Inter-observer agreement for assessment of faecal consistency in pigs was evaluated using a scoring system with 3 categories. In a pilot study, 3 observers performed an examination of faecal samples post-collection. The samples were obtained from pigs (12-13 weeks old) in 4 herds with a history of diarrhoea associated with Lawsonia intracellularis, Brachyspira spp. and/or Porcine Circovirus Type 2. Observer 1 examined all the faecal samples from the 4 herds. Observer 2 only examined the faecal samples from herds 1 and 2. Observer 3 only examined the faecal samples from herds 3 and 4. We observed a substantial agreement in faecal consistency scores between Observers 1 and 3 (kappa=0.64, 95% CI: 0.51-0.78). In contrast, only a fair agreement was observed between Observers 1 and 2 (kappa=0.24, 95% CI: 0.14-0.34). The variations in inter-observer agreement detected in the current study suggest that misclassification error can be a problem in studies assessing faecal consistency. Solutions may include developing a standardized system for scoring the consistency of pig faeces, calibration when more than one observer is involved in clinical studies and using a more objective measure of faecal consistency.  相似文献

Microwave drying as a procedure for determination of faecal dry matter in weaned pigs was evaluated and clinical relevant cut-off values between faecal consistency scores were determined. Repeatability and reproducibility were evaluated. Overall coefficient of variation was 0.03. The 95% confidence limits for any future faecal subsample examined by any operator in any replica were ± 0.85% faecal dry matter. Robustness in relation to weight of wet faeces was evaluated. The weight categories were 0.5, 1.0, 1.5, 2.0 and 3.0 g. Samples of 0.5 g gave significantly different mean faecal dry matter content compared to weighing of 1.0-3.0 g. Agreement with freeze-drying was evaluated. Lin's concordance correlation coefficient was 0.94. On average the faecal dry matter values was 1.7% (SD=1.99%) higher in freeze dried compared to micro waved samples. Non-parametric ROC analyses were used to determine optimal faecal dry matter cut-off values for clinical faecal consistency scores. The 4 consistency scores were score 1=firm and shaped, score 2=soft and shaped, score 3=loose and score 4=watery. The cut-off values were score 1: faecal dry matter content >19.5%, score 2: faecal dry matter content ≤ 19.5% and >18.0%, score 3: faecal dry matter content ≤ 18.0% and >11.3%, score 4: faecal dry matter content ≤ 11.3%. In conclusion, the microwave procedure has an acceptable repeatability/reproducibility and good agreement with freeze drying can be expected. A minimum of 1.0 g of wet faeces must be used for analyses. Faecal dry matter cut-off values between 4 different clinical consistency scores were determined.  相似文献

In 2004,five veterinarians examined 283 dairy cows from four Danish dairy herds and independently assigned scores for the clinical signs lameness, hock lesions and other cutaneous lesions to each cow using a clinical protocol. We evaluated the inter-observer and the intra-observer agreement using prevalence-adjusted, bias-adjusted kappa (PABAK). We chose two different cut-offs between the ordinal scores for classifying cows as healthy or diseased for each of the clinical signs, and we compared the ability of the observers to discriminate between healthy and diseased cows for the two cut-offs. Without any formal training of the observers we found PABAK in the range 0.36-0.88 and we concluded that the choice of cut-off had an effect on the observed agreement among the observers.  相似文献

The reliability of abdominal auscultation was investigated via an observer variation study. Clinicians listened to a variety of minute-long equine gut sound recordings. They evaluated the amount of gut sounds as 'absent', 'decreased', 'normal', or 'increased'. They subsequently evaluated the same recordings replayed in a different order. Intra- and inter-observer agreement was measured by the statistic kappa. There was significant intra-observer (kappa 0.57) agreement, but less agreement between observers (kappa 0.37). The best agreement was on the classification of sound tracks as 'absent' (intra-observer kappa 0.72 and inter-observer kappa 0.55). There was significant correlation between the clinicians' average assessment of the recordings and their acoustic energy levels. In this study abdominal noise was reliably assessed by auscultation. Standardised techniques and definitions would probably enhance the reliability of abdominal auscultation for the evaluation of gastrointestinal disease.  相似文献

