Summary: A 3 year old, female great Dane with atresia of the right external ear canal had recurrent episodes of ear pain. Radiography revealed absence of air in the right external acoustic meatus, thickened bone of the right tympanic bulla and increased radiodensity of the chamber of the bulla. Total ear canal ablation and lateral bulla osteotomy were performed. The superficial portion of the external ear canal was absent and the deeper segment of the vertical ear canal began as a blunt ended cartilage tube. A patent lumen in the existent portion of the external ear canal and the tympanic bulla contained wax, hair and exfoliated squames. The tympanic membrane was not intact. No bacteria were cultured from the contents of the external and middle ear. The dog responded well to surgery and was free of pain 11 months later. Failure to surgically correct atresia of the ear canal in young dogs may allow the accumulation of cellular and sebaceous debris with subsequent involvement of the middle ear in an inflammatory response.  相似文献

OBJECTIVE: To determine whether cats undergoing ventral bulla osteotomy (VBO) for removal of inflammatory polyps or nasopharyngeal masses have altered ability to hear and whether polyp or mass removal affects auditory function as measured via air-conducted brainstem auditory evoked response (BAER). DESIGN: Prospective case series. ANIMALS: 21 cats. PROCEDURES: Cats were sedated and had otic-oral examinations to confirm presence of nasopharyngeal masses; BAER testing was done immediately prior to and following the completion of a VBO and polyp or mass removal. Recheck examination, including otic examination and BAER in sedated cats, was performed when possible. Results-17 cats met final inclusion criteria, and long-term follow-up was available for 15. Six of 17 had deafness as measured via air-conducted BAER prior to surgery. Mean followup time was 161 days, and there was no change from presurgical status in auditory ability in any cat. Eleven of 17 developed ipsilateral Horner syndrome in the immediate postoperative period, and 1 of 16 had polyp regrowth. CONCLUSIONS AND CLINICAL RELEVANCE: Results suggested that in cats, VBO for removal of inflammatory polyps or masses is unlikely to affect hearing as measured via air-conducted BAER. Most cats developed short-term Horner syndrome. Cats with deafness prior to surgery did not regain auditory function. Ventral bulla osteotomy to remove nasopharyngeal polyps or masses provided no functional advantage with regard to restoration of hearing, compared with other surgical techniques. Polyp recurrence and long-term adverse effects were uncommon.  相似文献

Brainstem auditory evoked responses (BAERs) to click and tone-burst stimuli (0.5, 1, 2, 4, and 8 kHz) were recorded from 32 ears in 16 adult Beagles. BAERs were used to establish a normative database and describe response thresholds and latencies (wave I-IV) for the different stimuli. The frequencies of sound used were 8, 4, 2, 1, and 0.5 kHz. Sound stimuli were delivered by a closed headphone and masking stimuli were not applied. The BAER waveforms obtained were similar to those reported previously, except for a frequency-following response evoked by 1-kHz tone-bursts. The threshold was lowest when clicks and 4-kHz tone-bursts were used. Significant differences were observed between the mean values of the peak latencies for clicks and 4-and 8-kHz tone-bursts. The tone-burst stimulation used in our experiment was shown to be an appropriate method for detecting the BAERs. Thus, for investigating the frequency-specificity of the auditory sense, BAER data obtained using tone-burst sound in dogs may be meaningful.  相似文献

Three West Highland white terriers were presented for investigation of left‐sided para‐aural abscessation. CT revealed chronic otitis media with extensive osseous proliferation surrounding the horizontal and vertical ear canals contiguous with the expanded temporal bone, consistent with a unilateral variant of craniomandibular osteopathy. A left total ear canal ablation with lateral bulla osteotomy was performed in all dogs. An ultrasonic bone curette (Sonopet; Stryker) proved useful when removing the osseous proliferation in two dogs. Histopathological examination of the ear canals was consistent with craniomandibular osteopathy and the treatment led to resolution of the presenting clinical signs in all dogs. To our knowledge, this is the first report of craniomandibular osteopathy engulfing the external ear canal, presumably leading to chronic otitis media and para‐aural abscess formation. This is also the first reported use of an ultrasonic bone curette in canine otic surgery.  相似文献

