This study was conducted to validate an ultrasound-guided technique to block the sciatic nerve in cats. An anatomical study was first carried out in four feline cadavers to evaluate the feasibility of the glutea (cranial and caudal), femoris and poplitea ultrasonographical approaches for the sciatic nerve block. The results showed that the femoris approach was optimal because the region was free of vascular and bony structures, and the needle was easily visualised in-plane. Then, the efficacy of the femoris ultrasonographical approach to block the sciatic nerve was tested in six healthy adult experimental cats. A dose of 2 mg/kg lidocaine 2% diluted in saline to a final volume of 1 ml was administered in all cats. The blockade was successful in all cases and the cats recovered uneventfully. This study shows the usefulness of the femoris approach in performing an ultrasound-guided blockade of the sciatic nerve in cats.  相似文献

Objectives

To describe the ‘blind’ and ultrasound-guided approaches to block the maxillary nerve in donkeys. To compare the success and complication rates between the ‘blind’ and ultrasound-guided techniques based on staining of nerves and other structures in cadavers and assessing level of analgesia in live animals.

Study design

Prospective anatomical and experimental study.

Animals

Eighteen cadaver heads and nine adult live donkeys.

Methods

Phase 1: the anatomical characteristics of the maxillary nerve and its related structures were investigated within the pterygopalatine fossa in five cadavers. Phase 2: 0.1 mL of methylene blue dye was injected blindly and via ultrasound guidance in 13 cadavers to stain the left and right maxillary nerves, respectively. Nerve staining and dye spreading were evaluated through cadaver dissection. Phase 3: the former procedures were applied in nine live donkeys using lidocaine hydrochloride 2% and the onset of analgesia was verified through needle pricking at the naris.

Results

Ultrasound-guided deposition of methylene blue dye in cadavers and lidocaine injection in live animals were successful in all instances (accuracy = 100%) without inadvertent vascular penetration. Using the ‘blind’ technique, misdirection and intravascular deposition of dye were reported in four cadavers (accuracy = 69.2%) and neurovascular trauma was observed in live donkeys (five cases). Loss of cutaneous sensation in the ipsilateral naris was earlier in the ultrasound-guided approach (10.9 ± 1.8 minutes) than in the ‘blind’ technique (27.8 ± 3.2 minutes; p < 0.001).

Conclusions and clinical relevance

An ultrasound-guided maxillary nerve blockade proved very practical and can be used to block the maxillary nerve with a high degree of accuracy while avoiding vascular penetration. Further studies are mandatory to validate its analgesic effectiveness in clinical situations.  相似文献

ObjectiveTo describe an ultrasound-guided lateral quadratus lumborum (LQL) block technique and the spread characteristics of lidocaine–dye injected in the LQL plane using a transversal (LQL-T) or a longitudinal (LQL-L) approach.Study designExperimental anatomic study.AnimalsA total of eight canine cadavers.MethodsBilateral ultrasound-guided injections in the fascial plane lateral to the quadratus lumborum muscle and medial to the thoracolumbar fascia (LQL plane) with the needle directed at the first lumbar (L1) transverse process were performed using lidocaine–dye (0.3 mL kg−1). Anatomical dissection determined the dye distribution, sympathetic trunk staining and number of spinal nerves stained circumferentially >1 cm.ResultsThe LQL fascial plane was ultrasonographically recognized in all cadavers and filled with lidocaine–dye in all eight cadavers with the LQL-T approach and in six with LQL-L. The injectate spread ventral to the lumbar transverse processes, around the quadratus lumborum muscle and dorsal to the transversalis fascia, affecting the ventral branches of the spinal nerves and the sympathetic trunk. A median (range) of 4 (3–5) and 3 (0–4) ventral branches of the thoracolumbar nerves were dyed with LQL-T and LQL-L approaches, respectively (p = 0.04). The most cranial nerve stained was the twelfth thoracic (T12) with the LQL-T approach and T13 with LQL-L, and the most caudal was L3 with both approaches. The incidence of sympathetic trunk staining was significantly higher using LQL-T (six injections) compared with LQL-L (one injection; p = 0.04). Dye was not observed in the lumbar plexus, epidural space or abdominal cavity.Conclusions and clinical relevanceUltrasound-guided LQL-T approach resulted in a more consistent spread toward the spinal nerves and sympathetic trunk compared with LQL-L approach. Further studies are necessary to assess the LQL block effectiveness and success rate in live dogs.  相似文献

