Intra-abdominal pressure (IAP) was indirectly measured, using a transurethral catheterization technique, in 20 client-owned dogs before and after elective ovariohysterectomy. Mean preoperative IAP was 4.50 ± 0.44 cm H2O. Elective abdominal surgery caused significant elevations in mean postoperative IAP (mean 7.50 ± 0.45 cm H2O, range 0 to 15 cm H2O) that persisted for at least 24 hours. However, the increase in IAP caused no clinically evident complications; thus, after elective abdominal surgery an elevation in IAP up to 15 cm H2O is to be expected. Intra-abdominal pressure was also measured in 20 consecutive clinical cases with gross abdominal distension, before or after laparotomy, or both. Included in this group were dogs with gastric dilation and volvulus, closed pyometra, hemoperitoneum, acute ascites, and diaphragmatic hernias. All dogs with gross abdominal distension had an elevated IAP (> 16 cm H2O) either before or after surgery. Severe elevations of IAP were associated with anuria in two dogs, necessitating surgical decompression; one with hemoperitoneum (47 cm H2O) and one after repair of a chronic diaphragmatic hernia (30 cm H2O).  相似文献

Barotrauma, pneumothorax, and pneumomediastinum occurred in two anesthetized cats in which the waste gas outlet of a nonrebreathing circuit was occluded. To prevent any similar cases of barotrauma, we have modified our nonrebreathing circuit adapters by inserting a 15 cm H2O PEEP valve into the gas pathway of the nonrebreathing circuit adapter. This PEEP valve prevents the circuit and airway pressures from exceeding 15 cm H2O if the pop-off valve of the nonrebreathing circuit adapter is inadvertently left closed.  相似文献

Colonic luminal pressure (median, range) measured during ventral midline celiotomy in 69 horses with strangulating obstruction (SO) of the large colon (SO; ≥ 270° large colon volvulus; 27 cm H2O: 2 to 80 cm H2O) was greater ( P =.0023) than that measured in 37 horses with nonstrangulating obstruction (NSO) of the large colon (NSO; ≤ 180° volvulus or a nonstrangulating displacement; 18 cm H2O; 6 to 46 cm H2O). Sixty-five percent (45 of 69) of horses with SO and all horses with NSO survived. Survival analysis was restricted to 59 horses with large-colon SO that survived to hospital discharge or met the criteria specified for classification as nonsurvivors. Colonic luminal pressure in nonsurvivors (48.5 cm H2O; 34 to 80 cm H2O) was higher ( P =.0001) than that measured in survivors (18 cm H2O; 2 to 50 cm H2O) of SO. From response operating characteristic curves, a luminal pressure of 38 cm H2O optimized the distribution of horses with SO into survivor and nonsurvivor groups. Sensitivity, specificity, and positive and negative predictive values for a colonic luminal pressure greater than 38 cm H2O in predicting nonsurvival were 0.89, 0.91, 0.72 and 0.97, respectively. Measurement of colonic luminal pressure may be useful for predicting survival in horses with colonic volvulus.  相似文献

To assess the clinical applicability of pulse oximetry in the intensive care setting, a comparison was made of arterial hemoglobin saturation values determined by in vitro oximetry (SaO2) and pulse oximetry (SpO2) in 21 critically ill dogs. Single SaO2 measurements were compared to simultaneously obtained SpO2 readings. The correlation between these two methods was statistically significant (r = 0.8944, p = 0.0001). In addition, heart rates read by the pulse oximeter were compared to simultaneously obtained electrocardiograms (ECG). The correlation between these two methods was statistically significant (r = 0.9966, p = 0.0001). The pulse oximeter was easy to use, and recorded trends in oxygenation virtually instantaneously. Pulse oximetry appears to be an accurate and practical technique for the continuous non-invasive monitoring of oxygenation in critically ill dogs in the intensive care unit.  相似文献

The purpose of this study was to determine whether lingual vanous blood gas samples reflect arterial acid-base gas status in anethetized dogs. Heparinized blood samples were drawn simultaneously from the lingual vein and a peripheral artery in 50 anestheized dogs that were clinical surgical patients, as well as from four experimental dogs in which hemorrahaic shock was being studied. Blood pH, oxygen tension (PO2), and bicarbonate (HCO3-)) from the two sources in clinical patients showed significant liner correlation, although arterial PO2)(PaO2)) tended to be approximately 110mm Hg higher than lingual venous PO2). During hemorrahgic shock, however, PaO2) and PaCO2) were significantly different from lingual venous PO2) and PCO2), Lingula venous blood gas analysis may be useful in assessing acid-base and blood gas status in routline cases, but should not be relied upon in dogs with low cardiac output or poor perfusion.  相似文献

