The purpose of this study is to report our initial experience with the use of spring coils to close the patent ductus arteriosus in the dog. There are few large-patient series reported in the veterinary literature. Coil closure was attempted in 15 dogs (median weight, 6.5 kg; range, 1.2 to 38.7 kg) presenting with a patent ductus arteriosus between May 1997 and May 1999. Arterial catheterization followed by angiography was used to decide if coil placement was adequate. A 5- or 8-mm embolization coil, depending on the angiographic diameter of the ductus, was delivered, with 1 loop in the pulmonary arterial side and the remainder of the coil in the aortic side of the duct. Additional coils were used if a residual shunt was present, and closure was confirmed by aortography. Patients were discharged the day after the procedure. Successful coil closure, without residual shunt on angiography, was achieved in 11 of 13 dogs in which coils were released. In 6 dogs, a coil embolized to the pulmonary artery. Four of these dogs had successful closure with multiple coils, and 2 others had surgery. None of these dogs experienced adverse effects. In 2 dogs with conical patent ductus arteriosus >5 mm in minimal diameter, coil closure was not done. We conclude that the patent ductus arteriosus size and anatomical shape are crucial in deciding whether coil closure is the method of choice. In selected cases, coil closure represents an elegant alternative to surgical ligation. Although pulmonary embolism occurred commonly, it did not cause any obvious clinical problem.  相似文献

A purebred Ayrshire calf was born with multiple congenital cardiac defects. The major defects included absence of an interventricular septum (i.e., single ventricle), transposition of the aorta and pulmonary trunk, interatrial septal defect, patent ductus arteriosus, and aortic arch anomalies. Despite the severity of the anomalies, the calf survived to 5 days of age, when it was found dead. This particular combination of cardiac defects has not been previously described in domestic animals.  相似文献

OBJECTIVE: To present details of surgical management of an unusual vascular ring anomaly in two German Shepherd littermates. STUDY DESIGN: Clinical case report. STUDY POPULATION: Three-month-old intact male and female German Shepherd littermates. RESULTS: In each dog, the esophagus was obstructed by a vascular ring comprised of the left aortic arch, an anomalous patent right ductus arteriosus, and the pulmonary artery ventrally. Surgical treatment consisted of dividing and oversewing the patent right ductus arteriosus. Neither dog has clinical signs of esophageal disease one year after treatment. CONCLUSION AND CLINICAL RELEVANCE: This vascular ring anomaly should be considered a possibility in any young dog with esophageal obstruction and a machinery murmur.  相似文献

A necropsy diagnosis of hypoplastic left ventricular syndrome was made in a day-old foal. The cardiac abnormalities included mitral and aortic valve atresia, patent ductus arteriosus, and a secundum atrial septal defect. The left ventricle was hypoplastic and nonfunctional. The brief survival of the foal was a consequence of left-to-right shunting through the atrial septal defect and right-to-left shunting through the patent ductus. The information is presented to demonstrate the existence of the syndrome as a congenital defect in the horse and to clarify the necropsy findings for the practitioner to diagnose if seen.  相似文献

The heart of a 6-week old Arabian filly with a history of poor health and exercise intolerance revealed at postmortem examination, multiple cardiac malformations which included atresia of the right atrioventricular orifice (tricuspid atresia, with associated atrial and ventricular septal defects), complete transposition of the great arteries, anomalous drainage of the venae cavae and coronary sinus into the left atrium, coarctation of the aorta, and a small but patent ductus arteriosus. Surgical ligation of a left-to-right shunting patent ductus arteriosus was attempted in two animals. In both cases, a young cat and dog, ligation was complicated by poor visualization of the ductus resulting in unsuccessful ligation. Post-operatively, both the cat and dog underwent computed tomography angiography to characterize the location and morphology of the patent ductus arteriosus. In both cases, computed tomography angiography revealed a left-to-right shunting patent ductus arteriosus with an insertion location medial to the left pulmonary artery branch compared to the typical location. We hypothesize that this atypical location resulted in a difficult surgical visualization from the left thoracotomy approach. Transvenous coil embolization of the duct from the external jugular vein was performed in both cases and resulted in successful occlusion. Variations in the medial-lateral insertion of the ductus arteriosus may have consequences for surgical intervention. If an atypical location of a patent ductus arteriosus is suspected on transthoracic echocardiography, computed tomography angiography prior to ligation may be useful to further define ductal location and help guide the surgical approach.  相似文献

