Although the intramedullary nailing is thought to be the method of choice for treatment of closed tibial shaft, there is ongoing debate on the optimal surgical approach in patients with open types of these fractures. In addition, choosing between the reamed and unreamed intramedullary nailing is still an issue for the orthopedic surgeons. In present study, we aimed to compare the outcome and consequences of OA tubular external fixation vs. unreamed intramedullary nailing in open grade IIIA-IIIB tibial shaft fractures. In a randomized clinical trial, 50 patients with open tibial shaft fractures (grades IIIA-IIIB) were recruited in Tabriz Shohada teaching centre in a 2-year period of time. They randomized in two equal groups underwent either AO tubular external fixation or unreamed intramedullary nailing. These 2 groups were matched for sex, age and fracture-grade. The follow-up time was one year. Union time, surgical outcome, postoperative complications and the ambulation time were compared between the two groups. Twenty five patients, 20 males and 5 females with a mean age of 30.80 +/- 5.24 years were allocated in unreamed intramedullary group and 25 other patients, 22 males and 3 females with a mean age of 28.92 +/- 8.88 years were studied in the external fixation group (p = 0.70 and 0.37, respectively). The two groups were matched for sex (p = 0.70) and age (p = 0.37). The time of union was 3, 4, 5 and 6 < or = weeks after operation in 28, 12, 32 and 28% of the cases in unreamed intramedullary group vs. 4, 12, 48 and 36% of the cases in external fixation group, respectively (p = 0.14). Post-operative infection, soft tissue injury, malunion and nonunion were documented in 16, 8, 0 and 4% of the cases in unreamed intramedullary group vs. 32, 12, 24 and 8% of the cases in external fixation group, respectively (p = 0.19, 0.50, 0.02 and 0.50, respectively). The mean ambulation time after operation was 2.92 +/- 2.43 weeks in the unreamed intramedullary nailing group vs. 2.68 +/- 2.14 weeks in the external fixation group (p = 0.71). Our results are in favor of unreamed intramedullary nailing against external fixation in treatment of open tibial shaft fractures.  相似文献

To evaluate the impact of preemptive local analgesia at the incision site for postoperative pain in patients undergoing disc operation. In this prospective, randomized, double-blinded, placebo-controlled study 166 patients were assigned to either lidocaine (n = 83) or placebo (n = 83) groups. The incision site was infiltrated with either 20 mL of 2% lidocaine and 0.9% saline in lidocaine group or 0.9% saline before the incision. Morphine (5 mg) was used for postoperative pain treatment. Postoperative pain was measured with Visual Analog Scale (VAS) in 6, 12, 24 and 48 h. Data were analyzed with SPSS software, using Chi-square and t-tests. The groups were matched for age, sex, type of operation, mean length of hospital stay and mean length of operation. Statistical analysis revealed no significant difference in visual analog scores of pain severity at 6, 12, 24 and 48 h after surgery between lidocaine and placebo groups (6 h: 38.22 +/- 26.87 vs. 34.52 +/- 24.43, p = 0.35; 12 h: 33.26 +/- 28.83 vs. 28.01 +/- 24.71, p = 0.20; 24 h: 26.71 +/- 23.31 vs. 22.85 +/- 22.48, p = 0.27; 48 h 16.35 +/- 10.16 vs. 15.23 +/- 8.90 p = 0.45). The amount of narcotics used post operatively had no meaningful difference in the groups (lidocaine 10.07 +/- 8.24 mg vs. placebo 10.54 +/- 9.31 mg p = 0.73). Preemptive analgesia with lidocaine 2% used subcutaneously before skin incision has no effect in reducing postoperative pain, narcotics demand and duration of hospital stay.  相似文献