ObjectiveTo develop an ultrasound-guided dorsal approach to the brachial plexus and to investigate the nerve distribution and staining of a dyed injectate in common kestrel (Falco tinnunculus) cadavers.Study designProspective, cadaver study.AnimalsA group of three common kestrel cadavers (six wings).MethodsAll cadavers were fresh-frozen at –20 °C and thawed for 10 hours at room temperature before the study. The cadavers were placed in sternal recumbency and their wings were abducted. A 8–13 MHz linear-array transducer was placed over the scapulohumeral joint, at the centre of a triangle formed by the scapula and the humerus. The brachial plexus was identified between the scapulohumeralis muscle and the pectoralis major muscle, as hypoechoic structures lying just cranially to the axillary vessels. After ultrasound-guided brachial plexus identification, a 22 gauge, 50 mm insulated needle was advanced in-plane using ultrasound visualization. A volume of 0.5 mL kg–1 of a 3:1 (2% lidocaine:methylene blue) solution was injected. Following cadaver dissection, the pattern of the spread was assessed, and the extent of nerve staining was measured with a calliper and deemed adequate if more than 0.6 cm of the nerve staining was achieved.ResultsThe brachial plexus was clearly identified in all wings with the dorsal approach. After dye injection, all the branches of the brachial plexus defined as nerves 1–5 (N1, N2, N3, N4 and N5) were completely stained in five (83%) and partially stained in one (17%) of the six wings.Conclusions and clinical relevanceThe ultrasound-guided dorsal approach allows a clear visualization of the brachial plexus structure. The injection of 0.5 mL kg–1of a lidocaine/dye solution produced complete nerve staining in most cases. Further in vivo studies are mandatory to confirm the clinical efficacy of this locoregional anaesthesia technique in common kestrels (Falco tinnunculus).  相似文献

ObjectiveTo describe an approach for ethmoidal nerve block (EBLOCK) and to compare the effects of a maxillary nerve block (MBLOCK), EBLOCK and their combination (M-EBLOCK) on heart rate (HR), systolic (SAP), mean (MAP), diastolic (DAP) arterial pressures and respiratory rate (fR) during nasal stimulation in dogs.Study designProspective, blinded, randomized, crossover placebo-controlled study.AnimalsBeagle dogs (five cadavers, nine live dogs), with a median (interquartile range) weight of 10.5 (10.3–11.0) kg.MethodsThe accuracy of iohexol injections (each 1 mL) at the maxillary and ethmoidal foramina in cadavers was evaluated using computed tomography. Then, anesthetized dogs were administered four bilateral treatments separated by 1 week, saline or 2% lidocaine 1 mL per injection: injections of saline at the maxillary and ethmoidal foramina (Control), injections of lidocaine at the maxillary foramina and saline at the ethmoidal foramina (MBLOCK), injections of saline at the maxillary foramina and lidocaine at the ethmoidal foramina (EBLOCK) and injections of lidocaine at all foramina (M-EBLOCK). The ventral nasal meatus was bilaterally stimulated using cotton swabs, and HR, SAP, MAP, DAP and fR were continuously recorded. Values for each variable were compared before and after stimulation using Wilcoxon signed-rank test. Changes in variables among treatments were analyzed using Mann–Whitney U and Kruskal–Wallis tests (p ≤ 0.05).ResultsComputed tomography revealed iohexol distribution around the openings of the target foramina in all cadavers. In living dogs, HR, SAP, MAP, DAP and fR significantly increased after stimulation within each treatment (p < 0.03). Physiologic responses were significantly attenuated, but not absent, in the M-EBLOCK [HR (p = 0.019), SAP, MAP, DAP and fR (all p ≤ 0.001)] compared with those in the Control.Conclusions and clinical relevanceConcurrent injections of lidocaine at the maxillary and ethmoidal foramina attenuated HR, arterial pressure and fR responses to nasal stimulation in Beagle dogs.  相似文献

