03), and residents had higher accuracy than interns (P=0.02). Accuracy of diagnosis was reduced significantly when based on lateral radiographs compared with dorsoventral (DV)/ventrodorsal (VD) views (P=0.02) mainly because of low sensitivity of lateral radiographs of dogs with rib fractures. Rotating radiographs 90° clockwise was associated with increased specificity of one intern (from 74 to 90%, P=0.03), but had no effect on accuracy of diagnosis by other observers (P>0.16). Turning DV/VD views upside down had no effect on accuracy of diagnosis of any observers (P>0.40). Changing the orientation of a thoracic radiograph may make it easier for some novices to examine the ribs; hence it could be considered a training aid rather than a technique that will benefit an experienced radiologist.  相似文献

Objectives— To report rib fracture repair in a neonatal calf using the Securos® Cranial Cruciate Ligament Repair System (SCCLRS).
Study Design— Case report.
Animals— A 2-day-old female Holstein calf with fracture of right ribs 4–10.
Methods— On the day of admission the calf was anesthetized and rib fractures were repaired using open reduction and the SCCLRS.
Results— Rib fractures were successfully stabilized and the calf discharged from the hospital 8 days postoperatively.
Conclusions— Modification of previously reported use of the SCCLRS to repair rib fractures in foals was required because of the different anatomy in the calf. This new technique was rapidly and easily performed for a large number of fractured ribs in this case.
Clinical Relevance— Effective rib fracture repair in calves can be readily performed using the SCCLRS with modification of the technique reported in foals.  相似文献

The effects of dam parity, age at weaning, and preweaning diet were examined in the ontogeny of serum insulin-like growth factor-I (IGF-I) concentrations in foals. Foals born to 13 primiparous and 19 multiparous draft-cross mares were weighed and bled near birth. About one-half of the foals in each group were weaned early (about 13 wk old); the remaining foals were weaned late (about 16 wk of age). Pooled values for serum IGF-I concentrations between birth and 17 wk of age were higher (P < 0.065) for foals born to multiparous (386 ng/ml) than to primiparous mares (237.5 ng/ml). Colts (378 ng/ml) had higher (P < 0.05) serum IGF-I concentrations than fillies (254.5 ng/ml), regardless of dam parity. Colts (173.5 kg) also tended (P = 0.12) to be heavier than fillies (159.2 kg). Weaning, whether at 13 or 16 wk of age, reduced (P < 0.05) growth rates and serum IGF-I concentrations. Serum IGF-I values recovered to preweaning values within 1–3 wk postweaning concurrent to an improved weight gain. Fifteen 1-d-old foals in a second study were fed milk replacer for 7 wk and were compared with five foals that nursed their mares for 8 wk. During the first 2 wk, replacer-fed foals (0.46 kg/d) did not gain as rapidly (P < 0.03) as mare-nursed foals (1.73 kg/d). The associated serum IGF-I values for replacer foals (139.4 ng/ml) were lower (P < 0.0001) than values for mare-nursed foals (317.4 ng/ml). Despite similarity in gains for both groups thereafter, serum IGF-I concentrations of replacer-fed foals were only 36 and 60% of values obtained for mare-nursed foals at 8 (weaning) and 18 wk of age, respectively. The intrinsic differences between mare-nursed and milk-replacer foals in serum IGF-I concentrations persisted to 1 yr of age despite similarities in dietary management and body weight of the foals. At 1 yr of age, the serum IGF-I concentration of mare-nursed foals (1,203 ng/ml) was 48% higher than that of replacer-fed foals (815 ng/ml). These data indicate that dam parity, sex of foal, and preweaning nutrition affect the ontogeny of serum IGF-I concentration in the foal. The chronic, persistent difference in serum IGF-I values created by the early nutritional management of growing animals has implications in the interpretation of longitudinal serum IGF-I studies in all species.  相似文献

Objective – To characterize the clinical features and population differences among cats sustaining traumatic and nontraumatic rib fractures. Design – Retrospective clinical study. Setting – University small animal hospital. Animals – Thirty‐three cats with radiographic evidence of rib fractures. Interventions – None. Measurements and Main Results – Cats with rib fractures were identified by performing a computer search of the radiology database. Thirty‐three cats that sustained rib fractures were identified between January 2000 and September 2009. Seventeen cats had fractures due to trauma and 16 were deemed to occur from nontraumatic causes. A Mann‐Whitney rank‐sum test revealed statistically significant differences in the median ages between the 2 groups. Older cats were more likely to sustain rib fractures as a result of a presumed nontraumatic causes. A Chi‐square analysis showed that nontraumatic fractures occurred significantly more often in the midbody region and involved the 9th–13th ribs. The majority of cats with presumed nontraumatic rib fracture had respiratory disease; the remaining cats had chronic renal disease or neoplasia. Cats with traumatic rib fractures had external signs of trauma. Conclusion – Rib fractures in cats may be clearly associated with trauma, or may be an incidental finding in cats with comorbidities. Cats with diseases that cause prolonged respiratory effort or coughing, metabolic diseases, or certain neoplasms, are at increased risk of spontaneous nontraumatic rib fractures.  相似文献