The main objective of this study was to evaluate the potential protective effect of red Grape Seed Extract (GSE) on lead induced hypertension (HTN) and Heart Rate (HR) in male Wistar rats. The rats were randomly assigned to one of 4 groups: Each group received lead acetate (100 ppm in drinking water), GSE (100 mg kg(-1), orally) or Lead + GSE for 45 days. Another group assigned as control group provided with tap water and regular pellet food. The Systolic Blood Pressure (SBP) and heart rate were determined by tail plethysmography coupled to a computer system. There was a sustained elevation of SBP in lead exposed rats that significantly increased at day 18 (lead treated, 112.7 +/- 2.7 mmHg, vs. control, 105.6 +/- 2.6 mmHg, n = 10, p < 0.05) and reached a maximum level at day 36 (lead treated, 124.9 +/- 2.3 mmHg, vs. control, 103.6 +/- 3.1 mmHg, n = 10, p < 0.001). However, the other three groups; showed no significant changes in SBP. Furthermore, the heart rate was increased sustainly in lead exposed animals that was statistically significant at days 36 and 45 (lead treated group, 404.5 +/- 9.4 vs. control group, 381.7 +/- 6.7, n = 10, p < 0.05). The blood lead level in both lead and lead + GSE treated groups was increased significantly compared with control and GSE treated groups (p < 0.001). However, GSE administration had no effect on the blood lead level in lead treated group. According to the result of this study, it may be concluded that GSE could have beneficial effect in protecting the cardiovascular system through its antioxidant activity against oxidative stress.  相似文献

Tibia fractures are the most common type of long bone fractures in US. This study aimed at comparing the therapeutic results of closed tibial shaft fracture with intramedullary nails inserted with and without reaming. In this randomized clinical trial study, 60 patients with a fracture of the tibia were examined. The patients were randomly divided into two groups. Thirty patients treated through inserting intramedullary nail with reaming technique (group A). The other 30 patients treated through inserting intramedullary nail without reaming technique (group B). After operation physical examination and control radiography were taken up to 6 month and results were compared. Sixty patients suffering from closed tibial diaphysis fractures were studied. Mean age of the group A and B were 40.24 +/- 12.32 and 38.42 +/- 14.28, respectively. Group A consisted of 24 (80%) males and 6 (20%) females while group B consisted of 24% females and 76% males. Considering fracture based on OTA criteria (p = 0.4) and severity of soft tissue damage based on Tscherne classification (p = 0.6), there was no statistically meaningful difference between groups A and B. The study demonstrated that degree of horizontal displacement, mean time of surgery, post-operation infection, organ shortness at the end of the follow-up period, organ deviation in patients of the group A was significantly more than that of the group B. Time required for callus formation (mean time of union), mean time of full weight bearing time and mean time of return to normal activities in group B was significantly more than that of the group A.  相似文献

To determine the effect of adding ketamine to pethidine in reducing post-operative pain in patients undergoing major abdominal operations, in a double blind randomized controlled trial, 100 patients aged 15-60 years who were candidate for elective major abdominal surgery allocated into two groups of pethidine + ketamine group (5 mg pethidine and 0.25 mg kg(-1) ketamine) or pethidine and placebo group (10 mg pethidine and NS) according to the regimen prescribed in postanesthesia care unit. Severity of pain (using visual analogue scale), prescribed dose of pethidine and side effects were recorded until 24 h after operation. Regarding post-operative pain, pethidine + ketamine group showed significant lower scores in all the times except 0 min, 2, 6 and 16 h. Nausea was significantly less frequent amongst pethidine + placebo group at times of 0, 15, 30 and 45 min (p < 0.05). Comparison of two groups did not show significant differences in prescribed pethedine dose in 0, 9, 12, 16, 20 and 24 h (p > 0.05). Yet, the mean dose of administered pethidine as rescue analgesic was significant lower in pethidine + ketamine group compared to pethidine + placebo group (112 +/- 31.5 mg vs. 133.5 +/- 24.5 mg, p < 0.001). In conclusion, our results showed that co-administration of ketamine and pethidine in postanesthesia care unit will improve postoperative pain and reduce narcotic consumption. It may, however, increase rate of postoperative nausea in the first hour after operation.  相似文献

