A steer examined because of obstructive urolithiasis and urethral rupture underwent laser lithotripsy, using a chromium-thulium-holmium:yttrium-aluminum-garnet (Ho:YAG) laser inserted through an ischial urethrotomy. Procedures were performed with caudal epidural anesthesia. Six months after surgery, the urethra was patent with no clinical evidence of urethral stricture or fistula. Ischial urethrotomy provided rapid access to the bladder for catheterization and to the obstructive urolith for lithotripsy. Laser lithotripsy was a rapid and effective means of urolith removal in this steer.  相似文献

A 19-month-old entire male French lop rabbit was presented with a two-week history of a depressed appetite and lethargy. Clinical and laboratory findings, together with abdominal radiographic studies, suggested a diagnosis of right-sided calcium ureterolithiasis. Management included the surgical removal and analysis of the urolith. Confirmation of a calcium-containing urolith and the presence of hypercalcaemia necessitated the introduction of a low calcium diet. Recovery was complicated by the occurrence of a second urolith within the left ureter. This was also removed successfully by surgical means. The rabbit made a full recovery and assessment of serial serum calcium concentration has since confirmed the long-term maintenance of calcium levels within their normal range.  相似文献

BACKGROUND: Laser lithotripsy has been used as an alternative to surgical removal of uroliths in a number of species. OBJECTIVES: To determine the effectiveness of laser lithotripsy for removing urocystoliths, urethroliths, or both in dogs, and to determine the influence of dog (chronological order of lithotripsy, endoscope type, sex, body weight) and urolith (total urolith and urocystolith number, presence or absence of urethroliths, largest urolith dimension, and urolith composition) factors on outcome. ANIMALS: Twenty-five client-owned dogs with urocystoliths, urethroliths, or both were included. Male dogs were required to weigh > or =6.8 kg. METHODS: A single-arm prospective clinical trial. Laser lithotripsy was performed under general anesthesia. Main outcomes included procedure success and lithotripsy time. Predictors of procedure success and lithotripsy time were selected using logistic and linear regression modeling, respectively. RESULTS: The procedure was successful in 21 of 25 dogs. The procedure was successful more often and in less time in female spayed than in male neutered dogs. Sex was identified as a predictor of success. Univariable analysis showed that sex, endoscope type, and chronological order in which the dog received lithotripsy were significantly associated with lithotripsy time. Stepwise linear regression modeling identified sex, body weight, >10 uroliths, and largest urolith dimension as predictors of lithotripsy time. CONCLUSIONS AND CLINICAL IMPORTANCE: Laser lithotripsy is an effective procedure for the removal of urocystoliths, urethroliths, or both with rare important adverse effects and some limitations in male dogs.  相似文献

Objective: To describe successful surgical treatment of urinary incontinence caused by a ruptured and/or transected urethral sphincter in a mare. Study Design: Clinical report. Animal: A 7‐year‐old, Swedish Warmblood mare with urinary incontinence. Methods: The urethral sphincter, which had been damaged during removal of a cystic urolith, was repaired by apposing the ends of the disrupted urethralis muscle and tunica muscularis. Results: The mare was no longer incontinent after repair of the defect by apposition of the ends of the urethralis muscle and tunica muscularis. Conclusions: Transection and/or rupture of the urethral sphincter of a mare may result in urinary incontinence. Clinical Relevance: Apposition of the ends of the ruptured or transected urethralis muscle and tunica muscularis can correct urinary incontinence caused by this defect.  相似文献

Holmium:yttrium-aluminum-garnet (Ho:YAG) laser lithotripsy was attempted in a mare and a gelding with calculi in the urinary bladder. The procedure was unsuccessful in producing adequate fragmentation of the calculi. In the gelding, pulsed dye laser lithotripsy was subsequently used to fragment the urolith. Background: Laser lithotripsy has been used as an alternative to surgical removal of uroliths.
Objective: To describe the procedure and efficacy of laser lithotripsy for removal of lower urinary tract uroliths in horses. Additionally, the ultrastructure and the differences in mineralogy and microstructure from 1 successful and 1 unsuccessful laser lithotripsy case are described.
Animals: Six client-owned horses with 7 episodes of naturally occurring urocystoliths, urethroliths, or both.
Methods: Retrospective study of all horses treated between 2006 and 2008 by laser lithotripsy. All horses were sedated followed by laser lithotripsy. Quantitative urolith analysis was performed in all cases. Ultrastructure and microstructure analyses were performed on uroliths from 2 horses.
Results: Procedural success was achieved in 5 of 7 laser lithotripsy procedures. No complications occurred as a result of laser lithotripsy. One horse developed uroabdomen likely as a result of manual lithotrite disruption of the bladder after failure of laser lithotripsy. There were differences in microstructure between 1 urolith that was successfully fragmented by laser lithotripsy and 1 urolith that was resistant to laser fragmentation.
Conclusions and Clinical Importance: Laser lithotripsy is an effective procedure for removal of some urocystoliths, urethroliths, or both in horses.  相似文献