A nine-month-old Labrador retriever was referred to the Clinica Veterinaria Privata San Marco because of frequent headshaking and downward turning of the right ear. Clinical examination revealed that there was no external acoustic meatus in the right ear. Computed tomography confirmed that the vertical part of the right auditory canal ended blindly, providing a diagnosis of external auditory canal atresia. Cytological examination and culture of fluid from the canal and the bulla revealed only aseptic cerumen; for this reason, it was assumed that the dog was probably affected by a congenital developmental deformity of the external auditory canal. Reconstructive surgery was performed using a "pull-through" technique. Four months after surgery the cosmetic and functional results were satisfactory.  相似文献

Elephant ear mites, not previously described in North America, were eradicated in two African elephants (Loxodonta africana) after six otic instillations of ivermectin at 2-wk intervals. The microscopic examination of a clear, mucoid discharge collected from the external ear canals of two wild-born African elephants housed in a New York State zoo for 25 yr revealed live mites (Loxoaneotus bassoni). The cytologic examination demonstrated no evidence of inflammation or infection. Both elephants were asymptomatic with normal hemograms and serum chemistry panels. A diagnosis of otoacariasis was made. Each elephant was treated six times with 5 ml of 1% ivermectin syrup instilled in each ear canal once every 2 wk. Microscopic examinations of clear mucus collected from each elephant's ear canals 9 days after the first instillation of ivermectin were negative for any life stages of ear mites. Microscopic examinations of mucus collected from both elephants' ear canals at 6, 11, and 16 wk, as well as annually post-treatment for 7 yr, confirmed eradication of the ear mites. The L. bassoni ear mite was first identified in the external ear canals of wild, asymptomatic, lesion-free, African elephants culled in Kruger National Park in South Africa. However, a new species in the same genus of mites (Loxoanoetus lenae) was identified at the necropsy of an 86-yr-old Asian elephant (Elephas maximus) living in a circus in Australia. The autopsy revealed a marked, ballooning distension of bone around the left external acoustic meatus, suggestive of mite-induced otitis externa, as seen in cattle infested with ear mites (Raillieta auris). Elephant health care providers should identify the prevalence of, and consider treatment of, elephants in their care infested with ear mites, given the possible risk for adverse health effects.  相似文献

A 7-month-old female llama was examined because of chronic otitis media and externa of 7 months' duration. Radiographically, the tympanic bullae appeared thicker than normal, and the ventral borders were poorly defined; the left external acoustic meatus (ear canal) appeared to be narrower than the right. The llama was treated with penicillin, and the ear canals were lavaged daily. Contrast radiography was performed on day 15 to determine the shape and size of the ear canals and evaluate the integrity of the tympanic membranes. Contrast medium was visible radiographically in the left tympanic bulla, indicating that the left tympanic membrane was ruptured, but the right tympanic membrane appeared to be intact. The left ear canal was narrower than the right, and the bony ear canals had a well-defined sigmoid shape. The right ear improved with medial treatment alone, but the left ear did not. Therefore, lateral ear canal resection was performed. After surgery, however, exudate was still evident in the left ear canal, and the llama became more lethargic and more reluctant to eat. Lateral bulla osteotomy was attempted, but no purulent material was obtained, and curettage of the bulla resulted in hemorrhage. Because of this and because of the llama's poor physical condition, a decision was made to euthanatize the llama. The sigmoid shape of the bony ear canal and the multicompartmental nature of the tympanic bulla make surgical treatment of otitis media and externa in llamas difficult. Further study of surgical treatments for otitis media in llamas is needed.  相似文献

Five dogs were diagnosed as having unilateral separation of the otic cartilages (auricular and annular). Four dogs had sustained prior trauma (hit by a car). Clinical signs were typical of ear canal stenosis and included swelling at the base of the ear (n = 2), periotic fistulation (n = 2), head tilt toward the affected side (n = 2), and pain when opening the mouth (n = 1). Palpation revealed separation of the horizontal and vertical ear canals. Otoscopic findings included a shallow ear canal and a pseudotympanic membrane across the proximal end of the auricular cartilage. Findings at surgery confirmed the diagnosis of cartilage separation and included an exudate-filled horizontal ear canal and a blind-ending vertical ear canal. Surgical treatment involved isolating the distal end of the annular cartilage (horizontal ear canal), opening and evacuating the horizontal ear canal, and suturing the open end of the annular cartilage to the skin. The length of annular cartilage was variable among the dogs, although each dog had sufficient horizontal canal to appose skin to otic epithelium. The vertical ear canal was not surgically dissected or manipulated. The mean follow-up period was 46 ± 14 months (range, 31 to 68 months). All five dogs had patent horizontal ear canals when examined at the follow-up examination.  相似文献