Distal radius fractures are among the most common bone fractures all over the world. Close reduction and fixation by percutaneous pinning is a less invasive method comparing with other open surgeries. This study aims at evaluating the functional outcome of this treatment in distal radius fractures. In this prospective study, 50 patients with distal radius fractures underwent percutaneous pinning during a 16 month period in Tabriz Shohada Hospital. Follow-up for 3 months was possible in 48 (96%) patients. Possible complications (such as infection, nonunion, pin loosening and pain), functional outcomes (including range of motion, grip and pinch strengths) and radiological outcome were documented in intervals during follow-up. Forty eight patient, 22 males and 27 females with a mean age of 47.1 +/- 13.7 (19-80) years were enrolled. Local pain, infection and pin loosening were documented in 14.6, 16.7 and 2.1% of cases, respectively. Based on radiological reports, unacceptable palmar tilt, articular step, radial inclination and radial shortening were present in 6.3, 0, 2.1 and 6.3% of cases, respectively three months post-operation. The clinical outcomes according to Cooney's modification of the Green and O'Brien scheme were as excellent, good, fair and poor in 52.1, 31.3, 10.4 and 6.3% of cases, respectively. According to these results, closed reduction and percutaneous pinning of distal radius fractures is a rather successful method with minor complications.  相似文献

Preeclampsia is a disease regarding with altered vascular reactivity leading to hypertension of the mother and metabolic alterations in the fetus. This study aimed to assess nitric oxide and bleeding time following administration of magnesium sulfate to preeclamtic patients compared to normotensive pregnant women. A total of 112 subjects (56 preeclamtic patients and 56 normotensive pregnant controls) were enrolled in this case-control study. Cases and controls were matched for age, BMI, gestational age, parity and gravidity. Total concentration of nitrite and nitrate (NOx) was measured before and during magnesium sulfate (MgSO4) treatment using a modified Griess-based method. RESULTS: Systolic and diastolic blood pressures were significantly decreased during MgSO4 treatment in preeclamtic patients (p < 0.0001). NOx levels were significantly increased in preeclamtic women after MgSO4 administration (33.7 +/- 18.5 vs. 50.2 +/- 21.6, p < 0.0001) but it was not seen in normotensive parturients (52.4 +/- 28.9 vs. 57.3 +/- 21.7, p = 0.362). The bleeding time was scarcely increased following magnesium sulfate treatment in preeclamptic patients compared to normotensive pregnant women but it was not significant (p = 0.18). In addition, there was only a significantly reverse correlation between NOx levels and systolic or diastolic blood pressure in preeclamtic parturients after MgSO4 treatment (r = -0384; p = 0.003 and r = -0.29; p = 0.03, respectively). This study demonstrates that administrating MgSO4 to preeclamtic patients induced significant changes in NOx production which had a major role in modulating vasculature changes in preeclamsia.  相似文献

The goal of this study was to investigate the role of procalcitonin (PCT) in diagnosis of neonatal sepsis and its correlation with C-Reactive Protein (CRP). One hundred and seventeen neonates with the gestational age > or = 35 weeks with clinically suspected diagnosis of neonatal sepsis were studied during one year from 2007 in Tabriz Children's Hospital. Conventional sepsis workup was done in all cases and the diagnosis of neonatal sepsis was proved based on the results of blood culture. The serum procalcitonin was measured by quantitative Chemo-luminance methods and the results were compared with CRP levels between the neonates with and without proven sepsis. The results showed among in 117 neonates with suspected sepsis 27 (23.1%) cases have positive blood culture (proven sepsis). The mean levels of PCT in neonates with and without proven sepsis was 4.42 +/- 6.66 vs. 2.06 +/- 4.03 ng mL(-1) and CRP 33.98 +/- 36.81 vs. 12.30 +/- 20.42 mg L(-1) were significantly higher in neonates with proven sepsis (p = 0.026 and p < 0.001). The sensitivity, specificity, positive predictive value and negative predictive value of PCT (more than 2 ng mL(-1)) were 66.7, 50, 28.6, 83.3 and CRP (more than 3.5 mg L(-1)) were 70.4, 72.2, 43.2 and 89%, respectively, in diagnosis of neonatal sepsis. There was a meaningful correlation between the level of PCT and CRP in the sepsis group (r = 0.797, p < 0.001). The results of the current study showed that more relying on the level of PCT and CRP for planning the management of neonates with suspected sepsis is not logical, but a negative result may be helpful in ruling it out.  相似文献

