The objective of this case report is to describe the diagnostic and surgical techniques for removal of a metallic foreign body in the tongue of three adult horses. The three horses were presented for evaluation and treatment of dysphagia and marked hypersalivation of 3–5 days duration. Radiographs of the head revealed the presence of a metallic foreign body in the tongue of each horse. The foreign bodies could be precisely localised under general anaesthesia using palpation and lingual ultrasonography and/or lingual radiography in combination with a forceps as a marker. The foreign bodies were successfully removed using laparoscopic instruments creating minimal soft tissue trauma. The use of long (43 cm) small laparoscopic (5 mm) instruments enabled good visualisation of the surgical field, providing optimal conditions for successful minimally invasive surgical treatment of horses with foreign bodies in the tongue. The three horses made uneventful recoveries and 12 months after surgery were eating normally and could be ridden with a bit as per usual routine. It was concluded that using long laparoscopic instruments in combination with palpation, ultrasonography and/or radiography allowed removal of the foreign body creating minimal soft tissue trauma and allowing optimal conditions for a fast recovery.  相似文献

Survey radiography is used in diagnosis of different affections in buffaloes and cattle. The aim of the present study was to assess the role of radiography in diagnosis of reticular diaphragmatic hernias and traumatic pericarditis in buffaloes and cattle. The present study was carried out on 69 animals (51 buffaloes and 18 cattle). Reticular diaphragmatic hernias (40 buffaloes, 4 cattle) and traumatic pericarditis (11 buffaloes, 14 cattle) were evaluated. Lateral right-left survey radiography of the thorax was performed. In diaphragmatic hernia, radiography revealed presence of a rounded or vertical oval mass of soft tissue opacity superimposed over the heart. Radiopaque foreign bodies of variable shape and size were seen within the herniated part of the reticulum. The apex of the heart was difficult to visualize. With traumatic pericarditis, survey radiography of the thorax revealed poor differentiation of thoracic contents. The contour of the diaphragm was lost and the cardiac silhouette was obscured. In several animals radiopaque foreign bodies (sewing needles, nails, and pieces of wire) were detected at the level of the heart or in the area connecting the dome of the diaphragm with the heart.  相似文献

A question frequently asked by clinicians who are treating small animals suspected of having gastrointestinal foreign bodies is whether one imaging test such as survey radiography or ultrasonography is sufficient to make the diagnosis. A study was undertaken to try and answer this question. Survey abdominal radiography and ultrasonography was performed on 16 small animals (11 dogs, five cats) with clinical signs of an obstruction because of a confirmed gastrointestinal foreign body. The majority of the foreign bodies (14/16) were confirmed by surgical removal and were located in the small intestine. A gastric foreign body was retrieved endoscopically and a colonic foreign body was passed in the feces. Radiographically identifiable foreign bodies were evident in nine animals. Small intestinal overdistension was present radiographically in seven animals. Ultrasonography detected a foreign body in all 16 animals. The foreign bodies were identified by their distal acoustic shadowing and variable degrees of surface reflection. An intestinal perforation was detected sonographically but not radiographically. The value of additional sonographic findings including thickening of the gastrointestinal wall and loss of layering, free peritoneal fluid, and lymphadenopathy in these animals is discussed. The findings in this series suggest that in a small animal with a gastrointestinal foreign body, ultrasonography alone could be used to make the diagnosis and may be a more appropriate choice than survey radiography.  相似文献

Results obtained from clinical, endoscopic, bacteriological, and roentgenological investigations, following artificial introduction of foreign bodies into the tracheobronchial system of dogs, are reported in this paper. The foreign bodies included gauze pads, plastic halma pieces, larch, alder, and wood cones. Twenty of the 22 foreign bodies artificially placed into the bronchial tree of the experimental dogs were spontaneously coughed up, 16 of them between the 1st and 14th days from introduction. The cause underlying such high spontaneous healing rate of tracheobronchial foreign bodies in dog is discussed in some detail.  相似文献