OBJECTIVE: To investigate effects of ear type, sex, age, body weight, and ambient climatic conditions on external acoustic meatus (external ear canal) temperatures in dogs. ANIMALS: 650 dogs without clinical signs of ear disease. PROCEDURE: Dogs were assigned to 5 groups on the basis of auricular conformation and amount of hair in the external ear canal and 4 groups on the basis of body weight or age. External ear canal temperatures (EECT) were measured, using an infrared thermometer. Mean EECT determined for each group were compared to evaluate effects of ear type, age, weight, and sex, and EECT measured at different ambient temperatures and humidities were compared to determine climatic effects. RESULTS: Dogs with hirsute ear canals had significantly lower EECT than dogs with hairless ear canals, whereas significant differences were not detected between dogs with erect and pendulous auricles. Dogs < 6 years old had higher EECT than older dogs, and dogs that weighed < 6 kg had lower EECT than larger dogs. External ear canal temperatures measured when ambient temperatures were < 25 C were less than those measured at warmer temperatures. Sex and relative humidity did not affect EECT CONCLUSIONS AND CLINICAL RELEVANCE: EECT of dogs without ear disease were affected by weight and age, amount of hair in the external ear canal, and ambient temperature. However, hirsute ears had lower, not higher, EECT than hairless ears, suggesting that EECT may play less of a role in the development of otitis externa than believed.  相似文献

COMPUTED TOMOGRAPHIC APPEARANCE OF INFLAMMATORY POLYPS IN THREE CATS   总被引:1,自引:0,他引:1  
This report describes the use of computed tomography (CT) to evaluate the inner ear, tympanic bullae, nasopharyngeal area, and external ear canals of three cats. All cats presented for evaluation of upper respiratory signs or chronic ear infection. Nasopharyngeal masses were present in two cats, and a mass in the external ear canal was present in the third cat. In all three cats, CT was able to define the extent of osseous bulla involvement, which was confirmed at surgery. Computed tomography also defined the extent of the polyp in the nasopharyngeal area in two cats, and in the external ear canal in one cat. Surgical removal of the polyps was accomplished with a combined oral approach and ventral bulla osteotomy in cats 1 and 2. The polyp was removed from cat 3 using a combination of ventral bulla osteotomy and excision of the mass through the external ear canal. Microscopic examination confirmed all masses as nasopharyngeal (inflammatory) polyps.  相似文献

A 6-year-old French spaniel and a 14-month-old German shepherd dog were diagnosed with ear canal atresia. Based on presentation, computed tomography, and auditory function evaluation, the first dog underwent excision of the horizontal ear canal and bulla curettage, and the second underwent re-anastomosis of the vertical canal to the external meatus. BACKGROUND: The brainstem auditory-evoked response (BAER) is currently the standard evaluation method of hearing in dogs. In asymmetrical hearing loss in human patients, simultaneous presentation of masking noise to the nontest ear is routinely performed during BAER to eliminate the crossover effect. HYPOTHESIS: The crossover effect occurs during canine BAER, and masking noise of 20 decibels (dB) below click stimulus intensity is sufficient to abolish this effect. ANIMALS: Fifty-six Dalmatian puppies with confirmed unilateral deafness. METHODS: The BAER was elicited with 80 and 100 dB normalized hearing level (dBnHL) stimulus intensity in the deaf ear. The 100 dBnHL stimulus was repeated while simultaneously applying 80 dBnHL white masking noise to the nontest ear. RESULTS: Ten dogs were excluded because of BAER trace baseline fluctuation. In the remaining 46 dogs, 8 dogs had no waveforms, but 38 dogs had an identifiable wave-V in the deaf ear BAER at 80 dBnHL intensity stimulus. At 100 dBnHL intensity stimulus, all but 1 dog had a discernible wave-V in the deaf ear BAER. The deaf ear BAER waveforms were abolished by white masking noise at 80 dBnHL in the nontest ear in all dogs. CONCLUSIONS AND CLINICAL IMPORTANCE: Abolition of BAER wave-V in the deaf ear by white masking noise in the nontest ear suggests that this wave is caused by the crossover effect. beta distribution indicates 95% confidence that white masking noise, at 20 dB below click stimulus intensity, would abolish this crossover effect in over 90% of the dogs. This supports using masking noise in the nontest ear during canine BAER.  相似文献