Keratoconus is pathological condition of eye characterized by paraxial stromal thinning leads to corneal surface distortion. It is well documented that the corneal thickness is abnormally altered in keratoconus; but it seems that racial differences are exist in the amount of changes. This study compared the central and peripheral corneal thicknesses (CCT and PCT, respectively) in eyes with and without keratoconus in a large Iranian population. CCT and PCT were measured by ultrasonographic pachometry in ten thousands eyes undergoing refractive surgery during a six-year period (2003-2010) in Tabriz Nikookari Teaching Eye Hospital. These two variables were compared between the eyes with and without keratoconus. Two hundred and two eyes out of ten thousand eyes were diagnosed with keratoconus. The mean CCT and PCT were significantly lower in the eyes with keratoconus comparing with the others (CCT: 479.90 +/- 28.07 microm vs. 534.12 +/- 39.35 microm; p < 0.001; PCT: 515.04 +/- 148.44 microm vs. 560.78 +/- 32.93 microm; p < 0.001). This study confirmed that both CCT and PCT are lower in the eyes with keratoconus. Likewise, due to large sample size, the results can be used for epidemiological purposes.  相似文献

This study aims at assessing the relation between hypoalbuminemia and inhospital mortality in patients with acute stroke. In this cross-sectional study, a total of 112 consecutively admitted patients with first acute stroke in Ahvaz Jundishapur Hospital were enrolled. All patients hospitalized for 7-9 days or expired in this period of time. Admission hypoalbuminemia (serum albumin < 3.5 mg dL(-) as well as other prognostic factors were determined and compared between the two groups. One hundred and twelve patients, 74 males and 38 females with the mean age of 69.2 +/- 12.1 (66-79) years were enrolled. Twenty nine (25.9%) patients expired during the hospitalization. Totally, admission hypoal buminemia was present in 49 (43.8%) patients. 43% of the patients were Hypoalbuminemia. Frequency of patients with admission hypoalbuminemia and is chemic heart disease was significantly higher in the nonsurvivors (75.8-32.5%, p = 0.001; 34.5 vs. 16.9%, p = 0.047, respectively). The mean Glasgow Coma Scale score was significantly lower in the nonsurvivors (8.1 +/- 1.8 vs. 9.4 +/- 0.5, p = 0.003). These three parameters remained significant in multivariate analysis. Sex, age, history of hypertension, diabetes mellitus, hypertriglyceridemia, hypercholesterolemia, anemia, heart failure and smoking habit, admission atrial fibrillation, ethnicity and stroke type were comparable between the two groups. In conclusion, hypoalbuminemia is an independent predictor of inhospital mortality in patients with acute stroke.  相似文献

Previous studies have shown that inflammatory factors increases in pregnancy and is associated with several complications of pregnancy. The aim of this study was to assess effects of daily consumption of probiotic yoghurt on inflammatory factors in pregnant women. In a randomized clinical trial, seventy primigravid (the first pregnancy) and singleton pregnant women aged 18-30 years were assigned to two groups. Subjects consumed daily 200 g probiotic yoghurt containing Lactobacillus acidophilus La5 and Bifidobacterium animalis BB12 (10(7) CFU g(-1) for each) or 200 g conventional yoghurt for 9 weeks. Fasting blood samples were collected at baseline (28 weeks of gestation) and after intervention (37 weeks of gestation). Inflammatory factors, hs-CRP and TNF-alpha, were measured by Enzyme-linked Immunosorbent Assay (ELISA). Independent t-test was used to compare the two groups after intervention and paired-sample t-test compared variables before and after treatment. The results showed that the probiotic yogurt brought about a decrease in the serum hs-CRP level, from 10.44 +/- 1.56 to 7.44 +/- 1.03 microg mL(-1) (p = 0.041). There was no significant change in the conventional yogurt group in the serum hs-CRP level (12.55 +/- 1.57 to 14.51 +/- 1.62 microg mL(-1), p = 0.202). The probiotic yogurt had no effect on TNF-alpha (from 73.75 +/- 6.59 to 77.91 +/- 5.61 pg mL(-1), p = 0.633). Serum TNF-alpha did not change in the conventional yogurt group (p = 0.134). In conclusion probiotic yogurt significantly decreased hs-CRP in pregnant women but had no effect on TNF-alpha.  相似文献

