Clinical diagnosis of renal tumors in the horse has been difficult because the traditional diagnostic techniques are inadequate. Because of this shortcoming, early publications rarely reported antemortem diagnosis of primary renal tumors. Conventional radiographic techniques cannot effectively outline kidneys in the adult horse. With the advent of ultrasonography, a new dimension to the evaluation of the large-animal kidney has been introduced. Percutaneous diagnostic ultrasonography provides a noninvasive method of examining both kidneys. This report describes a 15 year-old Tennessee Walking Horse mare presented for evaluation of a mass in the sublumbar region, polyuria and polydipsia. Clinical examination and laboratory test results suggested intra-abdominal neoplasia. Two-dimensional ultrasonographic images of the mass were consistent with renal neoplasia. Histologic diagnosis of ultrasound-guided renal biopsy was renal tubular cell carcinoma.  相似文献

Prostatic masses were detected in two geldings that were presented to our referral hospital for evaluation of dysuria. The masses were detected during transrectal palpation and ultrasonography of the prostate gland and were further evaluated using urethral endoscopy. A 10-year-old gelding underwent intraurethral debulking of the prostatic mass. Histopathological diagnosis was prostatic cystadenoma. The mass regrew within 9 months, and the horse was euthanized due to deteriorating clinical control of urinary continence. In another case, a 12-year-old gelding was scheduled for transurethral debulking; however, the owners decided against treatment and the horse was euthanized due to progression of clinical disease. Postmortem examination revealed the prostate to be nearly completely obliterated by the mass. Histopathological diagnosis was adenocarcinoma incorporated into a spindle cell population of unknown histogenesis. To our knowledge, these represent the first reported cases of equine prostatic cystadenoma and adenocarcinoma. In both horses, referring complaint was dysuria, and identification of the prostatic mass was made on palpation and ultrasonographic examination per rectum. Treatment options for prostatic masses in horses are limited because of the difficulty of obtaining prostatic tissue via biopsy, the surgical inaccessibility of the gland, and the apparent low incidence of occurrence. We have subsequently identified prostatic masses in two additional geldings, both also presenting for dysuria. However, the exact nature of these two apparent prostatic masses has not been definitively diagnosed with histopathological examinations to date. The prostate should be evaluated in male horses presenting for dysuria.  相似文献

A 4-month old, 200 kg, grey warmblood colt presented for a firm, non painful mass on the distal medial aspect of the left third metatarsus. Excisional biopsy revealed a diagnosis of haemangiosarcoma. Equine haemangiosarcoma is uncommon and only limited reports of successful treatment are available. The prognosis for survival is therefore considered to be poor. After two separate incidences of recurrence with incomplete excision of the tumour, intralesional treatment with cisplatin without excision or debulking was performed on three separate occasions. Intralesional cisplatin injection was performed at monthly intervals for three treatments. Four years post treatment with cisplatin, the horse remained in remission. This case report describes the diagnostic and treatment challenges for successful treatment of a primary haemangiosarcoma on the distal limb of a warmblood foal using intralesional cisplatin chemotherapy.  相似文献

Three Thoroughbred horses with unilateral progressive ethmoid haematomas were treated using intralesional injections of 10% formalin (4% formaldehyde solution). Injections were performed in the standing sedated horse through the nasal passages under endoscopic guidance and, when the ethmoid haematoma involved the paranasal sinuses, through holes trephined into the affected sinus. Regression of the lesions occurred in all cases after repeated injections. This technique appears to be a safe and effective treatment for progressive ethmoid haematomas in the horse.  相似文献

The use of real-time ultrasound for examination of the bladder was a useful diagnostic aid in 2 cases of cystic calculi. The ultrasound procedure revealed that a firm mass palpated per rectum in the bladder of one horse was a calculus. In the other horse, ultrasound revealed additional small uroliths. To remove the uroliths in both horses, laparocystidotomy via a ventral paramedian approach was chosen because it provided the best access to the bladder lumen. The calculi were analyzed by qualitative chemical analysis and quantitative crystallography. Results differed between analysis methods. Crystallography revealed that the mineral composition of both calculi was pure calcium carbonate. Chemical analysis erroneously indicated that the calculi contained phosphate, magnesium, and oxalate.  相似文献