Cats may demonstrate deafness due to a variety of aetiologies and the current preferred method for assessing auditory function is the brainstem auditory evoked response (BAER). The BAER has largely been replaced by otoacoustic emission (OAE) testing in human neonatal deafness screening as the equipment is more readily available, is cheaper and the test is less invasive and simpler. This is the first study to demonstrate that transient evoked OAEs (TEOAE) and distortion product OAEs (DPOAE) can be recorded in cats using commercially available equipment. Protocols for recording the emissions and analysing the results are given. DPOAE testing is suggested to be quicker in this population of healthy cats and shows promise in rapidly providing detailed information about auditory function at a variety of different frequencies.  相似文献

Canalography was performed to measure the diameter of the horizontal ear canal and to assess the clinical relevance of the measurements in identifying abnormalities of the ear canal. The diameter of the proximal and distal end of the annular cartilage in the horizontal ear canal was measured using canalography in 222 ears. The tympanic membrane could not be visualized with otoscopic examination in 70 ears even after ear cleansing. These canals were classified as being stenotic. The diameter of the proximal annular cartilage in the stenotic canals ranged from 0.8 to 3.1 mm (mean = 2.6 +/- 0.8 mm). The proximal annular cartilage was consistently smaller in diameter than the distal annular cartilage. The ratio between the diameter of the proximal and distal annular cartilage varied between dogs. In stenotic ear canals (70 ears) the ratio was less than 0.65. Total ear canal ablations and histopathological analysis were performed in 70 ear canals classified as stenotic after canalography. Hyperplasia of the epidermal layer was found in 56 stenotic canals. Stenosis of the canal due to otitis externa was found in the other 14 canals. Three canals with severe otitis externa were also diagnosed with canalography as having a ruptured tympanic membrane. The results suggest that canalography can be used to measure the diameter of the horizontal ear canal and to assess whether or not the tympanic membrane is ruptured prior to conservative or surgical therapy.  相似文献

Objective To use the brainstem auditory evoked response (BAER) to test the hypothesis that auditory function could be worse in older horses than in younger horses. Procedure BAER waveforms in response to click stimuli were measured in five younger horses (5–8 years) and four older horses (17–22 years). Results Compared with the younger horses, the older horses showed significantly (P < 0.02) worse BAER thresholds and significantly (P < 0.02) worse BAER wave V amplitudes to the 90 decibels above normal hearing level stimulus. These results were consistent with partial deafness in the older horse group. Conclusion BAER assessment can be used to identify partial deafness in older horses. Such horses should be managed appropriately, with particular care taken in noisy environments where hearing loss could put the horse and/or its owner at risk of harm.  相似文献

In a survey of 900 Dalmatian dogs, brainstem auditory-evoked responses (BAER) and clinical observations were used to determine the incidence and sex distribution of bilateral and unilateral BAER abnormalities and their association with heterochromia iridis (HI). To assess the efficacy of BAER testing in guiding breeding programs, data from 749 dogs (subgroup A), considered to be a sample of the population at large, were compared with data from a subgroup (subgroup B; n = 151) in which selection of breeding stock had been based on BAER testing from the beginning of the 4-year survey. Brainstem auditory-evoked responses were elicited by applying click stimuli unilaterally, while applying a white noise masking sound to the contralateral ear. Under these conditions, BAER were either normal, unilaterally absent, or bilaterally absent. Dogs with bilaterally absent BAER were clinically deaf; dogs with unilaterally absent BAER were not clinically deaf but appeared dependent on their BAER-normal ears for their auditory-cued behavior. Dogs with unilaterally absent BAER often were misidentified as normal by uninformed observers. Among the 900 dogs, 648 (72.0%) were normal, 189 (21.0%) had unilateral absence of BAER, and 63 (7.0%) had bilateral absence of BAER or were clinically deaf and assumed to have bilaterally absent BAER (n = 4). Total incidence in the population sampled was assumed to be higher, because some bilaterally affected dogs that would have been members of subgroup A undoubtedly did not come to our attention. Among females, 24.0% were unilaterally abnormal and 8.2% were bilaterally abnormal whereas, among males, 17.8% were unilaterally abnormal and 5.7% were bilaterally abnormal.(ABSTRACT TRUNCATED AT 250 WORDS)  相似文献

