Objective
The goal of this study was to investigate the short-term safety and diuretic efficacy of furosemide constant rate infusion (CRI) diluted with 5% dextrose in water (D5W) compared to dilution with 2.4% hypertonic saline in healthy dogs.Animals
Six healthy dogs.Methods
Dogs were studied in a randomized, blinded, crossover manner. Furosemide 3.3mg/kg was diluted to 2.2mg/mL with either 1.5mL/kg D5W for the DEX method or with 1.0mL/kg D5W and 0.5mL/kg of 7.2% hypertonic saline for the H-SAL method. After a 0.66mg/kg furosemide IV bolus, the infusion rate was 0.3 mL/kg/hr for 5 h such that both methods delivered 0.66 mg/kg/hr (total 3.3mg/kg) furosemide in equal volume for the study duration. Urine output, water intake, central venous pressure (CVP), physical parameters, furosemide concentrations, blood and urine electrolytes, and urine aldosterone to creatinine ratio (UAldo:C) were evaluated.Results
Measured variables were not different between methods but showed changes over time consistent with diuresis. Mean CVP decreased over time similarly for both methods. Plasma furosemide and urine concentrations were stable and not different between methods. Both furosemide CRI methods showed an increase in the UAldo:C, however, the rise was greater for DEX than for H-SAL.Conclusions
Diuresis was similar for both furosemide CRI methods; however, the H-SAL method induced less renin-angiotensin-aldosterone system activation than the DEX method. The absence of intravascular volume expansion based on CVP suggests that dilution of a furosemide CRI with 2.4% hypertonic saline may be well tolerated in heart failure. 相似文献2.
Introduction
Aldosterone breakthrough (ABT) is the condition in which angiotensin converting enzyme inhibitors (ACEIs) and/or angiotensin receptor blockers fail to effectively suppress the activity of the renin angiotensin aldosterone system. The objective of this study was to determine if ABT occurs in dogs with naturally occurring myxomatous mitral valve disease receiving an ACEI, using the urine aldosterone to creatinine ratio (UAldo:C) as a measure of renin angiotensin aldosterone system activation.Animals, Materials and Methods
This study includes 39 dogs with myxomatous mitral valve disease. A UAldo:C cut-off definition (derived from a normal population of healthy, adult, and client-owned dogs) was used to determine the prevalence of ABT in this population. Spearman analysis and univariate logistic regression were used to evaluate the relationship between UAldo:C and ABT (yes/no) and eight variables (age, serum K+ concentration, serum creatinine concentration, ACEI therapy duration and ACEI dosage, furosemide therapy duration and furosemide dosage, and urine sample storage time). Finally, the UAldo:C in dogs receiving spironolactone, as part congestive heart failure (CHF) therapy, was compared to dogs with CHF that were not receiving spironolactone.Results
The prevalence of ABT was 32% in dogs with CHF and 30% in dogs without CHF. There was no relationship between either the UAldo:C or the likelihood of ABT and the eight variables. Therapy with spironolactone lead to a significant elevation of the UAldo:C.Discussion
Using the UAldo:C and a relatively stringent definition of ABT, it appears that incomplete RAAS blockade is common in dogs with MMVD receiving an ACEI. The prevalence of ABT in this canine population mirrors that reported in humans. While the mechanism of ABT is likely multifactorial and still poorly understood, the proven existence of ABT in dogs offers the potential to improve the prognosis for MMVD with the addition of a mineralocorticoid receptor blocker to current therapeutic regimens.Conclusions
Approximately 30% of dogs being treated for heart disease and CHF satisfied the definition of ABT. Identifying patient subpopulations experiencing ABT may help guide future study design and clinical decision-making. 相似文献3.