Multiple techniques exist for removal of cystic calculi in the male horse, some of which use a perineal urethrotomy (PU) to facilitate access to the urinary bladder for removal of small uroliths in the standing, sedated horse. These techniques have limited efficacy in the removal of uroliths that have a diameter larger than the PU site, typically 4–5 cm. Pneumatic lithotripsy as reported in this article provides a novel way to fragment these larger uroliths while containing the fragments and protecting the urethra and urinary bladder. Ten geldings and one stallion underwent a lithotripsy procedure to remove one (n = 10) or several (n = 1) uroliths ranging in diameter from 4 to 9 cm. A PU was performed and a laparoscopic retrieval bag was inserted into the urinary bladder through the PU site. The urolith was manipulated into the bag using blind transrectal manipulation or endoscopic guidance and the opening of the bag was exteriorised out of the PU site. A pneumatic lithotrite was used to fragment the stone. When the urolith was adequately fragmented, the fragments were removed by the use of sponge forceps and by flushing the fragments from the bag with obstetrical lube. This was continued until the bag could smoothly be withdrawn from the urinary bladder. All uroliths were successfully removed. The PU sites healed without complication and none of the horses had recurrence of presenting clinical signs. This article also discusses the difference between two types of laparoscopic retrieval bags. Pneumatic impact lithotripsy in combination with a retrieval pouch offers a safe alternative to other methods for removal of cystic calculi in the horse.  相似文献

Urolithiasis has been reported in veterinary literature in some species of the Mustelidae family, including ferrets. In this species, the most common form of urolithiasis is that caused by struvite. The current study examined the case of mixed urolith in an 8-month-old female ferret (Mustela putorius furo) with antecedents of strangury and hematuria. A cystotomy was carried out to remove the urolith, which had a rough surface and a homogeneously porous interior and was formed by a mixture of struvite (60%) and calcium oxalate dehydrated (40%).  相似文献

Abstract: An 11‐year‐old female spayed domestic shorthair cat was referred to The Ohio State University Veterinary Teaching Hospital (OSU‐VTH) for evaluation of a 6 × 4 × 3.5 cm mass in the left midcervical region causing increased respiratory sounds and lateral deviation of the trachea. A fine needle aspirate of the mass was obtained before referral and the cytology results were compatible with a reactive lymph node. Immunocytochemistry showed increased numbers of CD3+ T lymphocytes and small numbers of CD20+ and CD79a+ medium to large lymphocytes. Differential diagnoses from the referral pathologist were T‐cell‐rich B‐cell lymphoma and feline Hodgkin's‐like lymphoma. A subsequent fine needle aspirate performed at the OSU‐VTH showed similar results. On flow cytometry the majority of cells were CD3+ T lymphocytes that were double positive for CD4 and CD8 (73%), compatible with either a double‐positive (CD4+CD8+) T‐cell lymphoma or lymphocytes from ectopic thymic tissue. The mass was surgically removed. Histopathology and immunohistochemistry of the mass revealed a predominant population of CD3+ small lymphocytes and small numbers of medium to large lymphocytes with moderate anisocytosis and anysokaryosis. A population of cytokeratin‐positive epithelial cells surrounded small microcystic structures filled with eosinophilic material and structures interpreted as Hassall's corpuscles. These findings were consistent with thymic tissue and a diagnosis of ectopic thymoma was made. PCR results for lymphocyte antigen receptor rearrangement (PARR) were negative. The cat had no evidence of disease 16 months after removal of the mass. To our knowledge this is the first report of an ectopic cervical thymoma in a cat. The clinical and diagnostic features of this unusual case will be useful in helping veterinarians and pathologists obtain a presurgical diagnosis and establish a prognosis for similar lesions.  相似文献

A 9-year-old Quarter Horse mare was referred for evaluation of continuous oestrus, polyuria and suspected pyometra after a glass marble was placed in the uterus by the owner. Palpation per rectum and transrectal ultrasonography confirmed the presence of a spherical object, but it was not possible to discern whether it was located within the lumen of the uterus or urinary bladder. Thus, an endoscopic examination of both uterus and bladder was carried out and a spherical, spiculated surface urolith was found on the floor of the bladder. The cystic urolith with suspected marble core was removed using a 12-mm specimen retrieval bag with endoscopic guidance. Follow-up with the owner one month after discharge revealed that the mare had not shown any signs of discomfort while urinating nor signs of oestrus.  相似文献