Patent ductus arteriosus with pulmonary hypertension and right to left shunting of blood flow was diagnosed in a 9-month-old female cat. Because of the pulmonary hypertension, the cat did not have typical signs of patent ductus arteriosus; thus, cardiac angiography and catheterization were utilized to confirm the diagnosis. Two cases of patent ductus arteriosus are described in 2 Murrah buffalos, a 7.5-month-old heifer calf and a 5-month-old bull calf. The main clinical signs consisted of exercise intolerance, dyspnea, and tachycardia. At necropsy, lesions were consistent with congestive heart failure as indicated by dependent subcutaneous edema, body cavity effusions, and nutmeg liver. The lungs were extensively atelectatic. The heart was enlarged and had a globous shape. The ductus arteriosus, measuring 0.8 cm in length and 0.4 cm in caliber, was patent. The proximal pulmonary artery had an approximately 50-mm-long aneurysm. Eccentric cardiac hypertrophy was evident primarily in the right but to a lesser degree also in the left ventricle. Additionally, there was diffuse subendocardial fibrosis in the left and right ventricle. To the best of the authors' knowledge, patent ductus arteriosus was not previously described in buffalos. Since both calves were sired by the same bull, it is possible that this cardiac anomaly has a hereditary basis.  相似文献

Two foals with a history of normalcy at birth developed a lack of exercise tolerance and weakness in the first few days of life. Weakness, inability to rise, and reluctance to suckle were common complaints. Physical examination of both foals revealed a cardiovascular abnormality, with a loud systolic murmur audible over both thoracic walls. Additional diagnostic techniques were blood gas analysis, radiography, cardiovascular catheterization, and necropsy. Necropsy findings were ventricular septal defect, atrial septal defect, patent ductus arteriosus, and congenital absence of the aortic arch. Additionally, 1 foal had anomalous drainage of the cranial vena cava into the left atrium.  相似文献

OBJECTIVES: The objective of this study was to assess the feasibility of transcatheter embolisation of a patent ductus arteriosus using an Amplatzer vascular plug (Amplatzer Medical UK) in six dogs. METHODS: The Amplatzer vascular plug is a self-expandable, cylindrical device attached to a delivery cable. In all the dogs, the device was delivered transvenously. Successful device implantation was defined as firm anchorage of the device squarely within the distal part of the ductus arteriosus with no intra- or postoperative dislodgement. Successful occlusion of the ductus arteriosus occurred if a disappearance of the continuous murmur was achieved 24 hours after placement of the Amplatzer vascular plug. RESULTS: The age of the dogs ranged from 16 weeks to 7.5 years. Their weights ranged from 2.9 to 27.6 kg (median 6 kg). Two dogs had congestive heart failure before embolisation. Successful device implantation was achieved in all dogs. Successful occlusion of the ductus arteriosus was achieved in four of the six dogs. Complications included mild lameness, residual shunting, and bruising and pruritus around the surgical wound site. At follow-up, two dogs had a continuous murmur and one required treatment for congestive heart failure. CLINICAL SIGNIFICANCE: This technique may represent a clinically effective and less expensive alternative to the use of an Amplatzer duct occluder (Amplatzer Medical UK) in dogs with medium-sized patent ductus arteriosus. Further investigations are required to fully evaluate its efficacy and safety in various sizes and types of patent ductus arteriosus, and to determine the optimal device size in relation to the size of the ductus.  相似文献

This is the first report of an aortic sinus aneurysm with a communication to the main pulmonary artery, resulting in left-to-right shunting, diagnosed in vivo in a dog. There was also a second left-to-right shunt through a patent ductus arteriosus. Computed tomography (CT) angiography was used to confirm both congenital anomalies and assess the relative contributions of the two left-to-right shunts to left-sided volume overload.  相似文献

A 12-week-old, clinically normal Chihuahua was referred for investigation for a continuous heart murmur. Cardiac evaluation revealed an anatomically and haemodynamically typical left-to-right shunting patent ductus arteriosus. The continuous wave Doppler measurement of peak ductal jet velocity of 5.6 m/s was suggestive of a normal pulmonary to systemic arterial pressure ratio. The dog returned 16 days later with right heart failure and severe pulmonary hypertension. Marked reduction in left-to-right shunting was demonstrated and the ductal jet velocity had decreased to 2.5 m/s. Immediate ductus ligation, oxygen therapy before and after the operation, and administration of hydralazine failed to reduce pulmonary hypertension, and the dog was euthanased. Histopathological examination of the lung showed pulmonary necrotising arteritis with acute and chronic arterial lesions. Chronic pulmonary vascular changes related to high flow have been associated with altered nitric oxide and endothelin responses. These changes may be responsible for the acute onset of pulmonary hypertension due to relatively minor vascular insults in some human and veterinary patients with left-to-right shunts. The potential for acute progression supports the recommendations for early ductus ligation and the prognostic importance of detecting pulmonary hypertension presurgically in patent ductus arteriosus patients.  相似文献

