A 3-day-old Quarter Horse colt was examined because of signs of severe depression, discomfort, and abdominal straining. The foal seemed disoriented, and the abdomen was tense and distended ventrally. The differential diagnoses included ruptured urinary bladder, retained meconium, septicemia/bacteremia, and neonatal maladjustment syndrome. Serum biochemical analysis revealed marked hyponatremia, hypochloremia, and moderate hyperkalemia, as well as mildly high urea, creatinine, and phosphorus concentrations. The primary differential diagnosis at this time was ruptured urinary bladder. Abdominocentesis was performed to confirm this diagnosis. Microscopic examination of abdominal fluid revealed calcium carbonate crystals, which originated from the urine of the foal. Biochemical analysis also confirmed the diagnosis of ruptured urinary bladder, because the ratio of peritoneal fluid creatinine to serum creatinine was 2.8:1. The foal died before surgical correction could be attempted.  相似文献

Two horses were presented with lethargy, weight loss, anorexia, and swelling of the limbs and ventral body wall. One horse, a 12-month-old American Paso Fino colt, also had acute abdominal pain. The other horse, a seven-month-old Tennessee Walking Horse (TWH) filly passed diarrheic stools during the initial examination. Each horse had low serum protein, neutropenia, and a normal packed cell volume (3.2 g/dl, 1300 cells/ul, and 38%, respectively, for the colt, and 2.4 g/dl, 696 cells/ul, and 44%, respectively for the filly). After intravenously administering plasma, the colt's PCV dropped to 23%, and the filly's dropped to 30%. During exploratory surgery, 3.5 and 2.0 meters of thickened terminal small intestine were removed from the colt and filly respectively, and a jejunocecostomy performed. The results of histologic examination of resected intestine were consistent with a diagnosis of equine granulomatous enteritis (EGE). Both horses showed clinical improvement within two days after surgery. The colt developed a neutrophilia (20,500 cells/ul) within 24 hours of surgery. Serum protein concentrations remained stable and gradually elevated to normal or near normal values of 7.0 g/dl (colt) and 5.8 g/dl (filly) by two weeks. The colt was killed four months after surgery because of signs of abdominal pain. Postmortem examination revealed a small intestinal volvulus associated with an adhesion. The TWH filly remains clinically normal 13 months after surgery.  相似文献

This case report documents uroperitoneum due to bladder and pelvic urethral rupture in a 9 h old Standardbred colt born after severe dystocia. Three surgical procedures were required to manage rupture of the urinary bladder, urethral rupture, and ventral abdominal hernia with evisceration of a loop of jejunum. After 40 days of hospitalisation the colt was discharged without any further complication.  相似文献

A 14-year-old thoroughbred gelding was presented for the evaluation of acute abdominal pain. Rectally, there was a soft fluctuant painful swelling dorsal to the bladder and to the right of the midline. The creatinine concentration of the peritoneal fluid was 15 mg/dl. Transrectal ultrasonographic examination of the urinary tract revealed a large collection of fluid dissecting from the pelvic portion of the right ureter ventrally through the right side of the bladder wall and into the retroperitoneal space, and a thickened right ureter and bladder wall at the level of the trigone. Cystoscopically there was moderate hemorrhage within the wall of the bladder. Ultrasonography revealed air within the retroperitoneal fluid collection after ureteral catheterization, confirming the preliminary diagnosis of a tear in the right ureter. The gelding was treated medically. After 48 h of hospitalization, nuclear scintigraphy revealed normal clearance from both kidneys and no apparent leakage from the right ureter. The ureteral tear and urinoma were monitored using transrectal ultrasound until resolution. The horse was successfully returned to racing. This case establishes the value of diagngstic ultrasound in the diagnosis and monitoring of a traumatic ureteral tear in a horse.  相似文献