ObjectiveTo describe the gross and ultrasound anatomy of the parasacral region and an ultrasound-guided greater ischiatic notch (GIN) plane approach aimed at staining the lumbosacral trunk (LST) in canine cadavers. To evaluate if the ultrasound-guided GIN plane approach is non-inferior to the previously described ultrasound-guided parasacral approach at staining the LST.Study designProspective, randomized, non-inferiority experimental anatomic study.AnimalsA total of 17 (23.9 ± 5.2 kg) mesocephalic canine cadavers.MethodsAnatomic and echographic landmarks, and the feasibility of performing a GIN plane technique were evaluated using two canine cadavers. The remaining 15 cadavers had each hemipelvis randomly assigned to be administered either parasacral or GIN plane injection of 0.15 mL kg–1 dye solution. The parasacral region was dissected after injections to assess the staining of LST, cranial gluteal nerve, pararectal fossa and pelvic cavity. The stained LST were removed and processed for histological evaluation of intraneural injections. A one-sided z-test for non-inferiority (non-inferiority margin –14%) was used to statistically evaluate the success of the GIN plane versus the parasacral approach. Data were considered statistically significant when p < 0.05.ResultsThe GIN plane and parasacral approach stained the LST in 100% and 93.3% of the injections, respectively. The success rate difference between treatments was 6.7% [95% confidence interval, –0.6 to 19.0%; p < 0.001 for non-inferiority]. The GIN plane and parasacral injections stained the LST for 32.7 ± 16.8 mm and 43.1 ± 24.3 mm, respectively (p = 0.18). No evidence of intraneural injection was found.Conclusions and clinical relevanceThe ultrasound-guided GIN plane technique resulted in nerve staining that was non-inferior to the parasacral technique and may be considered an alternative to the parasacral approach to block the LST in dogs.  相似文献

Objective To describe the location of the femoral nerve within the iliopsoas muscle and determine the feasibility and potential complications of an ultrasound-guided block. Study Design Prospective experimental trial. Animals Sixteen adult dogs, median weight 14.3 (range 3-37) kg. Methods Phase I. Computed tomographic images of the iliopsoas muscle and femoral nerve were analyzed in two dogs. Phase II. The location of the femoral nerve within the iliopsoas muscle was determined via ultrasonography in 11 healthy anaesthetized dogs. Phase III. Peripheral nerve stimulation and ultrasound were combined to perform femoral nerve blocks in three dogs. Results Using computed tomography, the femoral nerve and its L4-L5 roots were identified within the iliopsoas muscle. The nerve itself was traced until it branched off into the quadriceps femoris muscle. Using ultrasonography, it was possible to observe the femoral nerve in nine dogs (82%). Starting at the dorsal third of the iliopsoas muscle, its path was traced in a caudo-ventro-lateral direction, emerging from the iliopsoas muscle shortly before passing through the muscular lacuna where it became very difficult to identify. An ultrasound-guided femoral nerve approach was carried out successfully in all three dogs. Conclusions It is possible to approach the femoral nerve using combined ultrasound guidance and peripheral nerve stimulation to a closer proximal point than previously described. Clinical relevance The ultrasound-guided approach of the femoral nerve within the iliopsoas muscle has the potential to become an additional approach.  相似文献