Ten, anesthetized dogs were instrumented with three pulse oximeter probes; two lingual transmittance probes and one rectal reflective probe. Arterial oxygen desaturation was produced by decreasing the inspired oxygen concentration. Hypotension was produced with an infusion of nitroprusside. Simultaneous pulse oximeter readings (SpO2) were compared to co-oximeter measured arterial saturation (SaO2) collected over a range of SaO2 (50–100%) and mean arterial pressures (40–100mmHg). Each of the monitors and means of evaluating SpO2 studied provided accurate SpO2 measurements over a range of mean arterial pressure from 40–100mmHg. All of the monitors tested tended to overestimate the SaO2 when the arterial saturation was less than 70%.  相似文献

The purpose of this study was to determine the frequency of hypoxemia and pulmonary mineralization using 99mTc-methylene diphosphonate (99mTc-MDP) in dogs with pituitary-dependent hyperadrenocorticism (PDH). Twenty-one dogs with PDH were pro-spectively evaluated using thoracic radiography, arterial blood gas analysis, and bone phase and pulmonary perfusion scintigraphy (using 99mTc-macro-aggregated albumin [99mTc-MAA]). The radiographs and bone and perfusion studies were evaluated subjectively. An averaged quantitative count density ratio was calculated between the thorax and cranial thoraco-lumbar vertebrae from lateral thoracic 99mTc-MDP images. Thoracic: vertebral ratios were calculated using 99mTc-MDP studies from 21 control dogs. The thoracic: vertebral ratios were compared between the 2 groups (PDH and control). The mean age (±SD) of the 21 PDH dogs was 10.2 (±3) years, whereas the mean age of the control group was 9.8 (±3) years. Seven of the 21 dogs with PDH were hypoxemic (denned as an arterial partial pressure of oxygen [PaO2] <80 mm Hg) with an average PaO2 (±SD) of 62 (±15) mm Hg. Of the 7 hypoxemic dogs, 2 were found to have pulmonary mineralization based on bone scintigraphic images. Pulmonary perfusion abnormalities were not identified using 99mTc-MAA in any of the 21 PDH dogs. Six PDH dogs had an abnormal interstitial pulmonary pattern and 5 of these dogs were hypoxemic. The average quantitative thoracic: vertebral ratio was not significantly different between the PDH and control dogs (0.5 ± 0.4 versus 0.4 ± 0.1, P = .16). Causes of hypoxemia other than pulmonary thromboembolism should be considered in dogs with PDH. Pulmonary mineralization may contribute to hypoxemia in dogs with PDH.  相似文献

We prospectively studied 26 dogs that presented for intercostal thoracotomy. Dogs were pre-medicated with oxymorphone, induced with diazepam and etomidate, and anesthesia was maintained with isoflurane in oxygen. Preoperatively, animal patients were randomly assigned to one of two groups. Group 1 (n = 13) received buprenorphine (10 μg/kg intravenously [IV]) every 6 hours for 24 hours starting 10 minutes before tracheal extubation. Group 2 (n = 13) received 0.5% bupivacaine (1.5 mg/kg) administered interpleural (IP) by slow injection through a pediatric feeding tube fixed to the most dorsal aspect of the thoracotomy incision. Interpleural injections were administered with each dog placed in lateral recumbency with the incision positioned ventrally; IP injections were administered every 4 hours for 24 hours starting 10 minutes before tracheal extubation. All cases were monitored in the intensive care unit for 24 hours postoper-atively. The analgesic efficacy of each regimen was evaluated using a pain scoring system that included a subjective pain score, heart rate, and respiratory rate. Arterial blood pressure, arterial blood gases, oxygen saturation, body temperature, and changes in the electrocardiogram or neurological status were also noted. Significant increases in mean heart rate, respiratory rate, and total pain score occurred after surgery in dogs in the buprenorphine group. In contrast, dogs in the bupivacaine group had no significant changes when compared with their preoperative values. Dogs in the bupivacaine group had significantly decreased total pain scores and better PaO2 and oxygen saturation values when compared with the dogs receiving buprenorphine. Hypoventilation did not occur in either group.  相似文献