To determine the reliability with which inorganic phosphorus (phosphate) concentrations can be used to predict major intestinal injury associated with equine colic, phosphate concentrations were measured in serum, peritoneal fluid, or both from 9 clinically normal adult horses (group A), 37 horses successfully managed medically for signs of abdominal pain (group B), 26 horses with signs of abdominal pain and undergoing exploratory laparotomy without intestinal resection (group C), and 26 horses undergoing intestinal resection or euthanasia for extensive intestinal lesions (group D). Peritoneal fluid phosphate concentration was significantly greater in horses in group D (mean, 4.58 +/- 0.34 mg/dl) than in horses in group A (mean, 2.78 +/- 0.21 mg/dl), group B (mean, 2.92 +/- 0.27 mg/dl), and group C (mean, 2.98 +/- 0.28 mg/dl; P less than or equal to 0.01). Serum phosphate concentration was significantly greater in horses in group D (mean, 3.87 +/- 0.30 mg/dl) than in horses in group A (mean, 2.73 +/- 0.22 mg/dl), group B (mean, 2.80 +/- 0.21 mg/dl), and group C (mean, 2.78 +/- 0.22 mg/dl); P less than or equal to 0.05). There was significant (P less than or equal to 0.001) correlation between serum and peritoneal fluid phosphate concentrations within each group and when pairs from all groups were pooled. When peritoneal fluid phosphate concentrations exceeded 3.6 mg/dl, intestinal lesions requiring resection or euthanasia were predicted with sensitivity of 77% and specificity of 76%. When serum phosphate concentrations exceeded 3.3 mg/dl, such lesions were predicted with sensitivity of 60% and specificity of 73%.(ABSTRACT TRUNCATED AT 250 WORDS)  相似文献

A 3-month-old Thoroughbred colt was presented with signs of colic. Findings from physical examination, abdominal paracentesis, abdominal radiographs and clinical pathology revealed uroperitoneum secondary to a calculus obstructing the urethra and causing subsequent urinary bladder rupture. Analysis of the calculus demonstrated a tissue center with outer concretions composed primarily of struvite.  相似文献

A 2-month-old Standard-bred colt with signs of abdominal pain and large scrotum was found to have septic periorchitis involving the right testis. Surgical exploration of the abdomen and scrotum was performed; the colt was then castrated. Actinobacillus equuli was isolated from specimens obtained at surgery. The colt was treated with broad-spectrum antibiotics and flunixin meglumine after surgery, and fully recovered. The clinical signs of periorchitis in the colt were similar to an inguinal/scrotal hernia.  相似文献

The purpose of this study was to assess the diagnostic value of ultrasonography in the evaluation of abdominal distension in 52 camels (Camelus dromedarius). The conditions included trypanosomiasis (n=35), intestinal obstruction (n=12) and ruptured urinary bladder (n=5). Fifteen clinically normal camels were included as controls. Transabdominal and transrectal ultrasonography was carried out on all camels. In animals with trypanosomiasis, ultrasonographic findings included accumulation of massive amounts of hypoechoic abdominal fluids where liver, intestine, kidney, spleen and urinary bladder were imaged floating. Except in two cases of bile duct calcification and one of hepatic abscessation, no detectable abnormal sonographic lesions were detected while imaging the hepatic and renal parenchyma, and the heart and its valves and major blood vessels. In camels with intestinal obstruction, ultrasonographic findings included distended intestinal loops with markedly reduced or absent motility. In one camel, the intestinal lumen contained localised hyperechoic material that was consistent with a foreign body. Hypoechoic fluid with or without fibrin was seen between intestinal loops. In camels with ruptured urinary bladder, ultrasonographic findings included collapsed and perforated bladder, echogenic blood clots within the urinary bladder and peritoneal cavity, increased thickness of the bladder wall, floating intestines in hypoechogenic fluid and echogenic calculi within the urethra. Ultrasonography was considered a useful tool for the evaluation of dromedary camels with abdominal distension.  相似文献

A 15-day-old American Quarter horse colt was presented for depression and pyrexia. Peritonitis was diagnosed following peritoneal fluid analysis. Exploratory laparotomy revealed an area of focal necrosis over the dorsal wall of the urinary bladder leading to peritonitis and uroperitoneum. The affected area of the urinary bladder was resected and the peritonitis resolved with medical treatment.  相似文献

A stillborn bovine male fetus with abdominal distention, arthrogryposis and atresia ani was presented for diagnostic evaluation. At necropsy, this fetus had a large amount of ascites, urachal obstruction and marked bladder distention. The ventral surface of the bladder had ruptured and attached to the abdominal wall by fibrinous adhesions. There was bilateral hydronephrosis with moderate pelvic dilatation and cortical attenuation. The rectum was filled with meconium but the anus was imperforate. The right forelimb was contracted. The cause(s) of these abnormalities could not be determined; however, we believe that developmental abnormalities during embryogenesis may be the result of chromosomal abnormalities.  相似文献