Hearing function was measured in normal dogs and in dogs with otitis using brainstem auditory evoked response testing. Data were obtained from 86 normal ears and from 105 ears with otitis, cntegorised into four degrees of severity. The data were analysed to illustrate the differences between the hearing function in the normal and abnormal ears and to estimate the degree of impairment associated with differing degrees of pathology. While severe hearing loss seemed to be present in the dogs with more severe otitis, only two individuals were identified as being totally deaf in the affected ears and no dogs were identified in which the cleaning and examination processes had caused damage to hearing function. Cleaning the ear canal produced measurable improvements in hearing in several dogs, indicating the profound effect of physical obstruction of the external ear canal by debris. It is concluded that most dogs with chronic otitis externa are not totally deaf and that the hearing impairment that does occur has the characteristics of conductive hearing loss.  相似文献

This report describes the clinical findings, clinicopathology and treatment of otomycosis caused by Aspergillus spp. in an atopic dog affected by chronic unilateral purulent otitis externa unresponsive to topical and oral antibiotics and antifungal treatments. Cytology of otic exudate revealed neutrophils and septate fungal hyphae, and otic culture grew Aspergillus spp. and no bacteria. Treatments included allergen‐specific immunotherapy, topical and oral antifungal therapy and anti‐inflammatory steroid therapy. Final resolution occurred after treatment of the underlying hypersensitivity disorder, administration of topical ketoconazole and debridement of infectious ear exudate. Otomycosis due to filamentous fungi may, as in humans, occur in dogs with ear canals compromised by pre‐existing allergic or bacterial otitis, and possibly previous antibiotic therapy. Antifungal medications provided clinical improvement, but the key to successful treatment was the restoration of the normal physiology of the external auditory canal.  相似文献

OBJECTIVE: To measure impedance audiometric values in clinically normal dogs that were sedated or anesthetized, evaluate effects of ear flushing on tympanometric measurements, and determine effects of performing acoustic reflex testing in a sound-attenuated room. ANIMALS: 35 mixed-breed and purebred client-owned dogs and 21 laboratory-bred Beagles. PROCEDURES: Tympanometry and acoustic reflex testing were performed on 27 mixed-breed and purebred dogs under isoflurane anesthesia in a non-sound-attenuated room and 21 Beagles under sedation in a sound-attenuated room. Tympanometry was performed on 8 mixed-breed dogs under halothane anesthesia before and after ear canal flushing. RESULTS: Among impedance audiometric values, ear canal volume and compliance peak were smaller in Beagles than in mixed-breed dogs; differences among other values were not detected. Ear canal volume was dependent on body weight. Differences were not found for tympanometric values measured before and after ear canal flushing. CONCLUSIONS AND CLINICAL RELEVANCE: Results of this study established reference range values for impedance audiometric measurements in clinically normal dogs under isoflurane anesthesia or sedation. Acoustic reflex testing does not need to be performed in a sound-attenuated room. The ear canals of clinically normal dogs can be flushed prior to performing tympanometry without altering the results. Impedance audiometry may be a useful noninvasive procedure for the diagnosis of otitis media in dogs.  相似文献

Eight beagles were experimentally inoculated intraotally with Malassezia pachydermatis to induce acute otitis externa. Three or 4 days after the inoculation, the animals showed the symptoms of otitis externa. All ear canals were erythematous and the dogs were shaking their heads. A large number of M. pachydermatis was noticed in exudate taken from every ear canal. Clinical signs of otitis externa were reduced after treatment with 0.1 ml (per canal) of 1% pimaricin suspension twice a day for 3 days. The amount of exudate decreased gradually and 12 of the 16 ear swabs examined, thereafter, were found to be negative for M. pachydermatis within 10 days. No side effects were observed in all the treated cases. These results suggested that M. pachydermatis could induce the canine otitis externa, and that pimaricin is effective agent for M. pachydermatis infection in ear canals.  相似文献