ObjectiveTo evaluate the dye extent and distribution at the lumbar plexus (LP) of three volumes of local anaesthetic-methylene-blue solution administered close to the femoral nerve (FN) by the use of a ventral ultrasound (US)-guided suprainguinal approach (SIA).Study designProspective experimental trial.AnimalsTwenty mongrel canine cadavers weighing 17.7 ± 3.8 kg (mean ± SD).MethodsThe left and right LP of two cadavers were dissected to identify the FN, obturator nerve (ON) and lateral femoral cutaneous nerve (LFCN). The extent and distribution of dye at the LP of each of three volumes of injectate of 0.2, 0.4 and 0.6 mL kg?1 administered close to the FN by a ventral US-guided SIA then were studied in a further 18 dog cadavers (n = 6 per group). Staining of ≥2 cm along the target nerves was indicative of sufficient spread to produce a nerve block.ResultsThe ventral US-guided SIA allowed the observation of the FN within the iliopsoas muscle (IPM) in a total of 17 cadavers. The assessment of the dye extent and distribution revealed a similar pattern regardless of the injected volume. From the injection site, the spreading of injectate occurred in cranial, lateral and caudal directions. The FN and ON were effectively stained in all the cases. The LFCN was not effectively stained in any case.Conclusions and clinical relevanceA volume of 0.2 mL kg?1 administered close to the FN by a ventral US-guided SIA produced a sufficient distribution of the injectate within the IPM to produce effective staining of the FN and ON. This US-guided technique may be an appropriate alternative to previously reported techniques based on electrolocation to block the FN and ON in the dog.  相似文献

Objective

To develop a modified ultrasound-guided parasacral approach to the sciatic nerve and compare the effects of a volume of 0.2 mL kg–1 of 0.5% levobupivacaine with an equivalent volume of 0.9% saline injected near the sciatic nerve.

Study design

Cadaveric and experimental, blinded, randomized study.

Animals

Seven canine cadavers and seven experimental Beagle dogs.

Methods

Both sciatic nerves of seven cadavers were identified using a modified in-plane ultrasound-guided approach. Methylene blue solution (0.2 mL) was injected perineurally, and success was evaluated through dissection. The same approach was repeated in seven Beagle dogs sedated with dexmedetomidine (50 μg kg–1) injected intramuscularly (IM). After randomization, 0.2 mL kg–1 of 0.5% levobupivacaine (limb L) and 0.2 mL kg–1 of 0.9% saline (limb C) were injected perineurally on either right or left limb. Block success was determined by sensory deficits every hour for 8 hours after an atipamezole injection (0.2 mg kg–1) IM. Reaction to pinprick (binary score) over the course of the sciatic nerve (four locations) and locomotion were assessed.

Results

The overall sciatic nerve block success was 93% in cadavers and 86% in sedated dogs. It was impossible to localize the sciatic nerves in one obese sedated dog. Significant differences between limb L and limb C were observed for pinprick at great trochanter, caudal thigh and lateral tarsal joint (p < 0.0001). Reaction to pinprick was absent in all dogs at great trochanter and caudal thigh up to at least 3 hours on limb L. Locomotion was impaired in all but one dog for 60 (30–210) minutes (median; interquartile range). No complications were observed.

Conclusions and clinical relevance

A volume of ≥0.2 mL kg–1 and a concentration of 0.5% levobupivacaine can be recommended when using a modified ultrasound-guided parasacral approach to the sciatic nerve in dogs.  相似文献

ObjectiveTo describe a new approach to block the femoral nerve and to evaluate the distribution of a dye injected into the psoas compartment using a new femoral nerve block approach; to assess its clinical application, when combined with a sciatic nerve block, for surgical anaesthesia/analgesia of the pelvic limb in dogs.Study designProspective anatomical, research and clinical study.AnimalsTwo dog cadavers; two dogs that had to be euthanized for reasons unrelated to this study, and 15 dogs undergoing pelvic limb orthopaedic surgery.MethodsPhase 1: anatomical dissections were performed to determine a simple method to approach the femoral nerve within the psoas compartment. Phase 2: 0.1 mL kg?1 of a lidocaine-new methylene blue solution was injected bilaterally after successful electrolocation of the femoral nerve in two anaesthetized dogs. Colorant spread was evaluated through femoral nerve dissections after euthanasia. Phase 3: in 15 dogs undergoing pelvic limb orthopaedic surgery under light general anaesthesia with isoflurane, intra-operative analgesic effect (cardiovascular responses) and early post-operative pain score, of the novel femoral nerve block combined with a sciatic nerve block as the sole analgesic protocol, were evaluated.ResultsPhase 1: a needle inserted from the lateral aspect of the lumbar muscles, cranially to the iliac crest and with a 30–45° caudo-medial direction, reaches the femoral nerve in the caudal portion of the psoas compartment. Phase 2: Four femoral nerves were stained >2 cm. Phase 3: this novel lateral pre-iliac approach, combined with the sciatic nerve block, blunted the intra-operative cardiovascular response to surgical stimulation in 13 out of 15 anaesthetized dogs. In addition, rescue analgesia was not required in the early post-operative 2-hour period.Conclusion and clinical relevanceThe lateral pre-iliac femoral nerve block technique may provide adequate intra- and early post-operative pain relief in dogs undergoing pelvic limb surgery.  相似文献