OBJECTIVES: To report rib fracture repair using the Securos Cranial Cruciate Ligament Repair System (SCCLRS; Securos Veterinary Orthopedics, Charlton, MA) in a neonatal foal. STUDY DESIGN: Case report. ANIMALS: A 1-day-old Standardbred foal with fracture of left ribs 2-9. METHODS: Four days after admission the foal was anesthetized and rib fractures were repaired using open reduction and the SCCLRS. RESULTS: Rib fractures were successfully stabilized and the foal was discharged 7 days postoperatively without further complications. CONCLUSIONS: The SCCLRS provided a straightforward, effective method of rib fracture repair in neonatal foals. CLINICAL RELEVANCE: Effective rib fracture repair in neonatal foals can be achieved with the SCCLRS.  相似文献

Two six-week-old intact Welsh terrier littermates, a male and a female, were presented for congenital ventral thoracic wall deformities characterised by noticeable funnel-like depressions of the cranial sternum associated with inversion of the rib cage. No exercise intolerance or cardiac murmurs were noted. Thoracic radiographic examination revealed a significant dorsal deviation of the first to the fifth sternebrae. At 12 weeks of age, the thoracic depressions had improved markedly in both puppies. Thoracic radiography to reassess the sternal deviation was at this stage within normal limits, demonstrating complete radiographic resolution of the sternal deformity.  相似文献

Foetal electrocardiograms (ECG) were obtained from 39 of 50 Thoroughbred foaling mares close to delivery. The 50 newborn foals were studied electrocardiographically during their adaptive period, immediately after birth. In 48 foals there were paroxysmal arrhythmias or mixed arrhythmias. The most common arrhythmias were sinus arrhythmias including wandering pacemaker (32/50) and atrial premature contraction (30/50). The others observed were atrial fibrillation (15/50), ventricular premature contraction (10/50), partial atrioventricular block (7/50), ventricular tachycardia (4/50), atrial tachycardia (3/50) and idioventricular rhythm (1/50). The duration of the arrhythmias was approximately 5 min, and in all cases the arrhythmia disappeared within 15 min of birth. From foetal ECG recordings, no indication of the likelihood of neonatal arrhythmias was detected. With the exception of 2 cases, all foals have continued to grow and develop normally. These arrhythmias are considered normal physiological processes in newborn Thoroughbred foals during the adaptive period to extra-uterine life. High vagal tone and hypoxaemia at birth are probably the main contributing factors.  相似文献

The objective of this retrospective study was to compare the efficacy of azithromycin-rifampin, clarithromycin-rifampin, and erythromycin-rifampin for the treatment of pneumonia caused by Rhodococcus equi in foals. Eighty-one foals with naturally acquired pneumonia caused by R. equi were included in the study. Information on age, sex, breed, physical examination findings, laboratory testing, and thoracic radiography was abstracted from each medical record. Foals were divided in 3 groups based on the antimicrobial agent selected for therapy. Short-term (discharge from the hospital) and long-term (apparently healthy as a yearling) success rates, days of hospitalization, days with fever, days with tachypnea, and percentage of radiographic improvement were compared among groups. Foals treated with clarithromycin-rifampin had significantly (P = .02) higher odds of overall short-term (odds ratio [OR] = 12.2) and long-term (OR = 20.6) treatment success and significantly fewer days with fever than foals treated with erythromycin-rifampin. Foals treated with clarithromycin-rifampin had a significantly (P = .03) higher percentage of radiographic improvement and a tendency (P = .06) toward higher odds of overall short-term (OR = 8.1) and long-term (OR = 11.8) treatment success compared to foals treated with azithromycin-rifampin. Among foals with severe radiographic lesions, the success rates of foals treated with clarithromycin-rifampin both short-term (88%) and long-term (83%) were significantly (P = .02) higher than that of foals treated with azithromycin-rifampin (0%). For each treatment group, the only reported adverse effect was diarrhea that was mild and self-limiting in most cases. The combination clarithromycin-rifampin is superior to azithromycin-rifampin or erythromycin-rifampin for the treatment of pneumonia caused by R. equi in foals in a referral population.  相似文献