Objective— To evaluate 4 methods of cholecystostomy catheter placement and to report on laparoscopic (Lap) cholecystostomy for the management of extrahepatic biliary obstruction (EHBO) in 3 dogs.
Study Design— Experimental study and clinical report.
Animals— Cadaveric dogs (n=20); 2 canine and 1 feline patient.
Methods— Pigtail cholecystostomy catheters were inserted in 20 canine cadavers using ultrasound (US) or Lap guidance. Insertion routes were either transperitoneal or transhepatic. Methods studied included Lap-transperitoneal, US-transperitoneal, US-transhepatic, and US-Seldinger (n=5 dogs/group). Insertion success, pleural penetration, and insertion site leakage (Lap-transperitoneal group) were evaluated. Three clinical EHBO cases were treated by Lap-transperitoneal technique.
Results— Insertion success was 100% by Lap-transperitoneal but 0% with US-transperitoneal and US-Seldinger methods. US-transhepatic yielded 3 of 5 successful placements. The pleura was penetrated in all US-transhepatic and US-Seldinger insertions. Leakage pressure for Lap-transperitoneal catheters averaged 75 cm H2O (±20 cm H2O). Lap-transperitoneal cholecystostomy resulted in marked improvement in 2 dogs, but the catheter became obstructed in the cat. One dog spontaneously regained common bile duct patency and the remaining 2 animals had successful cholecystoenterostomy.
Conclusions— In cadaver testing, the Lap-transperitoneal cholecystostomy method was superior based on high insertion success with no pleural penetration. In 2 clinical cases, Lap-transperitoneal placement successfully provided biliary drainage for patient stabilization.
Clinical Relevance— The role for temporary cholecystostomy has yet to be established, but may aid patient stabilization and mortality reduction in EHBO.  相似文献

The purpose of this study is to report the aterial blood gas findings in dogs with bacterial pneumonia. Arterial blood gas samples were collected from 62 dogs with culture-confirmed bacterial pneumonia. These results were compared with 46 normal dog arterial blood gas samples. Results demonstrated that respiratory acidosis was not a problem in dogs with pneumonia in this study. Significant evidence of hypoxemia was noted with abnormal mean values in PaO2 (P<0.001) and the Alveolar-arterial (A-α) gradient (P<0.001).  相似文献

The cardiopulmonary effects of the intravenous administration of clonidine (15 μg/kg), ST-91 (30 μg/kg) and diazepam (0.4 mg/kg) were compared in five healthy sheep using a randomized cross-over design, to determine whether the hypoxaemic effects of α2 adrenoceptor agonists are due to sedation, or to peripheral α2 adrenoceptor stimulation. All three drugs significantly lowered arterial oxygen tension (PaO2) levels within 2 min of their administration; however, clonidine and ST-91 produced long lasting and severe hypoxaemia with mean PaO2 levels of ≈40 mm Hg and 50 mm Hg (5.3 kPa and 6.6 kPa), respectively. The fall in PaO2 was considerably less with diazepam (63 mm Hg or 8.4 kPa at 2 min) and by 15 min the values did not differ from placebo treated animals. None of the drugs increased arterial carbon dioxide tension (PaCO2) levels when compared to saline treatment and the acid base variables did not show any significant change. A significant increase was recorded in the packed cell volume of the ST-91 treated group throughout the study. Within 2 min of their administration, all drugs caused a significant increase in mean arterial pressure (MAP) as compared to the placebo treated group. The MAP remained significantly increased for 5 and 60 min after clonidine and ST-91 treatment, respectively. The study shows that ST-91 and clonidine produce a greater degree of hypoxaemia than occurs with diazepam sedation, and that the hypoxaemic effect of α2 adrenoceptor agonists in sheep are mainly mediated by peripheral α2 adrenoceptors.  相似文献

Arterial blood samples were obtained from thirty normal conscious dogs breathing air. The mean values and standard deviations recorded were PaO2 101.3±5.6 mmHg, PaCO2 34.0±3.9 mmHg, oxygen saturation 93.8 ±1.2%, oxygen content 19.3 ± 1.8 ml/100 ml. Ten dogs with respiratory problems were also examined and of these animals seven had lower than normal oxygen tensions while three had carbon dioxide levels higher than those found in healthy dogs. It was concluded that, in severe respiratory disease, measurement of arterial oxygen tension gives a useful assessment of respiratory failure in dogs.  相似文献