An interventional ultrasound technique to increase the safety of surgical treatment of the calf forelimb was tested. First, the brachial plexus was evaluated using ultrasonography and then 2% lidocaine was injected under ultrasound guidance. Ultrasonically, the brachial plexus appeared as multiple hypoechoic areas surrounded by a hyperechoic rim or a hyperechoic structure characterised by multiple discontinuous lines. It was located between the omotransverse muscle and axillary artery and vein. The sensitive effect in the forelimb was seen mainly in the area supplied by the musculocutaneous nerve, indicating successful blockage in the nerve plexus. Out of the eight forelimbs, the motor effect was observed in seven forelimbs. These results suggest the clinical feasibility of ultrasound-guided brachial plexus block in bovine medicine, although further studies are needed to examine various approaches, including the sites of needle insertion and the appropriate volume and dosage of anaesthetic.  相似文献

The aim of this study was to investigate the use of ultrasound (US) guidance to perform sciatic and saphenous nerve blocks in dogs. Five dogs were sedated with medetomidine and butorphanol. A high-resolution US transducer was used to locate the nerves, guide placement of the needle and visualise the perineural injection of lidocaine 2%. Electrostimulation was used to confirm correct placement prior to the sciatic block. Nerve functions were evaluated over a 3 h period following administration of atipamezole. Successful identification of the nerves and the quality of the blocks were recorded. Location of the nerves, complete sensory block of the saphenous nerve, and partial to complete sensory and motor blocks of the sciatic nerve were achieved in all dogs. The resultant US guidance is potentially valuable for blocking the sciatic and saphenous nerves in dogs, although further work will be required to ensure a complete block of the sciatic nerve.  相似文献

The goal of the present study was to provide the operator with additional information to locate the FN within the IPM in dogs and cats using US.Study designThe study was carried out in three phases: a dissection of the FN (phase 1); an in vivoUS-assisted nerve study (phase 2), and an anatomical cross-sectional study (phase 3).AnimalsNine healthy adult beagle dogs and nine healthy adult cats.MethodsDissections were performed to investigate the anatomical characteristics of the FN and its related structures in one dog and one cat. Ultrasound scans of the left and right FN were performed in eight dogs and eight cats. The FN diameter and the distance between the FN and the external iliac artery (EIA) in US images and in anatomical cryosections were measured.ResultsThe median FN diameter did not differ significantly between cats and dogs (1.1 mm versus 1.0 mm) or between the two techniques (US versus anatomical cross-sectional study) (1.1 mm versus 1.1 mm in dogs; 1.0 mm versus 1.1 mm in cats). The US and anatomical measurements of the median distances between the FN and EIA differed significantly between dogs and cats (8.2 mm versus 5.8 mm by US; 5.7 mm versus 4.8 mm in the anatomical study).Conclusions and clinical relevanceThe distance between the EIA and FN is reproducible in beagle dogs and cats and can be used in locating the FN within the IPM.  相似文献