Three studies were conducted to evaluate foot ash as an alternative method to quantify the degree of bone mineralization in broiler chicks. In Experiment 1, the objective was to evaluate whether degree of bone mineralization would be reflected by foot ash in broiler chickens at 14 d of age. Birds receiving 0.24, 0.32, or 0.40% of dietary available phosphorus (AP) demonstrated significant differences in foot ash (10.93, 13.46, and 15.45%, respectively). In Experiment 2, we determined that the time required for a foot sample to dry to a constant weight at 105°C was 48 h. In Experiment 3, the objectives were to compare the response of bone mineralization to dietary phosphorus levels as reflected by foot ash and tibia bone ash and to evaluate the effect of fat extraction on the determination of these parameters. Broiler chicks were fed graded levels of AP (0.25, 0.30, 0.35, and 0.40%) from 1 to 14 d of age. Significantly different responses between each dietary AP level could be detected by any method used. A significant linear response was observed between dietary AP and percentage of extracted or unextracted tibia or foot ash, with determination coefficients above 90% found for all 4 methods used. Results confirmed that the dietary phosphorus levels affected bone mineralization and that the degree of mineralization could reliably be reflected by foot ash. The assay was found to be as reliable as the tibia bone ash in reflecting degree of bone mineralization in chicks during the first 14 d of age. Fat extraction did not affect the reliability of either assay.  相似文献

OBJECTIVE: To describe parainguinal laparocystotomy for urolith removal and to report outcome. STUDY DESIGN: Prospective clinical study. ANIMALS: Ten geldings with uroliths. METHODS: Signalment, number, size, type of uroliths, surgical technique, and complications were recorded. Long-term follow-up was obtained by telephone interviews with the client and/or examination by the referring veterinarian. RESULTS: Cystic calculi, 3-9 cm in diameter, were removed by parainguinal laparocystotomy. Mean surgical time was 59 minutes (range, 40-100 minutes). With this approach, ligation of the pudendal or superficial epigastric vessels was not needed. Serous incisional drainage resolved without treatment in 3 horses and 2 horses developed moderate-to-severe incisional swelling. On long-term follow-up (mean, 52 months; range, 11-82 months) for 9 horses, owners reported no recurrence of clinical signs associated with cystic calculi. CONCLUSIONS: Parainguinal laparotomy represents a satisfactory alternative approach to the apex of the bladder for urolith removal. CLINICAL RELEVANCE: Parainguinal laparocystotomy avoids the pudendal and superficial epigastric blood vessels that are encountered when performing median or paramedian laparocystotomy in geldings. This approach minimizes dead space by avoiding the reflection of the prepuce in paramedian or median approaches.  相似文献

This report describes a modification of the parainguinal approach for removal of cystic calculi: a ventral midline laparotomy‐guided parainguinal laparocystotomy. The ventral midline approach to the abdomen is rapid and routinely used by equine surgeons. With an arm introduced to the abdomen via the ventral midline, the surgeon is able to select the ideal parainguinal laparotomy incision location that allows bladder exteriorisation with the minimum amount of tension. Because the surgeon's hand is introduced via the ventral midline incision, the parainguinal incision can be sized to just allow exteriorisation of the urinary bladder and urolith, increasing extra‐abdominal bladder security during cystotomy while reducing incision size in the parainguinal region. Finally, an assistant's hand via the ventral midline incision can maintain bladder stability within the parainguinal incision during cystotomy closure.  相似文献

The influence of a high phosphorus (1.5%) and high calcium (2.2%) diet on ectopic mineralization in boars was examined over a four month period. The high phosphorus diet caused metastatic mineralization in the left atrial endocardium in 84% of animals and in the pulmonary and diaphragmatic pleura in 21 and 58% of animals, respectively. Mineralization, that apparently commenced as deposits of extraneous calcium and progressed by metastatic deposition, occurred also in the lamina propria mucosae of the respiratory airways and in the lamina muscularis mucosae of fundic stomach. Hyperplasia of osteoclasts and microfractures in costochondral junction, morphological features of activated parathyroid cells and a significant drop in serum phosphorus during the fourth month in boars fed high phosphorus, suggest that nutritional hyperparathyroidism was experimentally induced. No systemic bone disease developed. Feeding a high calcium diet resulted in 20% incidence of discrete lesions in the left atrial endocardium and no pleural involvement. Also, lesions in respiratory airways and fundic stomach were mild. The fact that high phosphorus but not high calcium increased the incidence and extent of ectopic mineralization suggests that hyperparathyroidism under normocalcemic conditions might be involved in the pathogenesis of these lesions.  相似文献

