The risk of injury to veterinarians treating horses is high, particularly when performing painful procedures. The present retrospective study investigated this risk, including the frequency and severity of kick injuries, by submitting questionnaires to 700 veterinary practitioners in Switzerland. Evaluation of 216 completed questionnaires revealed that the frequency of kick injuries was affected by the professional experience of the veterinarian, veterinarians being an owner of a horse, the season, the type of procedure, the percentage of equine patients in the practice population, the treatment location (stall, pasture) and chemical restraint of the patient. Severe injuries incurred by veterinarians in recent years were few in number and the result of a number of unfortunate circumstances, which included hurried procedures due to time restraints and unforeseen events that frightened the patient. The risk factors derived from the full statistical model are the percentage of equine patients in the practice population, the treatment location, the type of procedure and chemical restraint of the patient. Using this information, it should be feasible to reduce the risk of injuries. Sedation of horses undergoing painful procedures, choosing a safe treatment location and caution on the part of the veterinarian are advised to reduce the number of injuries.  相似文献

Adverse drug reactions (ADRs) are part of the risks, professional responsibilities, and liabilities inherent to veterinary medicine. The incidence of proven ADRs is not known, but veterinarians should anticipate, plan, and practice a response for patients that experience adverse reactions. The attending veterinarian should work closely with professional services personnel at pharmaceutical manufacturers to provide care for those patients, to investigate causes of ADRs, and to factually improve reports of ADRs that can be of considerable medicolegal benefit to the client and the profession.  相似文献

As veterinarians, our responsibilities do not end with the care of our patients. The welfare of our clients and their families depends on our ability to detect and control potentially zoonotic diseases in their pets. Because some of these zoonoses can have devastating effects on the development of the unborn fetus or on family health in general, discussion about these diseases between veterinarian and client is often emotionally charged. Under such circumstances, the offering of inaccurate and erroneous information by the veterinarian can have drastic consequences. It is likely that other zoonotic diseases of domestic pets will be identified in the future, especially those that can cause opportunistic infections in debilitated and immunodeficient persons. In the meantime, the potential hazards of cat ownership can be significantly reduced through an increased understanding of feline diseases and an improved level of health care for our feline patients.  相似文献

OBJECTIVE: To describe the attitudes to their work and career of those Australian veterinarians who work with horses. METHOD: Questionnaires were mailed to 866 veterinarians who had been identified as working with horses, and 87% were completed and returned. Data were entered onto Excel spreadsheets, and analysed using the SAS System for Windows. RESULTS: The main attractions of veterinary work with horses were the horses themselves and the equine industries, but working outdoors and with rewarding clients, and the satisfaction of successful outcomes were attractions for many. The list of disadvantages was longer, and included unreasonable and disagreeable clients as well as those who provided inadequate facilities, could not control their horses or did not care for them. The physical demands and risks of injury, the amount of time required, low rates of return and difficulties in collecting payment, were other major disadvantages. Some mentioned concerns about litigation, unethical behaviour, and recruiting and retaining veterinarians competent with horses. For many in mixed practice, the difficulties in affording modern equipment, and of developing and maintaining their own competence with horses, was a real concern. More than three-quarters of the respondents reported that their careers had lived up to expectations and that they would become veterinarians again; 70% of equine veterinarians would become an equine veterinarian again. Almost all (93%) of the respondents were either very glad, or 'generally glad though with some misgivings' that they had done the veterinary course. Older veterinarians reported suffering less stress, and being more content with their career, than younger colleagues. CONCLUSIONS: The advantages of doing veterinary work with horses outweigh the disadvantages for most veterinarians, especially those well advanced in their careers.  相似文献

