一例人感染羊接触传染性脓疱皮炎的确诊

摘    要:2013年6月6日,云南省江川县动物疫病预防与控制中心的一名兽医在保定可疑羊接触传染性脓疱皮炎发病羊时不慎被羊咬伤手指,10 d后在伤口愈合处形成丘疹,随后发展为水疱,最后形成结痂。随着痂皮的逐渐脱落,直至丘疹出现后1个多月感染病灶才完全愈合。经过对发病羊口唇痂垢及人手指丘疹、水疱液样品进行PCR、Real-time PCR及序列比对分析,2份样品均扩增出1225 bp的预期大小片段,Real-time PCR也扩增出标准S形曲线和Rn指数增长峰,2份病料测定出的序列(1133 bp)完全一致,与NCBI GenBank中的羊口疮病毒参考毒株NC-005336.1(1133/1133)对应序列的相似性达到100%,与AY386264.1(1115/1133)、AY386263.1(1114/1133)、HM133903.1(1113/1133)、KF837136.1(1110/1133)及普洱毒株PUER/YN/CHIA/2011(1113/1133)对应序列相似性达到98%,与DQ184476.1(1104/1133)对应序列相似性达到97%,与NCBI GenBank中同为副痘病毒属成员的伪牛痘参考毒株GQ329669.1(1052/1140)、GQ329670.1(1051/1140)对应序列的相似性达到92%,与牛丘疹性口炎病毒参考毒株AY386265.1(905/1130)的对应序列的相似性仅达到80%。由此,确诊此次病例为人感染羊传染性脓疱病毒。

Abstract:The present report described a human case of contagious ecthyma due to a bite by a diseased goat. On June 6th, 2013, the right middle finger of a veterinarian at Jiangchuan Center for Prevention and Control of Animal Disease, Yunnan Province was bitten by a goat suspicious of contagious ecthyma when he was examining the animal. Pimples formed near the wound spot 10 days later, following by blisters and scabs. As scabs fell off gradually, local lesions were healed. The case lasted for more than one month since pimples emerged. Pimple samples were collected from the goat and human and tested by PCR, Real-time PCR and sequence analysis. A 1225 bp DNA fragment was amplified in PCR and S shaped curves and Rn exponential growth peaks were amplified in Real-time PCR. The nucleotide sequences of PCR products of human and goat samples were identical and 1133 bp in length. Alignment of this nucleotide sequence with other available sequences demonstrated its identity at 100%with Orf virus reference strain NC-005336.1(1133/1133) in GenBank, 98%with orf strains AY386264.1 (1115/1133), AY386263.1 (1114/1133), HM133903.1 (1113/1133) , KF837136.1 (1110/1133) and Yunnan strain PUER/YN/CHIA/2011 (1113/1133), and 97%with DQ184476.1 (1104/1133). In addition, the identity to Parapoxvirus members pseudo-cowpox virus reference strain GQ329669.1 (1052/1140), GQ329670.1 (1051/1140) was 92%and to bovine popular stomatitis virus reference strain AY386265.1 (905/1130) was 80%. Therefore, this case of human infection was confirmed via direct contact with diseased goat.