A seven point locomotion scoring scale, ranging from 0=normal locomotion to 6=unable to stand or move, has been developed. To test the between and within observer reliability of the scale, 65 movie clips of sheep with normal and varying degrees of abnormal locomotion were made. Three observers familiar with sheep locomotion were trained to read the movie clips. Thirty clips were randomly selected and used to test between and within observer agreement. There was high inter-(intra-class correlation coefficient [ICC]=0.93, weighted kappa [kappa(w)]=0.93) and intra-(ICC=0.90, kappa(w)=0.91) observer reliability, with no evidence of observer bias. The main between score differences were for scores 0 (normal) and 1 (uneven posture and shortened stride but no head movement). The results indicate that the locomotion scoring scale using groups of defined observations for each point on the scale was reliable and may be a useful research tool to identify and monitor locomotion in individual sheep when used by trained observers.  相似文献

Background

The risk of injuries is of major concern when keeping horses in groups and there is a need for a system to record external injuries in a standardised and simple way. The objective of this study, therefore, was to develop and validate a system for injury recording in horses and to test its reliability and feasibility under field conditions.

Methods

Injuries were classified into five categories according to severity. The scoring system was tested for intra- and inter-observer agreement as well as agreement with a ''golden standard'' (diagnosis established by a veterinarian). The scoring was done by 43 agricultural students who classified 40 photographs presented to them twice in a random order, 10 days apart. Attribute agreement analysis was performed using Kendall''s coefficient of concordance (Kendall''s W), Kendall''s correlation coefficient (Kendall''s τ) and Fleiss'' kappa. The system was also tested on a sample of 100 horses kept in groups where injury location was recorded as well.

Results

Intra-observer agreement showed Kendall''s W ranging from 0.94 to 0.99 and 86% of observers had kappa values above 0.66 (substantial agreement). Inter-observer agreement had an overall Kendall''s W of 0.91 and the mean kappa value was 0.59 (moderate). Agreement for all observers versus the ''golden standard'' had Kendall''s τ of 0.88 and the mean kappa value was 0.66 (substantial). The system was easy to use for trained persons under field conditions. Injuries of the more serious categories were not found in the field trial.

Conclusion

The proposed injury scoring system is easy to learn and use also for people without a veterinary education, it shows high reliability, and it is clinically useful. The injury scoring system could be a valuable tool in future clinical and epidemiological studies.  相似文献

Consistency of assessment is essential to the farm assurance process. This study evaluated the inter-observer reliability of 31 farm assurance assessors, six veterinarians and four researchers for five pig welfare outcome measures proposed for inclusion into the UK pig farm assurance schemes. These were (1) tail lesions, (2) body lesions, (3) lameness, (4) pigs requiring hospitalisation and (5) oral behaviour. The following inter-observer reliability testing methods against a gold standard Trainer were used: a comparison of farm prevalence and the numbers of affected pigs in each pen identified by observers, Cohen's kappa (κ), Kendall's W, proportional agreement, sensitivity, and specificity. All measures achieved potentially high levels of inter-observer reliability and it was concluded that none should be excluded from farm assurance at this stage. However, across all the measures, 45% of observers did not record an overall farm prevalence 'close' to that of the gold standard Trainer. With the level of training and testing that took place in this study there would be a danger of significant bias occurring in a national assessment scheme. The data collected enabled some comparison of the methods used to assess inter-observer reliability. It is suggested that when the aim is to achieve agreement between observers on the overall farm prevalence the inter-observer reliability testing should focus on the closeness of the overall farm prevalence recorded by observers, but that other types of analysis may be helpful during training.  相似文献

Background

Overconsumption of antibiotics in the pig industry is of concern in relation to antimicrobial resistance. False positive disease diagnosis may result in the treatment of healthy animals. In Denmark, diarrhoea is the most common cause of antibiotic treatment in pigs. Farm personnel are not professional clinicians, which could result in inappropriate antibiotic treatments of diarrhoea.The primary objectives of this pilot study using digital pictures of faecal pools was to evaluate farmers’ diagnostic performance in the assessment of faecal consistency in nursery pigs and to investigate the effect of different co-variables, including practical experience. A secondary objective was to compare the diagnostic performance of farmers with that of veterinarians.At a pig congress, observers (farm personnel and veterinarians) working professionally with pigs participated in a faecal consistency test consisting of 16 pictures of faecal pools (eight diarrhoeic and eight non-diarrhoeic). The faecal pools had previously been collected and subjected to faecal dry matter determination. The true status of the faecal pools was determined by the faecal dry matter content (diarrhoea: faecal dry matter ≤ 18%). The true status was used to evaluate the farmers’ and veterinarians’ diagnostic performance.

