Forty limbs with femoropatellar osteochondritis dissecans in 24 horses were treated with arthroscopic surgery. Lesions were bilateral in 16 horses and unilateral in eight horses. Diagnostic examination and surgical treatment were performed through a single arthroscopic portal; five different instrument portal locations and six instrument approaches were used. Lesions were localized to the lateral trochlear ridge of the femur in 31 affected joints, medial trochlear ridge in two joints, lateral and medial trochlear ridges together in two joints, lateral trochlear ridge plus patella in four joints, and patella alone in one joint. The lesions consisted of subchondral defects containing chondral or osteochondral flaps or fragments, or were seen as dimpling, cracking, fibrillation, or erosion of articular cartilage, or intact cartilage over a subchondral defect. Loose bodies were found in three joints. There was a poor correlation between radiologic and arthroscopic findings. Surgical manipulations included removal of flaps, fragments, and undermined articular cartilage, and debridement of the subchondral defect. Three horses were euthanized: one electively to assess the joint grossly, one because of complications following surgery and salmonellosis, and one because of unrelated forelimb abnormalities. Immediate clinical improvement after surgery was seen in the 22 horses permitted to survive. Long-term follow-up on seven of 10 racehorses revealed that two have raced successfully, two are "ready to race," three are training sound, two are sound at pasture (still in convalescence), and one has been reoperated. Of six horses used for show or pleasure, three are being shown sound, one is sound for pleasure, and two are training sound. The remaining horses are convalescing. 相似文献

Ureteroliths were removed on two occasions from a mare by different techniques. A 3 cm calculus was removed by right ureterolithectomy during a ventral midline celiotomy. Blood urea nitrogen (BUN) and serum creatinine were measured monthly and remained within normal limits for 5 months. During the fifth postoperative month, after 9 days of lumbar pain, the mare was represented with an increased BUN and creatinine and a ureterolith in the left ureter. Using a Dormia basket stone dislodger, the second ureterolith was removed through a vestibulourethral approach. Fourteen days after surgery, the BUN and creatinine had returned to normal limits and the mare remained asymptomatic for 3 months. The mare then developed severe laminitis after 2 days of strenuous exercise and was euthanized. At necropsy, both kidneys contained multiple calculi. 相似文献

A surgical approach to the ramus of the mandible was developed. Subperiosteal elevation of the masseter muscle from the ramus allowed reduction and plate fixation of ramus fractures in a bull and a horse. Results of follow-up examinations, 2 years after surgery, showed excellent functional and cosmetic results. 相似文献

Arthrodesis of the proximal interphalangeal joint was used to treat lameness resulting from osteoarthrosis and for closed subluxation in 35 horses. Six horses had bilateral arthrodeses. Several conventional internal fixation techniques and a three converging screw method were used. Criteria for success included the horse performing its previous or intended athletic activity and the owner being satisfied with the outcome. Using these criteria, a successful outcome was obtained in 65% of the 26 animals in which adequate follow-up was available. A successful outcome was observed in four of six horses with bilateral arthrodeses. The internal fixation technique used did not influence the success rate, and the duration of postoperative casting was similar in successful and unsuccessful cases. The success rate of proximal interphalangeal arthrodeses was 46% in the fore limbs and 83% in the hind limbs. Complications included infection, cast ulcers, bone or implant failure, implant loosening, and laminitis. 相似文献

A 5-year-old Thoroughbred gelding with recurrent cecal impaction refractory to medical management was treated with a side-to-side jejunocolic anastomosis. Cecal impaction did not recur after surgery. The horse gained weight and performed successfully for 14 months, but experienced three episodes of mild abdominal pain between 14 and 20 months after surgery. Mild cecal gas distention and firm ingesta in the colon were detected on rectal examination. The horse's feces remained soft after surgery, except during the colic episodes when dry, firm feces were passed. 相似文献

