BACKGROUND: While adult populations have been well described in terms of nutritional status, such as the concentration of nutrient biomarkers, little work has been done in healthy paediatric populations. OBJECTIVE: The primary objective of this analysis was to explore the determinants of plasma micronutrients in a group of healthy infants and children. DESIGN: The Diabetes Autoimmunity Study in the Young (DAISY) has enrolled 1433 newborns at increased risk for type 1 diabetes in Denver, Colorado. A representative random sample of 257 children from the DAISY cohort between the ages of 9 months and 8 years with a total of 815 clinic visits over time was used in this analysis. Annual dietary intake was assessed over time with Willett food-frequency questionnaires that were validated in this population. Environmental tobacco smoke (ETS) was assessed using a validated survey. Plasma samples were tested for vitamins, carotenoids and total lipids. Predictors of plasma micronutrients were evaluated using mixed models for longitudinal data, while adjusting for age, human leukocyte antigen genotype, type 1 diabetes family history and other potential confounders and covariates. RESULTS: Increased micronutrient intake was associated with increased levels of their respective plasma nutrient, with the exception of gamma-tocopherol. Independent of dietary intake, levels of alpha- and beta-carotene and beta-cryptoxanthin were significantly lower, and gamma-tocopherol was significantly higher, in children who were exposed to ETS. CONCLUSION: Dietary intake predicts plasma micronutrient levels. Exposure to ETS potentially could have negative health effects in this young population.  相似文献

A 15-y-old castrated male Maine Coon cat was evaluated for an ulcerated soft tissue mass on the right hindlimb that had been observed for 4 mo and had grown rapidly. A 3 × 3 cm soft, raised, amorphous, and ulcerated subcutaneous mass was observed on the lateral right metatarsus. In-house cytology via fine-needle aspiration was nondiagnostic. Incisional biopsy of the mass and further staging was declined, and amputation was elected. The amputated limb was submitted for histopathology, which revealed severe chronic nodular granulomatous dermatitis and multifocal granulomatous popliteal lymphadenitis with large numbers of intralesional fungal hyphae. Fungal PCR and sequencing on formalin-fixed, paraffin-embedded tissue identified Chalastospora gossypii. No adjunctive therapy was elected at the time. The patient has done well clinically 1 y post-operatively. C. gossypii is a rare microfungus found worldwide and is considered a minor pathogen of several plants. To our knowledge, infection by this fungus has not been reported previously in veterinary species. Features in our case are comparable to other mycotic infections. Nodular granulomatous mycotic dermatitis and cellulitis, although uncommon, should be a differential for soft tissue masses in veterinary species; C. gossypii is a novel isolate.  相似文献

Fourteen dogs with enlarged gallbladders and immobile stellate or finely striated bile patterns on ultrasound are described. Smaller breeds and older dogs were overrepresented, with 4/14 Cocker Spaniels. Most dogs presented for nonspecific clinical signs such as vomiting, anorexia and lethargy. Abdominal pain, icterus and hyperthermia were the most common findings on physical examination. All dogs except one had serum elevation of total bilirubin and/or alkaline phosphatase, alanine aminotransferase and gamma glutamyl transferase. All dogs were diagnosed with a gallbladder mucocele upon histologic and/or macroscopic evaluation. Ultrasonographically, mucoceles are characterized by the appearance of the stellate or finely striated bile patterns and differ from biliary sludge by the absence of gravity dependent bile movement. On ultrasound, gallbladder wall thickness and wall appearance were variable and nonspecific. The cystic or common bile duct were normal sized in 5 dogs although all 5 had evidence of biliary obstruction at surgery or necropsy. Loss of gallbladder wall integrity and/or gallbladder rupture were present in 50% of the dogs, all located in the fundus. Gallbladder wall discontinuity on ultrasound indicated rupture whereas neither bile patterns predicted the likelihood of gallbladder rupture. Pericholecystic hyperechoic fat or fluid were suggestive of but not diagnostic for a gallbladder rupture. Cholecystectomy appears to be an appropriate treatment for mucoceles, if not to treat a gallbladder rupture, at least in most dogs to prevent it since gallbladder wall necrosis was identified by histology in 9 of 10 dogs. Mucosal hyperplasia was present in all gallbladders examined histologically. Positive aerobic bacterial culture was obtained from bile in 6 of 9 dogs. Cholecystitis was diagnosed histologically in 5 dogs and 4 dogs had signs of gallbladder infection solely upon bacterial bile culture. Gallbladder infection was not present with all the mucoceles suggesting that biliary stasis and mucosal hyperplasia may be the primary factors involved in mucocele formation. Based on the results of our study, we suggest two alternate courses of action in the presence of a distended gallbladder with an immobile ultrasonographic stellate or finely striated bile pattern: a cholecystectomy when clinical or biochemical signs of hepatobiliary disease are present or a medical treatment (antibiotics and choleretics) and patient monitoring by follow-up ultrasound examinations when the patient does not have clinical or biochemical abnormalities. An aerobic bile culture should be obtained in all patients, by ultrasound-guided fine needle aspirate or at surgery.  相似文献