Transthoracic echocardiography (TTE) is the primary tool for the assessment of cardiac structure and function in dogs but is challenging in English bulldogs due to dorsoventral compression of the thorax, obesity, and narrow intercostal spaces. Prospective clinical trial with paired analysis of TTE and CTA studies.Results
Eight of the 25 linear cardiac dimensional measurements were significantly different between TTE and CTA (p<0.033). Intraobserver agreement was strong with average coefficients of variation (CV) of 5.34% for TTE and 2.50% for CTA. Interobserver agreement CV averaged 6.5% for TTE and 8.75% CTA. Ejection fraction, stroke volume, and end-systolic volume were significantly different between modalities (all p<0.002). No significant difference was present between end-diastolic volume for TTE compared with CTA.Discussion
High-quality cardiac angiographic studies were accomplished using CTA without the use of general anesthesia in English bulldogs. Multi-detector computed tomography angiography and TTE are not interchangeable modalities in the clinical setting.Conclusion
Multi-detector-CT ECG-gated cardiac angiography is possible in sedated, non-intubated English bulldogs. Differences were found between some cardiac dimensions as measured by TTE in the awake dog and compared with sedated CTA, indicating the two methodologies are not equivalent. Objective
To evaluate the ability of a noninvasive cardiac output monitoring system with electrical velocimetry (EV) for predicting fluid responsiveness in dogs undergoing cardiac surgery.Study design

A total of 30 adult Beagle dogs.Methods
Stroke volume (SV), stroke volume variation (SVV) and cardiac index were measured using the EV device in sevoflurane-anaesthetized, mechanically ventilated dogs undergoing thoracotomies for experimental creation of right ventricular failure. The dogs were considered fluid responsive if stroke volume (SVI; indexed to body weight), measured using pulmonary artery thermodilution, increased by 10% or more after volume loading (10 mL kg–1). Relationships of SVV, central venous pressure (CVP) and pulmonary artery occlusion pressure (PAOP) with SVI were analysed to estimate fluid responsiveness.Results
Better prediction of fluid responsiveness, with a significant area under the receiver operating characteristic curve, was observed for SVV (0.85 ± 0.07; p = 0.0016) in comparison with CVP (0.65 ± 0.11; p = 0.17) or PAOP (0.60 ± 0.12; p = 0.35), with a cut-off value of 13.5% (84% specificity and 73% sensitivity).Conclusions and clinical relevance
SVV derived from EV is useful for identification of dogs that are likely to respond to fluids, providing valuable information on volume status under cardiothoracic anaesthesia. 相似文献5.

To describe a transorbital approach to the maxillary nerve block in dogs and compare it with a traditional approach.Study design

A volume of 1 mL of methylene blue dye was injected by each of two techniques, a traditional percutaneous approach and a transorbital approach to the maxillary nerve block. Both techniques were used on each head, alternating the left and right sides after random assignment to the first head. The heads were dissected to reveal the maxillary nerve and the length of nerve stained was measured.Results
There was no significant difference (p = 0.67) in the proportion of nerves stained for a length >6 mm by either technique (88.2% transorbital versus 82.3% percutaneous). The mean length of nerve stained did not differ significantly between the techniques (p = 0.26).Conclusions and clinical relevance
The transorbital approach to the maxillary nerve block described here presents a viable alternative to the traditional percutaneous approach. Further study is required to confirm its efficacy and safety under clinical conditions. 相似文献6.

Objective
To describe ultrasound-visualized anatomy and the spread characteristics of a dye injected in the thoracic paravertebral (TPV) space under ultrasound guidance.Study design