A 5‐year‐old female spayed Shetland sheepdog presented for evaluation of bandage sores on the left pelvic limb after a splint was placed to stabilise a digit fracture incurred approximately 2 weeks previously. Multiple areas of necrosis were identified upon bandage removal. After debridement of devitalised tissue, a 4 cm wound on the plantar surface of the metatarsal bones (with exposed bone and tendons) remained. The Papineau technique (which involves removal of infected or necrotic tissue, placement of autogenous cancellous bone graft within a wound, and delayed skin closure) was used to treat this wound. Use of bone graft in this manner aims to promote early granulation tissue formation and reduce the time until wound reconstruction can be performed. Autogenous cancellous bone graft was harvested from the proximal humerus and placed within the metatarsal wound. Once a healthy granulation tissue bed was noted to cover the bone graft, a meshed full thickness skin graft was placed. To our knowledge, this is the first report of the Papineau technique being used for this purpose in a dog.  相似文献

A histological survey was conducted on 604 pairs of adrenal glands from yellow (Papio cynocephalus) and olive (Papio anubis) baboons used in drug safety evaluation studies. Spontaneous lesions were found in 372 glands--34 of which had more than one change. Cortical lesions consisted of accessory nodules (190), nodular hyperplasia (7), hepatoadrenal adhesion (18) and partial fusion (1), focal mineralization (20), ectopic bone marrow (6), and focal fatty change (60). Medullary lesions were confined to focal lymphocytic (66) and plasma cell (1) infiltrates.  相似文献

OBJECTIVE: To describe a technique for laparoscopic-assisted removal of cystic calculi in geldings and report outcome. STUDY DESIGN: Clinical report. ANIMALS: Four geldings with cystic calculi. METHODS: Laparoscopic-assisted cystotomy and urolith retrieval was performed in 4 anesthetized geldings positioned in dorsal recumbency. With a laparoscope portal located at the umbilicus, the abdomen was insufflated and then the surgical table was tilted (30 degrees head-down position) before an instrumental portal was created parallel and 2-3 cm medial to the left external inguinal ring. Laparoscopic grasping forceps were inserted to grasp the cranial aspect of the bladder and elevate it to the ventral abdominal wall. With the instrumental portal as mid-point, the parainguinal skin incision was longitudinally extended cranial and caudal (approximately 8-10 cm) to accommodate the size of the urolith. The apex of the bladder was exteriorized and sharply incised, the urolith extracted, and after cystotomy closure, the bladder was repositioned. The mini-laparotomy and trocar incisions were closed in layers. RESULTS: There were no intra- or post-operative complications. All horses had minor incisional swelling for 3-4 days. No signs of abdominal or incisional pain were observed. Hematuria and slight stranguria occurred until the 3rd or 4th day. Surgical time (skin incision to skin closure) was 35-40 minutes. On long-term follow-up (up to 12 months) no recurrence of clinical signs associated with cystic calculi occurred. CONCLUSION: Uroliths (6-8 cm diameter) can be removed by laparoscopic-assisted cystotomy in geldings. CLINICAL RELEVANCE: Laparoscopic-assisted cystotomy combines the advantages of the parainguinal laparocystotomy with laparoscopic technique for removal of cystic calculi while avoiding their disadvantages.  相似文献

A 10-month-old male chow chow mixed breed dog was presented for anuria secondary to inadvertent prostatectomy performed during unilateral cryptorchidectomy. Ammonium chloride was added to diets varying in Ca content to evaluate its potential in preventing silica urolith formation in sheep. A 2 x 2 factorial experiment involved wether lambs with ad libitum access to a diet of 50% grass hay and 50% ground oats plus supplement. The basal diet contained on a DM basis 3.3% SiO2, .31% Ca, .22% P, 11.6% CP, and 26% ADF. Treatments (38 to 39 lambs/treatment) consisted of a control (C), limestone to increase dietary calcium to .6% (L), 1% ammonium chloride (A), and L + A (LA). After a 118-d experimental period, siliceous kidney deposits were found only in C and L, with silica making up 93% to 95% of the urolithic ash. Urolith incidences were 13% (C) and 18% (L), respectively. The lack of urolith development in lambs fed A and LA (ammonium chloride effect, P less than .01) and a trend toward a lower urolith incidence in C vs L (P less than .02) support the hypothesis that acid-forming effects of the diet and a reduction in the dietary Ca to P ratio reduce silica urolith formation.  相似文献
