| 摘 要： | 目的探讨手术操作与腰椎间盘突出症手术疗效的关系,为提高治疗质量提供更有效的措施.方法对手术治疗的400例腰椎间盘突出症患者中出现术后并发症而再次手术的28例患者进行回顾性分析和对比性研究.结果影响腰椎间盘突出症手术疗效的近期因素有髓核摘除不彻底、椎间盘炎、椎管内血肿形成等;远期因素有神经根粘连、椎间盘再突出、腰椎不稳、医源性椎管狭窄等.结论为避免腰椎间盘突出症术后效果不佳,应术前充分准备,注重术中操作,术后密切观察病情变化,积极预防和及时处理并发症.

| Abstract: | Objective To study the relationship between surgical procedures with clinical effect in lumbar disc herniation in order to improve operative methods and obtain better outcome. Method Retrospective analysis was carried out in 400 patients with lumbar disc herniation who had been operated with removal of nucleus pulposus by using small incision and fenestration,28 cases of whom received the second operation because of postoperative complications. Results The short term complications included incomplete nuclear extirpation,discitis and canal hematoma compression and so on. The long term complications included nerve root adhesion,recurred disc herniation,segmental instability and iatrogenic spinal stenosis. Conclusion Many factors may influence the outcome of lumbar disc herniation,which can be abolished by sufficient preparation, careful operation and proper management. |