Seven dog cadavers.Methods
One cadaver was used to observe, identify, and describe the relevant TPV anatomy. In the remaining six, the left fifth TPV space was randomly assigned to be injected with either a low volume (LV; 0.05 mL kg?1) or high volume (HV; 0.15 mL kg?1) of dye. Subsequently, the contralateral side was injected with the alternative volume. Anatomic dissections were conducted to determine the incidence of complete spinal nerve staining (>1 cm circumferential coverage), number of contiguous spinal nerves dyed and the absence or presence of solution in particular locations.Results
The ultrasound-visualized anatomy of the TPV space was defined as the intercostal space abaxial to the vertebral body, delimited by the parietal pleura ventrally and the internal intercostal membrane dorsally. The endothoracic fascia divides the paravertebral space into dorsal and ventral compartments. The target nerve was completely dyed in five of six and six of six injections in the LV and HV conditions, respectively. In one LV injection, the nerve was partially dyed. No multisegmental spread affecting contiguous spinal nerves was found in either treatment. Multisegmental spread was found in the ventral compartment of the TPV space, affecting the sympathetic trunk on 3 (0–3) and 3.5 (1–6) vertebral spinal levels in the LV and HV conditions, respectively, but differences between volumes were not significant. No intrapleural, ventral mediastinal or epidural migration was observed.Conclusions and clinical relevance
Ultrasound-guided TPV block is a potentially reliable technique. The LV appeared sufficient to dye a single spinal nerve and multiple sympathetic trunk vertebral levels. Multiple TPV injections may be needed to provide adequate thoracic analgesia in dogs undergoing thoracic surgery. 相似文献7.

One hundred and thirty-seven healthy adult dogs; 115 dogs with myxomatous mitral valve disease (MMVD) but no PH; 91 dogs with PH.Methods
A combined prospective and retrospective study. Full echocardiographic evaluations were performed on all dogs; TAPSE was indexed to Ao to produce a unitless TAPSE:Ao. Reference intervals for TAPSE:Ao were generated, and TAPSE:Ao was regressed on tricuspid regurgitant jet velocity in dogs with PH and on LA:Ao in dogs with MMVD without PH. Diagnostic test analysis was used to examine the ability of TAPSE:Ao to identify severe PH. An adjusted TAPSE:Ao (TAPSE:Ao(adj)) was derived to account for MMVD in dogs with PH.Results
The ratio, TAPSE:Ao, removed the effect of bodyweight from TAPSE measurements. Healthy dogs had TAPSE:Ao > 0.65. The ratio, TAPSE:Ao, showed a linear negative relationship with tricuspid regurgitation velocity and positive relationship with LA:Ao. The adjusted ratio, TAPSE:Ao(adj), increased the sensitivity of diagnosis of PH in dogs with moderate-severe MMVD without affecting the diagnosis of PH in dogs with PH and with no or mild MMVD.Conclusions
The ratios, TAPSE:Ao and TAPSE:Ao(adj), are a bodyweight-independent means of assessing right ventricular systolic function in dogs and for identifying severe PH in dogs with or without MMVD. 相似文献8.

To evaluate the onset and duration of hematological changes and the use of Doppler ultrasound (spleen) in dogs sedated with acepromazine or xylazine.Study design

A total of 24 mixed breed dogs aged 1–4 years and weighing 15–25 kg.Methods
Dogs were randomly distributed into two groups: acepromazine group (AG) which were administered acepromazine (0.05 mg kg?1) intramuscularly and xylazine group (XG) administered xylazine (0.5 mg kg?1) intramuscularly. Sonographic evaluations (morphologic and hemodynamic splenic vascularization) and hematologic tests were performed before drug administration (baseline) and 5, 15, 30, 60, 120, 240, 360, 480 and 720 minutes after drug administration.Results
A significant reduction occurred in erythrogram variables in AG at 15–720 minutes corresponding with a significant enlargement of the spleen. In XG, a significant reduction was observed in the erythrogram variables at 30–60 minutes without a significant enlargement of the spleen. Hilar diameter did not change over time in either group. Flow alterations were found only in the splenic artery in AG, with a decreased final diastolic velocity observed at 60–120 minutes.Conclusions
Administration of acepromazine resulted in decreased red blood cell count, hemoglobin, packed cell volume and an increased diameter of the spleen. Xylazine administration resulted in similar hematologic changes but of smaller magnitude and duration and without splenic changes. The absence of significant changes in the Doppler flow parameters of the splenic artery and vein and the hilar diameter suggests that the splenomegaly that was observed in AG was not due to splenic vasodilation. No splenic sequestration occurred after xylazine administration.Clinical relevance
The results indicate that acepromazine decreases the erythrocyte concentrations by splenic erythrocyte sequestration and concomitant splenomegaly. Xylazine can cause slight hematologic changes, but without splenic changes. 相似文献10.