To investigate the effects of copper and superoxide dismutase (SOD) content of seminal plasma on buffalo semen characteristics, 54 semen samples collected from buffalo bulls by a bovine artificial vagina were used. Semen characteristics (motility, viability, morphology, concentration and volume) were recorded. Seminal plasma was harvested by centrifugation and kept frozen until analysis. Seminal plasma copper content was determined by atomic absorption procedure and SOD was measured by using a kit. The mean total copper value of seminal plasma was recorded as 2.51 +/- 0.04 mg kg(-1) (Mean +/- SEM) and the mean total SOD values was 39.02 +/- 0.81 IU mL(-1). To reduce the range of variability, the data were categorized according to their motility records in 3 groups of Excellent (Ex, >90% motile, n = 33), Good (Go, 80-89% motile, n = 15) and Moderate (Mo, < 79% motile, n = 6). The mean motility, viability, copper and SOD values in Ex group was recorded as 92.24 +/- 0.51%, 94.00 +/- 0.48%, 2.56 +/- 0.04 mg kg(-1) and 39.52 +/- 0.57 IU mL(-1), respectively. These values were 81.66 +/- 0.62%, 85.26 +/- 0.95%, 2.38 +/- 0.11 mg kg(-1) and 36.48 +/- 1.51 IU mL(-1) in Go group and 71.66 +/- 1.05%, 77.00 +/- 2.94%, 2.55 +/- 0.10 mg kg(-1) and 50.66 +/- 2.51 in Mo group, respectively. The mean copper value in Ex group was highly (r = 0.600) correlated with SOD and correlated with sperm motility (r = 0.372) and viability (r = 0.363), while, in Go group it was highly correlated (r = 0.945) with SOD and sperm viability (r = 0.652) and in Mo group it was correlated (r = 0.874) with semen volume only. The mean SOD values in Ex group was highly correlated with sperm motility (r = 0.492) and viability (r = 0.490) and mean copper values, in Go group, it was highly correlated whit sperm viability (r = 0.659) and mean copper values and in Mo group it had no significant correlations with semen parameters. These results suggest that copper and SOD content of the buffalo seminal plasma have an influence on the sperm motility and viability which are the most important factors in semen fertility.  相似文献

Changes of cervical ultrasonographic physical parameters and their association with outcome of cerclage have not been fully elucidated. This study aimed at evaluating association between ultrasonographic physical cervical shapes before and after McDonald cerclage with the length of pregnancy. Cervical length, width and funneling rate were assessed by transvaginal ultrasonography before and after the procedure. Mean pre-cerclage cervical length, width and funneling rate were 29.28 +/- 5.54, 19.92 +/- 10.30 mm and 12.90 +/- 18.87%, respectively. The mean post-cerclage cervical length, diameter and funneling rate were 31.68 +/- 4.21, 9.78 +/- 13.86 and 4.32 +/- 10.89%, respectively (p < 0.001 for all pre- and post-cerclage changes). The mean post-cerclage duration of pregnancy was 19.78 +/- 5.94 weeks. There were significant direct correlations between the pre-and post-cerclage cervical lengths with pregnancy duration (r = 0.24, p = 0.04 and r = 0.44, p < 0.001, respectively). There were significant indirect correlations between the pre- and post-cerclage funneling rate with pregnancy duration (r = -0.30, p = 0.01 and r = -0.20, p = 0.01, respectively). Similar correlations were not significant for the cervical widths. This study showed that the pre- and post-cerclage ultrasonographically determined cervical length and funneling are good predictors of pregnancy duration.  相似文献

