Caslick's, episioplasty, urethroplasty, cervical, and rectovestibular laceration repair are, by preference, performed in the standing patient. Normal castration is performed routinely uneventfully in the standing patient. The duration of the procedures and the ease with which they are accomplished in a standing horse of normal temperament make general anesthesia unnecessary. More invasive procedures, such as removal of urinary bladder calculi in male horses or removal of a neoplastic ovary in a mare, can be accomplished in a standing patient but serious consideration should be given to the use of general anesthesia. For these procedures, the potential complications are increased while the ability to effectively deal with those complications is diminished in a standing patient. For instance, imagine a mare that lays down in the stocks during a standing flank removal of an ovary or a stallion that defecates during a Gokel's approach to the bladder. Only extenuating circumstances justify these risks when general anesthesia is available.  相似文献

Summary Environmental variables affecting anaesthesia and surgery in the tropics are defined and discussed. It was concluded that the practical application of anaesthetic techniques in the tropics requires a wider appreciation of the physical environment than in temperate zones but there is no evidence that changes of either latitude or altitude have any effect on the principles of anaesthesia and surgery. The implications of these conclusions with regard to surgical practice and veterinary education are discussed.
Sumario Se discuten y definen variables ambientales que afectan la anestecia y la cirugia en los trópicos. Se concluyó que la aplicación práctica de las técnicas de anestecia en los trópicos requiere de una apreciación mas amplia del ambiente físico que en las zonas temperadas, pero no hay evidencia de que los cambios en altitud ó latitud, tengan algún efecto sobre los principios de anestecia y cirugía. Se discuten las implicaciones de estas conclusiones con referencia a la práctica de la cirugía y la educación veterinaria.

Résumé Les facteurs variables affectant l’anesthésie de la chirurgie sous les tropiques sont définis et discutés. On conclut que l’application pratique de techniques d’anesthésie sous les tropiques exige une appréciation plus étendue du milieu physique que dans les zones tempérées, mais il n’y a pas de preuve que des changements, soit en latitude, soit en altitude, ont un effet quelconque sur les principes de l’anesthésie et de la chirurgie. Les implications de ces conclusions par rapport à la pratique chirurgicale et l’éducation vétérinaire sont discutées.

Esophageal disorders are relatively uncommon in cattle. The most frequently encountered problem is foreign-body obstruction, or choke. Other disorders may include perforation, rupture, stricture, diverticulum, and megaesophagus. Factors responsible for the high rate of complications associated with esophageal surgery include the lack of a serosal layer, movement during deglutition and reverse peristalsis, and an easily interrupted segmental blood supply. Important considerations of esophageal surgery include minimization of blood supply disruption, avoidance of transverse esophageal incisions whenever possible, provision of leak-proof closure of an incision or anastomosis and adequate drainage for the surrounding soft tissues, and restriction of postoperative oral alimentation and handling.  相似文献

Periodontal surgery requires the operator to have specific knowledge and skills. The use of proper equipment will improve the operator's skill and will decrease surgical time. It will also result in less tissue trauma, thus increasing healing time and reducing postoperative complications and patient discomfort. Various surgical procedures can be enhanced with the usage of equipment that is designed for specific tasks of the procedure. Retinal detachment is common in dogs and presents major challenges for the veterinary ophthalmologist. This article reviews recent advances in both technology and technique that have greatly increased the success rate of retinal reattachment surgery. Reconstruction of facial defects can be very challenging, even in the hands of an experienced surgeon. Most defects can be repaired using local available tissues. Lip defects can be reconstructed using geometric closure techniques and advancement of local tissues. Forehead defects can be reconstructed using skin grafts, caudal auricular flaps, or rotational or transposition flaps. Care must be taken to minimize tension on eyelids and maintain the integrity of the facial nerve. Cheek defects can be reconstructed using local advancement, superficial temporal, omocervical, and caudal auricular flaps. The bridge of the nose can be very difficult to reconstruct due to a paucity of local tissues; however, skin grafts, indirect pedicle flaps, and superficial temporal flaps can be considered. .  相似文献

Based on the clinical impression and the current knowledge of the clinical variables (rectal examination, abdominal distention, abdominal fluid, intractable pain) most likely to differentiate between medical and surgical cases, three decision trees are provided as a guide to making the management decision in a horse with abdominal pain. Prior elimination of horses with limited life expectancy because of age or function or where financial constraints are present is understood. It must be emphasized that most of the information presently available originates from referral centers where the prevalence of surgical candidates is much higher than in general practice. This would influence the accuracy of the clinician in identifying horses with a surgical lesion or estimating survival (positive predictive value).  相似文献

A clear understanding of the surgical anatomy of the bovine intestinal tract is the most important factor in determining the degree of success to be achieved in the surgical management of intestinal disorders in cattle. However, bovine intestinal anatomy can be very difficult to comprehend, especially in cattle with intestinal distention and malposition. The abundance of mesenteric and omental fat and the compact nature of the mesentery make manipulation and exteriorization of many parts of the bowel difficult. Therefore, it is important to accurately identify specific surgical landmarks. A good working knowledge of the relevant surgical anatomy increases the level of confidence in one's surgical abilities and makes one more willing to consider surgical intervention in a cow with suspected GI dysfunction.  相似文献

Orthopedic surgery in rabbits poses several unique parameters for the veterinary surgeon. It is imperative for the veterinarian to be knowledgeable about the anatomic features of the surgical repair site and to become familiar with a rabbit's pain and discomfort often associated with orthopedic injuries. Avian surgery is an exercise of preparedness; this statement is never so true as in emergency and critical care situations. The techniques used in avian surgery are significantly different from those used in larger species and require microsurgical techniques. Before initiating a surgical procedure, the patient must be evaluated and, in many cases, prepared or preconditioned. Rarely is surgery such an emergency that the prognosis would not be improved by readying the patient.