OBJECTIVE: To define and describe the population of Australian veterinarians who work with horses. METHOD: Questionnaires were mailed to 866 veterinarians who had been identified as working with horses, and 87% were completed and returned. Data were entered onto an Excel spreadsheet, and analysed using the SAS System for Windows. RESULTS: About 12% of Australia's veterinarians were doing all the veterinary work with horses, and about 3% worked exclusively (> 90%) with horses, but did more than half (58%) of the horse work. Veterinarians working with horses included more males (80%) than the veterinary population as a whole (approximately 60%). Males had an average age of 47 years, females 35. Almost all (94%) worked in private practice, with 31% being employees, 28% partners and 41% sole owners. Females were more likely to be employees than males. Males reported working 55 hours/week; females 49. More females (44%) than males (16%) had worked less than full-time for more than a year. Males expected to work for another 12 years in full-time equivalents, and females for 16. One quarter (24%) saw only horses, but treated 58% of total horse cases. One-half had < 25% horses, and 29% had < 10% of horses in their caseloads. More of the older (54% of those aged > 60) than younger respondents (27% of those < 40) had grown up on farms with animals. One-quarter (24%) decided to become a veterinarian while in primary school, and females decided at a younger age than males. Overall, younger respondents decided at a younger age than did their older counterparts. A veterinarian contributed to the decision for 21% of these veterinarians. CONCLUSION: In this survey, Australian veterinarians who work with horses were found to be typically male, and advanced in their careers. As these older veterinarians retire, there may not be enough veterinarians who are committed to and competent with horses to take their places.  相似文献

To protect the health and welfare of horses, veterinarians must legitimately use therapeutic medications. Many drugs are appropriately used to treat various disease conditions of horses, yet many of them have the ability to affect athletic performance or leave residues in tissues. For performance horses, the primary reason to have drug rules is to ensure fair competition and to protect the welfare of the horses and riders. When medications are administered to horses intended for human consumption, it is the responsibility of the veterinarian to ensure that violative drug residues do not occur. If horses are to receive proper veterinary care, suitable information on drug detection times must be available to equine practitioners.  相似文献

Two horses were examined for large head wounds suspected to be the result of trauma and characterized by extensive necrosis of the skin and subcutaneous tissue, with abundant purulent exudate. Corynebacterium pseudotuberculosis was isolated from the facial wounds in both horses. Histopathologic examination revealed severe suppurative cellulitis and panniculitis with fistulous tracts and granulation tissue in 1 horse. Both horses were treated with local wound care, nonsteroidal anti-inflammatory drugs, and administration of antimicrobials. The concept of moist wound care was used in the second horse, with products that have recently become available for veterinary wound management. Outcome in both horses was good.  相似文献

Second-intention repair is faster in ponies than in horses and faster in body wounds than in limb wounds. To a large extent, the differences between horses and ponies can be explained by differences in the local inflammatory response, which are a result of the functional capacity of leukocytes. In ponies, leukocytes produce more inflammatory mediators,resulting in better local defense, faster cellular debridement, and a faster transition to the repair phases, with more wound contraction. In horses,leukocytes produce fewer mediators, initiating a weak inflammatory response, which becomes chronic. This inhibits wound contraction and gives rise to the formation of exuberant granulation tissue. The anatomic environment that influences the inflammatory response and wound contraction most probably determines the differences between body and limb wounds. In body wounds, better perfusion results in faster initiation of the inflammatory phase. The weaker local resistance results in a greater degree of contraction. In limb wounds, particularly of horses, the initial inflammatory response is weak and wound contraction is restricted. Both factors give rise to chronic inflammation, which further inhibits wound contraction and promotes exuberant granulation tissue. The high incidence of exuberant granulation tissue in limb wounds of horses can thus be explained by the chronicity of the inflammatory response as well as by the common use of bandages during treatment. Chronic inflammation is often not recognized as a cause of exuberant granulation tissue. It must be prevented and treated to promote the healing process. Bandages and casts stimulate the formation of exuberant granulation tissue; however, they are advantageous in many respects and play an important role in support of the overall healing process.  相似文献