Results

A total of 119 farmers and 18 veterinarians were included in the statistical analysis. For the farmers, the mean proportion of faecal pools assessed as diarrhoeic was 0.48, the mean proportion of correctly classified faecal pools was 0.84, the mean diagnostic sensitivity was 0.83 and the mean diagnostic specificity was 0.86. Farmers with less than four years of practical experience detected clinical diarrhoea more accurately than farmers with more than four years of practical experience (p < 0.05). No significantly differences between farmers and veterinarians was observed (p > 0.20).

Conclusions

The results, using digital pictures of faecal pools, suggest that farmers and veterinarians have similar diagnostic performance in relation to diarrhoea. False positive classification of non-diarrhoeic pigs appears to be a larger problem than false negative classification of diarrhoeic pigs under Danish conditions. If these results can be confirmed under practical conditions, training in, and validation of, clinical diagnoses may be an important factor in reducing antibiotic consumption in the pig industry.  相似文献

Eighteen observers were influenced to different extents in the grades of lameness they allocated to eight horses by whether they knew that a nerve block had been administered; on a scale from 0 to 10 the mean difference in grade allocated once the observer knew a horse had been nerve-blocked was increased by 0.4. The consistency of the assessments made by the individual observers was good, with a an average of 0.6 of a grade difference when grading the same horse on two occasions. The agreement between the assessments of four orthopaedic experts was reasonable (+/-1 grade), but significantly poorer for four non-experts and 10 final year veterinary students.  相似文献

Aims: To assess the inter-observer agreement for detecting bovine digital dermatitis (BDD) lesions in digital colour photographs of the hind feet of cows, which had been taken while the animals were standing to be milked, between two trained observers.

Methods: Thirty-six photographs were selected from a total of 184 photographs held by the first author (R1), who had classified them as negative (n=11) or positive (n=25) for BDD. They were delivered to a technician (R2) who had previously visually inspected cattle for BDD lesions, and who then recorded the photographs as being either BDD-positive or BDD-negative. The percentage agreement between R1 and R2, and two other inter-observer agreement statistics, Cohen’s κ and Gwet’s first-order chance correction agreement coefficient (AC1), were calculated. The cumulative membership probabilities of Cohen’s κ and Gwet’s AC1 were then calculated for different benchmark ranges of κ.

Results: The percentage agreement between R1 and R2 was 33/36 (92%), Cohen’s κ was 0.80 (95% CI=0.57–1.0) and Gwet’s AC1 was 0.86 (95% CI=0.69–1.0). Based on the cumulative membership probabilities for Gwet’s AC1, there was 75% probability that the two observers had almost perfect agreement (κ≥0.81). For both Cohen’s κ and Gwet’s AC1, there was >95% probability that the two observers had at least substantial agreement (κ≥0.61).

Conclusions: The two trained observers had at least substantial agreement in identifying from a digital photograph as to whether BDD lesions were present or absent. Therefore results from the two could be used interchangeably.

Clinical Relevance: Visual assessment for BDD lesions in the milking parlour can be subjective. However a high agreement between these two trained BDD inspectors means BDD prevalence reported from different regions in New Zealand by these two can be directly compared.  相似文献

Reasons for performing study: Criteria for the radiographic evaluation of navicular bones in horses have been published to standardise classification of radiographic signs. However, intra‐ and interobserver agreement have not been established. Objective: To determine intra‐ and interobserver agreement in the evaluation of radiographic and computed tomographic (CT) navicular changes. It was hypothesised that: 1) intraobserver agreement would be better than interobserver agreement; 2) agreement would be better for CT than for radiography; and 3) pathological changes would be recognised with greater certainty with CT. Methods: Radiographs and CT scans of 60 cadaver navicular bones were evaluated by 3 observers using published criteria. A subset of 30 studies was evaluated twice by one observer. Agreement was tested using the kappa statistic. Certainty about pathological changes was evaluated by giving the observers the option to choose ‘not sure’. Results: Agreement varied from poor to almost perfect for radiographic evaluation and from poor to substantial for CT evaluation. For radiographic evaluation mean interobserver agreement was fair, as it was for CT evaluation. For radiographic evaluation mean intraobserver agreement was moderate as it was for CT evaluation. Pathological changes were evaluated with greater certainty on CT scans compared to radiographs; however, this was not associated with improved agreement. Conclusions: Variations in classification of navicular lesions in radiographic and CT studies were considerable between and within observers and challenge the use of such studies for diagnostic and prognostic purposes. Potential relevance: The results of this study allowed the identification of evaluation criteria with sufficient precision to be useful for navicular bone evaluation.  相似文献