Nasomaxillary fibrosarcoma was diagnosed in 3 young horses. Clinical signs included epiphora, facial swelling, dyspnea, unilateral serosanguineous nasal discharge, or an external mass. Physical examination, radiography, and cytology were important diagnostic adjuncts. The definitive diagnosis in each case was based on biopsy. In one case, surgical removal of the tumor and extensive curettage of the affected sinus resulted in extended remission from neoplastic disease.  相似文献

An 11‐year‐old Warmblood gelding with chronic bilateral epistaxis was diagnosed with an adenocarcinoma of the right ethmoid, based on transendoscopic biopsy. On computed tomography images, the cribriform plate was considered intact and the tumour was minimally invasive in surrounding tissue. The palliative treatment included a combination of surgical exploration and laser ablation. Following recurrence of the clinical signs one month later, intralesional formalin injection was performed. In the immediate post injection period, the horse developed severe neurological signs. Because of the overall poor prognosis, the horse was subjected to euthanasia. On post mortem examination, changes were seen in the frontal lobe consistent with diffusion of formalin through the cribriform plate. Care is needed when treating horses with intralesional formalin because macroscopic appearance of the cribriform plate may not relate to its functionality.  相似文献

Two adult male dogs (a 7‐year‐old shorthaired Chihuahua and 14‐year‐old Shih Tzu) and one adult female dog (a 9‐year‐old Maltese) presented for evaluation of new‐onset seizure activity. Magnetic resonance imaging of the brain demonstrated a large, poorly marginated T2‐weighted hyperintense, and strong contrast enhancing extra‐axial mass in each case. A surgical biopsy for histopathologic evaluation was elected in all cases, and intraoperative impression smears were successfully obtained. Intraoperative cytology identified a homogenous population of round to polygonal cells with central to eccentric nuclei, coarse chromatin, and variably amphophilic to eosinophilic granular cytoplasm. Cytologic findings led to a suspected diagnosis of granular cell tumor (GCT) in all cases. Histopathologic review identified a densely cellular, unencapsulated neoplastic mass comprised of sheets of large round to polygonal cells with abundant eosinophilic cytoplasm containing numerous eosinophilic intracytoplasmic granules, confirming the diagnosis of GCT in all cases. The cases reported here are unique in that they reveal an accurate intraoperative cytologic diagnosis of a rare canine central nervous system neoplasm. Intraoperative cytology of the intracranial masses could provide clinicians with important and quick diagnostic and prognostic information; therefore, expediting decisions made intraoperatively. Further research is warranted to determine the diagnostic accuracy of intraoperative cytology for neoplasia in veterinary patients.  相似文献

This case report describes the laparoscopic approach for removal of cystic intra-abdominal testicles in the standing colt. One 3-year-old Tobiano and one 2-year-old Warmblood colts were referred for abdominal cryptorchidectomy. The horses were clinically and ultrasonographically examined and a presumptive diagnosis of unilateral abdominal cryptorchidism was made. A laparoscopic approach via the flank was used to localise each abdominal testicle. In both colts the abdominal testicle was enlarged and cystic. Each spermatic cord was ligated and fluid was aspirated from the testicle. By reducing the size of the mass minimally invasive removal through an enlarged instrumental portal was possible. Histopathology revealed a cystic rete testis in the Tobiano and a teratoma in the Warmblood. In these cases the cystic enlarged testicles were nonpainful and were incidental findings. A cystic testicle might be developmental (Tobiano case) or arise due to neoplastic transformation (Warmblood case). The laparoscopic approach for enlarged cyst-like testicles in the standing horse offers a secure minimally invasive method for removal.  相似文献