Prediction of outcome is difficult in patients with acute upper gastrointestinal bleeding (AUGIB). Some factors have been proposed in this regard with varying accuracy. This study aimed to investigate probable predictors of in-hospital outcome in patients with AUGIB. One hundred sixty four patients with AUGIB were studied prospectively in Tabriz Imam Reza Teaching Centre. All these patients were evaluated endoscopically by an expert. Patients' age, gender, presenting complains, transfusion, clinical findings and previous medical history were compared between survived vs. expired, re-bled vs. non re-bled and operated vs. non operated patients. There were 117 males and 47 females with the mean age of 57.12 +/- 17.32 (range: 32-78) years in this study. Hematemesis was the sole independent predictor of in-hospital mortality (82.1 vs. 100%; p < 0.001). In univariate analysis, however, female gender, major hemorrhage and previous neurological disease were associated with higher rate of expiration. Comparing two re-bled and non re-bled groups, hematemesis (76.5 vs. 95.9%; p = 0.003) and need of transfusion > 2U (36.1 vs. 71.4%; p = 0.006) were independent predictors of re-bleeding. In univariate analysis, hematocrit < 30%, major hemorrhage and previous history of hepatic disease or hypertension were predictive of re-bleeding. In comparison between operated and non operated groups no significant predictor was detected. In conclusion, this study showed that presence of hematemesis at the time of admission and need of transfusion > 2U were independent predictors of poor outcome in patients with AUGIB.  相似文献

This experiment was conducted to identify the annual changes in hair follicle activity, changes at the follicular level and to characterize some of the fibre-follicle characteristics of camels at different ages. A total of 28 camels were allocated at random on the basis of age to one of four groups (2, 4, 6 and older than 8 years). All groups of camels were fed a maintenance level of ration throughout the experiment. To determine hair follicle cycle and other follicle characteristics samples of skin were taken using a trephine from the right midside of animals at approximately 28 day intervals for a period of 12 months. Using a small hand clipper, 15 g of fibre sample was taken from the left midside region to determine fibre characteristics. Analysis of variance was performed using a one-way SAS package and the means and the standard deviations of means were generated with this program. Mean S/P ratio, primary and secondary and total follicle densities of all groups of camels were 6.85 +/- 0.75, 3.76 +/- 0.63, 22.29 +/- 3.57 and 25.33 +/- 3.85, respectively. Mean fibre diameter, percentage of medullated and non-medullated fibre and clean wool percentage of all groups were 18.98 +/- 1.64, 18.10 +/- 1.65, 81.89 +/- 6.98 and 77.58 +/- 4.58, respectively. Mean percentage of active primary follicles significantly (p < 0.05) decreased to lowest in February to a minimum of 83.1%, then significantly (p < 0.05) increased over spring. Secondary follicle activity decreased over winter and spring to a minimum of 60% in February.  相似文献

The role of Human Papillomavirus (HPV) has been suspected in pathogenesis of various malignancies; however, the available data are not conclusive. This study aimed to determine and compare the frequency of HPV infection in oral and lung Squamous Cell Carcinoma (SCC) by a sensitive method. Sixty specimens of oral and lung SCC (30 cases each one) were reevaluated in Tabriz Imam Reza Centre in a 24 month period. Following genomic DNA extract, the Polymerase Chain Reaction (PCR) amplification was performed in presence of specific MY11 and MY09 primers for HPV infection. Three cervical specimens and a combination of PCR solution lacking DNA plus healthy persons' DNA samples were employed as positive and negative controls, respectively. The oral group was significantly older than the lung group (68.90 vs. 56.67 y, p < 0.001) with more males in the latter (83.3 vs. 60%; p = 0.04). Percentages of HPV infection in the oral and lung groups were comparable (20 vs. 10%, respectively; p = 0.47). Majority of patients with HPV infection were older than 60 years (88.9%) or male (88.9%). In the oral group, all these cases were well differentiated and the majority was of lower lip origin (83.3%). In the lung group, 66.7% of these specimens were moderately differentiated and the origin was bronchus in all cases. In conclusion, the rate of HPV infection in lung and oral SCC samples is rather lower than the previous reports in the literature. This rate is apparently higher in the oral than the lung SCC specimens.  相似文献