The agreements between various culturing-method combinations and specimens to detect sheep naturally infected by Salmonella enterica IIIb 61:k:1,5,(7) were tested. Rectal swabs, faecal samples and ileo-caecal lymph nodes were collected from each individual. Rectal swabs called "group I" (n = 54) were cultured directly on selective media (selenite cysteine, SC). Rectal swabs called "group II" (n = 47) were pre-incubated in buffered peptone water. The four other combinations of culturing-method and specimen (one lymph node and three faecal) in each of the two groups were cultured directly on SC. The results from all the combinations of method and specimens were compared to the result from the rectal-swab test by kappa and McNemar's chi-square. Kappas for the agreement between rectal swabs and faecal sample tests were only 0.4. The lymph-node test agreed even-more poorly, with mean kappa value -0.02. The McNemar's test revealed that the discordancies between the rectal-swab test and all of the other tests went in both directions but sometimes did show bias (directionality) in the discordancies.  相似文献

Four observers performed a standard clinical examination of finisher pigs in two commercial finisher herds. In herd 1, 600 finisher pigs in 44 pens were examined. The observers assessed clinical signs of lameness, umbilical hernia and tail bite according to a standardized procedure. The prevalence of the clinical signs was estimated at the pen level. The procedure was repeated after 3 months in another herd, where 730 finisher pigs in 69 pens were examined. The agreement between observer pairs was assessed using prevalence-adjusted bias-adjusted kappa (PABAK) and proportionate-agreement estimates (Ppos and Pneg).

Observer bias was present despite training and standardization of the participating observers. The highest pen level agreement for the observer pairs was found for pens that had one or more pigs with tail bite (PABAK = 0.82–1.00) and umbilical hernia (PABAK = 0.77–1.00). The agreement was fair-to-moderate for identification of pens holding one or more lame pigs (PABAK = 0.27–0.71). In general, the average agreement of observer pairs on absence of clinical signs (Pneg) was higher than for presence (Ppos). The observer bias varied between observer pairs and with the clinical signs.  相似文献

Prevalence and seasonal variations of helminth infections and their association with morbidity parameters were studied in traditionally reared Cambodian cattle. Four villages in two provinces of West Cambodia were visited on monthly intervals over a period of 11 months, during which 2391 animals were faecal and blood sampled for parasitological and haematological examinations. The body condition score (BCS), faecal consistency (diarrhoea score, DS), colour of the ocular conjunctivae (FAMACHA(?)) and packed cell volume were determined for each individual animal. The overall proportion of samples that was positive for gastrointestinal nematodes was 52%, 44% and 37% in calves (from 1 to 6 months), young animals (6 to 24 months) and adults (over 24 months), respectively, while geometric mean faecal egg counts (FECs) for each of these age categories were 125, 66 and 15 eggs per gram, respectively. Six genera of strongyles were found in the faecal cultures, i.e. in descending order of occurrence, Cooperia, Oesophagostomum, Haemonchus, Trichostrongylus, Mecistocirrus and Bunostomum. The prevalences of Fasciola and Paramphistomum, estimated by coprological examination, varied between 5-20% and 45-95%, respectively. Logistic mixed models were used to investigate associations of morbidity markers with the presence of parasite infection. A low BCS was associated with gastrointestinal nematode and liver fluke infections, and soft faecal consistency with Paramphistomum infections. However, other factors such as nutritional deficiencies and intercurrent diseases are likely to enhance the effects of parasites and should therefore be considered when using these morbidity parameters as indicators of parasitism.  相似文献

Assessments of animal-based outcomes form the core of routine veterinary clinical examinations and are being increasingly used as indicators of animal welfare. A method of group observation that did not require gathering and handling of individual sheep, was used to assess eight animal-based indicators of sheep welfare: demeanour, skin irritation, wool loss, excessive panting, coughing, lameness, and cleanliness of the ventral abdominal and 'breech' (perineum/gluteal/caudal hindlimb) areas. The inter-observer reliability of two or three observers who independently assessed these indicators was tested on 2406 adult sheep and growing lambs across 36 farms and the intra-observer reliability of an experienced, veterinary assessor--the 'test standard observer'--was assessed on 88 adult sheep during four on-farm assessments. Observer reliability was evaluated using Cronbach's alpha (α) comparison of the recorded proportion (%) of sheep affected by each welfare condition and by binomial logistic regression modelling. High levels of inter-observer reliability were identified for the assessment of group lameness (α 0.76-1.00) and cleanliness of the breech area (α 0.97-1.00). Excellent intra-observer reliability was determined for lameness (α 0.99), cleanliness of the breech area (α 0.97), demeanour (α 1.00) and wool loss (α 1.00). In addition, proportion data and logistic regression models identified few between-observer differences. The results suggest that welfare outcomes based on observations of the behaviour and physical appearance of individual animals within a group may offer a reliable and feasible measurement tool for the on-farm assessment of sheep welfare.  相似文献