This study was designed to evaluate the feasibility and limitations of transcatheter embolization coil occlusion of patent ductus arteriosus in dogs using a carotid artery approach. Seven dogs examined at the University of California, Davis Veterinary Medical Teaching Hospital in 2002-2003 for evaluation of heart disease had congenital patent ductus arteriosus diagnosed by characteristic physical, electrocardiographic, radiographic, and anatomic and Doppler echocardiographic findings. Dogs were anesthetized for transesophageal echocardiography and transcatheter coil embolization of the ductus via the right external carotid artery. Coil embolization was achieved in all seven cases, using one to four detachable embolization coils. There were no major complications. Minor complications occurred in two dogs (additional coils placed using a femoral arterial approach and coil embolization of a left femoral artery branch). One dog was examined only 24 h post-operatively and had no murmur and trivial residual ductal flow by Doppler echocardiography. The other 6 dogs were clinically healthy when examined up to three years post-intervention. One dog had a very soft continuous murmur and mild residual ductal flow; the other five had no audible continuous murmur, with only one dog having trivial residual ductal flow identified by Doppler echocardiography. Although technically challenging, coil embolization via the carotid artery is a viable alternative approach for transcatheter closure of patent ductus arteriosus in some dogs.  相似文献

Contrast echocardiography has traditionally been used in conjunction with conventional echocardiography to document right-to-left intracardiac shunting congenital anomalies. This technique does not clearly demonstrate shunting of blood in patients with right-to-left patent ductus arteriosus, as the shunt is extracardiac. ObjectiveTo report on transvenous detachable coiling in small dogs deemed ineligible for traditional transarterial patent ductus arteriosus occlusion and compare transthoracic echocardiographic and angiographic measurements to determine their equivalence.Materials and methodsA retrospective study of 35 dogs that underwent transvenous coiling of a patent ductus arteriosus. Demographic information, echocardiographic and angiographic studies, surgery reports, and follow-up evaluation of residual flow were obtained. A Bland-Altman analysis was used to compare echocardiographic and angiographic measurements of the minimal ductal diameter (Echo-MDD, Ang-MDD) and ampulla diameter (Echo-A, Ang-A).ResultsThirty-four of 35 dogs had successful deployment of a coil, with one dog undergoing occlusion with a different device after the exteriorized coil pulled through the ductus. Complete occlusion was achieved in 18 dogs within 24 h; four dogs were lost to follow-up, and the remaining 12 dogs had no residual flow or a significant reduction in shunting with normalization in cardiac chamber dimensions by a median of 99 days. Thirty percent of dogs (11/35) experienced perioperative complications of which 10 were minor complications. The analysis of 26 dogs with both echocardiographic and angiographic ductal measurements showed a −0.14 mm mean difference (95% limits of agreement −1.08 to 0.8 mm) in minimal ductal diameter and −0.68 mm mean difference (95% limits of agreement −2.73 to 1.37 mm) in ampulla diameter.ConclusionsDogs less than 3 kg deemed too small for transarterial occlusion can successfully undergo transvenous coil embolization of patent ductus arteriosus.  相似文献

A 6‐month old female alpaca cria presented to The Ohio State University for evaluation of a cardiac murmur. Echocardiography revealed a left‐to‐right shunting patent ductus arteriosus, a restrictive left‐to‐right shunting perimembranous ventricular septal defect, and secondary moderate left atrial and ventricular dilation. Aortic root angiography demonstrated a type IIA patent ductus arteriosus (PDA). Interventional closure of the PDA was successfully performed, without complication, using an Amplatz canine duct occluder. This case report describes the materials and methods used for interventional closure of a PDA in an alpaca cria.  相似文献

An 18‐month‐old male Akita Inu dog developed fever and lameness 8 months after successful transcatheter closure of a patent ductus arteriosus with an Amplatz Canine Duct Occluder (ACDO). Corynebacterium species were cultured from 3 blood samples. Echocardiography showed a vegetative process on the aortic valves. The dog died spontaneously 3 days after development of the initial signs. Necropsy confirmed the presence of bacterial ductal arteritis and myocarditis, and revealed an incomplete endothelialization of the intraductal metal implant. The reason for the lack of (neo)endothelialization of the ACDO remains unknown. We conclude that late‐onset bacterial device‐related ductal arteritis can develop in dogs where the implant is incompletely covered by a protective endothelial layer.  相似文献

A 14-week-old border collie with a history of exercise intolerance was confirmed to have a patent ductus arteriosus (PDA) from colour flow Doppler echocardiography. Under general anaesthesia, angiography was performed to gauge accurately the width of the ductus, and then two intravascular embolisation coils were introduced into the ductus via percutaneous catheterisation of the femoral vein. The coils were both 8 mm in diameter, each with four loops. This led to an immediate disappearance of the murmur. Some slight residual flow was detected at the time of the procedure by angiography, but by 10 days postoperatively there was no PDA flow detectable. The time taken to complete the procedure was 55 minutes, with a total fluoroscopy time of 15 minutes. Placement of intravascular embolisation coils represents a viable alternative to traditional surgical methods of ductus closure.  相似文献

A 3.5-year-old intact female miniature poodle (weighing 2.7 kg) was referred to the Veterinary Teaching Hospital at Kangwon National University, because of inadvertent aortic embolization, by an occlusion coil used for the closure of patent ductus arteriosus (PDA). The coil was found at the site of the branching renal arteries in the abdominal aorta. A foreign body forceps with a three-wire nail tip was used, with fluoroscopic guidance, to retrieve the coil.