The medical records of 163 neonatal foals that had thoracic radiographs taken within 48 hours of admission to a referral hospital were reviewed. The objectives of this study were (1) to identify risk factors for the development of thoracic radiographic changes and (2) to identify prognostic indicators for survival in foals with radiographic evidence of pulmonary disease. Failure of transfer of passive immunity (IgG concentration < or = 400 mg/dL) was the only risk factor for radiographic evidence of respiratory disease identified by multivariate analysis. Hypoxemic patients (PaO2 < or = 60 mm Hg) were 4.9 times more likely to reveal radiographic abnormalities in a subset of foals for which arterial blood gas results were available. Foals with a serum creatinine concentration > 1.7 mg/dL upon presentation, dyspnea, and a history of dystocia were significantly more likely to die based on the multivariate statistical outcome analysis. An anion gap > or = 20 mEq/dL was strongly associated with nonsurvival in a subset of foals with arterial blood gas results. These hematologic and biochemical variables can be readily obtained during the initial evaluation of sick foals. The presence of a high anion gap appeared to have the greatest clinical impact and may be a useful prognostic indicator in foals with radiographic evidence of respiratory disease. In contrast, the majority of physical examination variables, including evaluation of tachypnea, abnormal respiratory sounds, fever, weakness, and milk reflux from the nares, which are usually obtained during the general respiratory evaluation of foals, were unrelated to outcome.  相似文献

Our objective was to characterize the association between types of radiographic findings and outcome in foals with pneumonia caused by Rhodococcus equi. Admission lateral thoracic radiographs of 62 foals with culture‐confirmed R. equi pneumonia were reviewed retrospectively. A scoring system was developed to individually assess the severity of alveolar pattern, interstitial pattern, tracheobronchial lymphadenopathy, pleural effusion, and the number of nodular opacities and cavitary lesions. Individual scores were added to obtain a total radiographic score ranging from 0 (normal) to 22. Forty‐three of 62 foals (69%) survived to discharge. The median total radiographic score of nonsurvivors (14; range, 9–16) was significantly (P = 0.007) higher than that of survivors (11; range, 4–15). Foals with a total radiographic score of greater than or equal to 15 were 6.15 times (95% CI: 1.35 to 28.2) less likely to survive than foals with a lower score (P = 0.019). A multivariate logistic regression model was used to identify the potential associations between specific types of radiographic lesions and outcome. The model was statistically significant (P = 0.002) and correctly classified 75.8% of foals. Only severity of alveolar pattern and number of cavitary lesions made statistically significant contributions to the model. There was no significant association between concurrent isolation of other bacteria along with R. equi and the types or severity of radiographic lesions. Based on the results of this study, severity of alveolar pattern and number of cavitary lesions are the radiographic findings significantly associated with a poor outcome in foals with R. equi pneumonia.  相似文献

The thoracic bellows mechanism consists of the rib cage and the diaphragm. The purpose of this study was to determine if nontraumatically acquired lesions of the bellows were secondary to underlying disease. Abnormalities of the bellows, specifically stress fractures of the ribs and hiatal hernia, were found in 21 dogs and cats with underlying cardiopulmonary disease, neuromuscular disease, or metabolic disease. A case-control study of Bulldogs demonstrated that hiatal hernia was associated with the more severe manifestations of brachycephalic syndrome. Stress fractures occurred mostly in females and in cats, and involved multiple ribs. Fractures were usually related to severe respiratory effort, but also occurred in association with metabolic disease. Hiatal hernia was also associated with severe respiratory effort, but may be exacerbated if a neuromuscular disorder affecting the diaphragm is present. Abnormalities of the thoracic bellows, such as rib stress fractures and hiatal hernia, may be signs of underlying disease, rather than being primary causes of disease.  相似文献

Serum complement activity and selected hematologic variables were evaluated in 5 newborn foals fed bovine colostrum (principal group) and 6 foals allowed to nurse their dam (control group). Also, bovine colostrum was evaluated for anti-equine antibodies. Precolostral serum hemolytic and conglutinating complement activities were low and increased similarly in foals of both groups to reach adult values between 1 and 3 weeks after birth. Bovine colostrum strongly agglutinated, but did not hemolyse principal foals' RBC and blood containing all known equine blood group alloantigens. Hemolysis was not detected after administration of bovine colostrum. Physiologic anemia developed in foals of principal and control groups during the first week of life. Erythrocyte osmotic fragility in foals of the principal group prior to and after the ingestion of colostrum remained unchanged. However, at 36 hours after birth, there was a significant decrease in erythrocyte osmotic fragility in foals fed homologous colostrum.  相似文献

Commercially available nylon cable ties were used as a method of internal fixation for displaced rib fractures in 8 equine neonates. The procedure was effective, well-tolerated, rapid, and inexpensive.
