BACKGROUND: Multi-frequency bioelectrical impedance analysis (MF-BIA) has been used to evaluate extracellular fluid volume (ECFV), but not fluid fluxes associated with fluid or furosemide administration in horses. If able to detect acute changes in ECFV, MF-BIA would be useful in monitoring fluid therapy in horses. HYPOTHESIS: The purpose of this study was to evaluate the ability of MF-BIA to detect acute fluid compartment changes in horses. We hypothesized that MF-BIA would detect clinically relevant (10-20%) changes in ECFV. ANIMALS: Six healthy mares were used in the study. METHODS: This is an original experimental study. Mares were studied in 3 experiments: (1) crystalloid expansion of normally hydrated subjects, (2) furosemide-induced dehydration followed by crystalloid administration, and (3) acute blood loss followed by readministration of lost blood. MF-BIA measurements were made before, during, and after each fluid shift and compared to known changes in volume calculated based on the intravenous fluids that were administered in addition to urinary fluid losses. Mean errors between MF-BIA estimated change and known volume change were compared using nonparametric analysis of variance. Estimated ECFV pre- and post-fluid administration similarly were compared. The level of statistical significance was set at P < .05. RESULTS: Results of the study revealed a statistically significant change in ECFV and total body water during crystalloid expansion and dehydration. Statistically significant changes were not observed during blood loss and administration. Mean errors between MF-BIA results and measured net changes were small. CONCLUSIONS AND CLINICAL IMPORTANCE: MF-BIA represents a practical and accurate means of assessing acute fluid changes during dehydration and expansion of ECFV using isotonic crystalloids with potential clinical applications in equine critical care. 相似文献

A2-year-old Thoroughbred filly presented to the Veterinary Medical Teaching Hospital (VMTH) at the University of California, Davis, for evaluation of a bleeding disorder. The bleeding tendency was 1st noted 3 weeks before presentation on the morning after the horse was pin fired. Prolonged bleeding was noted after intramuscular injection of procaine penicillin G, and the trainer noted blood from the horse's right nostril 2 days before presentation. The filly had been seen by a referral practice, which found a prolonged prothrombin time but no other problems. The horse received an injection of vitamin K intravenously the day before presentation. The current owners purchased the horse ˜1 year before the initial bleeding episode, during which time she had not injured herself or had any intramuscular injections. The filly had received 1 dose PO of a dexamethasone (5 mg) and trichlormethiazide (200 mg) combinationa and 1 or 2 doses of phenylbutazone (unknown amount) but had not received aspirin. CASE HISTORY: Lameness and limb pain associated with a laceration in the inner thigh of a Border Collie dog progressed over 4 days to extensive necrosis of the full-thickness of skin and subcutaneous (S/C) tissue. A successful outcome was achieved using surgical debridement and intensive supportive care, followed by limited local closure, axial pattern flap development, and free skin grafting. CLINICAL FINDINGS: Clinical findings included severe pain, depression, pyrexia and hypoalbuminaemia, and full-thickness loss of skin from the caudal thigh to the hock. Histopathologically, debrided tissue showed extensive necrosis of the dermis, adipose and muscle tissues, and the presence of numerous Gram-negative rods. Escherichia coli was cultured from deep tissue samples. DIAGNOSIS: Necrotising fasciitis (NF) associated with E. coli infection. CLINICAL RELEVANCE: NF associated with E. coli has not been previously reported in dogs. The importance of early diagnosis and surgical debridement is noted and the relevant medical literature is reviewed. 相似文献

A suspected inherited form of rickets was diagnosed in Corriedale sheep on a commercial sheep farm in Marlborough. Affected lambs were normal at birth but showed a reduced growth rate and a range of skeletal abnormalities, including both varus and valgus deformities and a characteristic dip in the shoulder region. Up to 20 affected lambs had been born on the property over the last two seasons. Skeletal lesions in affected lambs included swollen costochondral junctions, moderate to marked thickening of physes in long bones, and collapse of weakened subchondral bone, particularly in the proximal humerus. These lesions, together with the microscopic changes, strongly suggested a diagnosis of rickets. Affected lambs were hypocalcaemic and hypophosphataemic, consistent with a deficiency of vitamin D. Vitamin D assays revealed significantly higher 1,25(OH) vitamin D concentrations in affected lambs than in controls, suggesting a defect in the vitamin D receptor as the likely mechanism. DNA testing revealed that 4/10 Corriedale rams used during the previous season had sired affected lambs. Background: Botulism is a potentially fatal paralytic disorder for which definitive diagnosis is difficult. Objectives: To determine if repetitive stimulation of the common peroneal nerve will aid in the diagnosis of botulism in foals. Animals: Four control and 3 affected foals. Methods: Validation of the test in healthy foals for its comparison in foals with suspected botulism. Controls were anesthetized and affected foals were sedated to avoid risks of anesthesia. The common peroneal nerve was chosen for its superficial location and easy access. Stimulating electrodes were placed along the common peroneal nerve. For recording, the active and reference electrodes were positioned over the midpoint and distal end of the extensor digitorum longus muscle, respectively. Repeated supramaximal stimulation of the nerve was performed utilizing a range of frequencies (1–50 Hz). Data analysis consisted of measuring the amplitude and area under the curve for each M wave and converting these values into percentages of decrement or increment based on the comparison of subsequent potentials to the initial one (baseline) within each set. Results: A decremental response was seen at all frequencies in control foals. Decremental responses also were observed in affected foals at low frequencies. An incremental response was seen in all affected foals at 50 Hz. Conclusions and Clinical Importance: Decreased baseline M wave amplitudes with incremental responses at high rates are supportive of botulism. Repetitive nerve stimulation is a safe, simple, fast, and noninvasive technique that can aid in the diagnosis of suspected botulism in foals. 相似文献