Eleven horses with unilateral distension of the sheath are reported with emphasis on the use of plain and contrast radiography in diagnosis. Six had associated bony abnormalities of the sustentaculum tali, and two of these showed destructive or mixed destructive and proliferative lesions resulting from osteomyelitis. One horse had a fracture of the sustentaculum. In three horses proliferative changes were found. Bony changes were best demonstrated on a dorsomedial-plantarolateral oblique view. Contrast radiography of the tarsal sheath was performed in five cases without, and two horses with, associated bony lesions. In five cases ballooning with or without folding of the proximal aspect of the sheath was demonstrated. Additional band-like filling defects (two cases), triangular filling defect (one case), nodular intra-tendovaginal masses (two cases), or an extra-tendovaginal mass (one case) were noticed. The remaining two horses showed a semicircular collar shape dilation of the proximal aspect of the sheath. Contrast radiography appeared to be a helpful tool for the examination of tarsal sheath lesions. Two horses with associated bony changes were destroyed immediately after diagnosis, one failed to recover following surgical intervention and in three horses the outcome was unsatisfactory after conservative treatment. Three horses without associated bony lesions recovered completely after conservative treatment. One was operated on subsequently; the results are promising but the outcome of this case is not yet known. One was sold shortly after diagnosis and lost to follow up.  相似文献

A one‐year‐old, Thoroughbred colt presented for evaluation due to a one month history of fever of unknown origin and progressive weight loss. On initial presentation, the horse was febrile and showed signs localised to the respiratory tract. These included bilaterally increased bronchovesicular sounds and a moderate, diffuse interstitial pattern on thoracic radiographs. A transtracheal wash yielded mucopurulent debris, culture of which grew small numbers of Staphylococcus epidermidis and Aspergillus spp. The horse was discharged with a diagnosis of bronchointerstitial pneumonia and placed on antibiotic therapy. Ten days after initial presentation, he developed abdominal pain that was unresponsive to on‐farm treatment. The horse was febrile, displayed increased respiratory rate and effort, and showed moderate signs of abdominal pain. On rectal examination, a firm, 8–10 cm mass was palpated on midline. The colt was admitted to the hospital and scheduled for exploratory laparotomy, but died a short time later before surgery could be performed. This report describes the clinical, diagnostic and histopathological findings of a case of alimentary lymphoma in a yearling colt.  相似文献

Splenic and hepatic ultrasonography were beneficial for diagnosis of lymphosarcoma in 3 horses with anorexia, weight loss, and lethargy. Ultrasonographic abnormalities of the spleen included a large, complex, hypoechoic mass in 1 horse, multiple well-marginated, hypoechoic nodules in 1 horse, and diffuse hyperechogenicity in another horse. Ultrasonographic abnormalities of the liver included a spherical, hypoechoic nodule in 1 horse and diffuse hyperechogenicity in another. Histologic examination of ultrasound-guided biopsy specimens or aspirates revealed lymphosarcoma. Necropsy findings confirmed diagnosis of lymphosarcoma in all horses. Necropsy findings of the liver and spleen correlated well with antemortem ultrasonographic images.  相似文献

This retrospective study summarises the case details, presenting signs, management and outcome in cases of temporohyoid osteoarthropathy (THO) and describes the findings of diagnostic imaging modalities. The condition appears to be relatively rare in Europe and the objective of this study is to make clinicians aware that THO can have a range of various neurological and clinical presentations. The records of 2 referral equine practices in England were reviewed and 7 horses with THO diagnosed on guttural pouch endoscopy indentified. The clinical and neurological signs, diagnostic procedures, treatment and outcomes were reviewed. Although small, this group is the largest case series of THO from Europe. One horse was a yearling, whereas THO is generally considered usually to affect middle aged and older horses. Computed tomography was used to confirm the diagnosis and demonstrated stylohyoid bone fractures in 2 cases while there was mild increased radionuclide uptake in one of 2 cases undergoing nuclear scintigraphy. Treatment is still controversial, although ceratohyoidectomy led to complete resolution of signs in 2 of 5 cases thus treated and improvement in the other 3. One horse with mild signs treated with antimicrobials and nonsteroidal anti‐inflammatory drugs made a full recovery while another, presented with head shaking and managed with a phased exercise programme, improved but did not resolve completely.  相似文献