An 11-year-old mare presented 36 hours after foaling with a ruptured bladder. Uroperitoneum was diagnosed on ultrasound and from the creatinine concentration of the peritoneal fluid. Bladder endoscopy demonstrated tissue necrosis and a rent in the dorsocranial aspect of the bladder. Following stabilisation, including abdominal drainage and lavage, the mare was taken to standing surgery. Under continuous sedation and epidural anaesthesia, and after surgical preparation, a Balfour retractor was placed in the vagina. Using sterile lubricant and moderate force, it was possible to insert a hand into the bladder. The tear was easily palpable on the dorsal portion of the bladder. Two fingers were inserted through the tear and used to provide traction to evert the bladder completely into the vagina where it could grasped with the surgeons other hand to prevent further trauma. A second surgeon could then visualise the entire tear and repaired this using a single layer of size zero PDS suture in a single continuous pattern. As soon as the bladder was repaired, it was replaced via the urethra. The mare did well after surgery and was discharged after 48 hours, apparently normal.This report is the first describing repair of the bladder without an abdominal incision or incision into the urethral sphincter. This greatly reduces the chance of possible complications such as urine pooling after surgery with the previously described standing technique or bladder trauma due to traction with abdominal surgery.  相似文献

Two broodmares were diagnosed with rupture of the urinary bladder. One mare revealed abnormalities post partum and the other associated with ante partum uterine torsion. The clinical symptoms included mild abdominal pain, anorexia, decreased urinary volume and increased peritoneal fluid. In one mare, based on the creatinine level of the peritoneal fluid and serum biochemical abnormalities, uroperitoneum was diagnosed. In the other mare, the bladder rupture was found during the celiotomy for surgical repair of uterine torsion that was diagnosed upon rectal examination. Surgery was performed without a urethral sphincterotomy. The vaginal floor was incised in a standing position and the bladder was diverted into the vagina in order to suture the tears located in the ventrocaudal aspect of the bladder. Both mares survived after treatment for uraemia. Bladder rupture, although uncommon, may affect peripartum mares. Approach to the ruptured bladder without urethral sphincterotomy in a standing position should be considered as a choice for surgical repair.  相似文献

An 18 h old Quarter Horse filly, while being treated for diarrhoea, was evaluated for a distended abdomen and electrolyte abnormalities. Peritoneal and pleural fluid was detected by ultrasound and a presumptive diagnosis of a ruptured bladder was made. Intravenous fluid therapy was instituted to correct the electrolyte abnormalities prior to surgical repair of the bladder tear. Anaesthetic complications included hypoxaemia and decreased compliance secondary to the pleural effusion; therefore, a thoracocentesis was performed. Analysis of the pleural fluid revealed a pleural fluid to serum creatinine ratio of >1.0, indicative of urinothorax. The filly recovered from anaesthesia and was discharged on systemic antimicrobials. Urinothorax should be considered in cases of uroperitoneum with concurrent pleural effusion. Early detection may decrease patient morbidity and anaesthetic complications.  相似文献

Ruptured bladder was diagnosed in a 90-day-old Thoroughbred colt that had suffered a open, comminuted tibial fracture 2 days earlier. The bladder rupture was identified by laparoscopic examination of the abdomen and was repaired using a laparoscopic stapling instrument. This technique provided good visualization and allowed repair of the rupture with minimal intervention. Ten months after surgery, the foal was admitted to a referral surgical practice because of colic and stanguria. A urinary calculus was removed from the penile urethra by urethrotomy. Laparoscopic repair of the bladder with nonabsorbable staples may be contraindicated because of possible urolith formation.  相似文献

A 5‐month‐old Warmblood cross colt was presented with focal swelling of the ventral abdomen extending from the umbilicus to the scrotum in the absence of colic signs. Palpation and ultrasound examination revealed the presence of incarcerated large intestine within the subcutaneous space adjacent to the caudal ventral abdomen and prepuce. Surgery was performed and revealed that the umbilical hernia sac had ruptured, and confirmed that the left dorsal and ventral colon were present in the subcutaneous space. The mild degree of vascular compromise of the large colon did not necessitate resection and so it was replaced within the abdomen. The abdominal wall defect was closed and the subcutaneous dead space was reduced by using a walking suture pattern. Herniation of the large colon through the umbilicus with dissection through the subcutaneous tissues of the ventral abdominal wall and prepuce has not been previously reported in foals. Ultrasonography permits differentiation of herniated small intestine from large intestine.  相似文献