Objective: To evaluate a technique for midhumeral peripheral nerve blockade in the dog. Study Design: Cadaveric technique development; in vivo placebo‐controlled, prospective crossover study. Animals: Canine cadavers (n=38) and 8 clinically healthy, adult hound dogs. Methods: A technique for peripheral block of the radial, ulnar, musculocutaneous, and median nerves (RUMM block) was evaluated using cadaver limbs. Eight purpose‐bred, research dogs were anesthetized; a RUMM block was performed on each thoracic limb. One limb from each dog randomly received 0.5% bupivacaine and the opposite limb was assigned to receive sterile saline solution as a control. After recovery from anesthesia, skin sensation at selected dermatomes was evaluated for 24 hours using a mechanical stimulus. Weight‐bearing, conscious proprioception, and withdrawal reflex were also evaluated. One month after initial testing, each dog was reanesthetized and each limb received the opposite treatment. Results: Sensory thresholds were significantly increased over baseline measurements when compared with control limbs for all nerves. Complete sensory block was achieved in radial (15/16), ulnar (3/16), musculocutaneous (8/16), and median (11/16) nerves, using a mechanical stimulus of analgesia. Complete simultaneous block of all nerves was only obtained in 1 of 16 limbs. Conclusion: RUMM block resulted in desensitization of the skin in the associated dermatomes for 4–10 hours. Complete sensory block of the dermatomes supplied by the radial nerve was most consistent. Clinical Relevance: RUMM block may be an effective technique to provide adjunctive analgesia for dogs undergoing surgery of the distal aspect of the thoracic limb.  相似文献

ObjectiveTo compare palpation-guided with ultrasound-guided brachial plexus blockade in Hispaniolan Amazon parrots.Study designProspective randomized experimental trial.AnimalsEighteen adult Hispaniolan Amazon parrots (Amazona ventralis) weighing 252–295 g.MethodsAfter induction of anesthesia with isoflurane, parrots received an injection of lidocaine (2 mg kg?1) in a total volume of 0.3 mL at the axillary region. The birds were randomly assigned to equal groups using either palpation or ultrasound as a guide for the brachial plexus block. Nerve evoked muscle potentials (NEMP) were used to monitor effectiveness of brachial plexus block. The palpation-guided group received the local anesthetic at the space between the pectoral muscle, triceps, and supracoracoideus aticimus muscle, at the insertion of the tendons of the caudal coracobrachial muscle, and the caudal scapulohumeral muscle. For the ultrasound-guided group, the brachial plexus and the adjacent vessels were located with B-mode ultrasonography using a 7–15 MHz linear probe. After location, an 8-5 MHz convex transducer was used to guide injections. General anesthesia was discontinued 20 minutes after lidocaine injection and the birds recovered in a padded cage.ResultsBoth techniques decreased the amplitude of NEMP. Statistically significant differences in NEMP amplitudes, were observed within the ultrasound-guided group at 5, 10, 15, and 20 minutes after injection and within the palpation-guided group at 10, 15, and 20 minutes after injection. There was no statistically significant difference between the two groups. No effect on motor function, muscle relaxation or wing droop was observed after brachial plexus block.Conclusions and clinical relevanceThe onset of the brachial plexus block tended to be faster when ultrasonography was used. Brachial plexus injection can be performed in Hispaniolan Amazon parrots and nerve evoked muscle potentials were useful to monitor the effects on nerve conduction in this avian species. Neither technique produced an effective block at the doses of lidocaine used and further study is necessary to develop a useful block for surgical analgesia.  相似文献

ObjectiveTo investigate the distribution and nerve staining of two volumes of lidocaine–dye solution after ultrasound-guided erector spinae plane (ESP) injections in canine cadavers.Study designExperimental cadaveric study.AnimalsA total of nine canine cadavers.MethodsESP injections were performed between the longissimus thoracis muscle and the dorsolateral edge of the ninth thoracic transverse process. Two cadavers were transversally cryosectioned after unilateral ESP injections [0.6 mL kg–1; high volume (HV)]. In seven cadavers, bilateral ESP injections with HV or low volume (0.3 mL kg–1; LV) were performed. Gadodiamide was added to the injectate for two cadavers and magnetic resonance imaging (MRI) was performed pre- and post-injection. Injectate distribution and nerve staining of the branches of the spinal nerves were recorded after gross anatomical dissection. The thoracic paravertebral and epidural spaces were examined for dye solution.ResultsCryosections, MRI and gross dissections showed that the injectate spread dorsally to the transverse processes, over the ventromedial aspect of the longissimus thoracis muscle where the medial and lateral branches of the dorsal branches of the spinal nerves are located. LV and HV stained a median (range) of 4 (2–7) and 4 (3–8) medial branches, respectively (p = 0.52). LV and HV stained 4 (2–5) and 5 (4–7) lateral branches (p = 0.26), respectively. Ventral branches were not stained, and dye was not identified in the epidural or paravertebral spaces.Conclusionsand clinical relevance Medial and lateral branches were consistently stained over several spinal segments. The number of nerves stained was not different with HV or LV, and the ventral branches of the spinal nerves were not stained in any cadaver. ESP block may find a clinical application to desensitize structures innervated by the medial and lateral branches of the dorsal branches of the thoracic spinal nerves.  相似文献