This clinical report describes 8 cases of branchial remnant cysts (BRC) in the horse. The horses presented with bimodal age distribution, with 5 cases in mature horses (age 8–21 years) and 3 in foals (age 1, 6 and 10 months). Mature cases presented for dysphagia or intermittent oesophageal obstruction (2/5), and retropharyngeal swelling (3/5), whereas respiratory stridor and visible mass were presenting complaints in the foals. Presence of a right‐sided (5/8) or dorsally located (2/8) palpable retropharyngeal mass of 3–35 cm diameter was noted clinically; one left‐sided mass was identified as an incidental finding at necropsy. Ultrasonography typically revealed a thick‐walled cyst containing hypoechoic fluid with dependent hyperechoic masses consistent with blood clots. Radiographs and upper airway endoscopy were also consistent with a retropharyngeal mass. Fluid cytology revealed chronic haemorrhage in 6/8 cases, and squamous epithelial cells in one case. Histopathology in all cases demonstrated an epithelium‐lined cyst with no smooth muscle or thyroid tissue. Two cases was subjected to euthanasia; one due to concurrent laryngeal anomalies and one due to financial constraints. The remaining 5 cases were treated via surgical excision. Post operatively, right laryngeal hemiplegia was observed in 4/5 cases. All previous reports of BRC in the horse have described juvenile individuals. Brachial remnant cyst should be considered a differential diagnosis for mature horses with masses of the throatlatch area and can be definitively diagnosed by the presence of squamous epithelium in aspirated fluid or by histopathology of the excised mass. Right recurrent laryngeal nerve damage is a common complication of surgery.  相似文献

Background: Renal biopsies are uncommonly performed in horses and little is known about their diagnostic utility and associated complication rate. Objective: To describe the techniques, the complication rate, risk factors, and histopathology results; as well as evaluate the safety and diagnostic utility of renal biopsy in the horse. Animals: One hundred and forty‐six horses from which 151 renal biopsies were obtained. Animals ranged in age from 48 hours to 30 years. Methods: Multicenter retrospective study, with participation of 14 institutions (1983–2009). Results: Renal biopsy in horses was associated with a similar rate of complications (11.3%) to that occurring in humans and companion animals. Complications were generally associated with hemorrhage or signs of colic, and required treatment in 3% of cases. Fatality rate was low (1/151; 0.7%). Biopsy specimens yielded sufficient tissue for a histopathologic diagnosis in most cases (94%) but diagnoses had only fair (72%) agreement with postmortem findings. Risk factors for complications included biopsy specimens of the left kidney (P= .030), a diagnosis of neoplasia (P= .004), and low urine specific gravity (P= .030). No association with complications was found for age, sex, breed, institution, presenting complaint, other initial clinicopathologic data, biopsy instrument, needle size, or use of ultrasonographic guidance. Conclusions and Clinical Importance: Renal biopsy in horses has low morbidity and results in a morphological histopathologic diagnosis in 94% of cases. However, this procedure might result in serious complications and should only be used when information obtained would be likely to impact decisions regarding patient management and prognosis.  相似文献