Babesiosis was diagnosed in a three-year-old racingThoroughbred colt with recurrent, intermittent fever, poor performance, malaise, and one occasion of diarrhea. Other signs included congestion of mucous membranes and petechial hemorrhages of conjunctival mucous membranes. Babesia caballi and B. equi were identified on blood smear, when the colt had fever. The colt was treated with Imidocarb (4 mg/kg four times at 72 intervals) and returned to regular exercise and competition four months after treatment. The colt and two other horses which showed babesial infection had been transported from Turkmen district to Mashhad city.  相似文献

The mean urea nitrogen concentration in vitreous humor (VUN) in 97 healthy steers after death was 15.7 mg/dl. The mean serum urea nitrogen concentration in the same cattle was 20.0 mg/dl. The mean vitreous creatinine (VC) concentration was 0.7 mg/dl, and the serum creatinine value was 1.5 mg/dl. The VUN and VC were both significantly lower than serum urea nitrogen and serum creatinine, respectively, but varied in a consistent, predictable manner. In tests with 10 cows, postmortem intervals as long as 36 hours and ambient temperatures up to 30 C had no significant effects on VUN and VC. Evaluation of 8 animals with uremia confirmed that the vitreous humor is a stable fluid compartment after death, usable as an indicator of serum urea nitrogen and creatinine.  相似文献

A 5-week-old Fell Pony colt was examined for fever, lethargy, and anemia. The colt had been lethargic for 1 week before examination, had continued to nurse, had a temperature of 104°F (40°C), and was treated with ceftiofur (5 mg/kg IM q12h). Approximately 36 hours before examination, the colt developed watery diarrhea. Blood work performed by the referring veterinarian on the day of admission revealed a PCV of 10%.  相似文献

Chylothorax associated with congestive cardiomypathy was diagnosed in a 10-year-old male castrated domestic shorthair cat via analysis of pleural fluid. The triglyceride concentration in the pleural fluid was high (579 mg/dl), compared with that in serum (87 mg/dl), and the fluid cleared with ether. Evidence of atrial and left ventricular enlargement was found via electrocardiography and thoracic radiography. Left-sided cardiac dilatation and reduced systolic motion of the left ventricle were revealed by echocardiography. The cat was treated with digoxin, furosemide, and aspirin. Two months after initiation of treatment, fluid (265 ml) was aspirated from the pleural cavity. Two months after this procedure, the cat was doing well, with minimal pleural fluid production.  相似文献

REASONS FOR PERFORMING STUDY: Analysis of human hair for drug residues is being used increasingly as a diagnostic tool in the investigation of drug use and abuse. Hair analysis is complementary to urine/blood testing in that it can provide an extensive historical record of drug use, is noninvasive, impersonal and can facilitate retesting. However, the technique has not been studied in horses. HYPOTHESIS: That the systemic administration of drugs in horses could be identified by the detection of drug residues in hair. OBJECTIVE: To evaluate hair analysis as a potential retrospective diagnostic test for drug administration in horses by studying the deposition of systemically administered drugs in tail hair. METHODS: Tail hairs (n = 40-50) from 4 horses with known drug histories were washed, chopped into 3-5 mm fragments and extracted overnight, in 0.1 mol/l hydrochloric acid, prior to solid-phase extraction and analysis by high-performance liquid chromatography. Horse 1, a 3-year-old Thoroughbred colt (gastric ulcer), was treated for 14 days with omeprazole; Horse 2, a 3-year-old Thoroughbred colt (anaerobic infection), was treated for 5 days with metronidazole; Horse 3, an 8-year-old Thoroughbred gelding (sinusitis), was treated for 10 days with trimethoprim/sulphadiazine; and Horse 4, a 3-year-old Thoroughbred colt (respiratory infection), was treated for 5 days with procaine benzylpenicillin. RESULTS: Omeprazole was not detected in tail hair. Metronidazole was detected in tail hair at a concentration of 0.57 ng/mg, trimethoprim and sulphadiazine at concentrations of 9.14 and 2.26 ng/mg, respectively, and procaine at a concentration of 1.66 ng/mg. CONCLUSIONS: The data presented suggest that hair analysis may become a useable technique for the retrospective detection of drug administration in horses. POTENTIAL RELEVANCE: This technique could ultimately be used as part of a prepurchase veterinary examination to identify misuse of anti-inflammatory and sedative drugs, in an in-training testing programme to identify use of anabolic agents, or to provide evidence to support post race blood or urine test results. Clearly, more extensive research will be required to evaluate the effectiveness of the technique over a much broader range of drugs.  相似文献
