Cat scratch disease (CSD) is a zoonotic infection caused primarily by the bacterium Bartonella henselae. An estimated 12,000 outpatients and 500 inpatients are diagnosed with CSD annually, yet little is known regarding clinician experience with and treatment of CSD in the United States. Questions assessing clinical burden, treatment and prevention of CSD were posed to 3,011 primary care providers (family practitioners, internists, paediatricians and nurse practitioners) during 2014–2015 as part of the annual nationwide DocStyles survey. Among the clinicians surveyed, 37.2% indicated that they had diagnosed at least one patient with CSD in the prior year. Clinicians in the Pacific and Southern regions were more likely to have diagnosed CSD, as were clinicians who saw paediatric patients, regardless of specialty. When presented with a question regarding treatment of uncomplicated CSD, only 12.5% of clinicians chose the recommended treatment option of analgesics and monitoring, while 71.4% selected antibiotics and 13.4% selected lymph node aspiration. In a scenario concerning CSD prevention in immunosuppressed patients, 80.6% of clinicians chose some form of precaution, but less than one‐third chose the recommended option of counseling patients to treat their cats for fleas and avoid rough play with their cats. Results from this study indicate that a substantial proportion of U.S. clinicians have diagnosed CSD within the past year. Although published guidelines exist for treatment and prevention of CSD, these findings suggest that knowledge gaps remain. Therefore, targeted educational efforts about CSD may benefit primary care providers.  相似文献

ObjectiveTo determine the cardiorespiratory and anesthetic effects of 2, 6, and 20 mg kg−1 IV alfaxalone in hydroxypropyl beta cyclodextrin (Alfaxan) in dogs.Study designBlinded four-way crossover randomized by dose.AnimalsEight healthy adult purpose-bred mixed breed dogs (four male, four female) weighing between 12 and 28 kg.MethodsFour (0, 2, 6, 20 mg kg−1) IV treatments of alfaxalone were administered to each dog with a 3-hour washout period between doses. Measurements of heart rate, aortic systolic, mean, and diastolic blood pressures, pulmonary arterial and right atrial mean pressures, cardiac output, respiratory rate, tidal and minute volumes, and arterial blood pH, blood gases (PaO2, PaCO2) were performed prior to and at predetermined intervals after drug administration. Systemic vascular resistance and rate pressure product were calculated. The quality of induction, maintenance, and recovery from anesthesia were categorically scored as was the response to noxious stimulation.ResultsThe administration of alfaxalone resulted in dose-dependent changes in cardiovascular and respiratory parameters. Decreases in arterial blood pressure and increases in heart rate occurred at higher doses with most variables returning to baseline in 15–30 minutes. Respiratory rate, minute volume, and PaO2 decreased and apnea was the most common side effect. The duration of anesthesia increased with dose, and induction, maintenance, and recovery were judged to be good to excellent with all doses studied.Conclusions and clinical relevanceAlfaxalone produced good to excellent short-term anesthesia in unpremedicated dogs. Cardiorespiratory effects were minimal at lower doses. Anesthesia was judged to be good to excellent and associated with unresponsiveness to noxious stimulation for the majority of anesthesia. Hypoventilation and apnea were the most prominent and dose-dependent effects.  相似文献

After imposition of fees for disposal of cattle carcasses by the rendering industry, on-farm methods of mortality disposal are being investigated. Three open-air mortality compost windrows were constructed in January (JAN) and February (FEB) of 2004 in the cold, semiarid climate of southern Alberta, Canada. Windrow A included a base of barley straw a minimum of 46 cm thick, a layer of cattle mortalities (n=12) and a layer of stockpiled manure (minimum of 46 cm) covering the mortalities. Windrows B and C also had a bottom layer of barley straw, with windrow B containing 9 mortalities in 2 layers and windrow C containing 12 mortalities in 3 layers. Due to ambient temperatures > 0°C, carcasses (range 236 to 673 kg) were frozen at the time of windrow construction. Type ‘T’ thermocouples were embedded at the base of B and C windrows and temperatures within 120 cm of the surface were measured using a stainless steel dial probe. Windrows were turned 3 times at 3 mo intervals. Samples were collected from initial compost amendments and prior to each turning for determination of dry matter (DM), organic matter (OM), N, C and coliform bacteria. Ambient temperature was 13°C higher (P < 0.05) for the initial heating of FEB as compared to JAN compost, although the rate of compost temperature decline did not differ between replicates in this period. Days at maximum temperature did not differ between replicates and maximum temperatures in all windrows exceeded 55°C. After 3 heating periods and 9 mo, flesh was not evident and only fragments of bones (max wt. 740 g) were found. Results of this study demonstrate that ambient temperatures < 0°C and frozen mortalities provide no barrier to the use of open-air windrows for disposal of cattle mortalities. As well, stacking up to 3 layers of full-sized cattle mortalities in a windrow did not affect temperature profiles, residual flesh or bone, or analyses of the finished compost. Under the climatic conditions of southern Alberta, layering mortalities would reduce space requirements for large-animal mortality composting in a feedlot setting.  相似文献