Objective
To evaluate the cardiorespiratory effects of a 7° reverse Trendelenburg position (RTP) in anaesthetized horses.Study design

A total of 125 horses undergoing elective surgery in dorsal recumbency.Methods
Horses were allocated to one of three weight classes and assigned to be positioned either on a horizontal table or on a table in 7° RTP, according to a randomized block design. In all horses, anaesthesia was maintained with isoflurane in oxygen and a constant rate infusion of romifidine. All horses were mechanically ventilated throughout anaesthesia, and routine cardiovascular monitoring and arterial blood gas analysis were performed at 15-minute intervals and relevant variables calculated. Data from the first 60 minutes of anaesthesia were compared between both positions using a mixed model analysis of variance.Results
A significant interaction was found between position and weight class for the alveolar to arterial oxygen tension gradient and F-shunt: these variables were lower in RTP than in horizontal position in the two lowest weight classes and vice versa in the highest weight class. Arterial oxygen tension and oxygenation indices were significantly worse in the horses in the higher weight classes.Conclusions and clinical relevance
A 7° RTP did not result in clinically relevant changes in gas exchange or cardiovascular function. Horses with a higher body weight are at increased risk for hypoxaemia during anaesthesia in dorsal recumbency. 相似文献11.

Objectives
The objective is to describe the clinical, radiographic, echocardiographic and angiographic findings in dogs with systemic-to-pulmonary arteriovenous fistula (SPAVF).Animals
Thirteen medical records of client-owned dogs with a diagnosis of SPAVF were reviewed/analysed.Methods
This is a retrospective study of case records. Thoracic radiography, transthoracic echocardiography (TTE), transesophageal echocardiography (TEE), three-dimensional TEE, intracardiac echocardiography, fluoroscopy-guided or computed tomography (CT) angiography were carried out.Results
Based on the TTE, SPAVF was identified in seven of the included dogs. In eight cases, TEE and angiography were both performed and confirmed the diagnosis. Computed tomography angiography was performed in three dogs. A case was diagnosed by TEE alone, another one by three-dimensional TEE and the latter by intracardiac echocardiography.Conclusions
Transthoracic echocardiography identified seven cases of SPAVF, while definitive diagnosis in the remaining dogs required selective angiography or computed tomography angiography. 相似文献12.
Introduction
The time from the onset of the P wave on electrocardiogram to the peak of late diastolic wave signal (PA-TDI interval) recorded by left atrial pulsed-wave tissue Doppler imaging (PW-TDI) is a surrogate of the total atrial conduction time, and it can predict the development of new-onset atrial fibrillation (AF) in people. This study investigated whether PA-TDI interval measured with PW-TDI at the level of lateral aspect of the mitral valve annulus could identify dogs which developed AF within 6 months after echocardiography.Animals
Forty-two dogs with different cardiac diseases were included; 21 dogs developed AF within 6 months after echocardiography (AF group) and 21 dogs did not (non-AF group). Each AF case was matched with a non-AF case for body weight and left atrium:aortic root ratio.Methods
This was a retrospective study. Review of signalment, underlying disease and echocardiography data were included. PA-TDI interval was measured offline from acquired PW-TDI images. PA-TDI interval and standard echocardiographic variables were compared between groups. Receiver operator characteristic curves were used to identify the best AF predictor. Univariate and multivariate regression were used to evaluate predictors of PA-TDI interval.Results
The AF group had significantly greater 2D left atrial maximal diameter, left-ventricular (LV) end-diastolic volume, M-Mode LV internal diameter and LV end-systolic volume index. PA-TDI was significantly longer in the AF group, and it was superior to other echocardiographic variables in predicting AF development within 6 months (AUC = 0.896).Conclusions

Objective
To investigate the dose-dependent effects of isoflurane and dobutamine on haemodynamics in dogs with experimentally induced mitral valve insufficiency (MI).Study design

