Three postoperative analgesic protocols were assigned randomly to 24 healthy dogs after thoracotomy at the left fourth intercostal space. Morphine was administered parenterally to eight dogs after tracheal extubation; selective intercostal nerve blocks with bupivacaine hydrochloride and epinephrine were administered to eight dogs before closure of the thorax; and bupivacaine hydrochloride and epinephrine were administered through an interpleural catheter to eight dogs after tracheal extubation. Heart rate, respiratory rate, rectal temperature, hematocrit, plasma protein, blood gas, and pain score evaluations were recorded before surgery and 30 minutes, 1 hour, 2 hours, and 3 hours after extubation. Morphine caused significant decreases in blood pH and blood oxygen tensions, and significant increases in carbon dioxide tensions. Dogs treated with intercostal nerve blocks had no significant changes in these parameters, and dogs treated with interpleural bupivacaine had significant decreases in blood oxygen tension. All dogs had significant decreases in rectal temperature, and hypothermia was prolonged after morphine. Analgesia was initially adequate in most dogs, but some dogs in each treatment group had recurrence of pain and were treated with interpleural bupivacaine. One dog developed pneumothorax. Interpleural administration of bupivacaine produced analgesia equal to that produced by systemic administration of morphine or selective intercostal nerve block with bupivacaine. Bupivacaine was easily readministered through an interpleural catheter. Respiratory compromise was less in dogs treated with bupivacaine than in dogs treated with morphine. After intercostal thoracotomy, interpleural bupivacaine provided prolonged analgesia with fewer blood gas alterations than morphine.  相似文献

Physical and clinicopathologic findings from six cows with proximal duodenal obstruction (PDO) and 58 cows with abomasal volvulus (AV) were compared retrospectively. Many of the physical signs were similar in cows with PDO and cows with AV, but the two conditions differed in the type of abdominal distention, and in the findings from rectal examination and abdominal auscultation. Cows with PDO had significantly lower mean values for serum sodium (Na+) and chloride ion (Cl-), and higher mean values for plasma bicarbonate [HCO3-], base excess, carbon dioxide pressure (pCO2), serum phosphate, urea nitrogen, and total protein than AV affected cattle. Cows with PDO showed hyperglycemia (range, 263-990 mg/dl; mean, 618 mg/dl) of unexplained etiology that was significantly higher than blood-glucose concentrations in AV cows (mean, 178 mg/dl). Although all AV cows with anion gap values greater than 32 mEq/l died;PDO cows with equally elevated anion gap survived. The anion gap elevations in PDO and AV cows resulted from accumulation of different anions. Although the site of obstruction of aborad flow of ingesta is similar in both disease conditions, the differences in physical and clinicopathologic findings appear to reflect differences in the degree of reticulo-omasal orifice obstruction and the degree of abomasal vascular compromise.  相似文献

A 2 cm partial ulnectomy was performed in twelve 4-month-old mongrel dogs with experimentally induced radius curvus. In four dogs, the periosteum was left intact; in four dogs, all of the periosteum was excised from the ulnectomy site; and in the remaining four dogs, the periosteum was sewn over the ends of the ostectomized bone. The unoperated limbs of all 12 dogs served as controls. Progress was determined monthly from radiographs until the dogs were 9 months of age. The ulnectomies performed when the periosteum was left in situ at the ulnectomy site healed quickly, resulting in progressive deformity of the foreleg. When the periosteum was excised or sewn over the ends of the bone, the ulnectomy sites did not heal and correction of the radius curvus resulted. Histopathologic examination confirmed the absence of bony healing.  相似文献

Partial maxillectomies were performed in 17 dogs and three cats to accomplish wide excision of oral tumors. The extent of the maxillectomy was dependent on tumor type, location, size, and invasiveness as determined by clinical and radiographic examination and incisional biopsy. The tumor and involved portions of adjacent facial bones, most commonly the maxillary and incisive bones, were removed. The resulting oronasal defect was closed by suturing a labial mucosal flap based on the lip margin to the hard palate mucoperiosteum. The major postoperative complication was partial suture line dehiscence, which occurred in three dogs. All four dogs with benign tumors were free of disease 7 to 34 months postoperatively (median 21.5 months). Of the 13 dogs treated for malignant neoplasia, seven were tumor-free at follow-up times ranging from 3 to 31 months (median 12 months). Five dogs with malignant tumors developed local recurrence 1 week to 10 months postoperatively (median 4.5 months). One cat with a benign tumor and two with malignant tumors were tumor-free at follow-up times of 7, 24, and 27 months. Partial maxillectomy can be effective in treating both benign and malignant oral tumors in dogs and cats.  相似文献