Insufficient agreement on scoring hip quality might be caused by differences in the assessability of a radiograph (exposure, contrast, positioning, and diagnostic quality). We studied the agreement in assessability of standard ventrodorsal hip-extended radiographs by experienced (nine) and inexperienced (21) observers, using the standard subjective method of quality control, currently applied in screening programs. The effect of assessability on the agreement of scoring hip quality [dysplastic vs. nondysplastic and the final Federation Cinologique International (FCI) score] was also investigated. There was a significant difference ( P <0.0001) in agreement on assessability between the experienced and inexperienced observers. In 68% of evaluations, experienced observers stated that the radiograph was assessable. Inexperienced observers evaluated the radiographs as being assessable in only 46.5% of evaluations. Increased interobserver agreement on assessability of a radiograph did not increase the overall interobserver agreement in the diagnosis of hip dysplasia, nor did it result in consistent scoring of the hip status from that radiograph, despite a significant ( P <0.05) increase in agreement of FCI scoring with an increasing agreement on assessability at a one to five ratio in the experienced group. The inconsistent evaluation of radiographic quality, as well as the inconsistent evaluation of the hip quality, caused differences in diagnosing hip dysplasia and FCI scoring in the same dog ranging from excellent hips to moderate hip dysplasia. Therefore, the credibility of the FCI screening method for canine hip dysplasia, using the standard hip-extended radiographic view, as currently applied in most European countries, is questionable.  相似文献

Experienced and inexperienced observers evaluated the assessability of 50 radiographs (25 dogs) and determined the hip status (dysplasia/nondysplasia and final scoring according Fédération Cynologique Internationale [FCI]‐criteria) individually. A radiographic technical quality assessment was performed in a separate reading session. Interobserver agreement in determining dysplasia/nondysplasia and FCI‐scoring did not significantly increase with the increasing quality of a radiograph, irrespective whether these observers are experienced or not. There was a significant agreement between the technical quality assessment and assessability (P<0.0005). Despite the effort to objectify radiographic quality and to present high‐quality radiographs to observers, interobserver agreement on dysplasia/nondysplasia and final scoring, remains low, even in the experienced group. Although increased radiographic quality narrows the range of scoring, the range remains unacceptably high.  相似文献

Canine meningiomas are currently graded using the human grading system. Recently published guidelines have adapted the human grading system for use in dogs. The goal of this study was to validate the new guidelines for canine meningiomas. To evaluate the inter-observer agreement, 5 veterinary surgical pathologists graded 158 canine meningiomas following the human grading system alone or with the new guidelines. The inter-observer agreement for histologic grade and each of the grading criteria (mitotic grade, invasion, spontaneous necrosis, macronucleoli, small cells, hypercellularity, pattern loss and anaplasia) was evaluated using the Fleiss kappa index. The diagnostic accuracy (sensitivity and specificity) was assessed by comparing the diagnoses obtained with the 2 grading systems with a consensus grade (considered the reference classification). The consensus histologic grade was obtained by agreement between 4 experienced veterinary neuropathologists following the guidelines. Compared with the human grading alone, the canine-specific guidelines increased the inter-observer agreement for: histologic grade (κ = 0.52); invasion (κ = 0.67); necrosis (κ = 0.62); small cells (κ = 0.36); pattern loss (κ = 0.49) and anaplasia (κ = 0.55). Mitotic grade agreement remained substantial (κ = 0.63). The guidelines improved the sensitivity in identifying grade 1 (95.6%) and the specificity in identifying grade 2 (96.2%) meningiomas. In conclusion, the new grading guidelines for canine meningiomas are associated with an overall improvement in the inter-observer agreement and higher diagnostic accuracy in diagnosing grade 1 and grade 2 meningiomas.  相似文献