Six healthy Beagle dogs.Methods
Dogs with surgically induced MI were anaesthetized once. First, anaesthesia was maintained at an end-tidal isoflurane concentration (Fe′Iso) 1.0% (ISO1.0) for 20 minutes. Then, dobutamine was infused successively at 2, 4, 8 and 12 μg kg?1 minute?1 (DOB2–12) for 10 minutes at each dose rate. Measurements were recorded at each stage. Dobutamine was discontinued and Fe′Iso was increased to 1.5% (ISO1.5) for 20 minutes. Dobutamine was administered similarly to ISO1.0, and cardiovascular variables were recorded. The same sequence was repeated for Fe′Iso 2.0% (ISO2.0). Aortic pressure (AoP) and left atrial pressure (LAP) were recorded by radiotelemetry. The combination method of the pressure–volume loop analysis and transoesophageal echocardiography was used to measure cardiovascular variables: end-systolic elastance (Ees), effective arterial elastance (Ea), Ea/Ees, forward stroke volume (FSV), heart rate (HR), and cardiac output (CO).Results
High isoflurane concentration resulted in reduced Ees and increased Ea/Ees, which indicated low arterial pressure. High-dose dobutamine administration resulted in increased Ees and FSV at all isoflurane concentrations. In ISO1.5 and ISO2.0, HR was lower at DOB4 than baseline (BL) but increased at DOB12 compared with DOB4. CO increased at ≥ DOB8 compared with BL. In ISO1.5 and ISO2.0, systolic and mean AoP increased at ≥ DOB4 and ≥ DOB8, respectively. LAP did not change under all conditions.Conclusions and clinical relevance
The dose-dependent hypotensive effect of isoflurane in MI dogs was mainly derived from the decrease in contractility. Dobutamine increased AoP without increasing LAP by increasing the contractility attenuated by isoflurane. Our findings may improve the cardiovascular management of dogs with MI undergoing general anaesthesia with isoflurane. 相似文献14.

Sound and lame chickens were gait scored and randomly assigned into four groups: sound-drug, sound-placebo, lame-drug, and lame-placebo in study 1. In study 2, an additional lame and sound handling control group was included. Chickens in drug groups were injected with either morphine or butorphanol 2 mg kg?1 intravenously. Chickens in placebo groups were injected with an equal volume of normal saline. All birds underwent an obstacle course (OC) and latency-to-lie (LTL) test before injection and at 30 minutes and 2 hours after injection, to assess their walking ability and their standing ability. The time taken to finish the OC and the standing time in the LTL test were recorded. Friedman tests with Dunn’s correction were used to identify significant differences.Results
Lame chickens finished the OC faster (mean ± standard deviation 36 ± 8 c.f. 69 ± 18 seconds) after the injection of butorphanol. Morphine caused sedation with an increase in time taken to finish the OC, even in sound chickens. In the lame handling control and placebo groups the OC times increased and the LTL times decreased with each observation.Conclusion
Intravenous butorphanol (2 mg kg?1) may be analgesic in chickens for up to 2 hours. Morphine caused sedation. 相似文献15.