An eight-month-old male Pomeranian was presented with hypovolaemic shock 7 days after successful oesophageal foreign body removal. On presentation the dog was anaemic and no coagulation abnormalities were detected. A fluid-filled oesophagus was visible on thoracic radiography and frank blood was aspirated from the oesophagus following oro-oesophageal intubation. Fluid resuscitation and blood transfusions were administered but attempts at stabilisation were unsuccessful and the dog died. At post-mortem examination, a 2-mm aortic oesophageal fistula was identified on the ventral aspect of the aorta at the level of heart base that communicated with the overlying oesophagus. Aortic oesophageal fistula has been reported in human medicine and occurs secondary to a number of conditions including oesophageal foreign bodies. These prove fatal without rapid stabilisation and surgery.  相似文献

Twenty-five small animal patients presenting with signs of orbital disease were investigated using magnetic resonance imaging (MRI) in an attempt to assess the value of this imaging technique for diagnosis. All patients were also examined using ultrasonography, and skull radiography was performed in 20 of these animals. The final diagnoses included neoplasia, inflammatory disease and foreign body penetration. MRI produced detailed images of orbital tissues and provided more information about the extent of pathology than the other imaging techniques; a correct diagnosis based solely on the MRI scan was made in 22 cases. Radiography was found to be helpful only in cases in which neoplastic disease extended markedly beyond the confines of the orbit into the nasal chamber and paranasal sinuses. Radiographic changes other than soft tissue swelling were not evident in other orbital disease processes. Ultrasonography gave both false negative and false positive diagnoses for neoplastic masses, although it allowed the correct diagnosis of both cases of foreign bodies and one of the three cases of retrobulbar abscesses in this series. MRI is recommended for patients in which radiography and ultrasonography fall to produce a confident diagnosis or for which surgery is proposed.  相似文献

Detection of wooden foreign bodies in dogs can be challenging. A retrospective, cross‐sectional study was done to describe computed tomographic (CT) signs associated with wooden foreign bodies, and to estimate the accuracy of CT for detection of wooden foreign bodies. Patient records and CT images were reviewed for 72 dogs that had a history of suspected stick injury and CT of the affected body part, or possible wooden foreign object reported on CT, and had surgical exploration during the same period of hospitalization. Duration of clinical signs was acute in 48 (67%) dogs and chronic in 24 (33%). Wood was removed from 55 dogs, including a piece of a tree or shrub in 33 (60%) instances, kebab stick in 8 (15%), piece of bamboo garden cane in 2 (4%), cocktail stick in 2 (4%), thorn in 1 (2%), and unidentified wood in the remaining nine instances. Based on review of CT images with knowledge of the surgical findings, sensitivity of CT for wooden foreign bodies was 79% (95% CI 65%–89%), specificity 93% (78%–98%), positive likelihood ratio 11.5 (2.9–44.1), and negative likelihood ratio 0.23 (0.13–0.41). Wooden foreign bodies were predominantly rectangular or linear, with median length 48 mm (range 2–270 mm), median thickness 3 mm (range 1–22 mm), and median attenuation 111 HU (range ?344 to +640 HU). A CT finding of gas in soft tissues was significantly associated with acute cases, whereas suspected foreign material, cavitary lesions, fat stranding, and periosteal reaction on adjacent bones were associated with chronic cases.  相似文献

The goal of this study was to describe the findings in four cows with non-magnetic reticular foreign bodies composed of copper. The cows were referred to our clinic because of reduced appetite and a marked decrease in milk production. Based on the clinical findings, a tentative diagnosis of traumatic reticuloperitonitis was made in all cows. The reticulum of all cows was then examined ultrasonographically and radiographically. In all cows, radiographs of the reticulum showed wire-shaped foreign bodies, ranging from 3 to 7 cm in length, which appeared to have penetrated the reticular wall. Two cows (No. 3, 4) had a magnet in the reticulum close to the foreign body but there was no direct contact between the two. A magnet was administered to cows No. 1 and 2, and radiography of the reticulum was performed for a second time the following day. The magnets were observed in the reticulum; however, they did not contact the foreign bodies. Because all the magnets were correctly placed in the reticulum yet, despite close proximity, did not contact the foreign bodies, the latter were thought to be non-magnetic. Cow No. 1 was slaughtered. Left flank laparoruminotomy was performed in the remaining three cows. In all cows, copper foreign bodies ranging in length from 3.0 to 7.0 cm, were found in the reticulum. They had penetrated the reticular wall and were not attached to magnets. The radiographic findings described in the present study are strongly indicative of a non-magnetic foreign body. Ruminotomy is the treatment of choice but slaughter may also be considered.  相似文献