The productivity and quality of malting barley were evaluated using factorial combinations of four preceding crops (faba bean, field pea, rapeseed, and barley) as main plots and four nitrogen fertilizer rates (0, 18, 36, and 54 kg N ha?1) as sub-plots with three replications at two sites on Nitisols of the Ethiopian highlands in 2010 and 2011 cropping seasons. Preceding crops other than barley and N fertilizer significantly improved yield and quality of malting barley. The highest grain yield, kernel plumpness, protein content, and sieve test were obtained for malting barley grown after faba bean, followed by rapeseed and field pea. Nitrogen fertilizer significantly increased yield, protein content, and sieve test of malting barley. All protein contents were within the acceptable range for malting quality. Inclusion of legumes in the rotation also improved soil fertility through increases in soil carbon and nitrogen content. We conclude that to maximize yield and quality of malting barley, it is critical to consider the preceding crop and soil nitrogen status. Use of appropriate break crops may substitute or reduce the amount of mineral N fertilizer required for the production of malting barley at least for one season without affecting its quality.  相似文献

Three rice cultivars, IR712, M1–48 and E425 were grown in solution culture for 10 weeks in temperature controlled glasshouses at IRRI, Los Baños, Philippines. Treatments consisted of two Mn levels, 0.5 and 50 μg.cm‐3 and two temperature regimes, 35/27 and 20/20 (day/night) in °C. Dry matter and Mn concentrations were determined at 2‐week intervals. In a second experiment, the M1–48 cultivar was grown in solutions containing 0.5, 1, 40 and 80 μg.cm‐3 Mn at 35/27, 29/21 and 20/20 °C (day/night) temperatures.

It was concluded that rice is more sensitive to excess Mn under cool conditions and that the concentration of Mn in the tissue at which toxicity symptoms appear or growth is decreased depends on the age of the tissues and the environmental conditions under which the plant is growing. Growth of two‐day old seedlings exposed to excess Mn was decreased to a greater degree than was that of 2‐week old seedlings. It is suggested that in screening rice cultivars for tolerance to excess Mn the environmental conditions should be rather rigidly controlled.  相似文献

AIMS: To screen tuatara undergoing translocation from a captive crèche to an island refuge for evidence of health and known diseases, and apply basic epidemiological techniques to assess the significance of disease test results.

METHODS: Tuatara (n=353) were physically examined and samples were taken from a random selection (n=30) for estimated white cell counts, screening for haemoparasites, and culture for Salmonella, Yersinia, Aeromonas and Campylobacter spp. Direct faecal smears were carried out on-site, and faecal floats were later performed to assess levels of endoparasitism with helminths and protozoa (n=69). Modified Ziehl-Neelsen staining was used to screen faecal smears, and positive specimens were further screened using an immunofluorescence antibody (IFA) test for Cryptosporidium oocysts.

RESULTS: There was no evidence of external parasites on any of the animals examined and only one animal had a gross abnormality. All estimated white cell counts were in the range 2.8– 17.5 × 109/L. No haemoparasites were observed. There were no enteric pathogens cultured, indicating the intestinal carriage of these bacteria in the tuatara was 9.4%. Of the 69 individual faecal samples examined, 12 (17%) had unidentified coccidial oocysts, 21 (30%) had nematode ova of various kinds, and 12 (17%) had intestinal carriage of motile protozoa consistent with Trichomonas spp and another unidentified organism. Nineteen (28%) tuatara had acid-fast oocysts present; however, IFA staining failed to detect any Cryptosporidium oocysts.

CONCLUSIONS: Our understanding of the diversity of gastrointestinal endoparasites affecting tuatara is inadequate as many of the parasite ova seen could not be identified. This is the first record of tuatara as a host for Trichomonas spp of protozoa in the gastrointestinal tract.  相似文献
