| 摘 要： | 目的:探讨甲状腺功能亢进症(甲亢)患者糖代谢及胰岛β细胞分泌功能、胰岛素抵抗的变化及意义。方法:对甲亢患者72例行口服葡萄糖耐量试验(OGTT)及胰岛素释放试验,并测定糖化血红蛋白(HbA1C)、血清游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)、促甲状腺激素(TSH)水平。经抗甲状腺药物治疗9～12月后复查以上指标,分别计算胰岛素敏感指数(ISI)、胰岛素抵抗指数(HO-MA-IR)、胰岛β细胞分泌功能指数(HOMA-IS)。结果:甲亢合并糖代谢异常者占52.78%(38/72),其中,糖耐量减低(IGT)占34.72%(25/72),空腹血糖受损(IFG)占6.94%(5/72),达糖尿病标准者占11.11%(8/72);甲亢组的餐后2h血糖(2hPG)、HbA1C及HOMA-IR指数显著高于正常对照组(P<0.05),而ISI、HOMA-IS低于对照组(P<0.05);经治疗甲状腺激素恢复正常以后,糖调节受损(IGR)中86.67%(26/30)的患者的血糖及HbA1C恢复正常;前后比较其2hPG、HbA1C及HOMA-IR明显下降(P<0.05),而ISI、HOMA-IS升高(P<0.05)。...