Seven horses with metallic foreign bodies in the mouth or pharynx were examined at the Colorado State University Veterinary Teaching Hospital from 1983 to 1989. The horses had variable clinical signs, such as purulent nasal discharge, swelling of the throatlatch area, and dyspnea. Most of the horses had clinical signs for more than 2 weeks, and had no or only temporary improvement with conservative medical treatment (antibiotics, nonsteroidal anti-inflammatory drugs). The definitive diagnostic test in all of the cases was radiography, which also aided in the plan for surgical removal of the foreign body. Once the foreign body was removed from each of the horses, their clinical signs resolved. Most of the foreign bodies were small pieces of wire, the sources of which could not be determined, but that may have been incorporated in baled hay.  相似文献

The tympanic bullae of 20 previously healthy cadaver animals were randomly filled with sterile saline or air. The heads were positioned using tape ties and a rostrocaudal open mouth radiograph taken. The resulting radiographs were assessed by a blinded ECVDI diplomate, with each tympanic bulla being recorded as being either air or fluid filled. Ultrasound examination of the bullae was performed, by a blinded assessor, using a B-mode 5 mHz sector probe (Sonotron-VingMed Sound A/S, PO Box 141, N3191, Horten, Norway) operating at 3 mHz. Each bulla was recorded as being either air or fluid filled. Results of radiography and ultrasonography were each compared to the actual status of the bulla in a chi square analysis. Sensitivity of radiography was 80%, with specificity of 65%. Ultrasonographic sensitivity was 100% and specificity 100%. Ultrasonography may in future provide a cheap, noninvasive, rapid and widely available method for diagnosing otitis media in clinical cases.  相似文献

The clinical, diagnostic and therapeutic features of a horse with a wooden foreign body embedded in the deep portion of the right masseter muscle adjacent to the right orbit are presented. The purpose of this report is to describe the clinical presentation, magnetic resonance imaging findings and treatment of a penetrating wooden foreign body in a horse that had no history of trauma or evidence of a puncture wound. This report documents the usefulness of magnetic resonance imaging to detect a wooden foreign body embedded in the soft tissues of a horse with a chronic copious ocular discharge. Two surgical procedures were necessary, which is a frequent complication encountered with wooden foreign bodies.  相似文献

This article describes the discovery of a chronic cervical wooden foreign body ventral to the left transverse processes of the cranial cervical spine using magnetic resonance imaging (MRI) in a dog that presented with chronic neck pain and lameness. The dog did not exhibit dysphagia or chronic draining tracts, the most common signs of the presumed cause, that of a penetrating oropharyngeal foreign body. The foreign body itself was represented on MR images as an oval straight-edged core within an inflammatory tissue reaction. The wood was slightly hyperintense on T2- and isointense on T1-weighted images relative to muscle. Surrounding this was a more conspicuous contrast-enhancing reactive tissue rim that was hyperintense on all pulse sequences. Adjacent musculature also exhibited diffuse edema and contrast enhancement that extended around the left cervical vertebral transverse processes and local intervertebral nerve roots. The foreign body was found to be a wooden stick upon surgical removal. MRI is an excellent method for visualizing the inflammatory tissue reactions associated with soft-tissue foreign bodies because of its contrast resolution and depiction of anatomy in multiple imaging planes.  相似文献