OBJECTIVE: To evaluate the efficacy of using serum total and ionized magnesium (Mg) concentrations and urine Mg concentrations to identify Mg deficiency in cats. ANIMALS: 6 healthy castrated male cats. PROCEDURE: A Mg-replete diet was fed for 37 days, followed by a Mg-deficient diet for 37 days. On days 1, 3, and 7 of the last week of each diet, serum ionized and total Mg concentrations were determined; in addition, urine Mg concentration was determined each day of the last week. Serum total and ionized Mg concentrations were compared with urine Mg concentration, amount of Mg excreted during 24 hours (24-hour urine Mg excretion), ratio of urine Mg concentration to urine creatinine concentration (Umg:Ucr), and urinary fractional excretion of Mg (FEmg) to determine which variable best predicted Mg status. RESULTS: Cats fed Mg-deficient diets had significantly lower serum total and ionized Mg concentrations and 24-hour urine Mg excretion values, compared with cats fed Mg-replete diets. Serum total Mg concentration was the best predictor of Mg status. Twenty-four-hour urine Mg excretion was a repeatable, reliable measurement and had the best correlation with serum total Mg concentration. Serum total Mg concentration also correlated with urine Mg concentration, Umg:Ucr, and FEmg. CONCLUSIONS AND CLINICAL RELEVANCE: Serum total and ionized Mg concentrations can be used to identify cats with dietary-induced Mg deficiencies. Twenty-four-hour urine Mg excretion and urine Mg concentration correlated best with serum total Mg concentration and, therefore, may be the most useful urine variables for identifying Mg deficiency.  相似文献

Most chaotic mixing experiments have been restricted to two-dimensional, time-periodic flows, and this has shaped advances in theory as well. A prototypical, bounded, three-dimensional flow with a moderate Reynolds number is presented; this system lends itself to detailed experimental observation and allows for high-precision computational inspection. The flow structure, captured by means of cuts with a laser sheet (experimental Poincare section), was visualized with the use of continuously injected fluorescent dye streams and revealed detailed chaotic structures with high-period islands.  相似文献

Rumen acidosis is a common metabolic disorder occurring when organic acid production exceeds clearance capacity, reducing ruminal pH. The occurrence of acidosis has been directly correlated to the ratio of concentrate to forage in the diet. However, rates of substrate fermentation and acid absorption vary at different locations in the reticulo-rumen. The objective of this study was to determine the pH and redox potential (Eh) in different locations of the reticulo-rumen using 16 ruminally cannulated steers (309 ± 43 kg) receiving different supplementation levels of quebracho extract (QT; Schinopsis balansae) within a grower type diet (CP: 13.4%; total digestible nutrients [TDN]: 70.4%; and ME: 2.55 Mcal/kg, dry matter [DM] basis). Animals were randomly assigned to one of four dietary treatments: QT at 0%, 1%, 2%, and 3% of DM (QT0, QT1, QT2, and QT3, respectively), containing about 0%, 0.7%, 1.4%, and 2.1% of condensed tannins (CT), DM basis, respectively. Animals were adapted to the basal diet for 12 d before being introduced to predetermined treatments for 4 weeks (wk), with diets provided twice daily to allow ad libitum intake. Weekly measurements of ruminal fluid pH and Eh were taken 4 h post-feeding using a portable pH meter with two probes (pH and redox) in four locations of the reticulo-rumen (reticulum, cranial sac, dorsal sac, and ventral sac). Data were analyzed using a random coefficients model with the pen as a random effect and wk as repeated measures, with DM intake included as a covariate. There was no interaction among treatments, location, and wk (P ≥ 0.882) on reticulo-ruminal pH. Overall, ruminal pH was lower for QT0 and QT1 compared to QT3 (P < 0.001). The pH in the reticulum was greater than those of the ventral and dorsal sacs (6.05 vs. 5.94, 5.89, respectively; P ≤ 0.001) but similar to cranial sac (6.00). Reticular pH was positively correlated with the ruminal locations (≥0.78; P < 0.001). The linear equation to estimate ruminal mean pH using reticulum pH had an intercept and slope different from zero (P ≤ 0.04), but CT (% DM) was not different from zero (P = 0.15), root mean square error of 0.15, and R2 of 0.778: 0.723 (±0.36) + 0.857 (±0.059) × reticulum pH + 0.033 (±0.023) × CT. The Eh was lower for QT0 in week 1 than all other treatments (P < 0.001). We concluded that reticulo-ruminal pH differs among locations in the rumen regardless of QT supplementation level and days on feed, with reticular pH being the highest.  相似文献