A standardbred gelding with a history of 10 days pyrexia and lethargy was referred to the Equine Hospital at the Swedish University of Agricultural Sciences in Uppsala, Sweden.The horse had tachypnea with increased respiratory effort and was in thin body condition. Laboratory findings included leukocytosis, hyperfibrinogenemia and hypoxemia. Thoracic radiographs showed signs of pneumonia with a multifocal nodular pattern, which in combination with lung biopsy findings indicated Equine Multinodular Pulmonary Fibrosis (EMPF). EMPF is a recently described disease in adult horses with clinical signs of fever, weight loss and respiratory problems. The pathological findings include loss of functional pulmonary parenchyma due to extensive nodular interstitial fibrosis which has been related to infection with the equine herpesvirus type 5 (EHV-5). In this case, lung biopsy and tracheal wash samples tested positive for both asinine herpesvirus type 5 (AHV-5) and EHV-5 using PCR assays. The horse failed to respond to treatment and was euthanized for humane reasons. Postmortem examination confirmed the diagnosis of EMPF. This case suggests that not only EHV-5 alone should be considered in association with the development of this disease.  相似文献

Gastric leiomyosarcoma was diagnosed in a 12-year-old Thoroughbred gelding. The horse was examined because of anorexia, weight loss, and intermittent fever of 1.5 months' duration. Antemortem diagnostic testing was extensive, but a definitive diagnosis could not be reached with noninvasive methods. Exploratory celiotomy confirmed an intra-abdominal mass, and the horse was euthanatized. Necropsy revealed a firm mass associated with the distal portion of the esophagus, cranial two thirds of the stomach, and visceral surface of the liver. The histopathologic diagnosis was leiomyosarcoma.  相似文献

Two grey mares, a 12-year-old Arabian and a 13-year-old Lippizaner cross, were referred for chronic pelvic limb lameness and abduction of the limb during the non supporting phase of the stride. The clinical examination revealed melanomas under the tail and on the perineum, and oedema of the proximal lame limb. A mass in the pelvic area was detected on rectal palpation. Laparoscopy was performed and the provisional diagnosis of malignant melanoma was made. Histological examination of the biopsy taken under laparoscopic control from the mass in the pelvis confirmed the diagnosis of malignant melanoma. In the postmortem examination a large black mass in the pelvis and metastases in several organs were diagnosed. The process under the tail showed microscopically the characteristics of dermal melanomatosis. The metastases showed the appearance of a malignant melanoma. The lameness and abduction during the non supporting phase of the stride were possibly caused by a combination of pain, a mechanical limitation, and paresis/paralysis. When an older grey horse with melanomas on the predilection sites is submitted with pelvic limb lameness and abduction of the limb during the non supporting phase of the stride, a malignant melanoma in the pelvis is one of the differential diagnoses and a rectal examination should be performed. Diagnostic laparoscopy in the standing horse can be useful to confirm the provisional diagnosis.  相似文献

A 14‐month‐old half‐breed horse showed a severe lameness due to a lesion of the right forefoot which appeared completely and asymmetrically cleaved, from the solar margin to the coronary band. The horse was presented to an abattoir for slaughter and both forelimbs were collected for diagnostic investigations. The hoof lesion described herein does not fulfil the diagnostic criteria of equine polydactyly since post mortem imaging and pathological investigations ruled out the presence of supernumerary digits. Based on the clinical signs, diagnostic imaging (computed tomography) and gross examination, a diagnosis of pseudo‐polydactyly was proposed. To the best of our knowledge, the features of the present case report are unique, thus widening the range of lesions observed at the level of the hoof, which is crucial for horse health and activity.  相似文献

Three horses with clinical signs compatible with pleural effusion were examined with diagnostic ultrasound. In one of the three horses, confirmed septic pleural effusion was monitored with ultrasound during a successful treatment regimen. Eficacy of thoracentesis and detection of recurrence of fluid were readily determined. In the second horse, diagnosis of diffuse pleural neoplasia (lymphosarcoma) was made. In the third horse presented for pleural effusion, the effusion was documented and concurrent ascites detected with ultrasound. Carcinomatosis was found at necropsy. Ultrasound examination proved to be a sensitive, noninvasive modality useful in diagnosis and treatment of pleural effusion.  相似文献