While playing or simply because of avidity, dogs may ingest a variety of foreign bodies. Ingested foreign bodies, which are not stopped in the mouth or oesophagus, enter into the stomach. Once a foreign body has passed through the pylorus, jejunum and ileum appear to be the most common sites of the small intestine obstruction. The records of 103 cases, treated at the Clinic for Surgery, Orthopaedics and Ophthalmology of the Veterinarian Faculty, University of Zagreb from January 1981 till December 1998 were analysed. The analysis included the incidence of ileus caused by foreign bodies and the distribution of patients by sex, age, breed, duration of illness, site of obstruction, types of foreign bodies and the interrelation between these parameters. The results of our research show that the number of patients with foreign body induced ileus is increasing. Males ingested foreign bodies more often than females. Foreign body induced ileus was more frequently found in animals below 2 years of age. Foreign bodies were mostly ingested by mongrels, but also by popular dog breeds such as Dobermanns, Poodles, Cocker Spaniels and Rottweiler. Most of these ileus cases were found in March and October and the predominant clinical signs included anorexia, dehydration, abdominal tenderness and absence of defecation. The most common site of small intestine obstruction by foreign bodies was the jejunum, and the most effective treatment was enterotomy. Dogs mostly ingested stones, plastic and rubber objects. The treatment was more successful in dogs below 2 years of age. Patients that died post-surgically, died mostly the first day after surgery.  相似文献

Eight dogs referred for the investigation of a persistent cough were found to have intrabronchial foreign bodies. The salient findings were haemoptysis, halitosis, eosinophilia and, on thoracic radiographs, an ill-defined peribronchial increase in radiodensity involving a caudal lobe bronchus. Bronchoscopy was essential to confirm the diagnosis as the results of radiography were equivocal. Removal of the foreign body via the trachea was achieved in seven patients resulting in complete recovery in six. Two dogs died of related complications.  相似文献

The diagnosis and treatment of nasal foreign bodies usually includes a combination of rhinoscopy and imaging techniques, such as CT. The purpose of this retrospective, multicenter study was to describe the CT characteristics of nasal foreign bodies in dogs and cats and to determine if different nasal CT features exist between acute and chronic cases. Twenty dogs and six cats met the inclusion criteria. Eleven nasal foreign bodies (42%) were detected confidently with CT. The foreign body had a linear shape in 81% of cases and displayed a “tubular‐like appearance” in 54% of cases. In five cases (19%), a foreign body was suspected but not clearly visible. Additional CT changes were present in the nasal passages in 96% of the cases. The presence of turbinate destruction (P = .021) and mucosal thickening (P = .014) on CT were associated with the presence of a chronic nasal foreign body. In this sample, the nature of the foreign body did not influence its visibility and was not associated with specific CT characteristics. Computed tomography may be useful in the investigation of nasal foreign bodies, however, a negative CT examination does not exclude their presence.  相似文献

The equine head is a complex structure prone to traumatic injuries. To determine the value and limitations of radiography and (CT) for the diagnosis of skull fracture, the differences between the two modalities were described. Two observers retrospectively reviewed the radiographic and CT images of 18 horses with a skull fracture. To allow direct comparison between the two modalities, a simplified fracture classification system was used. In 3/18 cases the evaluation of the radiographic examination concluded no injuries visible. In 2/15 cases soft tissue involvement was not detected and in 7/15 cases the extension of the fracture was underestimated with radiography. Radiography classified 4/10 multiple fractures incorrectly as single fracture and 5/15 comminuted fractures on CT were diagnosed as simple fracture with radiography. The number of fragments was underestimated with radiography in 14/15 cases. In conclusion, radiography is able to diagnose a skull fracture in most cases. Skull fractures however are not similarly classified after radiographic and CT evaluation, which causes a difference in interpretation and perception of the fractures. Therefore, CT should be the modality of choice for surgical planning and prognosis.  相似文献