A 6 month old domestic shorthaired cat died of suspected malignant hyperthermia. Anesthesia was induced and maintained with halothane vaporized in oxygen and nitrous oxide, after acepromazine premedication. Before an incision was made, the cat's heart rate dropped from 140 to 90 beats/min concomitant with a drop in blood pressure. Glycopyrrolate administration resulted in severe ventricular tachycardia (340 beats/min). Halothane and nitrous oxide were discontinued and the surgery was abandoned. Lidocaine administration resulted in a normal sinus rhythm. In recovery, the cat was tachypneic and struggling, with a rectal temperature of 40.1°C that quickly increased to 41.4°C. While the cat was being cooled, cardiac dysrhythmias progressed to ventricular fibrillation that was not responsive to cardiorespiratory resuscitation. Blood specimens obtained while the cat was being cooled showed hyperkalemia (10.0 mEq/L) and increased serum creatine kinase activity (780 IU/L). There was extreme extensor rigidity (rigor mortis) within 5 min of cardiac arrest. Results of microscopic and electron microscopic examination of muscle showed occasional perivascular infiltrates of lymphocytes with infrequent perimysial and epimysial neutrophils and a few sarcomeres with streaming of Z-bands (suggesting a contracted state). Histochemical evaluation of skeletal muscle showed no significant difference between type I and type II fibers. 相似文献

Medical records of 210 horses that survived ventral midline celiotomy for at least 4 months were examined and owners were queried to determine factors contributing to incisional hernia formation. The incidence rate of incisional hernias within 4 months was 16%. Factors significantly associated with occurrence of incisional hernias were incisional drainage, closure of the linea alba with chromic gut suture material, previous midline celiotomy, excessive incisional edema, castrated male sex, postoperative leukopenia, and postoperative pain (colic). Factors not significantly associated with occurrence of incisional hernias were suture pattern used for linea alba closure, concurrent enterotomy or intestinal resection, postoperative bandage or stent, postoperative fever, hypoproteinemia, diarrhea, respiratory disease (coughing), and peritonitis. Hernias developed in horses within 12 weeks of surgery, with the earliest hernia recognized at week 2. Of 30 horses for which information was available, only one hernia developed in 24 (80%) and two or more hernias developed in 6 (20%) along the incision. Ventrodorsal radiographs of the hips of 12 dogs were obtained. Copies of the radiographs were circulated to eight people with instructions for measurement of the angle of inclination of the femoral head and neck by two methods. Ureterocolonic anastomosis (UCA) was performed in 10 dogs with transitional cell carcinoma of the urinary bladder trigone or the urethra, or both. All grossly visible tumor was excised. All of the dogs recovered from anesthesia and surgery and had anal continence with no urine leakage. One dog died of undetermined causes 7 days after surgery. Nine dogs survived 1 to 5 months. The owners of eight of the dogs considered their dog's quality of life to be acceptable. Four dogs were euthanatized because of neurologic disease, three of which also had nausea and vomiting. The neurologic and gastrointestinal signs may have been caused by hyperammonemia, metabolic acidosis, and uremia. Blood ammonia levels were elevated in two dogs with neurologic signs. Hyperchloremic metabolic acidosis that was reversible with bicarbonate therapy was diagnosed in five dogs. All of the dogs were azotemic because of intestinal recycling of urea. Serum creatinine concentrations increased in four dogs after surgery. Drug-induced renal disease may have developed in two dogs. Pyelonephritis developed in five kidneys, two of which had outflow obstruction and two had bilateral hydroureteronephrosis before the UCA. In this small number of dogs, surgical excision of transitional cell carcinoma was not curative with six dogs having confirmed metastatic lesions at the time of death. 相似文献

Editor's note : The holding of the 19th Annual Meeting of the American College of Veterinary Surgeons in Philadelphia in conjunction with the Centennial Celebration of the School of Veterinary Medicine of the University of Pennsylvania provides an opportunity for reflection. Dr. Jacques Jenny, Professor of Orthopedic Surgery at the School, was Chairman of the Organizing Committee that gave rise to the American College of Veterinary Surgeons on December 16,1965, and was elected as first President of the College. The solid foundation and high ideals that the Organizing Committee established are in large part responsible for the success of the College. Dr. Jenny died on November 20, 1971. Those of us fortunate to have known him personally realize the immense gap that his premature death left. For those that did not know him, I hope that this article permits some appreciation of the contributions of Dr. Jenny the comparative orthopedic surgeon, as well as of the very special human qualities of Jacques Jenny the man. The first author, Dr. Mark Allam, was Professor of Surgery and Dean of the School of Veterinary Medicine during most of Dr. Jenny's career at the University of Pennsylvania, and was the first Chairman of the Board of Regents of the American College of Veterinary Surgeons. The second author, Dr. David Nunamaker, is the first holder of the Jacques Jenny Professorship in Veterinary Orthopedics at the University of Pennsylvania. 相似文献