Following the emergence of Hendra virus (HeV), private veterinarians have had to adopt additional infection control strategies to manage this zoonosis. Between 1994 and 2010, seven people became infected with HeV, four fatally. All infected people were at a higher risk of exposure from contact with horses as they were either veterinary personnel, assisting veterinarians, or working in the horse industry. The management of emerging zoonoses is best approached from a One Health perspective as it benefits biosecurity as well as a public health, including the health of those most at risk, in this case private veterinarians. In 2011 we conducted a cross-sectional study of private veterinarians registered in Queensland and providing veterinary services to horses. The aim of this study was to gauge if participants had adopted recommendations for improved infection control, including the use of personal protective equipment (PPE), and the development of HeV specific management strategies during the winter of 2011. A majority of participants worked in practices that had a formal HeV management plan, mostly based on the perusal of official guidelines and an HeV field kit. The use of PPE increased as the health status of an equine patient decreased, demonstrating that many participants evaluated the risk of exposure to HeV appropriately; while others remained at risk of HeV infection by not using the appropriate PPE even when attending a sick horse. This study took place after Biosecurity Queensland had sent a comprehensive package about HeV management to all private veterinarians working in Queensland. However, those who had previous HeV experience through the management of suspected cases or had attended a HeV specific professional education programme in the previous 12 months were more likely to use PPE than those who had not. This may indicate that for private veterinarians in Queensland personal experience and face-to-face professional education sessions may be more effective in the improvement of HeV management than passive education via information packages. The role of different education pathways in the sustainable adoption of veterinary infection control measures should be further investigated.  相似文献

OBJECTIVE: To give an account of the views held by Australian veterinarians who work with horses on the future of their professional field. METHOD: Questionnaires were mailed to 866 veterinarians who had been identified as working with horses, and 87% were completed and returned. Data were entered onto an Excel spreadsheet, and analysed using the SAS System for Windows. RESULTS: Their future prospects were believed to be very good or excellent by >60% of equine veterinarians but by only 30% of mixed practitioners seeing < 10% horses. The main factors believed likely to affect these prospects were the strength of the equine industries and the economic climate affecting horse owners, followed by the encroachment of cities into areas used for horses, competition from other veterinarians including specialist centres and from non-veterinary operators, and their ability to recruit and retain veterinarians with interest, experience and skill with horses. Urban encroachment, competition and recruitment were especially important for those seeing few horses. Concerns were also expressed about the competence and ethical behaviour of other veterinarians, the physical demands and dangers of horse work, the costs of providing equine veterinary services and of being paid for them, the regulatory restrictions imposed by governments and statutory bodies, the potential effects of litigation, and insurance issues. For many veterinarians in mixed practice these factors have reduced and are likely to reduce further the number of horses seen, to the extent that they have scant optimism about the future of horse work in their practices. CONCLUSION: Economic and local factors will result in an increasing proportion of equine veterinary work being done in specialised equine centres, and the future of horse work in many mixed practices is, at best, precarious. A key factor influencing future prospects will be the availability of competent veterinarians committed to working with horses.  相似文献

AIM: To determine the frequency and type of skin wounds encountered by New Zealand veterinarians in their equine patients, the duration and estimated costs of treatment as well as the expected outcomes for these wounds.

METHODS: An online survey was sent to all veterinarians registered with the New Zealand Equine Veterinary Association. The survey comprised questions on the location and experience of respondents, the number of wound-related cases in relation to the total equine caseload, the type and anatomical location of wounds treated, the frequency, duration and costs of treatments, the outcome of wound treatment and an estimate of the most common causes of death or euthanasia in their equine patients.

RESULTS: The survey response rate was 110/262 (41.9%). The median number of equine cases seen by respondents was 20 (interquartile range (IQR) 6–60) per month; of these, five (IQR 2–10) were wound related. Wounds ranked third after lameness and respiratory disease for the relative frequency with which respondents encountered them. Of 102 respondents 59 (58%) reported that their clients frequently treated wounds incurred by their horse without consulting a veterinarian. Wounds on the distal limb, whether involving only the skin or also deeper structures, were reported by 86/101 (85%) respondents as the most frequently encountered. Wounds in this location also incurred the longest treatment period and were the most prone to develop complications. Finally, wounds ranked second, after colic, as the most common cause of death/euthanasia in the veterinary respondents’ equine patients.