OBJECTIVE: To adapt and standardize neural tissue mobilization exercises, quantify nerve root movement, and assess the anatomic effects of lumbar spinal nerve and dural mobilization in dogs. ANIMALS: 15 canine cadavers. PROCEDURES: 5 cadavers were used in the preliminary part of the study to adapt 3 neural tissue mobilization physical therapy exercises to canine anatomy. In the other 10 cadavers, the L4 to L7 nerve roots and the dura at the level of T13 and L1 were isolated and marked. Movements during the physical therapy exercises were standardized by means of goniometric control. Movement of the nerve roots in response to each exercise was digitally measured. The effects of body weight and crownrump length on the distance of nerve root movement achieved during each exercise were also assessed. Each exercise was divided into 4 steps, and the overall distance of neural movement achieved was compared with distances achieved between steps. RESULTS: Neural tissue mobilization exercises elicited visible and measurable movement of nerve roots L4 to L7 and of the dura at T13 and L1 in all cadavers. CONCLUSIONS AND CLINICAL RELEVANCE: The physical therapy exercises evaluated had measurable effects on nerve roots L4 to L7 and the dura mater in the T13 and L1 segments. These exercises should be evaluated in clinical trials to validate their efficacy as primary treatments or ancillary postsurgical therapy in dogs with disorders of the thoracolumbar and lumbosacral segments of the vertebral column.  相似文献

ObjectiveTo describe a novel in-plane ultrasound (U/S)-guided temporal approach to peribulbar block in dogs.Study DesignProspective experimental cadaver study.AnimalsA group of 10 Beagle cadavers.MethodsAfter describing the U/S anatomy, peribulbar injection was performed bilaterally in 10 thawed Beagle cadavers by two randomly assigned operators. A 5–8 MHz microconvex U/S probe was positioned caudal to the orbital ligament in the longitudinal plane. Using an in-plane technique, methylene blue dye was injected in five dogs (10 eyes total), while methylene blue dye and iohexol contrast mixture (50:50) were injected in the remaining five dogs. Injection volume was 0.2 mL cm?1 of cranial length. A computed tomography (CT) scan was performed on dogs injected with dye and contrast to identify spread of contrast. Dissection to visualize dye spread in the orbit was performed in all dogs. Injection success was defined as spread of contrast into the peribulbar space. The pattern of distribution of contrast-dye was also assessed. Comparisons between operator and bilateral injections were assessed using a Student t test (p < 0.05). All other data are reported as number (n/N) and percentage (%).ResultsPeribulbar spread was noted in 19/20 injections (95%) on dissection. CT imaging (five dogs) illustrated peribulbar contrast spread in 9/10 injections (90%), with mixed peribulbar/retrobulbar spread for the remaining injection. Contrast was present at the rostral alar foramen in 4/10 (40%) injections, orbital fissure in 5/10 (50%), oval foramen in 1/10 (10%), maxillary nerve in 3/10 (30%) and intracranial in 5/10 (50%). Coverage of the maxillary nerve was noted on 3/20 (15%) injections on dissection. No further dye spread was noted.Conclusions and clinical relevanceThis technique demonstrated peribulbar spread of injectate in 100% of injections for the 10 canine cadavers studied.