Background: Foreign body aspiration is a differential diagnosis for acute or chronic cough that requires medical or surgical management in animals. Hypothesis: Success of bronchoscopy in airway foreign body removal is dependent on the size of the animal, duration of clinical signs, and location of the foreign body. Animals: Thirty‐two dogs and 5 cats with airway foreign bodies identified at the UC Davis Veterinary Medical Teaching Hospital. Methods: Retrospective case study evaluating the role of duration of clinical signs and body size in successful bronchoscopic removal of foreign bodies. In addition, radiographic localization of disease was compared with bronchoscopic identification. Bronchoalveolar lavage (BAL) culture and cytologic findings are reported. Results: Bronchoscopy was successful for removal of airway foreign bodies in 76% of animals (24/28 dogs and 2/5 cats), and in dogs was independent of duration of clinical signs or body size. One‐third of thoracic radiographs lacked distinctive features of an airway foreign body, and therefore radiography was unable to predict the affected site. BAL fluid at the site of the foreign body contained more neutrophils and more often had intracellular bacteria than lavage fluid from a separate site. Conclusions and Clinical Importance: Bronchoscopy was successful in removing airway foreign bodies regardless of animal size or long duration of clinical signs. Results of this study confirm the utility of bronchoscopy with lavage in management of suspected foreign bodies, even in the absence of localizing radiographic findings.  相似文献

The history, clinical signs, results of diagnostic imaging, treatment methods and outcome of 16 adult horses with a metallic foreign body in the tongue are reviewed. All the horses had a swollen tongue, they salivated excessively and were partially to completely anorexic. Less common clinical signs were fever, an enlarged and painful intermandibular space, dysphagia, unilateral tongue paralysis and halitosis. Most of the horses had shown clinical signs for less than 24 hours. The foreign bodies were diagnosed by oral examination, radiography and ultrasonography; they were removed from the tongue of four of the horses during the initial oral examination, and were removed surgically from nine others; the other three horses were treated medically without attempts being made to extract the foreign bodies. Twelve of the bodies were small pieces of wire and one was a hypodermic needle. All the horses received a combination of antimicrobial and non-steroidal anti-inflammatory drugs and they all made an uneventful recovery. Feeding hay and the use of cable-framed tractor tyres as feeders were commonly associated with the cases.  相似文献

Objective— To investigate the effect of tibial plateau leveling osteotomy (TPLO) on the proximal tibial soft tissue envelope with and without use of protective gauze sponges, and to determine whether the action of an oscillating saw blade on the gauze sponges would result in retention of particulate cotton debris. Study Design— Cadaveric study. Animals— Medium to large breed dog cadavers (n=10; 20 pelvic limbs). Methods— TPLO was performed using the currently recommended technique involving dissection of the proximal tibial soft tissue envelope and its protection using cotton gauze sponges. In paired limbs, the procedure was repeated but no attempt was made to retract and protect the proximal tibial soft tissue envelope. Damage to the soft tissue envelope and presence of gross particulate cotton debris were investigated by direct observation and photographic analysis. Presence of microscopic cotton debris was investigated using light microscopic analysis of wound lavage fluid. Results— No soft‐tissue trauma was found in gauze sponge‐protected specimens. When protective gauze sponges were not used, full‐thickness (sagittal plane) lacerations to the caudoproximal tibial muscle group occurred in all specimens with a mean craniocaudal width of 9.5 mm (range 2–12 mm). The cranial tibial muscle was traumatized in only 1 specimen without protective gauze sponges. Trauma to the popliteal vessels was not identified in any specimen. No gross cotton debris was identified, but microscopic cotton fibers (diameter, 7–35 μm) were identified in lavage fluid from all gauze sponge‐protected specimens. Conclusions— Use of protective gauze sponges is effective in protecting the proximal tibial soft tissue envelope from an oscillating TPLO saw blade, but results in retention of microscopic cotton particulate debris within the operative site. Significant soft tissue trauma is seen only in the caudoproximal tibial muscle group if protective gauze sponges are not used. Clinical Relevance— Retraction and protection of the caudoproximal tibial soft tissue envelope is recommended during TPLO; however, to prevent retention of microscopic particulate cotton debris, alternatives to cotton gauze sponges should be considered as protective devices.  相似文献

Wooden skewer foreign bodies were found in eight dogs. Five presented for evaluation of draining sinuses and two for inflammatory disease referrable to the thorax, abdomen or pelvic region. In an additional case, pneumothorax resulted from perforation of the stomach and diaphragm. Ingestion was considered the most likely mode of access of the foreign body in all cases. Although the wooden foreign bodies were seen radiographically in only two dogs, identification of soft tissue or bony changes, or the results of contrast sinography assisted diagnosis. Surgical retrieval led to resolution of signs in all cases.  相似文献