Changes in cardiopulmonary function and platelet count were determined in 22 dogs of various breeds that underwent total hip replacement with cemented femoral prostheses. In 11 dogs (group I) polymethylmethacrylate (PMMA) was inserted without venting the reamed and lavaged femoral canal. In a second group of 11 dogs (group II) a urethral catheter (ID: approximately 2.7 mm) was placed into the medullary cavity before the insertion of PMMA. The application of PMMA resulted in a decrease in end-tidal carbon dioxide tension (PETco2) until 5 minutes after insertion of bone cement. Increases in arterial to end-tidal pCO2 gradient [P(a-ET)co2] and physiological dead space (VD/VT) were recorded between 2 minutes before and 5 minutes after insertion of PMMA in 12 dogs. A significant decrease in platelet count occurred in both groups of dogs. Decreases in arterial pO2 (Pao2), arterial/alveolar oxygen tension ratio (Pao2/PAo2), and percent O2 saturation of hemoglobin in arterial blood (Sao2) were not statistically significant. No significant differences could be detected between data obtained from both groups of dogs. An increase in femoral intramedullary pressure caused by the insertion of PMMA and subsequent pulmonary microembolism by medullary contents has been considered the most likely cause for changes in pulmonary function. The lack of statistically significant differences in cardiopulmonary variables and platelet count between the two groups of dogs could have been related to inefficient pressure reduction by the method used.  相似文献

The question addressed was whether the influence of dietary copper concentration on selenium metabolism depends on the amount of selenium in the diet. Weanling, male rats were fed purified diets containing either 1 (low), 4 (normal) or 42 (high) mg Cu/kg diet and either 0.03 (low), 0.05 (normal) or 1.0 (high) mg Se/kg diet in a 32 factorial design. Extra copper was added to the diets in the form of CuSO4 · 5H2O and selenium as Na2SeO3 · 5H2O. In rats fed either the low or normal amounts of selenium, higher intakes of copper decreased the apparent intestinal selenium absorption and increased urinary selenium excretion. The effects of copper on selenium absorption, excretion and retention were not seen in rats fed the high-selenium diets. An increase in dietary copper concentrations elevated selenium concentrations in the liver and kidneys, but slightly lowered those in the spleen of rats that were fed the diets with the normal level of selenium. In rats that were fed the diets with either low or high selenium concentration, copper intake had no effect on organ selenium concentrations. Glutathione peroxidase activity in erythrocytes was raised by feeding the diets which contained either normal or high copper content instead of those that were low in copper. It is concluded that the amount of selenium in the diet determines whether or not an increase in dietary copper concentration affects selenium metabolism.  相似文献

Increasingly severe degrees of pneumothorax were produced in 6 adult anesthetized bloodhounds. Computed tomography (CT) of the thorax was performed on each dog to evaluate the effects of pneumo thorax on thoracic and on pulmonary cross-sectional area (TA and PA). Arterial PO2 (PaO2) and PCO2 (PaCO2), heart rate (HR), and mean arterial blood pressure (MAP) were determined and related to the severity of pneumothorax. Volumes of air equal to 1, 1.5 and 2 times functional residual capacity of the lung produced approximately 33%, 40%, and 50% reductions in pulmonary area respectively. These amounts of atelectasis correspond to a radiographically "moderate" degree of pneumothorax. As severity of pneumothorax increased, thoracic area consistently increased, PaO2 consistently decreased, and PaCO2 consistently increased, with all being statistically significant relationships (p<0.0001); but HR and MAP were variable and showed no statistical correlation to the degree of pneumothorax (p>0.2).  相似文献

Serum concentrations of thyroxine (T4), 3,5,3'-triiodothyronine (T3), free thyroxine (fT4), and free 3,5,3'-triiodothyronine (fT3) were compared between tumor-bearing dogs (with and without chronic weight loss) and non—tumor-bearing dogs (with and without chronic weight loss) (n = 83). Serum T4, T3, and fT3 concentrations were lower ( P < .05) in dogs with weight loss, whether or not they were tumor-bearing, than in dogs without weight loss. Serum fT4 concentrations did not vary among the groups. Serum albumin concentrations were lower ( P < .05) in cachectic dogs than in dogs not experiencing weight loss, regardless of their tumor-bearing status. Percentage of weight loss was found to be associated ( P < .05) with T4, T3, and fT3 concentrations. It appears that the low thyroid hormone concentrations are related to either an abnormal nutritional state or to the severity of illness, rather than to a tumor-related phenomenon.  相似文献