Eighteen cadaver heads and nine adult live donkeys.Methods
Phase 1: the anatomical characteristics of the maxillary nerve and its related structures were investigated within the pterygopalatine fossa in five cadavers. Phase 2: 0.1 mL of methylene blue dye was injected blindly and via ultrasound guidance in 13 cadavers to stain the left and right maxillary nerves, respectively. Nerve staining and dye spreading were evaluated through cadaver dissection. Phase 3: the former procedures were applied in nine live donkeys using lidocaine hydrochloride 2% and the onset of analgesia was verified through needle pricking at the naris.Results
Ultrasound-guided deposition of methylene blue dye in cadavers and lidocaine injection in live animals were successful in all instances (accuracy = 100%) without inadvertent vascular penetration. Using the ‘blind’ technique, misdirection and intravascular deposition of dye were reported in four cadavers (accuracy = 69.2%) and neurovascular trauma was observed in live donkeys (five cases). Loss of cutaneous sensation in the ipsilateral naris was earlier in the ultrasound-guided approach (10.9 ± 1.8 minutes) than in the ‘blind’ technique (27.8 ± 3.2 minutes; p < 0.001).Conclusions and clinical relevance
An ultrasound-guided maxillary nerve blockade proved very practical and can be used to block the maxillary nerve with a high degree of accuracy while avoiding vascular penetration. Further studies are mandatory to validate its analgesic effectiveness in clinical situations. 相似文献16.
Introduction
To report normal echocardiographic variables from a population of healthy Salukis in North America.Animals
The study included 83 healthy adult Salukis from North America with structurally normal hearts.Methods
All animals underwent a full physical examination and two-dimensional, M-mode, and Doppler echocardiography using the right parasternal and left apical views with the left ventricular volumes calculated using the Simpson's method of discs. Echocardiographic variables were compared among sex, body surface area, and body weight (BW) using linear regression. The 95% predictive intervals were calculated for both unadjusted and BW-adjusted data.Results
No relationship between sex and the echocardiographic variables was noted. Predictive intervals for echocardiographic variables are presented for 22 echocardiographic variables. Linear regression suggested that 16 of those variables were associated with BW. The 95% predictive intervals of echocardiographic variables adjusted for BW are reported.Conclusions

One hundred one dogs with thoracic radiographs and echocardiography performed on the same day at a veterinary teaching hospital were retrospectively reviewed.Methods
Thoracic radiographs were reviewed by two cardiologists, two radiologists, and two small animal rotating interns. Radiographs were evaluated for the subjective presence and severity of LAE and for seven Roentgen signs. Echocardiographic LA size was evaluated objectively by the left atrial-to-aortic root ratio and LA volume indexed to body weight. Interreader agreement of radiographic LAE evaluation and agreement between radiographic LAE assessment and echocardiographic LAE were assessed by linearly weighted kappa and intraclass correlation coefficient.Results
Interreader agreement for the presence and degree of radiographic LAE ranged from moderate to substantial. The agreement between subjective radiographic LAE and echocardiographic LAE was moderate. Single Roentgen signs had poor to fair correlation with echocardiographic LAE.Conclusions
The present study found that interreader agreement for radiographic classification of LAE was substantial among specialists and moderate for all readers. Subjective, global assessment of radiographic LAE is better than using any single Roentgen sign. 相似文献18.

Objective
To compare the effects of epidural injection of three volumes of lidocaine injected at the third (T3) or eleventh thoracic vertebra (T11) in conscious dogs to induce thoracic epidural anesthesia (TEA) and to measure the epidural dispersion of iohexol under similar conditions.Study design
Prospective crossover experiment.Animals
A group of five Beagle dogs weighing 10.4 ± 0.5 kg (mean ± standard deviation).Methods
Each dog was anesthetized twice, separated by 1 week, for inserting an epidural catheter at the lumbosacral space and advancing the tip to T3 (treatment TEAT3) or T11 (treatment TEAT11). For each treatment, three volumes of 2% lidocaine (0.05, 0.10 and 0.20 mL kg–1) were administered at 24 hour intervals, and sensory blockade (SB) of dermatomes was estimated by pinching the skin with mosquito forceps. Under identical conditions of injection volume and site, iohexol was administered 3 hours after lidocaine injection to identify epidural distribution (ED) using computed tomography. The effects of injection site and volume on SB of thoracic dermatomes and ED were analyzed using a linear mixed model (p < 0.05).Results
Thoracic SB and ED significantly increased as the volume increased (p < 0.001 and p < 0.001, respectively), and significantly decreased in TEAT3 than in TEAT11 (p = 0.011 and p = 0.002, respectively). Cervical SB was obtained in three of five dogs in TEAT3 and two of five dogs in TEAT11 injected with 0.20 mL kg–1. One dog showed temporary inspiratory stridor probably caused by bilateral laryngeal paralysis, but no hypoxia.Conclusions and clinical relevance
TEA induced at T3 produced less thoracic SB than did TEA at T11 with the same volumes of lidocaine. The cervical SB obtained with the highest volume of lidocaine may increase the risk of laryngeal paralysis and pulmonary aspiration. 相似文献20.
