ObjeCTIVE: To evaluate a total intravenous anaesthetic technique in dogs undergoing craniectomy. STUDY DESIGN: Prospective clinical study. ANIMALS: Ten dogs admitted for elective surgical resection of rostro-tentorial tumours. METHODS: All dogs were premedicated with methadone, 0.2 mg kg(-1) intramuscularly 30 minutes prior to induction of anaesthesia. Anaesthesia was induced with propofol administered intravenously (IV) to effect, following administration of lidocaine 1 mg kg(-1) IV and maintained with a continuous infusion of propofol at < or =0.4 mg kg(-1) minute(-1) during instrumentation and preparation and during movement of the animals to recovery. During surgery, anaesthesia was maintained using a continuous infusion of propofol at alfentanil < or =1 microg kg(-1) minute(-1). Lidocaine was administered at 1 mg kg(-1) IV immediately prior to extubation. Arterial blood pressure and heart rate (HR) were recorded prior to induction and every 5 minutes throughout preparation and surgery. Central venous pressure was recorded every 5 minutes throughout surgery. RESULTS: Administration of propofol and lidocaine prevented significant increases in mean arterial blood pressure (MAP) and HR during endotracheal intubation and extubation. Adequate MAP was maintained throughout anaesthesia. Recovery was smooth and excitement free. There was no association between duration of anaesthesia, total drugs administered, or severity of neurological disease and recovery times. Postoperatively there was no deterioration in neurological function in the immediate postoperative period with complete resolution of pre-existing neurological deficits within 7 days of surgery. CONCLUSION: This technique provided minimal response to intubation and extubation, adequate arterial blood pressure and a smooth predictable recovery. All animals were neurologically improved by the time of discharge, suggesting that this technique had not caused significant neuronal damage. CLINICAL RELEVANCE: Total intravenous anaesthesia with propofol and alfentanil appears to be a satisfactory anaesthetic technique for use in dogs undergoing surgery for debulking/removal of rostro-tentorial tumours.  相似文献

ObjectiveTo compare induction targets, and the haemodynamic and respiratory effects, of propofol, or as an admixture with two different concentrations of alfentanil, delivered via a propofol target-controlled infusion (TCI) system.Study designProspective blinded randomized clinical study.Animals Sixty client-owned dogs scheduled for elective surgery under general anaesthesia. Mean body mass (SD) 28.5 kg (8.7) and mean age (SD) 3.5 years (2.4).MethodsDogs received pre-anaesthetic medication of acepromazine (0.03 mg kg−1) and morphine (0.2 mg kg−1) administered intramuscularly. Animals were randomly assigned to receive one of three induction protocols: propofol alone (group 1), a propofol/alfentanil (11.9 μg mL−1) admixture (group 2), or a propofol/alfentanil (23.8 μg mL−1) admixture (group 3), via a TCI system. Blood target concentrations were increased until endotracheal intubation was achieved, and induction targets were recorded. Heart rate (HR), respiratory rate (fr) and non-invasive arterial blood pressure were recorded pre-induction, at endotracheal intubation (time 0) and at 3 and 5 minutes post-intubation (times 3 and 5, respectively). Data were analysed using anova for normally distributed data or Kruskal–Wallis test, with significance assumed at p < 0.05.ResultsThere were no significant differences between groups with respect to age, body mass, HR, fr, systolic and diastolic blood pressure. The blood propofol targets to achieve endotracheal intubation were significantly higher in group 1 compared with groups 2 and 3. Mean arterial blood pressure (MAP) was significantly higher in group 1 at time 0 when compared with groups 2 and 3.Conclusions and clinical relevanceInduction of anaesthesia with a TCI system can be achieved at lower blood propofol targets when using a propofol/alfentanil admixture compared with using propofol alone. However, despite reduced targets with both propofol/alfentanil admixture groups, MAP was lower immediately following endotracheal intubation than when using propofol alone.  相似文献

Different structurally related phenylpiperidine opioids exhibit different isoflurane-sparing effects in cats. Because minimum alveolar concentration (MAC) in cats is affected only by very high plasma concentrations of some phenylpiperidine opioids, we hypothesized these effects are caused by actions on nonopioid receptors. Using a prospective, randomized, crossover design, six cats were anesthetized with isoflurane, intubated, ventilated, and instrumented. Isoflurane MAC was measured in triplicate using a tail-clamp and bracketing technique. A computer-controlled intravenous infusion using prior pharmacokinetic models targeted plasma concentrations of 60 ng/ml fentanyl, 10 ng/ml sufentanil, or 500 ng/ml alfentanil, and isoflurane MAC was measured in duplicate. Next, naltrexone 0.6 mg/kg was administered to cats hourly during the opioid infusion, and isoflurane MAC was measured in duplicate. Blood was collected during MAC determinations to measure opioid concentrations. Responses were analyzed using repeated measures ANOVA with significance at p < .05. Alfentanil and sufentanil decreased isoflurane MAC by 16.4% and 6.4%, respectively, and these effects were completely reversed by naltrexone. Fentanyl had no significant effect on isoflurane MAC. Alfentanil and sufentanil modestly reduce isoflurane MAC via agonist effects on opioid receptors. However, these effects are too small to justify clinical use of phenylpiperidine opioids as single agents to reduce MAC in cats.  相似文献