A protocol for clean harvesting, ethylene oxide sterilization, and room temperature storage of canine and feline cortical bone for use as allografts in fracture repair is described. Three dogs and one cat received cortical allografts prepared in this manner for reconstruction of severely comminuted mid-diaphyseal femoral fractures. Function of the affected limb returned to normal, and radiographic evaluations of the allografted femurs showed union at the host-graft interface and incorporation of the allograft.  相似文献

The formation of hematomas or seromas under skin grafts is reported to be the most common cause of graft failure. Several techniques have been developed to prevent fluid accumulation by increasing drainage, but their effectiveness has not been evaluated. In this study, four full-thickness skin grafting procedures were performed on both sides of the chests of 12 dogs, and the amount of wound drainage was quantitated for the first 10 postoperative days. The techniques evaluated were a sheet graft (control), continuous low level suction, piecrust incisions, and nonexpanded mesh graft. Significantly more drainage fluid was removed by continuous low level suction than the other techniques evaluated. Meshing allowed significantly more drainage than the piecrust or sheet grafts but was not as effective as continuous low level suction. Piecrust incisions had no significant effect on wound drainage.  相似文献

Four dogs (27.3–31.8 kg) with ilial body fractures associated with various degrees of acetabular comminution were treated using internal fixation. Anatomic reduction and internal fixation were achieved using a long 2.7 mm reconstruction plate on the hemipelvis. Based on the patient's pelvic radiographs, the reconstruction plate was precontoured before surgery to precisely fit a bony pelvis of similar size and shape. Excellent anatomic reduction of the fractures was achieved in all four dogs. Three dogs recovered uneventfully and regained full function of the affected leg. In one dog with comminution of the acetabulum, reduction of the most caudally located fracture was not maintained. Severe osteoarthritis developed, and excision arthroplasty was performed.
Reconstruction plates offer greater versatility than dynamic compression plates in the treatment of complex pelvic fractures such as ilial body fractures with acetabular comminution. The technique of precontouring the reconstruction plate prior to surgery decreases intraoperative time and corrects minor discrepancies in reduction.  相似文献

Partial rostral hemimandibulectomy was performed in 10 adult dogs. The temporomandibular joints (TMJs) were examined radiographically and tomographically before surgery, and mandibular stability was evaluated before and immediately after surgery. Radiographic, tomographic, and hemimandibular mobility assessments were made again at months 3 and 6. The TMJs were examined grossly and histologically in five dogs euthanatized at month 3 and in five dogs euthanatized at month 6. Statistically significant hemimandibular instability (p less than 0.05) persisted in all subjects throughout the study. The radiographic appearance of the joints remained unaltered; however, space asymmetry was identified in postoperative tomograms of three dogs at month 3 and four dogs at month 6. The TMJs were grossly normal at necropsy. Histologically, there were degenerative changes in articular cartilage and subchondral bone in all of the joints. The authors conclude that partial rostral hemimandibulectomy causes TMJ degeneration, as a consequence of hemimandibular instability or abnormal loading, or both.  相似文献

Single or multiple rib resection was performed in 40 dogs for the treatment of primary osteosarcoma or chondrosarcoma. The resulting thoracic wall defect was closed with polypropylene (12 dogs), primary muscle flap (16 dogs), diaphragmatic advancement (10 dogs), or a combination (2 dogs). Few immediate (less than 2 weeks) postoperative complications were observed. Twenty dogs with osteosarcoma had a median survival time of 3.3 months (range, 0.5 to 23 months), with a 20% 6-month survival time. Metastases occurred in all the dogs. Fourteen dogs with chondrosarcoma followed up longer than 2 weeks had a median survival time of 10.7 months (range, 0.5 to 36 months) with a 64% 6-month survival time. Eight dogs developed metastases, five died from concurrent disease, and one dog is alive. Dogs with chondrosarcoma survived significantly longer than dogs with osteosarcoma. Survival time was not related to tumor size or number of ribs resected.  相似文献

A ventral approach to the pelvic canal by bilateral pubic and ischial osteotomy was performed in five female dogs. The entire urogenital tract could be exposed and easily manipulated through this approach. All animals were ambulatory within 24 hours. Clinical union had formed in three of the four osteotomies by the time of euthanasia (months 2-4) in all animals. Incomplete union in some osteotomies indicated that exercise should be restricted for at least 4 months after the procedure. The procedure was determined to be clinically useful when wide exposure of the entire female urogenital tract is desired.  相似文献