The leukotrienes (LT) LTD4 and LTB4 have been shown to cause bronchoconstriction and neutrophil accumulation, respectively, in horse lungs. Such changes are characteristic of the equine allergic respiratory disease, chronic obstructive pulmonary disease (COPD). To further investigate the role of these putative mediators in the pathogenesis of equine COPD the effect of a 5-lipoxygenase inhibitor, fenleuton, on antigen-induced changes in horses with this condition has been examined. Six horses with COPD underwent a series of four antigen challenges, one month apart, with placebo pre-treatment on three occasions and fenleuton (4 days oral dosing 5 mg/kg) pre-treatment on one occasion. Three horses received fenleuton prior to the second challenge and three horses received the drug prior to the fourth antigen challenge. Changes in radiolabelled neutrophil distribution, lung function and peripheral leucocyte counts were monitored on each occasion for 7 h following the start of antigen challenge. Antigen challenge caused an increase in radioactive counts over the lungs and a decrease in peripheral leucocyte count. Neither response was affected by fenleuton pre-treatment. Mean maximal changes in pleural pressure (ΔPplmax) and respiratory rate were also unaffected by fenleuton pre-treatment. However, in the two horses which responded to antigen-challenge with a particularly marked increase in ΔPplmax (>15 cm H2O), prior administration of fenleuton reduced the response by 64 and 63%. These results suggest that 5-lipoxygenase inhibitors warrant further investigation as bronchodilators in equine COPD.  相似文献

Cardiopulmonary effects of laparoscopic surgery were investigated in five crossbred dogs (21 ± 1.9 kg). Premedicated dogs were anesthetized with thiopental and maintained with halothane at 1.5 times minimum alveolar concentration in oxygen. Controlled ventilation maintained partial pressure of end-tidal co2 at 40 ± 2 mm Hg. Vecuronium was used for skeletal muscle relaxation. After instrumentation and stabilization, baseline measurements were made of cardiac output (thermodilution technique), mean systemic, mean pulmonary arterial and pulmonary wedge pressures, heart rate, saphenous vein and central venous pressures, and minute ventilation. Baseline arterial and mixed venous blood samples were drawn for analysis of pH, Pao2, Paco2, Pvo2, Pvco2, and bicarbonate concentrations. Systemic and pulmonary vascular resistances, oxygen delivery and consumption, shunt fraction, and dead space ventilation were calculated using standard formulas. Abdominal insufflation using co2 to a pressure of 15 mm Hg for 180 minutes resulted in significant ( P <.05) increases in heart rate (15 to 180 minutes), minute ventilation (75 to 135 minutes), and saphenous vein pressure (15 to 180 minutes), and decreases in pH (60 to 180 minutes) and Pao2 (60 to 180 minutes). For 30 minutes after desufflation, there was a significant decrease in Pao2, and increases in cardiac output, o2 delivery, and heart rate, compared with baseline. There was a significant increase in shunt fraction and decrease in pH at 15 minutes after desufflation only. The changes were within physiologically acceptable limits in these healthy, ventilated dogs.  相似文献

Cerebrospinal fluid pressure (CSFP) was measured as part of the neurologic assessment of dogs with suspected intracranial disease. Because propofol has not been shown to cause an increase in intracranial pressure in humans, the authors examined its effect on (CSFP) in dogs to determine if it would be an appropriate substitute for thiopental as an anesthetic agent for the measurement of CSFP. The CSF pressure in eucapnic propofol-anesthetized dogs (105 ± 5.6 mm HPO) was not significantly different (p < .05) from CSFP in eucapnic thiopental-anesthetized dogs (103.8 ± 6.6 mm HPO).  相似文献

Objective— To evaluate stance phase limb use after cementless (BFX®) total hip replacement (THR) in dogs and to relate postoperative radiographic variables to static bodyweight distribution after surgery.
Study Design— Prospective clinical study.
Animals— Dogs (n=35) that had THR.
Methods— THR was performed using the BFX® THR technique. Postoperative pain management regimens were similar for all dogs. Standing bodyweight distribution (%BWdistrib) was measured using a pressure sensitive walkway and radiographs made before surgery and at 3, 6, and 12 months after surgery. Repeated measures models (with backwards-stepping to obtain the final model) were used for statistical analysis.
Results— Temporally, %BWdistrib to the operated limb increased ( P <.0001; normal by 3 months) and decreased to the unoperated limb ( P =.0001) and also increased to the pelvic limbs and decreased to the thoracic limbs. %BWdistrib to the unoperated limb was significantly less than the operated limb at 3, 6, and 12 months after surgery. Postoperative canal fill and femur flare were significantly negatively correlated with change in %BWdistrib (estimate=−0.24, P =.0413).
Conclusion— BFX® THR results in normal %BWdistrib to the operated limb by 3 months after surgery. A greater fill of the proximal femur may be associated with a suboptimal outcome.
Clinical Relevance— BFX® THR normalizes standing bodyweight distribution dogs with hip osteoarthritis. Objective evaluation of THR outcome and radiographic features may reveal factors that could be improved through changes in prosthesis design or surgical technique.  相似文献