CONCLUSIONS: The data obtained via the survey indicate that skin wounds, particularly on the distal limb, are a common occurrence in horses in New Zealand and, when they involve structures underlying the skin, are costly and time-consuming to manage and may lead to decreased performance, retirement or euthanasia. Consequently, we recommend that more effort be devoted to the education of equine veterinarians and owners, and that appropriate research funds be allocated to help improve patient outcomes.  相似文献

A questionnaire was designed to determine the incidence of major injuries, severe colic, and acute laminitis in horses at American Horse Shows Association (AHSA) A- and B-rated shows, ie, those shows requiring a veterinarian in attendance. The questionnaire was sent to the show stewards and filled out by the attending veterinarians of the AHSA A- and B-rated shows held from January 1982 to October 1984. Of 2,952 questionnaires sent out, 799 (27%) were completed and returned by veterinarians; 99 of them were discarded because of incomplete or misleading information. The study revealed a low incidence of major injuries, severe colic, and acute laminitis in horses at AHSA A- and B-rated shows. In 700 horse shows surveyed, there was a total of 231 major injuries, severe colic, or acute laminitis. Considering that the shows surveyed had a median number of 193 horses and a median duration of 3 days, the 231 injuries represent a relatively small number, compared with the general population at risk.  相似文献

OBJECTIVE: To describe the attitudes of veterinarians to their work, career and profession during the 10 years after graduation. DESIGN: Longitudinal study of students who started their course at The University of Queensland in 1985 and 1986, and who completed questionnaires in their first and fifth year as students, and after one, five and 10 years as veterinarians. METHODS: Data from 129 (96%) questionnaires completed after 10 years as a veterinarian were coded numerically then analysed, together with data from previous questionnaires, with SAS System 7 for Windows 95. RESULTS: After 10 years, almost all respondents were either very glad they had done the veterinary course (57%) or generally glad, though with some misgivings (37%). Despite this, only 55% would definitely become a veterinarian if they 'had to do it over again'. The responses for about one-third were different from those given five years earlier. The views of many were related to the level of support and encouragement received in their first job after graduation. There were 42% who were working less than half-time as veterinarians, and their main reasons were, in order, raising children, long hours of work, attitudes of bosses and clients, and poor pay. A majority was concerned about the ethics and competence of some colleagues, and almost all believed that consideration of costs must influence the type of treatment animals receive. CONCLUSIONS: Most veterinarians were glad to have done the veterinary course, but for about one-quarter their career had not lived up to expectations and almost half would not do it again in another incarnation. Stress, hours of work, difficulties in balancing personal life with career and low income were important concerns for many. Low income may contribute to the low number of males entering the veterinary profession.  相似文献

Over time, evidence-based veterinary medicine (EBVM) should integrate with normal clinical practice. Also, clinical knowledge increases with EBVM, reducing the need for information in one area and allowing veterinarians to explore new areas of specialty or cutting-edge advances in the profession. Textbooks, journals, veterinary conferences, and web sites provide nearly unlimited information about EBVM for the practicing veterinarian to help with the transition to EBVM use in daily practice life. Nonhealing wounds in cats are a source of frustration for veterinarians and pet owners alike. These wounds are known for their insidious onset and tendency to recur after standard surgical closure. The presence of a feline wound that fails to heal in the face of appropriate therapy should initiate an extensive diagnostic evaluation for an etiologic agent. Initially animals should be screened for causes of immunosuppression or systemic disease. The local wound environment should then be evaluated through a combination of wound exploration, cytology, histopathology and tissue culture for evidence of a foreign body, neoplasia, or other less common etiologies of these wounds. Many of the less common etiologic agents require specific tissue handling for successful microscopic identification and culture. It is therefore essential for veterinarians to be familiar with the various causes of nonhealing wounds in cats so that laboratories can be alerted of the need for specific diagnostic techniques. Recent surgical and medical advances have the potential to facilitate the successful treatment of these wounds. With improved veterinary awareness of the potential etiologies of nonhealing wounds in cats and improved medical and surgical management techniques many wounds can now successfully be treated.  相似文献