This study was made on the effects of a normal diet containing 12.8 mg Cu kg(-1) DM which added gradually molybdenum and sulphur on the copper status and fibre quality in eight 1-year Merghoze goat. One group (n = 4 mean weight 31 +/- 2.0 kg) was treated with Mo and S supplements for 20 weeks, the second group (n = 4 mean weight 32 +/- 2.1 kg) served as controls. In addition of blood sampling for measuring copper status in plasma, the copper content and quality of fleeces were measured every 6 weeks. Mohair measurements were carried out by taking patch samples (10 x 10 cm2) from the mid-side area of the goat. The analytical set consists of plasma copper concentrations (Pl Cu), Trichloroacetic acid soluble copper concentrations (TCA-Cu), and fleece copper content. The results indicated that the addition of 20-30 mg Mo and 2-2.5 g S kg(-1) DM to the normal diet did produce sub clinical copper deficiency in treated goats. One such visual symptom was the loss of fleeces pigmentation and poorer crimp being observed. The Pl Cu minus TCA-Cu plasma became more than 2 microM in the blood of treated goat, indicating that there was a significant thiomolybdate formation in the body. The results showed that there was a significant decrease in Pl Cu (p < 0.05) along with a significant increase in thiomolybdate (MoS) production after 4 months. The sub clinical signs of copper deficiency and mohair quality are likely to be from high molybdenum intake and thiomolybdate formation in the body.  相似文献

Background: Selective phosphodiesterase (PDE3) inhibitors improve cardiac contractility and may use in congestive heart failure. However, their proarrhythmic potential is the most important side effect. Methods: In this research, we evaluated the potential cardiotonic activity of six new synthesized selective PDE3 inhibitors (6-hydroxy-4-methylquinolin-2(1H)-one derivatives) using the spontaneously beating atria model. In each experiment, atrium of reserpine-treated rat was isolated and the contractile and chronotropic effects of a synthesized compound were assessed. The 3-isobutyl-1-methylxanthine, a non-selective PDE inhibitor, was used for comparison. Results: The results showed that, among new compounds, the best pharmacological profile was obtained with the compound 6-[4-(4-methylpiperazine-1-yl)-4-oxobutoxy]-4-methylquinolin-2(1H)-one, C6, which displayed selectivity for increasing the force of contraction (168 +/- 5% change over the control) rather than the frequency rate (138 +/- 5% change over the control) at 300 muM. However, C6 at concentrations of 10 and 100 muM produced left and upward shift in the positive inotropic concentration-response curve of isoprenaline. The -log EC50 of isoprenaline was 8.843 +/- 0.171 in the absence, 9.448 +/- 0.138 and 9.456 +/- 0.107 in the presence of 10, 100 muM of C6, respectively (P<0.001, n = 6). Also, amrinone, a selective PDE3 inhibitor, shifted the isoprenaline concentration-response curve to the left and upward. The concentration of 10 and 100 muM amrinone decreased -log EC50 of isoprenaline to 9.527 +/- 0.287 and 9.423 +/- 0.243, respectively (P<0.001, n = 6). Moreover, the positive chronotropic effect of isoprenaline was not affected by amrinone or C6. Conclusion: This study provides functional evidence for the positive inotropic effect of C6. Considering the augmentation of isoprenaline positive inotropic concentration-response with C6 and amrinone, we conclude that C6 produces its effect via potentiation of cAMP-dependent signaling system and possibly by inhibition of PDE3 activity.  相似文献
