Objective – To discuss 3 potential mechanisms for loss of peripheral vasomotor tone during vasodilatory shock; review vasopressin physiology; review the available animal experimental and human clinical studies of vasopressin in vasodilatory shock and cardiopulmonary arrest; and make recommendations based on review of the data for the use of vasopressin in vasodilatory shock and cardiopulmonary arrest. Data Sources – Human clinical studies, veterinary experimental studies, forum proceedings, book chapters, and American Heart Association guidelines. Human and Veterinary Data Synthesis – Septic shock is the most common form of vasodilatory shock. The exogenous administration of vasopressin in animal models of fluid‐resuscitated septic and hemorrhagic shock significantly increases mean arterial pressure and improves survival. The effect of vasopressin on return to spontaneous circulation, initial cardiac rhythm, and survival compared with epinephrine is mixed. Improved survival in human patients with ventricular fibrillation, pulseless ventricular tachycardia, and nonspecific cardiopulmonary arrest has been observed in 4 small studies of vasopressin versus epinephrine. Three large studies, though, did not find a significant difference between vasopressin and epinephrine in patients with cardiopulmonary arrest regardless of initial cardiac rhythm. No veterinary clinical trials have been performed using vasopressin in cardiopulmonary arrest. Conclusion – Vasopressin (0.01–0.04 U/min, IV) should be considered in small animal veterinary patients with vasodilatory shock that is unresponsive to fluid resuscitation and catecholamine (dobutamine, dopamine, and norepinephrine) administration. Vasopressin (0.2–0.8 U/kg, IV once) administration during cardiopulmonary resuscitation in small animal veterinary patients with pulseless electrical activity or ventricular asystole may be beneficial for myocardial and cerebral blood flow. 相似文献

Objective To compare the cardiopulmonary effects and sensory blockade of epidural bupivacaine and ropivacaine. Study Design Prospective randomized study. Animals Six young adult medium‐sized crossbred dogs weighing 25.7 ± 7.1 kg. Method Dogs were chronically implanted with a lumbosacral epidural catheter. Acepromazine sedated dogs received all treatments: 0.5% bupivacaine at 0.14 mL kg?1 (LB5) or 0.22 mL kg?1 (HB5); 0.5% ropivacaine at 0.14 mL kg?1 (LR5) or 0.22 mL kg?1 (HR5); 0.75% bupivacaine at 0.22 mL kg?1 (HB7.5) or 0.75% ropivacaine at 0.22 mL kg?1 (HR7.5). Loss of sensation was tested at the level of the perineum, hind toe webs, flank, and caudodorsal rib areas before injection, and post‐injection (PI) up to 150 minutes PI. Systemic arterial blood pressure and heart rate were recorded before injection, and every 10 minutes PI until 150 minutes PI. Arterial blood gas analyses were performed prior to injection, and at 30, 60 and 150 minutes PI. Results No statistical differences existed between groups for the cardiopulmonary data or time to onset of block. Group HR7.5 had lower systolic (10–70 minutes PI) and diastolic (10–70 minutes PI) blood pressures and group HR5 had lower mean (10–90 minutes PI) and diastolic (10–90 minutes PI) blood pressures compared to baseline. Heart rate was lower compared to baseline in groups LR5 and HB7.5. A significant, but mild metabolic acidosis developed in groups LR5 and HB7.5 (150 minutes PI). No differences were present for the duration of block between groups, but duration of block in the dorsocaudal rib area was shorter in group HR5 compared to HR7.5. Conclusion Epidural ropivacaine and bupivacaine at the doses used have mild effects on the cardiopulmonary system, and extent of block are similar. Clinical Relevance The 0.75% concentration of bupivacaine and ropivacaine at 0.22 mL kg?1 appeared to contribute to greater success of block (>80%) at dermatomes L5–L7. 相似文献

AIM: To investigate the effect of ulinastatin (UTI) on inflammatory cell infiltration, expression of tumor necrosis factor-α (TNF-α) and concentration of oxidative stress product malondialdehyde (MDA) in the kidney medulla after cardiopulmonary resuscitation (CPR) in rabbits and to access its potential mechanisms in kidney injury after CPR.METHODS: Twenty-four male New Zealand adult rabbits were randomly divided into control group and UTI group after return of spontaneous circulation (ROSC) from 5 min ventricular fibrillation induced by alternating current. The UTI at the dose of 2.5×104 U/kg was administered immediately after ROSC to the animals in UTI group, the same volume of saline were injected in control group. The urinary output was recorded and the serum concentration of BUN and creatinine were detected every 4 h after ROSC. The animals were sacrificed 24 h after ROSC and the kidney medullas were obtained to observe the degree of inflammatory cell infiltration, the expression of TNF-α and the concentration of MDA.RESULTS: Six rabbits in control group and 6 animals in UTI group survived to the end point of experiment. The urinary output was decreased gradually to the lowest 8 h-12 h after ROSC and then increased in both groups. The urinary output in UTI group was more than that in control group 8 h after ROSC (P<0.05). The serum concentrations of BUN and creatinine were significantly lower in UTI group than those in control group 4 h after ROSC (P<0.05). The myeloperoxidase-positive cells in control group were much higher than that in UTI group (P<0.05). The expression of TNF-α and concentration of MDA in the kidney medullas in UTI group were lower than those in control group (P<0.05, P<0.01).CONCLUSION: The standard dose of UTI (2.5×104 U/kg) administered in rabbits suffered from ventricular fibrillation for 5 min may alleviate the degree of inflammatory cell infiltration, decrease the expression of TNF-α and reduce the concentration of MDA in kidney medulla. ObjectiveTo determine the anaesthetic and cardiorespiratory effects of a constant rate infusion of fentanyl in sheep anaesthetized with isoflurane and undergoing orthopaedic surgery.Study designProspective, randomised, ‘blinded’ controlled study.AnimalsTwenty healthy sheep (weight mean 41.1 ± SD 4.5 kg).MethodsSheep were sedated with intravenous (IV) dexmedetomidine (4 μg kg−1) and morphine (0.2 mg kg−1). Anaesthesia was induced with propofol (1 mg kg−1 minute−1 to effect IV) and maintained with isoflurane in oxygen and a continuous rate infusion (CRI) of fentanyl 10 μg kg−1 hour−1 (group F) or saline (group P) for 100 minutes. The anaesthetic induction dose of propofol, isoflurane expiratory fraction (Fe’iso) required for maintenance and cardiorespiratory measurements were recorded and blood gases analyzed at predetermined intervals. The quality of recovery was assessed. Results were compared between groups using t-tests or Mann–Whitney as relevant.ResultsThe propofol induction dose was 4.7 ± 2.4 mg kg−1. Fe’iso was significantly lower (by 22.6%) in group F sheep than group P (p = 0). Cardiac index (mean ± SD mL kg−1 minute−1) was significantly (p = 0.012) lower in group F (90 ± 15) than group P (102 ± 35). Other measured cardiorespiratory parameters did not differ statistically significantly between groups. Recovery times and recovery quality were statistically similar in both groups.Conclusions and clinical relevanceFentanyl reduced isoflurane requirements without clinically affecting the cardiorespiratory stability or post-operative recovery in anaesthetized sheep undergoing orthopaedic surgery. 相似文献
